    症例は12歳男児で、左頬部腫脹を主訴とした。穿刺吸引細胞診や生検では診断できず、造影CT・MRI所見より多形腺腫、神経鞘腫、横紋筋肉腫、炎症性筋繊維芽細胞腫を疑った。腫瘤摘出術を行い、免疫組織化学染色でCDK4およびMDM2が陽性であったことより脱分化型脂肪肉腫と診断した。術後5年現在、再発や転移は認めない。本例は傍咽頭間隙から発生した脱分化型脂肪肉腫で、脂肪成分が僅かであったため画像所見で診断できず、免疫組織化学染色により診断を得られた。

    <p>　Progressive supranuclear palsy (PSP) has been reported as a disorder mainly characterized by a tendency to fall, vertical supranuclear gaze palsy, and constriction of the body axis, akinesia, and cognitive impairment. Patients presenting with these typical clinical features are diagnosed as having the PSP-Richardson syndrome (PSP-RS). Electronystagmography (ENG) is useful for the detection of ocular motility disorder, which is the main feature of PSP. In cases of PSP, ocular motility disorder in the vertical direction occurs from the initial stage of the disease, and is characterized by prolonged latency and reduced velocity of the saccadic eye movement in the vertical direction. Horizontal saccadic eye movement disorder also occurs after the intermediate stage. Square wave jerks (SWJ) are often recognized from the initial stage of the disease. In terms of nystagmus, the quick phase is impaired due to impaired saccadic eye movements, and impairment of optokinetic nystagmus also starts from the quick phase movements. Pursuit (smooth) eye movement may also be impaired as the disease progresses. On the other hand, the vestibulo-ocular reflex is maintained even after disease progression to a late stage.</p>

    茎状突起過長症は咽頭痛,顎下部痛,耳下部痛など非特異的な症状をきたすが,複数の医療機関を受診しても原因が分からず長期の経過を辿る例がある.保存的加療で症状が改善しない場合には手術が検討される.手術には口内法と頸部外切開法があるが,それぞれ利点と欠点があり,どちらを選択するかについて議論の余地がある.今回われわれは,多根総合病院と大阪市立大学(現・大阪公立大学)医学部附属病院で経験した4例を報告する.診断には頸部CTの3D構築画像に加え,本疾患を念頭において身体所見を取ることが有用であった.3例は保存的加療で症状は軽快し,1例は保存的加療で改善せず手術加療を行った.口内法で茎状突起を切除したが,咽頭痛が軽快せず,さらに外切開による全摘出を行って咽頭痛を消失させることができた.4症例の茎状突起過長症の診断方法,外科的治療法の選択について若干の文献的考察を加えて報告する.(著者抄録)

    <p>Proliferating trichilemmal tumor (PTT) is a rare skin tumor and is more common in women than in men. More than 90% of PTT lesions arise on the scalp, but here we report a case of PTT on the right lower ear. </p><p>The patient was a 40-year-old man who had been aware of the tumor for about 10 years. He came to our hospital after the lesion had increased in size over the previous six months. An elastic hard mass measuring approximately 3cm in diameter was observed on the lower part of the right ear. We initially suspected a parotid tumor, but imaging did not show typical signs of a parotid tumors, Warthin tumor, or pleomorphic adenoma. The tumor was resected, and a histopathological diagnosis of PTT was made based on the characteristic presence of trichilemmal keratinization. </p><p>PTT can recur and become malignant, but it has not recurred in our patient during the 3-year postoperative follow-up period. We are continuing to monitor the patient’s course with careful follow-up examinations. This case serves as a reminder that skin tumors or skin appendage tumors can occur in the head and neck area.</p>

    <p>　Auditory steady-state response (ASSR) is widely used in clinical practice as an objective audiometric test with frequency specificity. Eclipse version 1.02 by Interacoustics, which uses CE-Chirp<sup>®</sup>, is available in two modulation frequencies: 40 Hz and 90 Hz. The stimulation frequency of 40 Hz is recommended for adults in the awake state, while that of 90 Hz is recommended for children in the asleep state. However, in actual clinical practice, some children could be awake during the testing, and some adults could be asleep during the testing. In this study, we measured the ASSR thresholds in the same subject under both awake and asleep conditions and examined the relationship between the results of each test and the pure tone hearing thresholds. The results showed that the results of ASSR measured at both stimulation frequencies were strongly correlated with the pure tone hearing thresholds. In addition, the results for each level of hearing loss showed that the ASSR thresholds were higher than the pure tone thresholds in the normal-hearing group and lower in the severe hearing loss group at all frequencies.</p>

    聴性定常反応(auditory steady-state response,以下ASSR)は周波数特異性のある他覚的聴力検査として臨床の場で広く用いられている。CE-Chirpを用いたInteracoustics社のEclipse version 1.02は40Hz刺激と90Hz刺激の2種類の変調周波数で検査が可能で,40Hz刺激は覚醒状態で成人に,90Hz刺激は睡眠下で小児に施行することが推奨されている。しかし実際の臨床においては小児でも覚醒状態で検査が可能な場合もあり,また成人でも検査中に睡眠状態となることも有り得る。そこで我々は同一被験者に対して,40Hz刺激覚醒下,90Hz刺激睡眠下の両方の条件でASSR閾値を測定した症例を対象に,それぞれの結果について純音聴力閾値との関連について検討した。その結果,どちらの変調周波数で測定したASSRの結果も純音聴力閾値との間に高い相関が見られた。さらに難聴程度別における検討を行ったところ,正常群は純音聴力閾値よりASSRの閾値が高く,高重度難聴群では低いという結果が全周波数を通じて得られた。(著者抄録)

    症例は66歳女性で、嚥下困難を主訴とした。17歳時にクローン病を発症し回盲部切除術が施行され、メサラジンにより寛解を維持していた。喉頭内視鏡検査では喉頭蓋から咽頭腔にかけて1～2時方向を除き亜全周性に膜様癒着を認めたが、声帯の可動性は両側とも良好で披裂部の癒着も認めなかった。嚥下造影検査では喉頭蓋付近に造影剤が貯留し、正中部から漏斗状に流入していく様子を認めた。頸部CTでは喉頭蓋が尾側に引きつれるように変形していた。10年前の上部消化管内視鏡検査所見とほぼ同じ病変であったため、クローン病の喉頭病変と診断し経過観察方針となった。食事に時間がかかるが体重減少もなく、初診から4年経過して増悪を認めていない。

    症例は58歳男性で、左視力障害を主訴とした。左眼窩痛と霧視の増悪、左視力の低下にて受診し、球後視神経炎の疑いで脳神経内科と脳神経外科に紹介となり、造影CTでは左Onodi蜂巣に接する眼窩先端部で内直筋より内側の筋円錐外に腫瘍性病変を認めた。MRIではT1強調画像で等信号、T2強調画像で高信号を呈し、造影効果を伴う腫瘍(長径5.6mm)を認め、ダイナミックMRIでは一点から徐々に周辺へと造影される辺縁部結節性濃染を示した。画像所見などから海綿状血管腫が疑われ、診断と治療を兼ねて内視鏡下に経鼻アプローチで腫瘍を摘出し、病理組織検査所見より海綿状血管腫と診断された。術後は霧視の消失と左視力の回復が得られ、術後2年経過時点で再発を認めていない。眼窩内側の筋円錐外に存在する腫瘍に対し、低侵襲な経鼻アプローチによる摘出はよい適応であると考えられた。

    頭部外傷性めまい症例2例を呈示した。症例1は26歳男性で、高さ2～3mから転落して左側頭骨骨折をきたした。意識障害軽快後に左顔面神経麻痺、左難聴、回転性めまいを自覚したためステロイド漸減加療を開始したが、難聴、ふらつき、顔面神経麻痺が持続した。聴力検査、眼振検査、温度眼振検査、vHIT、側頭骨単純CT各所見より、頭部外傷性外リンパ瘻による感音難聴、めまい(左前庭機能低下)と考え前庭リハビリとした。2ヵ月後にふらつき症状、左顔面神経麻痺は改善した。症例2は46歳女性で、交通事故でむち打ち、頸椎症を受傷し、3ヵ月後に左耳鳴り、ふらつき症状が増悪した。各種検査所見で明らかな異常は認めなかったが、立位・歩行時、電車に乗るなどの行動で症状が増悪した。むち打ち後から自覚した症状であり、外傷後に発症した持続性知覚性姿勢誘発めまいと診断し、前庭リハビリにより自覚症状は徐々に軽快した。

    58歳男性。頭痛および複視を主訴に近医の脳神経外科を受診、頭部CTにて鼻内から頭蓋底にかけての腫瘤性病変を指摘され、精査目的で当科へ紹介となった。造影CT、Gd造影MRIでは蝶形骨洞を主座とするT1強調像で軽度低信号、T2強調像で等信号の腫瘤が認められ、局所麻酔下での経鼻的生検にて蝶形骨洞原発の小細胞癌と診断された。治療はシスプラチン+エトポシド(PE療法)を2コース施行されたが、効果は乏しく、放射線療法が行われた。効果はわずかな腫瘍の縮小であったため、追加治療としてアムルビシンの投与も行われた。だが、10コース終了時点で腫瘍の再増大を認めた。そこで、再度PE療法、イリノテカンの投与が行われたが、最終的に患者は初診20ヵ月後に死亡となった。

    発話文中の不自然な箇所に休止点(ポーズ)が置かれた発話の内容を理解しようとした時に、それに関連してみられる神経活動過程を脳磁図で調べた。日本語を母国語とする健康な男性24名(平均23.1±2.6歳)を被験者とした。論理的内容および非論理的内容の文章を用い、さらに不適切な箇所にポーズをはさんで読み上げた発話文を被験者に聞かせ、理解するよう努力してもらった。その間に脳磁図計測を行った。ポーズの箇所が適切であった時と比較したところ、文章が論理的・非論理的のどちらの場合でも、ポーズが不適切な場合は発話内容理解の水準が低下していた。脳磁図計測の結果、論理的に整合しポーズは不適切であったストーリーの発話を聞いた時にブロードマン領野での10番領野ではガンマ帯域のパワーが増大しており、そのパワー増大は内容理解の悪化と負に関連していることが明らかになった。

    56歳男性。右顔面麻痺を主訴に当科へ受診となった。顔面麻痺はステロイド内服で回復傾向にあったが、初診時に右中耳から隆起する病変を認めた。CTでは中耳に充満する軟部影がみられ、患者に精査を勧めるも希望がなく、経過観察とした。しかし、初診2年後に右耳痛が出現し、側頭骨CT検査を行ったところ、右外耳道内から鼓室内にかけ軟部影が充満し、側頭骨の腫瘍が中頭蓋窩に及んでいた。脳神経外科にて減圧術および側頭骨腫瘍生検、中耳腫瘍生検を施行した結果、中耳カルチノイドと診断された。患者へ手術を勧めるも希望せず、化学放射線療法が行われたが、頭蓋内進展および多発転移をきたし、初診6年後に患者は死亡となった。

    <p>A 56-year-old man visited our hospital with the chief complaint of facial palsy. Although a mass was detected in his right middle ear, he refused to undergo further examination and failed to return for follow-up. When he revisited the hospital 2 years later due to right otalgia, a tumor invading the skull was detected. Because the patient did not wish to undergo surgery, chemoradiotherapy was administered, which resulted in a reduction of the tumor volume. After another 2 years, the tumor regrew and the facial palsy became more severe. Chemotherapy was administered again; however, it was less effective this time, and the tumor gradually became larger and gave rise to multiple metastases in the lung, liver, and bone.</p><p>Middle ear carcinoids are relatively rare, and only a few cases of this condition presenting with facial palsy have been reported. These carcinoids are generally considered to be slow-growing, and often follow a favorable course. However, our patient did not wish to receive standard care, and ultimately developed multiple metastases, with an unfortunate outcome.</p>

    <p>The subject was a 63-year-old female patient who visited the neurosurgery outpatient clinic with the chief complaint of speech impairment. As she also had a fever and earache, she was referred to the otorhinolaryngology clinic. Examination at that department revealed several cotton-like foreign bodies from the left external auditory meatus to the tympanic cavity, and when they were removed, the patient exhibited malodorous otorrhea. CT of the temporal bone showed soft-tissue densities from the left external auditory meatus to the middle ear and a defect in the tegmen of the temporal bone from the left tympanic cavity to the mastoid antrum. Head MRI showed high-intensity signals in the mastoid air cells of the left middle ear to the temporal lobe and in the ventricles, and signal abnormalities with low values of the apparent diffusion coefficient (ADC) on diffusion-weighted imaging (DWI). On contrast-enhanced MR imaging, mass shadow containing a region with a low signal intensity in the central area with peripheral enhancement was observed in the left temporal lobe. The patient was diagnosed as having otitis media and cerebral abscess caused by foreign bodies in the external auditory meatus. Drainage of the brain abscess was performed and antibiotic therapy was initiated. The speech impairment gradually improved. Several bone defects in the tegmen of the left temporal bone were observed during the left tympanoplasty. No exacerbation has been observed during the three years since the surgery.</p>

    症例は81歳女性.2014年9月某日より咳嗽を自覚.呼吸困難感,嗄声が徐々に増悪し2014年9月某日に救急外来を受診した.Vital sign,血液検査,画像所見上も異常は認められなかったため,処方帰宅となった.しかし,呼吸困難悪化し,翌日の午前4時に再び救急外来受診,頸部X線にて気道の軽度狭窄が認められた.その際,耳鼻科の受診を勧められたため,同日の午前,耳鼻科外来を受診した.頸部X線を再度施行したところ,気道の狭窄の悪化を認めた.また,喉頭内視鏡を実施したところ,声門下浮腫を指摘され,気道閉鎖の危険性があったため同日緊急入院となった.入院後は喉頭内視鏡で確認しながらステロイドの治療で軽快,気道確保はせず退院となった.このように成人に発症から声門下浮腫が認められるまでの経過をX線,喉頭内視鏡で追うことができた例は稀であり,若干の文献的考察を含めて報告する.(著者抄録)

    Conclusion: The Japanese version of the Tinnitus Handicap Inventory-12 (THI-12), Tinnitus Rating Scale (TRS), TRS 1-week version (TRSw), Tinnitus Severity Scale (TSS), and TSS 1-week version (TSSw), which were developed in this study, showed high reliability and validity, suggesting their usefulness in clinical practice. Based on the THI severity grades, we propose that the severity grades of THI-12 (draft) are categorized into four groups: 0-4 points, 5-9 points, 10-14 points, and 15-24 points. Objectives: We developed Japanese versions of new questionnaires for evaluating the level of psychological distress and difficulty in activities of daily living due to tinnitus, and performed their psychometric validation to determine the reliability and validity. The THI-12 is an assessment consisting of 12 items, each of which is rated on a 3-point scale that was created by reducing the number of questions from the 25 items of the THI. The TRS, TRSw, TSS, and TSSw, which were self-evaluation questionnaires of tinnitus on an 11-grade integer Likert scale from 0 to10 points, were used as additional instruments to assess tinnitus severity and distress. Methods: The subjects were healthy adults, and patients with subjective tinnitus who were examined at the Otolaryngology Department of Keio University Hospital, Osaka City University Hospital, or Nagoya City University Hospital with a chief complaint of tinnitus between September 2010 and January 2011. In all, 38 healthy adult subjects and 113 patients with subjective tinnitus were included. We examined the reproducibility and the internal consistency for reliability. We also examined the relationship with the available scales (THI and Hospital Anxiety and Depression Scale, HADS) and group divergence for validity. Results: The psychometric validation showed high reliability and validity of the THI-12, TRS, TRSw, TSS, and TSSw.

    Vertigo, or dizziness, is a known manifestation of temporal lobe epilepsy. We report a case of temporal lobe epilepsy with vertigo. A 65-year-old man had repeated vertigo attacks with right ear tinnitus. Upon undergoing an otoacoustic emissions examination, he was found to have moderate sensorineural hearing loss in the right ear, an absence of caloric responses in the right ear, and an inner auditory hearing loss in the right ear. A brain MRI examination was unremarkable. Although we treated him for peripheral vertigo during the early stage of disease, the remission of the vertigo attacks was not achieved. At three years after his first visit, the vertigo attacks were still occurring but were also accompanied by a clouding of consciousness. An electroencephalographic examination suggested temporal lobe epilepsy. An MRI examination revealed severe atrophy of the cerebrum and hippocampus. Our findings suggest that temporal lobe epilepsy is still an important part of the differential diagnosis of intractable vertigo and vertigo of unknown origin.

    症例は65歳男性で、高血圧、高脂血症、自律神経失調症で内服中であった。間欠的右耳鳴りから回転性めまいが出現し、1日中続くめまい発作となって紹介受診した。両側鼓膜は正常で、右耳の水平型中等度感音難聴、左耳の高音漸傾型感音難聴が認められた。足踏み検査では回転角度右160°、移動距離50cmであった。眼振は認めず、温度刺激検査では右耳30℃で反応なく、右半規管麻痺が疑われた。耳音響放射検査では右耳障害を示唆する所見であった。当院神経内科での精査で異常は認めず、右メニエール病疑いでイソソルビドを投与するも変化はなかった。その後、天地がひっくりかえるようなめまいとなり、短時間の意識減損発作を伴って転倒することが多くなった。てんかんを疑い、脳波検査で右前側頭部と中側頭部で除鋭複合波を認め、MRIで大脳の著明な萎縮と両側海馬に萎縮が認められた。神経外科で側頭葉てんかんと診断され、バルプロ酸ナトリウム内服でめまい発作の消失を得た。

    Joubert syndrome is a rare autosomal recessive disorder characterized by a specific congenital malformation of the cerebellar vermis. Diagnostic criteria for it include hypotonia, ataxia, episodic hyperpnoea, psychomotor delay, abnormal eye-movement, and the molar tooth sign on magnetic resonance imaging (MRI). We present a 12-month-old boy with Joubert syndrome referred to our hospital because of vertigo. On equilibrium examination, he exhibited left-beating gaze nystagmus, and right-beating horizontal nystagmus, up-beating nystagmus, and alternating nystagmus with a torsional component on positional nystagmus examination. Saccades were preceded by slow eye movement ending with a glissade, and were followed by postsaccadic drift. His smooth pursuit to the left was saccadic, and optokinetic nystagmus gains were reduced. Three months later, his symptoms gradually resolved, his gaze nystagmus nearly disappeared, and only right-beating nystagmus on positional examination was detected.

    症例は12歳男児で、生下時に発作性多呼吸がみられたが受診時には消失し、hypotoniaが強く、運動発達の遅れを認めた。MRIで小脳虫部の欠損を認め、Joubert症候群(JBTS)と診断された。4ヵ月で頸定、9ヵ月で寝返り、1歳7ヵ月で座位、2歳6ヵ月でつかまり立ち、4歳10ヵ月でつたい歩き、5歳で歩行可能となった。8歳頃、時折、頭痛、めまい感を訴え、1ヵ月前より毎日、光を見たとき、臥位になったとき、起きるときに1分間ほど持続するめまい感と締め付けられるような頭痛を自覚した。体幹失調を認め、歩行はスタンスが広く、体幹の動揺のみられる失調性歩行で、会話は数語ずつ細切れにしゃべる断綴性言語を認めた。側方の視標をとらえる際には回転しすぎた頭位を戻すように反対方向に頭を回転するhead thrustがみられた。MRIで小脳虫部の著明な低形成を認め、両側の上小脳脚がやや開き中脳と併せた形が臼歯のように見えるmolar tooth signが認められた。衝動性眼球運動および衝動性眼球運動後の眼位の保持の障害を認め、滑動性眼球運動も一部障害を認めた。視運動性眼振検査(OKP)も触発不良であった。また、めまい症状が出現し、消退したことより小脳および脳幹の機能はある程度の変化が予測された。

    Objectives: Alexithymia is a psychological characteristic involving difficulty expressing and identifying feelings. It has been associated with a variety of psychiatric disorders as well as physical illnesses. The purpose of this study was to investigate the prevalence of alexithymia in patients with vertigo.<br>Methods: The 20-item Toronto alexithymia scale (TAS), the SF-36, the dizziness handicap inventory (DHI), and the state trait anxiety inventory (STAI) were administered to a total of 146 vertigo patients. We also determined any correlation between the length of the disease and alexithymia.<br>Results: Vertigo patients were found to have significantly more alexithymia than controls. There was a positive correlation between TAS and SF-36, DHI and STAI scores in patients with vertigo. A positive correlation between alexithymia and duration of vertigo was found.<br>Conclusion: Alexithymia appears to be related to the progression of vertigo.

    A 56-year-old man with bilateral hearing impairment who had taken betamethasone combined with dexchlorpheniramine maleate for 7 years to treat chronic sinusitis developed a dry cough after discontinuing this medication and was diagnosed with asthma, and after which he sensed impaired bilateral hearing. Based on the presence of numerous eosinophilic leukocytes in otorrehea, we made a diagnosis of eosinophilic otitis media, and he was prescribed predonisolone to control the asthma, but discontinued it on his own. He then developed fever, maniphalanx stiffness, testicular pain, and facial hyperesthesia, eruptions, and the lower-limb numbness. The detection of a positive serum reaction for MPO-ANCA and evaluated of eosinophilic leukocyte levels yielded a definitive diagnosis of CSS, for which the man was treated with predonisolone and cyclophosphamide. His symptoms were relieved, even though the onset of neutropenia, necessitated the discontinuation of cyclophosphamade administration.

    Congenital external auditory canal stenosis (CAS) is usually considered one of the slight malformations related to congenital external auditory canal atresia (CAA). However, the clinical symptoms caused by the slight malformation may not be mild. Destructive external ear canal cholesteatoma commonly occurs in CAS, but is infreguent in CAA. For several reasons CAS tends to receive an inappropriately low estimate of risk compared to that for CAA. The other malformations accompanying CAS and CAA such as microtia confound the decision regarding future treatment for doctors as well as for the patients themselves. A consensus regarding the clinical entity of CAS is required among doctors not only in otorhinolaryngology but also in relevant fields such as pediatrics and plastic surgery. These two malformations, the pathological conditions and clinical treatment of CAS and CAA should be considered separately.

    The purpose of this study was to determine factors associated with recurrence of Benign Paroxysmal Positional Vertigo (BPPV). We studied 122 patients with BPPV. Recurrence was observed in 46 patients. We examined patient age, smoking habit and alcohol consumption, canal paresis (CP) on caloric test, complications, type of BPPV, positional nystagmus observed after recovery and duration of BPPV.<BR>Complications or CP was recognized with higher frequency in patients with recurrence than in those without recurrence. Complications such as hypertension or hyperlipemia were significantly associated with recurrence of BPPV. Duration of BPPV was significantly longer in patients with recurrence. There were no differences in age, smoking habit and alcohol consumption, type of BPPV and positional nystagmus after recovery between patients with recurrence and those without recurrence.<BR>It appeared that patients with recurrence of BPPV might have pathophysiological changes causing detached otoconia.

    A 64-year-old woman consulted our hospital complaining of repeated attacks of vertigo over a six-year. We diagnosed granulomatous angiitis because P-ANCA titers were positive, CT and MRI examinations demonstrated granulation in the middle ear and histopathological study of the kidney demonstrated crescentic glomerulonephritis. We therefore administered the same treatment as for Wegener's granulomatosis but did not use immunosuppressive drugs.<br>Prior to treatment, the frequency of her vertigo attacks had gradually increased and mixed (conductive and sensorineural) hearing loss appeared. We considered these symptoms were mainly due to infection, which can trigger worsening of Wegener's granulomatosis. We therefore first administered antibiotics, then prednisolone (PSL) and sulfamethoxazole and trimethoprim (ST) complex drugs.<br>Cyclophosphamide (CY) was not used even though it has effects on autoimmune disease, since it may promote infectious disease.<br>In such patients, it is important to determine whether autoimmune disease should first be treated with PSL and CY or infection should first be treated with antibiotics.

    経外耳道的内視鏡下耳科手術は侵襲が少なく視野が得やすいことより試験開放術および内耳窓閉鎖術のよい適応と考える。外リンパ瘻に対する試験開放術および内耳窓閉鎖術を行う時には、卵円窓と正円窓を十分に明視下におくことが重要である。正円窓、卵円窓および卵円窓前方に存在するfissula ante fenestramよりの外リンパの漏出を確認する。リンパ液の漏出の有無や部位を確認できるように、できるだけ出血を抑えるようにする。アブミ骨底板や卵円窓周囲、正円窓周囲の粘骨膜をピックにて掻破、アブミ骨底板上、底板後方、fissula ante fenestramを十分に覆うように結合組織、筋膜、軟骨膜を留置しフィブリン糊で固定する。正円窓窩は充填するように結合組織、筋膜、軟骨膜を留置してフィブリン糊で固定する。リンパ液の漏出が明らかでない場合にはcochlin-tomoprotein(CPT)検査を行う。後日にCTP陽性と結果が得られることもあり、リンパ液の漏出がみられなくても正円窓、卵円窓およびfissula ante fenestramへの操作を行うべきとされている。(著者抄録)

    小児めまい患者は成人のめまい患者と同様に中枢性疾患や全身性疾患を除外することが第一に必要である。小児では、問診や検査所見で得られる情報が不十分となることが多く、診察にあたる際にはより注意が必要となる。頻度別でいうと、良性発作性めまい(BPV)、前庭性片頭痛(VM)が多く、約30～40%を占める。これらの頻度を理解することも重要であると同時に年齢層別にも疾患を理解することも重要である。新生児期～乳児期は先天性疾患の除外が必要であり、幼児期の頃に反復するめまいでは小児良性発作性めまい(BPV)が多いとされる。起立性調節障害は10歳以上の思春期の時期に多く特に女児に多いとされている。これらのように疫学的理解を持ちつつ、実際の診察、検査結果で得られる情報を正確に評価し診療を行うことが必要となる。(著者抄録)

    良性発作性頭位めまい症(BPPV)の診断、治療を行う際には患者に病態の理解、検査・治療について説明し理解が得られることが重要である。診断には頭位・頭位変換検査により診断を行う。症状および神経学的所見の確認がBPPVと合致しており典型的な眼振所見が得られればそれ以上の精査は必ずしも必要ではない。治療については耳石浮遊置換法を行うべきである。非特異的な運動療法を行うよりも高い効果がある。薬物治療は効果がなく薬物の投与は利点より欠点が多いと考えられている。再発を繰り返すことも多く、繰り返し治療を行うこともある。高齢者の転倒リスクの1つでもあることより疾患について理解が得られるように説明する必要がある。(著者抄録)

    持続性知覚性姿勢誘発めまい(persistent postural perceptual dizziness;PPPD)は、慢性めまいを主訴とする機能性疾患である。本邦で策定されためまいを起こす疾患にはPPPDに該当する疾患は含まれていない。特徴としては3ヵ月以上、ほぼ毎日持続する浮遊感・不安定感・非回転性めまいのいずれかが主訴となる。立位姿勢、能動的・受動的な動き、および複雑な視覚刺激で増悪する。急性・発作性・慢性の前庭疾患が先行して起こった際に発症する場合もある。PPPD発症頻度の高い病態としては、末梢性または中枢性の前庭疾患(25～30%)、前庭性片頭痛の発作(15～20%)、顕著な浮動感を示すパニック発作または不安(それぞれ15%)、脳しんとうまたはむち打ち症(10～17%)、自律神経障害(7%)である。今までは、原因不明の「めまい症」とされてきた患者の中に一定数は存在しており、的確に診断が行えれば患者の不安を軽減させ治療意欲の増加にもつながると考えられる。(著者抄録)

    ストレスとめまいの関連のパターンとしては、(1)メニエール病のように疾患そのものは内耳の器質的障害であるがめまい発作の出現にストレスが強く関与する場合、(2)めまい疾患そのものがストレスとなり発症する恐怖性姿勢めまい、(3)ストレスや心理因子そのものが原因の心因性めまいや自律神経機能障害によるめまいなどがある。メニエール病では器質的障害に加えてストレスが発作の誘因になっていると考えられる。抗利尿ホルモンは心理ストレスに反応し放出され、ADHの慢性的な高値が内耳の水代謝に影響を与え内リンパ水腫を増長することが示唆されている。恐怖性姿勢めまいではめまい体験より引き起こされる錯認知が原因と考えられるため、心因機序の説明と共に行動療法が有用である。ストレスは中枢自律神経線維網へ作用することで自律神経系へ影響する。自律神経機能障害により起立性低血圧や起立性血圧調節障害が発症しめまいの原因となりえる。(著者抄録)
