    <p> In cooperation with the Kinki branch office of the Japan Marrow Donor Program, we verified the problems in current coordination and constructed a WEB system to provide a notification of the harvest schedule at each hospital. After introducing this system, the median time between donor selection and collection and actual transplant were 70-78 days in 2013-2016 and 57-63 days in 2017-2020 without refusal. The collection rate in the first-choice week of patients increased 29-38% in 2013-2016, to 32% in 2017, and 63-73% in 2018-2020. Additionally, the role of the hematopoietic cell transplant coordinator was important for improving the efficiency of collection and reducing the workload of hematologists. This system helped improve the efficiency of harvesting. In contrast, there was disparity in the harvest experience of various hospitals. In the future, the development of teaching harvest techniques and skills to improve donor safety will be an important problem.</p>

    <p>Monoclonal gammopathy of undermined significance (MGUS) is usually asymptomatic, and untreated follow-up is the standard treatment. However, MGUS progresses to multiple myeloma or related malignancy at a frequency of 1.5% per year. It is sometimes difficult to diagnose the progression of the disease via usual examinations. We herein report a case wherein rapid renal dysfunction led to a diagnosis of disease progression to multiple myeloma in a patient with MGUS that was asymptomatic for a long time. A 66-year-old woman developed rapid renal dysfunction requiring continuous hemodiafiltration 8 years after the diagnosis of IgA-κ type MGUS. A complete examination led to the diagnosis of IgA-κ type multiple myeloma. Chemotherapy was not effective, and she died due to sepsis on the 19th day of admission. A pathological autopsy revealed systemic amyloidosis and multiple abscesses positive for <i>Staphylococcus aureus</i>. An abnormal free light chain κ/λ ratio and M protein other than IgG are reportedly risk factors of disease progression of MGUS. In cases with these risk factors, it is important to always keep in mind the possibility of disease progression and to monitor the patient carefully for an early diagnosis.</p>

    <p>There is currently no evidence that a severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) mRNA vaccine might be associated with the development of autoimmune hemolytic anemia or disease progression in patients with mature B-cell neoplasm. Our patient was a 71-year-old man with indolent mature B-cell neoplasm who had been monitored for many years without treatment. After receiving the second dose of the BNT162b2 mRNA COVID-19 vaccine, he developed severe warm autoimmune hemolytic anemia. Although steroid therapy improved his anemia, he continued to develop IgM-monoclonal gammopathy, renal insufficiency, and splenomegaly. He was diagnosed with splenic marginal zone lymphoma after undergoing splenectomy. The splenectomy improved the patient’s symptoms. We assessed his SARS-CoV-2 specific antibody response, but the patient’s serologic response to the vaccine was impaired. In patients with mature B-cell neoplasm, a non-specific immune response after vaccination might be associated with paraneoplastic syndromes.</p>

    <p>Chylothorax is a rare clinical sign in patients with diffuse large B-cell lymphoma (DLBCL), which is often challenging to manage and has a poor prognosis. We report the case of a 59-year-old woman who presented with right pleural effusion at the time of DLBCL diagnosis. Lymphadenopathy rapidly improved in response to chemotherapy. However, the pleural effusion progressed and was identified as chylothorax by thoracentesis. Because attempts to manage the condition with fasting and central venous nutrition were unsuccessful, we performed ultrasound-guided intranodal lipiodol lymphangiography from the inguinal lymph node. Although leak sites were not detected, the pleural effusion markedly improved on the day after the examination and resolved after 2 months. Lymphangiography is a minimally invasive examination with few complications. It contributes not only to the identification of leak sites but also to the improvement and resolution of chylothorax. Therefore, lymphangiography should be considered for refractory chylothorax that is unresponsive to chemotherapy or nutritional management.</p>

    <p>Bone marrow necrosis (BMN) is a rare but important complication of hematological malignancies. We report the case of a 52-year-old male patient with a recurrence of acute lymphoblastic leukemia (ALL) accompanied by BMN. After re-induction therapy, bone marrow aspiration (BMA) and biopsy from the iliac bone showed necrotic cells and eosinophilic debris, respectively. Magnetic resonance imaging (MRI) showed heterogeneous signals in the bilateral iliac bone, possibly reflecting various stages of BMN. BMA from the sternum eventually revealed the recurrence of ALL after a few weeks. Comprehensive assessments, including MRI and repeated bone marrow tests, are required when evaluating the underlying hematological malignancies of patients with BMN. </p>

    <p><b>Objective </b>Hemorrhagic pneumonia due to <i>Stenotrophomonas maltophilia</i> (SM) in severely immunocompromised patients has a very poor prognosis. However, the risk factors for hemorrhagic pneumonia are not clear. </p><p><b>Methods </b>This study assessed the predictive factors of hemorrhagic pneumonia caused by SM. The medical records of patients admitted to Osaka City University Hospital with SM bacteremia between January 2008 and December 2017 were retrospectively reviewed. </p><p><b>Patients </b>All patients who had positive blood cultures for SM were included in this study. They were categorized into two groups: the SM bacteremia with hemorrhagic pneumonia group and the SM bacteremia without hemorrhagic pneumonia group. The clinical background characteristics and treatments were compared between these groups. </p><p><b>Results </b>The 35 patients with SM bacteremia included 4 with hemorrhagic pneumonia and 31 without hemorrhagic pneumonia. Hematologic malignancy (p=0.03) and thrombocytopenia (p=0.04) as well as the prior use of quinolone within 30 days (p=0.04) were more frequent in the SM bacteremia patients with hemorrhagic pneumonia than in those without hemorrhagic pneumonia. The mortality of the SM bacteremia patients with hemorrhagic pneumonia was higher than that of those without hemorrhagic pneumonia group (p=0.02). </p><p><b>Conclusion </b>Patients with SM bacteremia who have hematologic malignancy, thrombocytopenia, and a history of using quinolone within the past 30 days should be treated with deliberation. </p>

    <p>A 65-year-old woman was urgently admitted to our hospital for antibiotic-resistant fever, hypoxemia, and hyperleukocytosis and was diagnosed with acute monoblastic leukemia. Chest computed tomography revealed interlobular septal thickening, central ground-glass opacity, and a nodular shadow in the left lower lobe. Although several treatments for infectious disease and acute heart failure were administered, they were less effective. Transbronchial lung biopsy was performed on day 7 of hospitalization, and subsequently, pulmonary leukemic infiltration was confirmed. Based on the diagnosis, we decided to start intensive chemotherapy. Consequently, the abnormal lung shadow on computed tomography vanished, and complete hematological remission was achieved. Although acute myeloid leukemia is frequently associated with lung infiltration during onset, it is often difficult to distinguish it from other pulmonary complications. In clinical practice, intensive chemotherapy is often initiated based on the clinical evaluation without pathological confirmation of the lung disease. Our patient was accurately diagnosed based on the pulmonary leukemic infiltration observed pathologically and recovered well. Here we report our case along with a discussion of the relevant literature.</p>

    <p>Post-transplant erythrocytosis (PTE) following allogeneic hematopoietic stem cell transplantation (alloHSCT) is rare, and the clinical characteristics of this condition remain unknown. In this study, we examined the clinical characteristics of three PTE cases among 321 patients who received allo HSCT from January 1992 to December 2011 at our institution. All three patients exhibited normal levels of white blood cell and platelet counts when their hemoglobin levels reached their peak. Two patients exhibited normal levels of erythropoietin. No thrombosis or hemorrhage was observed in any of the three patients without cytoreductive therapy or an antiplatelet agent. All three patients tested negative for JAK2V617F mutations. Two patients had high levels of IL-13, an upstream signal for the JAK/STAT pathway. JAK2 is known to significantly contribute to the pathology of polycythemia vera; however, this pathology may differ from that of PTE. We believe that it is necessary to construct a more appropriate management structure for PTE by analyzing more case data in the future.</p>

    <p>A 53-year-old man presented with uncontrolled bleeding caused by acquired platelet dysfunction accompanied by calreticulin-mutated primary myelofibrosis. Based on the detection of abnormal platelets, including large gray platelets, under light microscopy and the loss of the second wave of aggregation observed by light transmission aggregometry, the patient was diagnosed with platelet dysfunction accompanied by myeloproliferative neoplasms (MPNs). In addition, the absence of platelet α-granules was confirmed by electron microscopy. Therefore, this condition may be termed "acquired gray platelet syndrome." Acquired platelet dysfunction must be ruled out when abnormal platelets are observed in patients with MPNs. </p>

    <p>Refractory viremia/viral disease is a major life-threatening complication that may arise among patients undergoing allogeneic hematopoietic cell transplantation (allo-HCT). This study aimed to clarify the therapeutic effect of high-dose polyclonal intravenous immunoglobulin (IVIG) against viremia/viral diseases after allo-HCT. We conducted a pilot study to investigate the therapeutic effect of 400 mg/kg of IVIG given for 5 consecutive days against refractory viremia/viral disease after allo-HCT. Overall, 7 patients were drug-resistant and the other 7 had not previously received any drug for their viremia/viral disease. All patients completed the 5-day therapy regimen of IVIG. A complete response at Day 56 was observed for 8 of 14 patients (57.1%). Additionally, 10 of 14 patients (71.4%) were alive at Day 56, although only one death occurred due to the viremia/viral disease. Remarkably, all 3 cases who developed exogenous viremia/viral diseases including respiratory syncytial virus pneumonia/bronchitis and human parvovirus B19 viremia achieved a complete response, suggesting that high-dose polyclonal IVIG may be more effective against exogenous viruses rather than endogenous ones. Congestive heart failure was observed in 1 patient. High-dose polyclonal IVIG could be an effective and feasible therapy for refractory viremia/viral disease after allo-HCT.</p>

    <p>In allogeneic hematopoietic stem cell transplantation (HSCT), ascites may develop owing to several causes, including sinusoidal obstruction syndrome, infections, malignancies, and malnutrition. However, it is often difficult to determine its precise cause. Here, a 59-year-old male developed chylous ascites three months post allogeneic bone marrow transplantation for relapsed acute myeloid leukemia. None of the attempted treatments resulted in improvement. Lymphangioscintigraphy revealed a lymphatic flow disorder at the level of the cisterna chyli. Autopsy revealed no leukemic cell infiltration or graft-versus-host disease of the liver or pancreas. The pancreatic specimen revealed parenchymal fibrosis and infiltration of plasma cells, suggesting chronic inflammation in addition to pathological changes caused by acute pancreatitis. These findings indicate that acute or chronic pancreatitis caused a lymphatic flow disorder that developed into refractory ascites. Although we could not diagnose pancreatitis while the patient was alive, it is important to recognize that asymptomatic pancreatitis can develop after HSCT. Furthermore, one should attempt to make an accurate diagnosis as early as possible.</p>

    当施設で骨髄破壊的前処置による同種造血幹細胞移植(HSCT)を受けた患者において、類洞閉塞症候群(SOS)の予防目的で行った凝固薬とウルソデオキシコール酸(UDCA)の併用投与の有効性を後ろ向きに評価した。2000年6月〜2014年5月にHSCT後のSOS予防のためUDCAとダルテパリンまたはダナパロイドのいずれかが併用投与された患者(I群)59例とUDCA単剤を投与された患者(II群)193例を対象とした。Seattle基準、Baltimore基準によるそれぞれのSOS発症率、および移植後30日、100日以内の出血イベント発症率を検討した。I群の59例中9例(15.3%)とII群の193例中28例(14.5%)がSOSを発症した。SOS発症率に関して両群間に有意差は認めなかった。移植後30日または100日以内の出血イベントの発症率においても、両群間で有意差を認めなかった。以上から、HSCT後のSOS予防にUDCAを使用する場合、抗凝固薬を併用投与することの意義は乏しいと考えられた。

    <p>Chronic graft versus host disease (cGVHD) of the central nervous system is a rare condition that could occur after allogeneic hematopoietic stem cell transplantation (SCT) but has been poorly documented. Here, we report a patient diagnosed with recurrent acute disseminated encephalomyelitis (ADEM) with longitudinal extensive transverse myelitis (LETM) as cGVHD after HLA haploidentical peripheral blood SCT using posttransplantation cyclophosphamide for mixed-phenotype acute leukemia. We observed clinical and radiological improvement after the rituximab treatment of the condition that had been refractory to steroids. To the best of our knowledge, no report of cGVHD presented recurrent ADEM with LETM after allogeneic SCT and successfully treated with rituximab. Hence, ADEM should be included in the differential diagnosis of neurological symptoms in posttransplant patients with cGVHD.</p>

    血圧コンポーネントである収縮期血圧(SBP)、拡張期血圧(DBP)、脈圧、平均動脈圧(MAP)のうち、蛋白尿発症の最も有用な予測因子を、健康な日本人中年男性を対象とする前向きコホート研究により検討した。2000年4月〜2001年3月の登録時に糖尿病および蛋白尿がなく、推算糸球体濾過量≧60mL/分/1.73m2で、降圧薬未服用の40〜55歳の日本人男性9341名(登録時平均年齢48.2±4.2歳)を解析対象とした。まず、交絡因子から成るベースラインモデルに各血圧コンポーネントを個別に追加した。次に、各血圧コンポーネント追加によって持続性蛋白尿発症の予測能がそれぞれどの程度向上するのかを赤池情報量規準(AIC)により比較した。その結果、84587人・年の観察期間中に151名で持続性蛋白尿発症が確認された。いずれの血圧コンポーネントも持続性蛋白尿発症の有意な予測因子であった。第1四分位に対する第4四分位のハザード比は、SBPでは3.11(95%信頼区間[CI] 1.79-5.39)、脈圧は1.87(95%CI 1.18-2.94)、MAPは2.21(95%CI 1.33-3.69)と有意であった。DBPと持続性蛋白尿発症の関係はU字カーブであり、DBP第2四分位に対する第4四分位のハザード比は2.69(95%CI 1.65-4.38)と有意であった。ΔAICは、SBP追加モデルとDBP追加モデルにおいて最大であった。以上から、日本人中年男性において、SBPとDBPは持続性蛋白尿発症の最も有用な予測因子であると考えられた。

    <p><i>Background:</i> We examined prospectively which of the four blood pressure (BP) components (systolic BP [SBP], diastolic BP [DBP], pulse pressure [PP], and mean arterial pressure [MAP]) was best in predicting the risk of proteinuria.</p><p><i>Methods:</i> This prospective study included 9341 non-diabetic Japanese middle-aged men who had no proteinuria and an estimated glomerular filtration rate ≥60 mL/min/1.73 m<sup>2</sup> and were not taking antihypertensive medications at entry. Persistent proteinuria was defined if proteinuria was detected two or more times consecutively and persistently at the annual examination until the end of follow-up. We calculated the difference in values of Akaike's information criterion (ΔAIC) in comparison of the BP components-added model to the model without them in a Cox proportional hazards model.</p><p><i>Results:</i> During the 84,587 person-years follow-up period, we confirmed 151 cases of persistent proteinuria. In multiple-adjusted models that included a single BP component, the hazard ratios for persistent proteinuria for the highest quartile of SBP, PP, and MAP were 3.11 (95% confidence interval [CI], 1.79–5.39), 1.87 (95% CI, 1.18–2.94), and 2.21 (95% CI, 1.33–3.69) compared with the lowest quartile of SBP, PP, and MAP, respectively. The hazard ratio for the highest quartile of DBP was 2.69 (95% CI, 1.65–4.38) compared with the second quartile of DBP. Of all models that included a single BP component, those that included SBP alone or DBP alone had the highest values of ΔAIC (14.0 and 13.1, respectively) in predicting the risk of persistent proteinuria.</p><p><i>Conclusions:</i> Of all BP components, SBP and DBP were best in predicting the risk of persistent proteinuria in middle-aged Japanese men.</p>

    症例は69歳女性で、2ヵ月前に原因不明の皮疹が出現し、その後、高熱、咽喉痛、全身疲労がみられた。今回、進行性白血球減少症と持続性発熱のため入院となった。数年間にわたり降圧薬、抗脂質異常薬、血栓予防目的でアスピリンを服用しており、1年前からは非ウイルス性肝炎に対してウルソデオキシコール酸を処方していた。所見から薬剤誘導性無顆粒球症と仮診断し、全ての処方薬を中止した。4日目にG-CSF投与とラミナエアフロー隔離を行った。広域抗生物質投与により高熱と咽頭痛は改善したが、好中球数は回復しなかった。その後、重度肺炎を発症し、11日目のCTで前縦隔腫瘍が明らかになった。G-CSF投与を継続した上で、20日目から高用量メチルプレドニゾロン静注療法(1g/日を3日間)を開始し、その後プレドニゾロン(1mg/kg)を投与した。40日目には好中球数が次第に正常範囲に改善したが、アスペルギルス肺炎と脳出血により46日目に死亡した。

    ダサチニブ、イマチニブ、ニロチニブによる治療を行った慢性期の慢性骨髄性白血病(CML)患者30例を対象に、ダサチニブ治療中の免疫細胞数とナチュラルキラー(NK)細胞反応性の変化、免疫学的変化と治療奏効の関連性、大顆粒リンパ球(LGL)の抗CML作用を評価した。さらに、ダサチニブ治療中のLGLおよびNK細胞の拡大に関与する可溶性因子を特定し、LGLリンパ球増加を発症する患者の特性を検討した。NK細胞反応性とLGL数は、ダサチニブ治療中に有意に増強した。NK細胞反応性と制御性TNF細胞数は、ダサチニブ治療の奏効の程度と有意に相関した。ベースラインとダサチニブ治療開始後3、6ヵ月時のT-LGL数には有意な正の相関が認められた。ダサチニブ治療により増強したNK細胞の細胞毒性が抗CML作用の根拠であり、ダサチニブ治療前のT-LGL数はダサチニブ治療の予後的指標となりうることが示唆された。

    <p>Although anti-neutrophil antibodies (ANAs) often exist and immunoreaction may be involved in agranulocytosis, few reports have so far described ANA-positive cases of agranulocytosis with an unknown etiology. We herein describe the case of a 69-year-old woman who presented with ANA-positive agranulocytosis. In this case, both the withdrawal of the drugs that had possibly caused neutropenia and the use of granulocyte-colony stimulating factor (G-CSF) were ineffective treatment measures. Approximately 2 weeks after the discontinuation of the suspected drugs, we initiated corticosteroid pulse therapy; the neutrophil count recovered by day 19 of steroid therapy. High-dose methylprednisolone therapy should thus be considered for patients demonstrating ANA-positive agranulocytosis with an unknown etiology that is refractory to G-CSF treatment. </p>

    <p><b>Objective </b>Recent studies suggest that presepsin (soluble CD14-subtype) is a useful diagnostic and prognostic marker for sepsis, with secretion by activated macrophages potentially dependent on phagocytosis of microorganisms. As "hemophagocytosis" is one of the major characteristics in patients with hemophagocytic syndrome (HPS), we hypothesized that presepsin may reflect the phagocytic activity and be a useful prognostic marker for HPS. Therefore, we aimed to assess the prognostic potential of presepsin in secondary HPS in adult patients with hematological malignancies. </p><p><b>Methods </b>Between April 2006 and August 2014, we retrospectively examined consecutive patients with HPS whose blood samples were available at our institution and compared the prognostic value of the following in HPS, singly and in combination: plasma presepsin, serum soluble interleukin (IL)-2 receptor (sIL-2R), ferritin, tumor necrosis factor-α (TNF-α), interferon-γ (IFN-γ), IL-6 and IL-10. </p><p><b>Results </b>A total of 14 patients were enrolled. The median age of the patients was 46.5 years (range, 22-65). In univariable Cox models, there were no significant variables associated with the prognosis. However, in 12 evaluable patients, only the combination of higher median values of presepsin (>1,935 pg/mL) and sIL-2R (>4,585 U/mL) at the onset of HPS was significantly associated with the 90-day mortality (hazard ratio 14.5; 95% CI, 1.47-143.36; p=0.02). </p><p><b>Conclusion </b>These results suggest that a composite model of plasma presepsin and serum sIL-2R levels at the onset of HPS might be a novel predictor of the prognosis of patients with hematological malignancies and secondary HPS. </p>

    <p>We herein describe a case of myelodysplastic syndrome with chronic graft-versus-host disease (cGVHD)-related polymyositis. On approximately day 1,570 post HLA-identical sibling bone marrow transplant, the patient presented with a fever, myalgia and liver dysfunction. A muscle biopsy revealed destruction of the muscle fibers and infiltration of CD20<sup>+</sup> B cells and CD4<sup>+</sup> and CD8<sup>+</sup> T cells, and a liver biopsy confirmed the findings of cGVHD. An analysis of plasma cytokine profiles indicated elevation of not only T-helper (Th)1 and Th2, but also Th17 cytokines. Increases in these cytokines in addition to the invasion of inflammatory cells might be associated with the pathophysiology of cGVHD involving the muscle and liver. </p>

    症例は64歳男性で、主訴は発熱と疲労であった。末梢血では汎血球減少症が認められ、骨髄検査、形態学的検査、細胞遺伝学的検査から、骨髄異形成症候群(MDS)と確定診断した。MDSの治療計画を行ったが、患者が通院を中断してしまったため、実施することが出来なかった。しかし、3ヵ月後に右眼の視力障害を訴えて入院となり、両眼に軽度核白内障、右眼に相対性求心性瞳孔反応欠損が認められた。蛍光眼底撮影、細隙灯顕微鏡検査所見から、虚血性および非虚血性変化に起因する視神経炎と診断した。血液学的検査では汎血球減少症の改善と末梢血および骨髄で好酸球数の著増が認められたが、視神経炎に対してはメチルプレドニゾロンパルス療法を開始したところ、2週間以内に視神経乳頭浮腫が消失し、末梢血の好酸球数も急速に減少し、視野も部分的に改善が得られた。その後、低用量プレドニゾロンの維持療法が行われているが、視野欠損の悪化もみられず、11ヵ月間良好に維持されている。

    症例は43歳、女性。前医にて前縦隔腫瘤を指摘され、胸腺癌の疑いで切除術を施行された。術後病理検査の結果、悪性リンパ腫の診断であった。当院にて精査の結果、縦隔原発大細胞型B細胞性リンパ腫(primary mediastinal large B-cell lymphoma、PMBL)の臨床病期IV期と診断した。初回治療としてCHOP-R療法を選択し、現在3コースまで施行し、経過良好である。PMBLはまれな疾患であり、リスク分類および治療選択に関してはまだ十分に確立されていない。今回我々は若年女性に発症した初発PMBLの1例を経験した。初回治療選択および長期治療戦略に関して文献的考察を加えて報告する。(著者抄録)

    Myelodysplastic syndrome (MDS) is known to often be complicated by a range of autoimmune diseases. We herein present a case with MDS complicated by cold autoimmune hemolytic anemia (cold AIHA). The patient was a 51-year-old woman. She was diagnosed with MDS (refractory cytopenia with multilineage dysplasia) in May 2009. In January 2010, she underwent unrelated allogeneic bone marrow transplantation but was re-admitted in October 2010 for treatment of relapsed MDS. Despite daily transfusions of red blood cells, her anemia failed to improve. Her laboratory examinations showed a low haptoglobin level and elevation of indirect bilirubin and LDH. The direct Coombs test was positive at a low and at room temperature and cold agglutinin was negative. After confirming the diagnosis of cold AIHA, all transfusion fluids were warmed but her anemia still failed to improve. In addition to the warmed transfusion fluids, we administered corticosteroids, immunosuppressive agents and high-dose intravenous immunoglobulin infusions. This management strategy ameliorated the patient's hemolytic anemia. To our knowledge, MDS cases complicated by cold AIHA are rare. Our patient thus provides a valuable contribution to medical knowledge.

    Myelodysplastic syndrome (MDS) was diagnosed in a 64-year-old man. Three months later, he presented with right-sided visual loss. A diagnosis of optic neuritis caused by both ischemic and non-ischemic changes was established. Concurrently, prominent eosinophilia was seen in both the peripheral blood and bone marrow. A partial improvement of visual loss was obtained concomitant with a rapid decrease of the eosinophils after treatment with corticosteroids. Optic neuritis related to MDS is a rare condition and its etiology has not yet been identified. We herein report a case of optic neuritis associated with MDS and accompanied by an eosinophilic crisis.<br>

    症例は65歳女性,1992年5月にクームス試験陽性自己免疫性溶血性貧血と赤芽球癆の合併と診断された。その後赤芽球癆は改善したが,新たに血小板減少を認め,骨髄検査にて特発性血小板減少性紫斑病の合併と考えられ,Evans症候群と診断された。Prednisoloneおよびazathioprineを投与し軽快,以後約20年に渡り投薬を漸減しつつ寛解増悪を繰り返していた。2012年10月,溶血発作の再増悪とB症状及びリンパ節腫脹が出現した。リンパ節生検の結果DLBCLと診断された。一方クームス試験は陰性,精査にてクームス陰性自己免疫性溶血性貧血も否定的であり,リンパ腫に関連した他の原因による溶血の可能性が疑われた。CHOP療法を施行したところ,溶血およびB症状は改善し,リンパ節病変も縮小を認めた。Evans症候群後DLBCLの発症の報告は稀であり,示唆に富む症例と考えられ,今回報告する。(著者抄録)

    We describe an 18-year-old man with acute leukemia who presented with posterior reversible encephalopathy syndrome (PRES) shortly after developing acute pancreatitis. On day 15 after the third consolidation course with high-dose cytarabine, treatment with broad-spectrum antibiotics was initiated for febrile neutropenia. On day 16, he developed septic shock, and subsequently, acute respiratory distress syndrome (ARDS). After adding vancomycin, micafungin and high-dose methylprednisolone (mPSL) to his treatment regimen, these manifestations subsided. On day 22, he received hemodialysis for drug-induced acute renal failure. On day 24, he developed acute pancreatitis possibly due to mPSL; the following day he had generalized seizures, and was intubated. Cerebrospinal fluid findings were normal. Brain MRI revealed hyperintense signals on FLAIR images and increased apparent diffusion coefficient values in the sub-cortical and deep white matter areas of the bilateral temporal and occipital lobes, indicative of vasogenic edema. Thus, we diagnosed PRES. Blood pressure, seizures and volume status were controlled, with MRI findings showing improvement by day 42. He was extubated on day 32 and discharged on day 49 without complications. Although little is known about PRES following acute pancreatitis, clinicians should be aware that this condition may develop.

    A 65-year-old woman was diagnosed with Coombs-positive autoimmune hemolytic anemia (AIHA) and pure red cell aplasia (PRCA) in May 1992. One month later, her PRCA went into remission following treatment but she developed idiopathic thrombocytopenic purpura and was diagnosed with Evans syndrome. Although her condition resolved with administration of prednisolone and azathioprine, it was necessary to continue treatment with gradual tapering over the following two decades. In October 2012, her hemolytic anemia again worsened, and lymph node swelling, splenomegaly and B symptoms developed. She was diagnosed as having diffuse large B-cell lymphoma (DLBCL) based on lymph node biopsy. However, AIHA was not considered to be the cause of her hemolytic anemia, but rather to be related to DLBCL. This was because a Coombs test and other extensive investigations for Coombs negative-AIHA yielded negative results. The patient underwent CHOP therapy, and all of her symptoms improved. Herein, we report this rare case in which DLBCL developed after the onset of Evans syndrome.

    Cerebral toxoplasmosis is a rare, potentially fatal, complication of hematopoietic cell transplantation. Early definitive diagnosis is very difficult and it may be associated with a poor prognosis. Herein, we describe a 60-year-old woman who developed cerebral toxoplasmosis after cord blood transplantation for myelodysplastic syndrome. During treatment with tacrolimus and methylprednisolone for relapsed grade 2 acute gut GVHD, fever and disturbance of consciousness occurred on day 210. Brain MRI showed multiple ring-enhancing nodular lesions in the thalamus, basal ganglia, brainstem, and subcortical white matter. Cerebrospinal fluid (CSF) assessment revealed elevations of both anti-to-xoplasma IgM and IgG, which were also elevated in serum, but no evidence of other infections or malignancies. Notably, the IgM level was higher in the CSF than in serum. Thus, cerebral toxoplasmosis was diagnosed. Soon after administration of oral sulfamethoxazole/trimethoprim and intravenous clindamycin in combination with short-term dexamethasone for the cerebral edema, her symptoms and signs began to improve. On day 229, both IgM and IgG titers in CSF had clearly decreased but remained essentially constant in serum. She was discharged without clinically significant neurological disorders. This case suggests that CSF specific anti-toxoplasma IgM titers might be useful for early diagnosis of cerebral toxoplasmosis after transplantation.

    66歳女性。10日前に発熱および上気道症状が出現し、他院にて肺炎および急性骨髄性白血病(AML)と診断され、当院に転院となった。血小板輸血を行うも血小板数低値が持続し、ヒト白血球抗原(HLA)適合血小板輸血、副腎皮質ステロイド投与、高用量免疫グロブリン静注を行っても血小板輸血不応状態は改善しなかった。へパリン起因性血小板減少症の可能性を考慮し、9日目にヘパリンフラッシュを中止、ELISA法にて抗血小板第4因子(PF4)/ヘパリン複合体抗体を測定したところ、吸光度(OD)は2.1と高値を示した。21日目(ヘパリンフラッシュ中止12日後)に血小板輸血不応状態は消散し、AMLに対する化学療法を開始した。2コース実施後に完全寛解が得られたが、地固め療法実施下で再発をきたし死亡した。その後の検討で、抗PF4/ヘパリン複合体抗体のOD値の漸減と、OD値が1.0未満へ至ったときに血小板輸血不応状態が消散していた。

    A 30-year-old female developed fever and multiple lymphadenopathy in September 2011. Her symptoms improved with antibiotic treatment. However, she again presented with fever and multiple lymphadenopathy in December 2011. In addition, she suffered from nephrotic syndrome with severe edema. She was therefore hospitalized to undergo detailed examinations. Renal biopsy revealed endocapillary proliferative glomerulonephritis. Since her renal function deteriorated rapidly, she was given steroid pulse therapy with methylprednisolone, followed by maintenance therapy with prednisolone. After treatment, her renal function improved but multiple lymphadenopathy persisted. Biopsy of a left axillary lymph node was then performed and revealed angioimmunoblastic T-cell lymphoma (AITL). She received CHOP therapy but showed no response. Therefore, she was given ESHAP therapy. A partial response was achieved and the nephrotic syndrome also resolved completely. We report this extremely rare case of renal dysfunction due to endocapillary proliferative glomerulonephritis complicated by AITL.

    A 20-year-old female presented with thirst, polyposia, and polyuria and was referred to our hospital because of leukocytosis and anemia. Bone marrow aspiration revealed 66.8% myeloperoxidase-positive blasts and trilineage myelodysplasia. The karyotype was 45, XX, inv(3)(q21q26.2), -7[19]. Therefore, a diagnosis of AML with inv(3)(q21q26.2) complicated by -7 was made. Moreover, hyposthenuria and a low anti-diuretic hormone (ADH) level were observed. Although cerebrospinal fluid analysis was normal, magnetic resonance imaging (MRI) revealed the absence of hyperintensity in the neurohypophysis in T1-weighted images. Therefore, she was also diagnosed with diabetes insipidus. After she was administered a desmopressin nasal spray, the volume of urine produced decreased. Following treatment with second induction therapy containing high-dose cytarabine for AML, she achieved complete remission in the bone marrow. Moreover, when the abnormality on MRI and the volume of urine were normalized, she discontinued desmopressin. Although diabetes insipidus is a rare complication of AML, the majority of AML patients who have diabetes insipidus have the abnormal karyotypes with inv(3)(q21q26.2)/t(3;3)(q21;q26.2) and monosomy 7. Further study is required to clarify the pathogenesis and develop a strategy for the treatment of this category of AML.

    症例は40歳、女性。分子学的寛解を得たPhiladelphia染色体陽性急性リンパ性白血病に対して、非血縁者間骨髄移植を実施した。day 90ごろより知覚異常、膀胱直腸障害および下肢筋力低下を認め、MRI所見も含めて急性横断性脊髄炎と診断した。ステロイドパルスおよび免疫グロブリン製剤投与を行い、症状の改善を得た。横断性脊髄炎は運動、知覚および自律神経系に異常を来す炎症性疾患であるが、造血幹細胞移植での合併例は比較的まれである。早期の診断および治療開始が機能的予後に影響するため、移植後の脊髄症には急性横断性脊髄炎を鑑別診断として念頭に置く必要性がある。(著者抄録)

    当院で治療を受けた初発症候性多発性骨髄腫51例について、ボルテゾミブとデキサメタゾン併用療法(BD療法)が全く実施されなかった群(Non-Bor群、15例)、狭義の再発・難治症例に対して実施された群(RR-Bor群、17例)、および、初回治療として短期間のDex大量療法(HD-Dex)を先行させた上で早期に実施された群(Upfront-Bor群、19例)に分けて、治療成績を後方視的に検討した。RR-Bor群とUpfront-Bor群のVGPR以上の奏効率は、それぞれ17.6%と31.6%で、PFSもUpfront-Bor群がRR-Bor群に対して有意に優れていた(p=0.039、ハザード比0.39)。3群間でOSには差を認めなかった。また、Upfront-Bor群における先行HD-DexはM蛋白量を約30%減少させ、腫瘍崩解症候群などの重篤な合併症のリスクを軽減させる可能性が示唆された。(著者抄録)

    <b>Objective</b> Several recent studies report that, after allogeneic hematopoietic cell transplantation (allo-HCT), eosinophilia is a favorable factor for transplant outcomes. However, whether the degree of eosinophilia influences transplant outcomes is yet to be established.<br> <b>Methods</b> We studied 144 patients with hematological malignancy who received allo-HCT at our institution. The stem cell sources were bone marrow in 84 patients, peripheral blood stem cells in 32 patients, and cord blood in 28 patients. One hundred and twelve patients underwent myeloablative conditioning and 49 patients had high-risk disease. We performed semi-landmark analysis to examine the influence of eosinophilia.<br> <b>Results</b> Eosinophilia developed at a median of 47 days after transplantation in 63 patients (44%). The patients with eosinophilia showed significantly better overall survival (OS) and a lower relapse rate at three years, compared to those without eosinophilia (66% vs 55%, p=0.04 and 30% vs 50%, p=0.002). On analysis following division into groups with mild (500-1,500&times;10<sup>6</sup>/L) and hyper- (>1,500&times;10<sup>6</sup>/L) eosinophilia, three-year OS and relapse rates were 68% and 65% (p=0.92), and 31% and 28% (p=0.90), respectively. On multivariate analysis, eosinophilia was significantly associated with lower relapse rates [HR: 0.5 (95% CI: 0.3-0.9), p=0.01] and the same trend was preserved in the analysis of the mild and hyper-eosinophilic groups.<br> <b>Conclusion</b> The results suggest that eosinophilia after allo-HCT was associated with better OS and a lower relapse rate, regardless of the levels. The mechanism of this effect is still unclear, and requires study of the pathophysiological process to clarify the relationship between the higher levels of eosinophilia after allo-HCT and organ infiltration.<br>

    A 19-year-old man with Philadelphia chromosome-positive acute lymphoblastic leukemia received an allogeneic hematopoietic cell transplant with unrelated bone marrow. On day 20, the patient developed impaired consciousness and disorientation. Examination of the cerebrospinal fluid showed 2&times;10<sup>4</sup> copies/mL of HHV6B. HHV6 encephalitis was diagnosed, as had HHV6 myelitis based on symptoms that included lancinating pain/pruritus in the lower limbs and dysuria/dyschezia. Concurrently, he showed sinus tachycardia. Even after clearance of the HHV6 genome from the plasma and CSF was achieved by treatment with foscarnet, sinus tachycardia persisted for another 100 days. We suspected prolonged sinus tachycardia due to dysautonomia caused by HHV6 encephalomyelitis.<br>

    A 38-year-old man was diagnosed with acute lymphoblastic leukemia. We performed myeloablative bone marrow transplantation from an unrelated donor during the patient's first complete remission. After engraftment, he developed acute graft-versus-host disease involving the gastrointestinal tract on day 32. Steroids and mycophenolate mofetil were initiated from day 39. His symptoms improved and the dose of immunosuppressants was tapered and then discontinued on day 421. On day 491, he developed nephrotic syndrome (NS). Based on renal biopsy, membranous nephropathy was diagnosed. There were no apparent symptoms or abnormal laboratory data suggestive of chronic graft-versus-host disease (cGVHD). Steroid therapy was initiated from day 518 and proteinuria improved significantly. NS is very rare following allogeneic hematopoietic stem cell transplantation (allo-HSCT). When there is no concomitant cGVHD, as in this case, allo-HSCT-associated NS is difficult to distinguish from idiopathic NS.

    63歳女。骨髄異形成症候群からの急性骨髄性白血病と診断され、化学療法で寛解後に非血縁骨髄移植を施行したが骨髄再発した。再寛解導入療法を施行し、第2寛解後に再度非血縁骨髄移植を施行した。day28に好中球生着を確認したが、day32以降にアデノウイルスによる出血性膀胱炎およびウイルス血症、サイトメガロウイルス抗原血症、腎出血を発症し、ribavirin、gancyclovir、血小板輸血などを使用した。day83にCRP高値が続き、liposomal amphoptericin B(L-AMB)を5mg/kg/dayに変更した。いったん改善したが、day104より発熱が出現し、β-Dグルカン著明上昇を認め、静脈血液培養で酵母様真菌が検出され、後日Trichosporon spp.と判明した。day108よりvoriconazole 400mg/dayとmicafungin 300mg/dayの点滴に変更したところ、翌日には解熱し、β-Dグルカンも減少し、血液培養陰性を確認した。しかし、day 122より再び発熱し、翌日Stenotrophomonas maltophilia敗血症で死亡した。

    24歳女。急性骨髄性白血病にて骨髄移植を受けたが再発し、再移植目的で入院となった。同種増血幹細胞移植を受け寛解状態であったが、下剤抵抗性の便秘、上腹部痛が出現し、更には汎血球減少、肝逸脱酵素の上昇、フェリチン高値を認めた。骨髄検査を行ったが、血球貪食像は認めなかった。その後、全身に散在性の水疱が出現したためアシクロビル投与を開始したが、翌日に血球減少の進行、肝逸脱酵素、フェリチンの急激な上昇を認めた。高熱なく、診断基準を十分に満たしていなかったが血球貪食症候群と診断し、メチルプレドニゾロンを追加したところ、翌日より肝機能、フェリチン値ともに改善を認めた。後日、皮疹の生検で巨細胞を認め、VZV-DNA高値であり、内臓播種性水痘帯状疱疹ウイルス感染症と確定診断した。ウイルス量は次第に減少し、難治性であった腹痛も改善して退院となった。

    同種造血幹細胞移植の移植前処置の強度の最適化を検討する目的で静注busulfan 8mg/kg+cyclophosphamide 120mg/kg+total lymphoid irradiation(TLI)7.5Gyによるmodified myeloablative conditioning regimenにて進行期の急性、慢性白血病9例に対して同種造血幹細胞移植を行った。年齢の中央値は30(18〜59)歳。造血幹細胞源は血縁骨髄2例、血縁末梢血幹細胞1例、非血縁骨髄6例。急性graft-versus-host disease(GVHD)予防はcyclosporin+短期methotrexate療法で行った。急性GVHDは9人中6例(67%)に出現し、全例でgrade 2であった。広範型慢性GVHDは評価可能症例7例中3例に認められた。移植後観察期間中央値は813(248〜1702)日で、9例中5例で移植後に原疾患の再発、再燃を認めたが、移植後100日以内の再発例はなかった。移植全観察期間を通じて移植関連死亡は認めなかった。2年の全生存率は88.9%、無病生存率は50.0%であった。今回の結果から骨髄破壊的前処置を最適化することによって、移植関連死亡を減少させると同時に病勢をコントロールし得る至適な移植前処置が存在する可能性があると考えられた。(著者抄録)

    A 64-year-old man was diagnosed as having acute myeloid leukemia. We performed sequential treatment with chemotherapy and reduced-intensity stem cell transplantation from an unrelated donor while the patient was in partial remission. After engraftment, he developed acute graft-versus-host disease of the gut on day 42 and steroid therapy was started. Despite transient aggravation of diarrhea, his symptoms slowly improved and the dose of steroid was tapered. On day 159, he complained of acute left lower abdominal pain. A CT scan showed perforation of the digestive tract and ileectomy was performed. At surgery, multiple ulcers of the intestine were found and one of the ulcers was perforated. Pathologically, transmural and diffuse proliferation of atypical cells in the ulcer were confirmed. Since these cells were positive for CD20 and Epstein-Barr-virus (EBV) encoded RNA, we made a diagnosis of EBV-associated post-transplant lymphoproliferative disorder (PTLD). Reduction in the dose of immunosuppressive agents and rituximab led to complete remission of PTLD. PTLD after allogeneic hematopoietic stem cell transplantation (allo-HSCT) is relatively rare, and the development of gastrointestinal perforation after allo-HSCT is very rare.

    An 18-year-old female with myelodysplastic syndrome underwent an allogeneic cord blood transplantation in May 2005. The conditioning regimen consisted of total body irradiation, cytarabine and cyclophosphamide. The day of the cord stem cell transfusion was counted as Day 0. For acute GVHD prophylaxis, cyclosporine A (CsA) and methotrexate were used. Engraftment was achieved on Day 30, acute GVHD grade II developed on Day 45 and treatment with methylprednisolone for acute GVHD was started. On Day 68 the patient had generalized seizures accompanied by loss of consciousness, hypertension and left hemiparesis, and was intubated. A cranial CT scan showed a mass effect on the right basal ganglia, and high signal intensities on the T2-weighted and FLAIR images of a MR examination were detected in the bilateral basal ganglia and posterior lobes, the findings of which were compatible with a brain tumor or infectious disease. Since an increased level of apparent diffusion coefficient (ADC) values on the bilateral basal ganglia was noted, we suspected that vasogenic edema had caused the mass effect. She went into remission immediately after CsA treatment was discontinued. Therefore, this case was diagnosed as atypical reversible posterior leukoencephalopathy syndrome (RPLS) associated with CsA. CsA-induced encephalopathy presenting a mass effect in clinical imaging findings is very rare, and an ADC map may be useful for the diagnosis of RPLS.

    抗菌薬不応性の発熱性好中球減少症に対する抗真菌薬の経験的治療は標準的治療法であるが、実際の真菌感染症は半数にも満たず、多くの不必要な投与が行われるため、薬剤の有効性に加えて安全性も重要である。カスポファンギンはキャンディン系抗真菌薬としては本邦で初めて、発熱性好中球減少症に対して保険承認を獲得し、有効性・安全性の強力なエビデンスからも、第一選択薬として使用しやすい。また、抗アスペルギルス作用を有し、近年増加しているアスペルギルス症に対しても治療選択肢の一つになり得る。本稿では自験例を中心に、投与の実際について概説する。(著者抄録)
