    <p><b>Objective </b>Therapeutic predictors derived from the venous pressure before therapy have not been identified for Budd-Chiari syndrome (BCS). The aim of this study was to determine whether or not measuring the distal pressure or pressure gradient was useful for predicting treatment efficacy in BCS. </p><p><b>Methods </b>We retrospectively analyzed seven consecutive patients diagnosed with symptomatic BCS at our hospital between 2008 and 2017. Distal and proximal venous pressures at occlusion sites of BCS were measured before treatment in all cases. The pressure gradient was defined as the difference between distal and proximal venous pressures. A receiver operating characteristics (ROC) analysis was performed for venous pressures. </p><p><b>Results </b>Percutaneous old balloon angioplasty (POBA) was performed in seven cases, with technical success achieved in all cases (100%). No complications were encountered. The median primary patency was 574 (interquartile range, 439.5-1,056.5) days. The 1-year primary patency rate was 71.73%. Six cases (85.7%) showed resolution of symptoms, representing clinical success. The ROC analysis revealed a high distal pressure (area under the ROC curve = 0.83, cut-off=12 mmHg) as a predictor of treatment efficacy of POBA for symptomatic BCS. In addition, the pressure gradient was considered significant from a clinical perspective, because the 6 successful cases with resolution of symptoms showed a large pressure gradient (range, 8-21 mmHg) before treatment, whereas the failed case showed a relatively small pressure gradient (7 mmHg). </p><p><b>Conclusion </b>High distal pressure and a large pressure gradient might predict the treatment efficacy of balloon angioplasty for symptomatic BCS. </p>

    <p><b>Objective </b>Balloon-occluded retrograde transvenous obliteration (B-RTO) for gastric varices (GV) is associated with drawbacks including a postoperative increase in portal pressure and the risk of subsequent worsening of esophageal varices (EV). Selective B-RTO that embolizes only the varices may have the potential to minimize such risks. The aim of this study is to retrospectively compare the postoperative course of patients after selective B-RTO (Group S) and conventional B-RTO (Group B). </p><p><b>Methods </b>One hundred four patients treated from January 2007 to April 2012 were classified into Groups S (n=5) and B (n=99). In the univariate analysis, the volume of 5% ethanolamine oleate iopamiodol (EOI) administered at baseline and the GV blood flow on endoscopic ultrasound after B-RTO were considered as covariates. The rates of GV recurrence and EV aggravation was also compared between Groups B and S. </p><p><b>Results </b>In Group S, the volume of 5% EOI was significantly lower (Group S vs. Group B: 14.6±5.5 vs. 28.5±16.4 mL; p=0.0012) and the rate of EV aggravation was lower in comparison to Group B (p=0.045). However, in Group S, the rate of complete eradication of GV blood flow was significantly lower (Group S vs. Group B: 0% vs. 89.9%; p<0.001) and the rate of re-treatment for GV was higher in comparison to Group B (Group S vs. Group B: 60% vs. 1.0%; p<0.001). </p><p><b>Conclusion </b>Selective B-RTO for GV could minimize the risk of a worsening of EV or reduce the amount of sclerosants; however, the rate of recurrence was high in comparison to conventional B-RTO. </p>

    肝動脈合併膵頭十二指腸切除術(PD-CHAR)では術後に肝臓の血流が乏しくなり、肝血流を保つためには、空腸動脈や胃動脈などの血管と固有肝動脈の吻合(血流改変)が必要となる。今回、当院におけるPD-CHAR術前血流改変の方法について紹介し、2症例を提示した。症例1では術前CT像から胃十二指腸動脈と総肝動脈が浸潤を受け、右胃動脈から末梢の固有肝動脈は浸潤を受けていないと考えられた。血流改変は、左胃動脈→胃→右胃動脈からの肝血流温存を期待して実施し、予定どおりPD-CHARを施行することができた。症例2では術前CT像から総肝動脈が浸潤を受けていると考えられた。右肝動脈は上腸間膜動脈から分岐し、左肝動脈は左胃動脈から分岐していたため、右肝動脈と総肝動脈の術前コイリングにより、左胃動脈→左肝動脈→左右吻合枝→右肝動脈の流れで肝血流を左胃動脈から1本化することを目指し、予定どおりPD-CHARを施行できた。

    Distal pancreatectomy with en bloc celiac axis resection (DP-CAR) was now employed for locally advanced cancer of the body of the pancreas involving the common hepatic artery and/or celiac trunk. Recently, pancreaticoduodenectomy with common hepatic artery resection (PD-CHAR) is employed for locally advanced cancer of the head of the pancreas. In this operation, reconstruction of the hepatic artery is generally needed. In some cases, reconstruction of the hepatic artery is not needed due to hepatic arterial variants. Herein, we discussed the strategy for preoperative coil embolization of PD-CHAR in our institute by reporting two cases.

    Postpancreatectomy hemorrhage (PPH) is a rare but often severe complication after pancreatic resection. In consequence the incidence and mortality rates in the literature reveal a large range 3-8% and 15-60%, respectively. Treatments for PPH include laparotomy and endovascular interventions. We describe endovascular management that includes embolization and stentgraft placement. Embolization is based on the "isolation technique", in which both the proximal and distal parts including the bleeding point are filled with embolic materials to avoid retrograde blood flow. It is minimally invasive and associated with a high success rate. Stentgraft placement is a good option for complete hemostasis and the preservation of parental artery flow.

    A 40-year-old male with a 10-year history of repeated acute exacerbations of chronic alcoholic pancreatitis who had been diagnosed 4 months earlier as having a hemorrhagic pancreatic pseudocyst, was admitted with a one-week history of melena. At the time of admission, his blood pressure was 98/65 mmHg, and blood examination revealed slight anemia. Abdominal contrast-enhanced CT showed a hemorrhagic pancreatic cyst measuring 50 mm in diameter in the pancreatic tail. Furthermore, the cyst was found to penetrate the transverse colon. The melena persisted after the patient was hospitalized, and the anemia worsened. Therefore, emergency angiography was performed, which showed a pseudoaneurysm of the inferior branch of the splenic artery ; the patient was treated by coil embolization for the pseudoaneurysm, which led to control of the melena. To address re-bleeding and infection caused by the cyst penetrating the colon, distal pancreatectomy with left colectomy was performed. A hemorrhagic pancreatic cyst penetrating the colon and causing gastrointestinal hemorrhage is rare ; only nine cases, including ours, have been reported so far in the Japanese literature. We report this experience with a review of the pertinent literature.

    Purpose: We investigated the ability to detect the articular disk and joint effusion of the temporomandibular joint (TMJ) of a method of dual echo volumetric isotropic turbo spin echo acquisition (DE-VISTA) additional fusion images (AFI).<br> Methods: DE-VISTA was performed in the 26 TMJ of 13 volunteers and 26 TMJ of 13 patients. Two-dimensional (2D) dual echo turbo spin echo was performed in the 26 TMJ of 13 volunteers. On a workstation, we added proton density-weighted images (PDWI) and T<sub>2</sub> weighted images (T<sub>2</sub>WI) of the DE-VISTA per voxel to reconstruct DE-VISTA-AFI. Two radiologists reviewed these images visually and quantitatively.<br> Results: Visual evaluation of the articular disk was equivalent between DE-VISTA-AFI and 2D-PDWI. The sliding thin-slab multiplanar reformation (MPR) method of DE-VISTA-AFI could detect all articular disks. The ratio of contrast (CR) of adipose tissue by the articular disk to that of the articular disk itself was significantly higher in DE-VISTA-AFI than DE-VISTA-PDWI (<i>P</i><0.05) in patients and volunteers with closed or open mouth. In volunteers, the CR between adipose tissue and the disk on DE-VISTA-AFI was marginally significant to that on 2D-PDWI at opened mouth (<i>P</i>=0.071) and not significantly different (<i>P</i>=0.18) from that at closed mouth. Joint effusion could be identified in DE-VISTA-AFI in all 8 joints that had joint effusion in DE-VISTA-T<sub>2</sub>WI but in only 3 of those joints in 2D-T<sub>2</sub>WI. The CR of joint effusion to adipose tissue on DE-VISTA-AFI did not differ significantly from that on DE-VISTA-PDWI. However, using DE-VISTA-T<sub>2</sub>WI in addition to DE-VISTA-PDWI, we could visually identify joint effusion on DE-VISTA-AFI that could not be identified on DE-VISTA-PDWI alone.<br> Conclusion: DE-VISTA-AFI can depict the articular disk and a small amount of joint effusion by the required plane of MPR using the sliding thin-slab MPR method.<br>

    <B>PURPOSE:</B> The purpose of this study was to report clinical behavioral survey of trainees before and after the Japanese Society of Interventional Radiology 8th Academic Summer Seminar.<BR><B>MATERIALS AND METOODS:</B> 27 trainees participated in the seminar. 26 trainees included in this study. The seminar consisted of a total of 5 hours of lectures on B-RTO and NBCA embolization, and following three types of practical training conducted for 120 minutes each: venous embolization and NBCA Embolization using swine 1, venous embolization and NBCA Embolization using swine 2 and a simulated B-RTO procedure using a venous vascular model. We surveyed to trainees clinical behavior about B-RTO and NBCA embolization of six months before and after the seminar.<BR><B>RESULT:</B> The number of trainees who had already experienced the procedures as operator of B-RTO and NBCA embolization at the time of the seminar were 15 and 13, respectively.<BR>No trainees newly experienced operator and assistant of B-RTO in six months after the seminar. One and 2 trainees newly experienced operator and assistant of NBCA embolization in six months after the seminar, respectively.<BR><B>CONCLUSION:</B> We reported clinical behavioral survey of trainees before and after the Japanese Society of Interventional Radiology 8th Academic Summer Seminar.

    Background. We apply percutaneous CT-guided radiofrequency ablation (RFA) for unresectable lung tumors. With this new treatment, we encountered 2 cases of cavity formation followed by inflammation, 3 weeks after RFA for lung metastases from colorectal cancers. Cases. Two patients, 61-and 57-year-old men with lung metastases from colorectal cancers were considered inoperable. During RFA, severe complications were not encountered, and they were discharged on day 8 after ablation. Three weeks after ablation, however, they both contracted a fever and abscess formation on chest X ray and CT scans. They were treated with IV and oral antibiotics, and recovered after the treatment. Conclusion. We should bear in mind that cavitation usually changes after RFA and may often overlap infection.

    This is a case report of a 77-year-old male with giant gastric ulcer, at the bottom of which there was a large exposed vessel with the arterial beat. As a possibility of inducing the bleeding by the endoscopic hemostasis, we performed abdominal angiography. It showed an aneurysm at the parietal branch of left gastric artery and transcatheter arterial embolization (TAE) was carried out and achieved complete hemostasis. Although hemorrhagic gastric ulcers with aneurysm are rare, their hemostasis seemed to be difficult and TAE may prove to be a useful method in the treatment of these ulcers.

    近年、膵切除術後出血に対してドレナージやコイル塞栓をはじめとするインターベンショナルラジオロジーが積極的に導入されている。また、わが国では2016年12月よりステントグラフト内挿の臨床応用が可能となった。ステントグラフト内挿に関しては至適サイズの選択、デリバリー、術中ヘパリン化や留置後の抗血小板療法、長期開存性、感染などの問題があり、今後の課題である。膵切除術後出血の原因と診断、塞栓ならびにステントグラフト内挿について概説した。
