    女性型脱毛症(FPHL)のマウスモデルについて検討した。3週齢と7～8週齢のC57BL/6雌マウスを2群に分け、1群には卵巣摘出(OVX)を行い、他の群には偽手術を行った。3週齢マウスでは7週齢目に背部皮膚を採取し、7～8週齢マウスでは12と24週齢目に採取した。前群では、休止期の毛包の孔径と毛幹の直径はOVXによって低下し、後群ではこれらの値が上昇した。特に真皮と皮下組織の厚さはOVXで有意に上昇した。OVXマウスがFPHLの理想的なマウスモデルとして使用できるかはさらに検討する必要があるが、皮膚の厚さに対するこれらの知見は、閉経関連皮膚状態のような非毛髪関連疾患の新規治療を確立するために使用できると考えられた。

    39歳,男性.約3年前に草刈り機の破片が左膝にあたったが放置していた.次第に同部位が腫大してきたため近医を受診し,単純X線にて金属片様異物を認めた.当院にて摘出術を行ったところ,中央部に黒色の硬い異物が迷入し,その周囲に結節が生じていた.病理組織像では異物周囲に密なリンパ球の浸潤を認めリンパ濾胞様構造が観察された.免疫組織化学的検査ではB細胞の単クローン性の増殖は認めず,異物によるlymphocytoma cutis(LC)と診断した.金属片はタングステン,コバルト,チタンからなる超硬合金であり,その他亜鉛など19種類の成分で構成されていた.金属パッチテストの結果コバルトと亜鉛に陽性であり,自験例の成因に金属アレルギーが関与していることが推察された.(著者抄録)

    抗BP180型粘膜類天疱瘡(BP180-MMP)の病理学的機序を明らかにするため、BP180-MMP患者(男性2例、女性5例)の皮膚組織におけるBP180の分布や、BP180の分布に及ぼすBP180-MPP患者由来上皮細胞における免疫グロブリンG(IgG)の影響について検討するため、免疫組織化学分析を行った。その結果、BP180は、正常上皮細胞では基底膜部(BMZ)に線状に沈着を認めるが、BP180-MMP検体ではBMZや基底細胞表面に沈着し、基底細胞内に沈着を認める検体と認めない検体が存在していた。また、IgG投与したケラチン細胞株のライブセルイメージングにおいては、正常上皮細胞IgG投与ではBP180の内在化はみられなかったが、BP180-MMP-IgG投与では、7例中3例にBP180の内在化が認められた。以上の結果から、BP180-MMP患者由来IgGはBP180は、上皮細胞に内在化を起こす場合と起こさない場合があり、不均一であることが示された。

    弾性線維腫の組織学的特徴を明らかにするため、弾性線維腫患者3例(75歳男性、65歳女性、80歳男性)の検体を用い、定量的免疫組織化学分析により、TGF-β1および塩基性FGF(bFGF)の発現を調べた。患者由来の検体を健常者の皮膚検体5例、皮膚線維腫検体10例、ケロイド検体10例と比較検討した結果、弾性線維腫で16〜59%、皮膚線維腫で96%、ケロイドで93%、正常皮膚で2%の線維芽細胞が細胞質においてTGF-β1陽性が示された。また、弾性線維腫で26〜67%、皮膚線維腫で97%、ケロイドで97%、健常者皮膚で22%の線維芽細胞が細胞質においてbFGF陽性が認められ、腫瘍サイズや増殖率は、bFGF陽性細胞の割合に比例することが明らかにされた。以上の結果から、弾性線維腫は、細胞質にTGF-β1及びbFGFを高発現することが示され、TGF-β1とbFGFの組み合わせは、弾性線維腫のマーカーとなる可能性が示唆された。

    69歳男。尋常性乾癬の既往があった。水疱形成を伴うそう痒性紅斑にて来院した。2ヵ月前に外用薬を中断していた。落屑を伴う紅斑が顔面以外の全身にみられ、皮膚びらんおよび緊満性水疱が前腕および項部にみられた。紅斑の病理組織所見は乾癬に一致し、表皮下水疱の内部および周辺に好中球および好酸球の浸潤がみられた。間接免疫蛍光法にてIgG抗体での1M NaCl-splitヒト皮膚の表皮側および真皮側の反応が見られ、免疫ブロットにてラミニンγ1、BP180およびBP230陽性であった。以上より、尋常性乾癬を合併した抗ラミニンγ1、抗BP180、抗BP230抗体を伴う水疱性類天疱瘡と診断し、シクロスポリンを投与した。水疱は徐々に消失したが、6ヵ月後の酵素結合免疫吸着測定法にてBP230は陽性であった。

    マウス胎生期ステージ0〜3において皮膚検体を採取し、ラミニン-511および-332とその受容体蛋白質であるα3およびβ4インテグリンに対する免疫染色を行って、それぞれの局在を調査した。また、皮膚検体から蛋白質を抽出し、ラミニン-511および-332レベルを免疫ブロット法で定量し、半定量的RT-PCR法で内因性mRNAを評価して転写発現を解析した。ラミニン-332蛋白質は、ステージ0で検出され、ステージ1〜2の間に下方制御され、ステージ3で回復することが明らかになった。ラミニン-511のα5鎖の発現はステージ0〜3を通じて一定であった。ラミニン-332の全てのサブユニットのmRNAは発現ステージ0〜2の間に徐々に増加し、ラミニン-511の全てのサブユニットのmRNAはステージ0〜3を通じて一定であった。ラミニン-332とラミニン-511の適切な発現が毛髪形態形成の適切な制御を行うことが示された。

    メチルマロン酸血症は先天性有機酸代謝異常の一つで、治療は、症状に対する対症療法とともに代謝不可能なアミノ酸摂取制限が不可欠となり、その治療に伴う必須アミノ酸不足によりまれに腸性肢端皮膚炎様皮疹を呈する。メチルマロン酸血症に発症した腸性肢端皮膚炎様皮疹の創傷管理の報告は極めて少ないが、今回、その創傷管理に奏功した症例を経験した。メチルマロン酸血症に発症した腸性肢端皮膚炎様皮疹の創傷管理やスキンケアの方向性が示唆され、同様の症例に対し、ケアの指標の一つになると考えられた。(著者抄録)

    31歳,男性.初診2ヵ月前から右上腕に無症候性皮疹が出現した.初診時に直径2cmと5mmの隣接する紅色の結節および丘疹を認めた.皮膚生検では,Grenz zoneを伴い,真皮全層に稠密に細胞が浸潤しており,一部に濾胞様構造を形成していた.腫瘍細胞はB細胞性の小型リンパ球様細胞が主体で,形質細胞様の細胞も混在していた.免疫グロブリン軽鎖はκ鎖のみが陽性だった.PET-CTで全身に有意な集積像を認めず,皮膚原発marginal zone B-cell lymphomaと診断した.皮膚腫瘍を3cmマージンで切除し遊離植皮術で再建した.術後1年8ヵ月現在まで再発はない.皮膚原発marginal zone B-cell lymphomaは皮膚原発悪性リンパ腫の7%を占めるのみであり,比較的稀な疾患である.限局性の病変では手術もしくは放射線療法が第一選択とされているが,切除マージンなどのコンセンサスはいまだなく,今後の症例の蓄積が待たれる.(著者抄録)

    A 26-year-old Japanese woman was admitted to our hospital because of spiking fever, eruption, and liver dysfunction. High fever continued after admission, and antibiotics were not effective. The patient was diagnosed as adult-onset Still's disease after excluding leukemia and malignant lymphoma. Because the patient was complicated with acute liver failure, steroid pulse therapy was started. Although steroid pulse therapy was transiently effective for clinical symptoms, oral predonisolone was not. Moreover, the patient was complicated with severe cytopenia. We diagnosed with macrophage activation syndrome complicated with adult-onset Still's disease. She was treated with cyclosporine A and liposteroid, resulted in complete remission for macrophage activation syndrome and adult-onset Still's disease. In summary of our case, combination of cyclosporine A and liposteroid was effective for the treatment of macrophage activation syndrome with acute liver failure in a patient with adult-onset Still's disease.<br>

    症例は26歳、女性。発熱および肝機能障害にて入院。抗生剤治療不応の弛張熱と皮疹を特徴とし、肝生検などにより白血病や悪性リンパ腫などの疾患を除外して成人発症Still病と診断した。急性肝不全を呈し、ステロイドパルス療法によりいずれの臨床症状も軽快したが、ステロイドの減量によりすぐに再燃した。更に血球減少を呈するマクロファージ活性化症候群(macrophage activation syndrome、MAS)に移行した。シクロスポリンおよび静注用リポ化ステロイドの投与を開始したところMAS症状は改善し、第41病日に退院した。成人発症Still病に合併したMASにはシクロスポリンおよびリポ化ステロイドの治療が有用であった。(著者抄録)

    90歳男性。体幹および大腿部に紅色皮疹が出現し、これが拡大したため近医を受診、ステロイド外用薬と抗アレルギー薬が処方されたが改善はなく、やがて39℃台の発熱を伴い著者らの施設にある内科へ入院、翌日に皮膚科転科となった。初診時、胸腹部から両鼠径部、大腿にかけて皮疹がみられ、粟粒大の紫赤色ないし紅色の丘疹が集簇して激しい痒みを伴っていた。血液検査ではLDH値やCRP値の上昇と可溶性IL-2レセプターの異常高値がみられたが、胸部-骨盤部CTでは明らかな異常は認められなかった。一方、鼠径部の紅色丘疹の皮膚生検ではH-E染色で表皮に海綿状態がみられ、真皮浅層から中層に腫瘍細胞が稠密に浸潤していた。また、免疫染色ではCD3・CD4・CD45Rは陽性、CD20・CD68は陰性で、PCR法による遺伝子再構成検索ではモノクローナルなT細胞レセプターの遺伝子再構成が確認された。以上の所見を踏まえ、治療は入院時に内科で投与されたソル・メドロールにより皮疹がやや軽快したため、以後も125mg×4日間、80mg×3日間を継続、更に抗ヒスタミン薬の内服が行われた。その結果、皮疹は徐々に痂皮化して脱落し、入院17日目には退院となった。尚、現在まで発熱や皮疹の再燃はみられていない。

    57歳、男性。7年ほど前より右下腿に有痛性の皮下腫瘤が出現した。初診時、右下腿内側に10×10mm大の境界明瞭な弾性軟の皮下腫瘤を認め、揉むと硬くなり疼痛が出現した。超音波所見では、結節はlow echoic lesionで内部エコーはほぼ均一であった。カラードプラでは血流が豊富で、揉むと腫瘍は硬くなって血流シグナルが消失し、温めると再び血流シグナルが出現した。病理組織学的所見では、血管と不規則に交錯する紡錘形細胞から構成されており、血管平滑筋腫と診断した。血管平滑筋腫は発作性に疼痛が出現することが知られるが、疼痛出現前後の超音波所見の変化についての報告はいまだない。本稿では、自験例の超音波所見を記載するとともに文献的考察を加えた。(著者抄録)

    68歳男。4ヵ月前頃より陰茎に痛みとしこりが出現し増悪した。陰茎冠状溝付近左側に直径約2cm程度の皮膚潰瘍があり、周囲の中枢側背面皮下には硬結を触れ、自発痛・圧痛を伴っていた。病理組織所見で、表皮は一部で脱落し、真皮全層にリンパ球、好中球、組織球などの著明な炎症細胞浸潤を認めた。痛みが強いためプレドニゾロン5mg/dayを投与し、更にQFT-TB陽性よりイソニアジド、リファンピシン、エタンブトール、ピラジナミド投与を試みたが症状は改善せず、壊疽性膿皮症と診断した。3日間メチルプレドニゾロン1g/dayを点滴静注し、プレドニゾロン60mg/dayを開始したところ、炎症に伴った陰茎の浮腫、潰瘍部からの排膿や滲出液は減少し、1週間後には赤色肉芽形成を認めた。その後、プレドニゾロンは漸減し、創部処置にトラフェルミンを用いたところ創部の上皮化傾向を認めたため退院とした。プレドニゾロン減量中に潰瘍が拡大したが、増量および塩酸ミノサイクリン200mg/day併用で改善し、2ヵ月後に創部は上皮化した。壊疽性膿皮症に関連した合併症はなかった。

    28歳女性。自宅で就寝中、突然の激痛で目が覚めた。翌朝の初診時、右前腕と左下肢の激痛・発汗過多が見られ、右前腕の疼痛部の紅斑も観察された。臨床所見からは診断できなかったが、帰宅後、ベッドの上でセアカゴケグモの死骸を発見し診断がついた。文献的考察を加え、刺咬部位以外の症状が診断上重要であることを強調した。(著者抄録)

    ＜症例のポイント＞Parry-Romberg症候群(Parry-Romberg syndrome、以下、PRS)はまれな疾患であり、緩徐に進行する皮膚、軟部組織の萎縮を伴う後天性疾患である。PRSの経過としてはまず顔面片側に色素斑が生じ、次第に皮膚および軟部組織の萎縮へと進行する。色素斑内部に白斑を伴うことはあるが、脱色素斑と皮膚萎縮が完全にオーバーラップした報告はない。今回、21歳女性に生じたPRSの症例を報告する。患者の皮疹は1)下顎の白斑に完全に一致して陥凹していた、2)上背部、腰部にびまん性の茶褐色斑が存在したが、その内部に陥凹病変を伴う白斑が複数個存在していた点で特異的であった。病理組織学的には膠原線維の軽度の均質化、弾性線維の減少が皮疹部で認められた。(著者抄録)

    C57BL/6脱毛モデルマウスからの組織を用いて、毛髪退行期におけるラミニン-511と-332の局在性および役割について検討した。下部毛嚢の基底膜(BM)周辺のラミニン-332分布は退行期に変化したが、その総発現量はmRNAおよび蛋白レベルの両方で退行期の間中安定していた。一方、ラミニンα5のin situハイブリダイゼーション、ウエスタンブロット、QRT-PCR研究により、ラミニン-511発現に関しては徐々にダウンレギュレートしたことが示された。更に、発育期における遺伝子組み換えラミニン-332の注入は毛髪退行進行に影響を与えなかったが、ラミニン-511に富んだ蛋白エキスはその遅延をもたらした。以上より、ラミニン-511のダウンレギュレーションが毛髪退行に大きく関与していることが示された。

    シクロホスファミド(CYP)投与により化学療法後脱毛(CIA)を誘導したマウスを用いて、ラミニン-332、-511の役割および局在性を検討した。中期から後期の毛髪萎縮退行期後に、毛嚢の下部1/3における外毛根鞘周辺で、ラミニン-332とその受容体α6β4インテグリンの量的、空間的アップレギュレーション(UR)が認められ、本URは転写レベルでも発生した。一方、ラミニン-511は中期萎縮退行期後に蛋白レベルでのダウンレギュレーションを示した。ラミニン-511発現は、CIAにおけるα3インテグリン発現に関連し、培養ケラチノサイトにおいてα3インテグリンプロモーターの活性をURし得ることが示された。マウスの背部皮膚へのラミニン-511に富んだ蛋白エキスの注入はCYP誘導CIAにおける毛髪喪失の遅延をもたらしたが、遺伝子組み換えラミニン-332は本作用を示さなかった。以上より、ラミニン-511はCIAに対し有用な補助療法になり得ると考えられた。

    51歳女。Basedow病の甲状腺亜全摘の術後2週目頃より両下腿に無症候性の皮疹、浮腫が出現し徐々に拡大し受診した。入院時、両下腿遠位側1/3から踵に掛けて全周性に足背から足趾まで淡紅色、弾性硬、表面凸凹不正の隆起性局面を形成していた。TSH受容体抗体(TRAb)およびサイログロブリン(Tg)は異常高値で、TSH値は低値を示したが、入院時にはレボチロキシンナトリウム内服によりfree T3、free T4、TSHはほぼ正常であった。病理所見では右下腿結節部は表皮の角化、真皮浅層から中層に掛けて膠原線維の断裂および著明な浮腫を認め、アルシアンブルー染色で膠原線維の縁に陽性部分が多数存在し多量の酸性ムコ糖類が沈着していた。粘液水腫と診断し約1年間ステロイド軟膏外用を行い下腿の腫脹は若干軽快したが、局注および漢方内服の効果なくトレパン打抜法を行った。術直後より多量のムチンの排出を認め、弾性包帯による圧迫を継続した。開口部に吉草酸ベタメタゾンローションの注入、トリアムシノロンアセトニドを局注し、術後8日目にトレパンで打ち抜いた周囲は平坦化し、12日目にムチンの排出、18日目に創部の上皮化を認め、前脛部を中心に著明に改善した。2ヵ月後の再入院で両下腿全周および足背に計60箇所の打抜術を行ったが感染はなく、両側足首の周囲径は術前に比べ著明に改善した。

    58歳,女性。53歳時,Overlap症候群と診断された8ヵ月後より背部に結節が出現し,その後5年間で徐々に増数した。初診時,前胸部から肩甲部に左右対称性,約2cm,正常皮膚色から褐色の硬い結節が多発していた。組織学的に真皮浅層から深層において,線維芽細胞はやや増殖し,結節状に増生する膠原線維を認めた。Overlap症候群に伴う結節状強皮症と診断し,原疾患に対するプレドニゾロン,シクロスポリン内服に加え,結節に対してメチルプレドニゾロン局所注射,トラニラスト内服を行うも結節は不変である。結節状強皮症は稀で,発症機序は不明であり,難治性の疾患である。(著者抄録)

    80歳、男性。79歳時より前額部および左前腕に紅色腫瘤が出現し、次第に増大してきた。左前腕部紅色腫瘤の皮膚生検で、真皮にGrenz zoneとその下層にび漫性の密な腫瘍細胞の浸潤を認めた。免疫染色では、腫瘍細胞はLCA(+),CD4(+),CD56(+),CD3(-),CD5(-),CD8(-),CD10(-),CD20(-),CD30(-),CD34(-),CD79a(-),TIA-1(-)、グランザイムB(-),TdT(-),MPO(-),bcl-2(+)であった。サザンブロット法にてT-cell receptorおよび免疫グロブリンH鎖の遺伝子再構成を認めなかった。造影CT,FDG-PETにてリンパ節腫脹や遠隔転移は存在しなかった。以上より、本症をCD4+/CD56+hematodermic neoplasmと診断した。(著者抄録)

    An 80-year-old Japanese male presented with tumors of the left forearm and forehead. A biopsy specimen from the skin lesion showed dense infiltrates of medium-sized lymphoid-appearing cells in the dermis with a Grenz zone. These cells were stained positively by LCA, CD4, CD56, bcl-2 and negatively by CD3, CD5, CD8, CD10, CD20, CD30, CD79a, TIA-1, granzyme B, TdT, MPO on immunohisto-chemical examination. There was no evidence of abnormal lymph node swelling or visceral involvement under image diagnosis. The patient was diagnosed with CD4+/CD56+hematodermic neoplasm.     We report a case of nodular scleroderma in a 58-year-old woman with Overlap syndrome (polymyositis + systemic scleroderma + Sj&ouml;gren syndrome). At the age of 53, eight months after diagnosis, asymptomatic nodules appeared on her back and gradually increased in number for five years. Multiple normal skin colored or pigmented firm elevated nodules of 2cm in diameter scattered on her shoulders and back. Histopathological findings showed increase of fibroblasts, and whorl-like collagen nest through the dermis. We diagnosed this case as a nodular scleroderma with Overlap syndrome. Along with oral steroid and ciclosporin for Overlap syndrome, topical injection of steroid and oral tranilast were given, but the size and number of these nodules were unchanged. Nodular scleroderma is a rare and refractory disorder, and the mechanism remains still unclear.

    Basal cell carcinoma (BCC) is a tumor that often arises from the skin on the face. When it occurs on the trunk, however, it is often difficult to diagnose it as BCC because of various clinical appearances.<br>We reported an 83-year-old Japanese female, who noticed a skin tumor in the right groin since 1999. It had gradually grown and showed intermittent bleeding and discharge, so she visited our hospital in July 2005. She had a pedunculated tumor of 53&times;45 mm in size in the right groin, and it looked blackbrown and was easy-bleeding. And swelled lymph nodes were palpated in the right groin. Clinically we suspected it as malignant melanoma, eccrine porocarcinoma, or basal cell carcinoma, and an incisional biopsy was carried out. Pathological examination revealed it as basal cell carcinoma. A pedunculated BCC in the groin region is relatively rare. The pathological process of pedunculated tumor formation has not yet been evaluated. We should take BCC into consideration when observing a pedunculated tumor on the trunk.     基底細胞癌は顔面に好発する腫瘍である。臨床的には典型的な臨床像をとることから診断は容易である。しかし、躯幹に生じた場合、様々な臨床をとり診断に苦慮することがある。患者は83歳、女性。1999年頃より右鼠径部の腫瘤に気付いた。徐々に増大し出血・滲出液を生じるようになり、2005年7月当科を受診した。初診時、右鼠径部に53×45mmの易出血性の黒褐色有茎性腫瘤が存在し、右鼠径部リンパ節は腫大していた。臨床的に、悪性黒色腫、エクリン汗孔癌、基底細胞癌などを疑い皮膚生検を行った。病理組織学的に基底細胞癌と診断した。鼠径部に有茎性基底細胞癌が生じることは比較的稀である。有茎となる要因はまだ明らかにされていない。躯幹では基底細胞癌が有茎腫瘤を呈する可能性も考慮すべきである。(著者抄録)

    症例は74歳女性で、慢性骨髄増殖性疾患にてヒドロキシウレアとプレドニゾロンによる治療中に右手背から前腕にかけて腫脹と疼痛が出現して急速に増悪し、重症軟部組織感染症の診断にて入院となった。入院当日にショック状態・多臓器不全に陥ったが、デブリドマンを含む外科的処置とデイコプラン投与により改善した。デブリドマン時に皮下組織・脂肪組織と筋膜浅層の壊死を認めたことから壊死性筋膜炎と診断され、起因菌として手背創部の壊死組織より黄色ブドウ球菌が同定された。第3病日に尿量減少・高カリウム血症・アシドーシス進行を認め人工呼吸管理下に持続血液濾過透析(CHDF)を導入した。抗菌薬をアンピシリンからテイコプラニンへの変更によりCRPが低下し、第9病日にCHDFから離脱できた。第12病日にデブリドマンを追加し壊死のため右第2指遠位末節骨部を切断し、第26病日にデブリドマンと遊離植皮手術を施行した。植皮生着は良好であったが、第35病日に緑膿菌肺炎を合併し第48病日に死亡した。以上より壊死性筋膜炎は死亡率が高く、稀ではあるが血液疾患治療中の合併が報告されており、早期診断・起因菌同定・抗菌薬選定・外科的処置と全身管理が重要と考えられた。
