    <p><b>Objective </b>Mapping the long-term prognosis of Budd-Chiari syndrome (BCS) is difficult, as the prognosis is associated with changes in the liver function. The present study evaluated the time course changes in the liver function in a treatment group with percutaneous old balloon angioplasty (POBA) and a non-treatment group using the albumin-bilirubin score (ALBI) and Child-Pugh score during long-term follow-up. </p><p><b>Methods </b>In this retrospective study, 13 consecutive patients diagnosed with BCS at our hospital between 2007 and 2020 were categorized into a treatment group (n=8), which received POBA, and a non-treatment group (n=5). Differences in the liver function in the ALBI and Child-Pugh scores between the initial visit and one- and three-year follow-up were calculated and statistically evaluated. We investigated the changes in the liver function during the long-term follow-up, including events such as re-stenosis and re-treatment. </p><p><b>Results </b>While the Child-Pugh scores in the treatment group did not differ significantly between the initial visit and 1- or 3-year follow-up, the ALBI scores in this group improved significantly between the initial visit and the 1- or 3-year follow-up visit (p=0.0078 and 0.0156, respectively). The liver function according to the ALBI score in the treatment group showed gradual improvement from the initial value but gradual worsening in the non-treatment group. The ALBI scores also revealed that the liver function varies according to re-stenosis and re-POBA in BCS patients. </p><p><b>Conclusion </b>Unlike the Child-Pugh score, the ALBI score was able to capture changes in the liver function of BCS patients during the long-term course of BCS. </p>

    Primary liver cancer is the third most common cause of cancer mortality in the world. Hepatocellular carcinoma (HCC) is the dominant type of liver cancer, accounting for approximately 75% of the total. Molecular targeted agent (MTA) therapy is the guideline-recommended treatment for patients with well-preserved liver function (Child-Pugh A) and with advanced-stage (Barcelona Clinic Liver Cancer [BCLC] Stage C) HCC. There is a high level of evidence supporting transarterial chemoembolization (TACE) as the current standard of care for intermediate-stage (BCLC-B) HCC. The development of MTAs encourages us to consider a new treatment strategy for intermediate-stage HCCs. We must maximize the benefits of both treatments, TACE and MTA, for patients with intermediate and advanced HCC. Recently, some reports showed that patients with intermediate-stage HCC beyond up-to-7 criteria received initial treatment with lenvatinib (LEN) followed by TACE (LEN-TACE sequential therapy). LEN-TACE sequential therapy was shown to prolong survival compared to LEN monotherapy. We have had cases in which re-challenge TACE was effective after LEN therapy for cases refractory to initial TACE. Our re-challenge TACE cases were in the state of LEN-refractoriness/intolerance. In our clinical results, LEN with re-challenge TACE prolonged survival compared to LEN monotherapy. Re-challenge TACE showed a good overall response rate (CR+PR) at 65.2%. As adverse events of LEN, it causes tumor-related hemorrhages despite rapid suppression of tumor blood flow. In our clinical results, five of 68 consecutive cases developed tumor hemorrhages. These five cases had larger tumors than those without hemorrhages.

    フォンタン手術を受けた患者における肝硬度(LS)と肝線維化ステージとの相関を調べた。フォンタン手術を受けた患者58名に対し、transient elastographyを用いてLSを前向きに測定した。58名うち、LSが11.0kPa以上(中央値19.2kPa、範囲12.2〜39.8kPa)の22名(男性12名、女性10名、年齢9.9〜32.1歳)を対象に、肝生検、心カテーテル検査、臨床検査を行った。その結果、組織学的には、LSが11.0kPa以上であった22名全てが軽度から重度の門脈および類洞の線維化を示したが、肝硬変はなかった。統計的には、LSは組織学的な肝線維化スコアを予測しなかった。LSは中心静脈圧(p=0.456)や肝静脈圧較差(HVPG、p=0.062)とは相関しなかったが、HVPGのp値は有意性の閾値を僅かに上回った。これらの結果から、フォンタン手術を受けた患者は、門脈および類洞の線維化の両方を発症しやすいことが示された。LCはHVPGの影響を受ける可能性があり、従来のLSのカットオフ値を使用すると、これらの患者の肝線維化を過大評価し、治療し過ぎてしまう可能性があることが示唆された。

    <p><b>Objective </b>Therapeutic predictors derived from the venous pressure before therapy have not been identified for Budd-Chiari syndrome (BCS). The aim of this study was to determine whether or not measuring the distal pressure or pressure gradient was useful for predicting treatment efficacy in BCS. </p><p><b>Methods </b>We retrospectively analyzed seven consecutive patients diagnosed with symptomatic BCS at our hospital between 2008 and 2017. Distal and proximal venous pressures at occlusion sites of BCS were measured before treatment in all cases. The pressure gradient was defined as the difference between distal and proximal venous pressures. A receiver operating characteristics (ROC) analysis was performed for venous pressures. </p><p><b>Results </b>Percutaneous old balloon angioplasty (POBA) was performed in seven cases, with technical success achieved in all cases (100%). No complications were encountered. The median primary patency was 574 (interquartile range, 439.5-1,056.5) days. The 1-year primary patency rate was 71.73%. Six cases (85.7%) showed resolution of symptoms, representing clinical success. The ROC analysis revealed a high distal pressure (area under the ROC curve = 0.83, cut-off=12 mmHg) as a predictor of treatment efficacy of POBA for symptomatic BCS. In addition, the pressure gradient was considered significant from a clinical perspective, because the 6 successful cases with resolution of symptoms showed a large pressure gradient (range, 8-21 mmHg) before treatment, whereas the failed case showed a relatively small pressure gradient (7 mmHg). </p><p><b>Conclusion </b>High distal pressure and a large pressure gradient might predict the treatment efficacy of balloon angioplasty for symptomatic BCS. </p>

    経皮的古典的バルーン血管形成術(POBA)を行った症候性Budd-Chiari症候群(BCS)患者7例(男性3例、女性4例、年齢31〜81歳)を後向きに評価し、遠位圧または圧較差がPOBA有効性の予測に有用であるか検討した。全例で治療前にBCS閉塞部位の遠位/近位静脈圧を評価し、遠位静脈圧と近位静脈圧の差異を圧較差とした。全例でPOBAが成功した(技術的成功率100%)。合併症の発生はなかった。一次開存日数中央値は574日で、1年後の一次開存率は71.73%であった。6例(85.7%)でPOBA後に症状が解消し、臨床的成功とした。症候性BCSに対するPOBA有効性の予測因子としての治療前遠位圧のカットオフ値を12mmHgとした場合、ROC曲線下面積は0.83であった。治療前の圧較差については、症状解消を認めた6例は比較的大きく(8〜21mmHg)、症状解消が認められなかった1例は7mmHgであったことから、圧較差も予測因子であると考えられた。

    <p><b>Objective </b>Balloon-occluded retrograde transvenous obliteration (B-RTO) for gastric varices (GV) is associated with drawbacks including a postoperative increase in portal pressure and the risk of subsequent worsening of esophageal varices (EV). Selective B-RTO that embolizes only the varices may have the potential to minimize such risks. The aim of this study is to retrospectively compare the postoperative course of patients after selective B-RTO (Group S) and conventional B-RTO (Group B). </p><p><b>Methods </b>One hundred four patients treated from January 2007 to April 2012 were classified into Groups S (n=5) and B (n=99). In the univariate analysis, the volume of 5% ethanolamine oleate iopamiodol (EOI) administered at baseline and the GV blood flow on endoscopic ultrasound after B-RTO were considered as covariates. The rates of GV recurrence and EV aggravation was also compared between Groups B and S. </p><p><b>Results </b>In Group S, the volume of 5% EOI was significantly lower (Group S vs. Group B: 14.6±5.5 vs. 28.5±16.4 mL; p=0.0012) and the rate of EV aggravation was lower in comparison to Group B (p=0.045). However, in Group S, the rate of complete eradication of GV blood flow was significantly lower (Group S vs. Group B: 0% vs. 89.9%; p<0.001) and the rate of re-treatment for GV was higher in comparison to Group B (Group S vs. Group B: 60% vs. 1.0%; p<0.001). </p><p><b>Conclusion </b>Selective B-RTO for GV could minimize the risk of a worsening of EV or reduce the amount of sclerosants; however, the rate of recurrence was high in comparison to conventional B-RTO. </p>

    胃静脈瘤(GV)に対してバルーン閉塞下逆行性経静脈的塞栓術(B-RTO)を行った患者104例を対象に、選択的B-RTOと従来型B-RTOの術後経過を後向きに比較した。選択的B-RTOは5例(S群:男性4例、女性1例、平均63.4±6.2歳)、従来型B-RTOは99例(B群:男性65例、女性34例、平均65.9±10.1歳)に施行した。S群はB群に比べてベースラインの5%エタノールアミンオレイン酸イオパミドール投与量が少なく(14.6±5.5 vs 28.5±16.4mL,p=0.0012)、術後1年目の食道静脈瘤(EV)悪化率が低かったが(20 vs 62.1%、p=0.045)、GV血流の完全根絶率は低く(0 vs 89.9%、p<0.001)、GV再治療率が高かった(60 vs 1.0%、p<0.001)。選択的B-RTOは従来型B-RTOに比べて、EV悪化リスクを最小限に抑え、硬化剤の使用量を減少するが、GV再発率が高いことが示された。

    2014年5月〜2018年9月にかけて、当院で薬物溶出性ビーズ(DEB)による経カテーテル動脈化学塞栓術(DEB-TACE)を受けた肝細胞癌(HCC)患者35例(男性27例、女性8例、年齢40〜88歳)を対象とし、局所再発の危険因子を後向きに調査した。その結果、単変量解析により、高齢、女性、腫瘍径、肝表面との接触およびデジタルサブトラクション血管造影(DBA)上に認められた残存血管が、局所再発の危険因子として挙げられた。なかでも、腫瘍径、肝表面との接触およびDBA上の残存血管に関しては、多変量ロジスティック解析においても、局所再発の危険因子であることが示された。以上の結果から、HCCのDEB-TACE後局所再発においては、腫瘍径、肝表面との接触およびDBA上の残存血管が危険因子と考えられた。

    肝動脈合併膵頭十二指腸切除術(PD-CHAR)では術後に肝臓の血流が乏しくなり、肝血流を保つためには、空腸動脈や胃動脈などの血管と固有肝動脈の吻合(血流改変)が必要となる。今回、当院におけるPD-CHAR術前血流改変の方法について紹介し、2症例を提示した。症例1では術前CT像から胃十二指腸動脈と総肝動脈が浸潤を受け、右胃動脈から末梢の固有肝動脈は浸潤を受けていないと考えられた。血流改変は、左胃動脈→胃→右胃動脈からの肝血流温存を期待して実施し、予定どおりPD-CHARを施行することができた。症例2では術前CT像から総肝動脈が浸潤を受けていると考えられた。右肝動脈は上腸間膜動脈から分岐し、左肝動脈は左胃動脈から分岐していたため、右肝動脈と総肝動脈の術前コイリングにより、左胃動脈→左肝動脈→左右吻合枝→右肝動脈の流れで肝血流を左胃動脈から1本化することを目指し、予定どおりPD-CHARを施行できた。

    肝動脈合併膵頭十二指腸切除術(PD-CHAR)では術後に肝臓の血流が乏しくなり、肝血流を保つためには、空腸動脈や胃動脈などの血管と固有肝動脈の吻合(血流改変)が必要となる。今回、当院におけるPD-CHAR術前血流改変の方法について紹介し、2症例を提示した。症例1では術前CT像から胃十二指腸動脈と総肝動脈が浸潤を受け、右胃動脈から末梢の固有肝動脈は浸潤を受けていないと考えられた。血流改変は、左胃動脈→胃→右胃動脈からの肝血流温存を期待して実施し、予定どおりPD-CHARを施行することができた。症例2では術前CT像から総肝動脈が浸潤を受けていると考えられた。右肝動脈は上腸間膜動脈から分岐し、左肝動脈は左胃動脈から分岐していたため、右肝動脈と総肝動脈の術前コイリングにより、左胃動脈→左肝動脈→左右吻合枝→右肝動脈の流れで肝血流を左胃動脈から1本化することを目指し、予定どおりPD-CHARを施行できた。

    Distal pancreatectomy with en bloc celiac axis resection (DP-CAR) was now employed for locally advanced cancer of the body of the pancreas involving the common hepatic artery and/or celiac trunk. Recently, pancreaticoduodenectomy with common hepatic artery resection (PD-CHAR) is employed for locally advanced cancer of the head of the pancreas. In this operation, reconstruction of the hepatic artery is generally needed. In some cases, reconstruction of the hepatic artery is not needed due to hepatic arterial variants. Herein, we discussed the strategy for preoperative coil embolization of PD-CHAR in our institute by reporting two cases.

    Postpancreatectomy hemorrhage (PPH) is a rare but often severe complication after pancreatic resection. In consequence the incidence and mortality rates in the literature reveal a large range 3-8% and 15-60%, respectively. Treatments for PPH include laparotomy and endovascular interventions. We describe endovascular management that includes embolization and stentgraft placement. Embolization is based on the "isolation technique", in which both the proximal and distal parts including the bleeding point are filled with embolic materials to avoid retrograde blood flow. It is minimally invasive and associated with a high success rate. Stentgraft placement is a good option for complete hemostasis and the preservation of parental artery flow.

    ステージB肝細胞癌に対する分子標的薬と肝動脈化学塞栓療法(TACE)の併用療法について、その背景と可能性を概説した。併用療法に関する近年の成績として、2020年Kuwamuraの報告、2021年Shimoseの報告、2021年Andoの報告を紹介した。大阪公立大学医学部の成績から、Re-challenge TACEの可能性について述べた。また、併用療法の手順と方法、レンバチニブ導入先行による腫瘍出血、免疫療法とTACEの併用療法についても述べた。

    近年、膵切除術後出血に対してドレナージやコイル塞栓をはじめとするインターベンショナルラジオロジーが積極的に導入されている。また、わが国では2016年12月よりステントグラフト内挿の臨床応用が可能となった。ステントグラフト内挿に関しては至適サイズの選択、デリバリー、術中ヘパリン化や留置後の抗血小板療法、長期開存性、感染などの問題があり、今後の課題である。膵切除術後出血の原因と診断、塞栓ならびにステントグラフト内挿について概説した。
