Objective: The objective of this study was to analyze how parental education modifies the genetic and environmental variances of BMI from infancy to old age in three geographic-cultural regions.

Methods: A pooled sample of 29 cohorts including 143,499 twin individuals with information on parental education and BMI from age 1 to 79 years (299,201 BMI measures) was analyzed by genetic twin modeling.

Results: Until 4 years of age, parental education was not consistently associated with BMI. Thereafter, higher parental education level was associated with lower BMI in males and females. Total and additive genetic variances of BMI were smaller in the offspring of highly educated parents than in those whose parents had low education levels. Especially in North American and Australian children, environmental factors shared by co-twins also contributed to the higher BMI variation in the low education level category. In Europe and East Asia, the associations of parental education with mean BMI and BMI variance were weaker than in North America and Australia.

Conclusions: Lower parental education level is associated with higher mean BMI and larger genetic vari-ance of BMI after early childhood, especially in the obesogenic macro-environment. The interplay among genetic predisposition, childhood social environment, and macro-social context is important for socioeco-nomic differences in BMI.

Background: The genetic architecture of birth size may differ geographically and over time. We examined differences in the genetic and environmental contributions to birthweight, length and ponderal index (PI) across geographical-cultural regions (Europe, North America and Australia, and East Asia) and across birth cohorts, and how gestational age modifies these effects.

Methods: Data from 26 twin cohorts in 16 countries including 57 613 monozygotic and dizygotic twin pairs were pooled. Genetic and environmental variations of birth size were estimated using genetic structural equation modelling.

Results: The variance of birthweight and length was predominantly explained by shared environmental factors, whereas the variance of PI was explained both by shared and unique environmental factors. Genetic variance contributing to birth size was small. Adjusting for gestational age decreased the proportions of shared environmental variance and increased the propositions of unique environmental variance. Genetic variance was similar in the geographical-cultural regions, but shared environmental variance was smaller in East Asia than in Europe and North America and Australia. The total variance and shared environmental variance of birth length and PI were greater from the birth cohort 1990-99 onwards compared with the birth cohorts from 1970-79 to 1980-89.

Conclusions: The contribution of genetic factors to birth size is smaller than that of shared environmental factors, which is partly explained by gestational age. Shared environmental variances of birth length and PI were greater in the latest birth cohorts and differed also across geographical-cultural regions. Shared environmental factors are important when explaining differences in the variation of birth size globally and over time.

Abstract
The mechanisms behind handedness formation in humans are still poorly understood. Very low birthweight is associated with higher odds of left-handedness, but whether this is due to low birthweight itself or premature birth is unknown. Handedness has also been linked to development, but the role of birthweight behind this association is unclear. Knowing that birthweight is lower in multiple births, triplets being about 1.5 kg lighter in comparison with singletons, and that multiples have a higher prevalence of left-handedness than singletons, we studied the association between birthweight and handedness in two large samples consisting exclusively of triplets from Japan (n = 1,305) and the Netherlands (n = 947). In both samples, left-handers had significantly lower birthweight (Japanese mean = 1,599 g [95% confidence interval (CI): 1,526-1,672 g]; Dutch mean = 1,794 g [95% CI: 1,709-1,879 g]) compared with right-handers (Japanese mean = 1,727 g [95% CI: 1,699-1,755 g]; Dutch mean = 1,903 g [95% CI: 1,867-1,938 g]). Within-family and between-family analyses both suggested that left-handedness is associated with lower birthweight, also when fully controlling for gestational age. Left-handers also had significantly delayed motor development and smaller infant head circumference compared with right-handers, but these associations diluted and became nonsignificant when controlling for birthweight. Our study in triplets provides evidence for the link between low birthweight and left-handedness. Our results also suggest that developmental differences between left- and right-handers are due to a shared etiology associated with low birthweight.

BACKGROUND: There is evidence that birth weight is positively associated with education, but it remains unclear whether this association is explained by familial environmental factors, genetic factors or the intrauterine environment. We analysed the association between birth weight and educational years within twin pairs, which controls for genetic factors and the environment shared between co-twins.

METHODS: The data were derived from nine twin cohorts in eight countries including 6116 complete twin pairs. The association between birth weight and educational attainment was analysed both between individuals and within pairs using linear regression analyses.

RESULTS: In between-individual analyses, birth weight was not associated with educational years. Within-pairs analyses revealed positive but modest associations for some sex, zygosity and birth year groups. The greatest association was found in dizygotic (DZ) men (0.65 educational years/kg birth weight, p=0.006); smaller effects of 0.3 educational years/kg birth weight were found within monozygotic (MZ) twins of both sexes and opposite-sex DZ twins. The magnitude of the associations differed by birth year in MZ women and opposite-sex DZ twins, showing a positive association in the 1915-1959 birth cohort but no association in the 1960-1984 birth cohort.

CONCLUSION: Although associations are weak and somewhat inconsistent, our results suggest that intrauterine environment may play a role when explaining the association between birth weight and educational attainment.

Abstract
It is well established that boys are born heavier and longer than girls, but it remains unclear whether birth size in twins is affected by the sex of their co-twin. We conducted an individual-based pooled analysis of 21 twin cohorts in 15 countries derived from the COllaborative project of Development of Anthropometrical measures in Twins (CODATwins), including 67,850 dizygotic twin individuals. Linear regression analyses showed that boys having a co-twin sister were, on average, 31 g (95% CI 18 to 45) heavier and 0.16 cm (95% CI 0.045 to 0.274) longer than those with a co-twin brother. In girls, birth size was not associated (5 g birth weight; 95% CI -8 to -18 and -0.089 cm birth length; 95% CI -0.202 to 0.025) with the sex of the co-twin. Gestational age was slightly shorter in boy-boy pairs than in boy-girl and girl-girl pairs. When birth size was standardized by gestational age, the magnitude of the associations was attenuated in boys, particularly for birth weight. In conclusion, boys with a co-twin sister are heavier and longer at birth than those with a co-twin brother. However, these differences are modest and partly explained by a longer gestation in the presence of a co-twin sister.

BACKGROUND: There is evidence that birth size is positively associated with height in later life, but it remains unclear whether this is explained by genetic factors or the intrauterine environment.

AIM: To analyze the associations of birth weight, length and ponderal index with height from infancy through adulthood within mono- and dizygotic twin pairs, which provides insights into the role of genetic and environmental individual-specific factors.

METHODS: This study is based on the data from 28 twin cohorts in 17 countries. The pooled data included 41,852 complete twin pairs (55% monozygotic and 45% same-sex dizygotic) with information on birth weight and a total of 112,409 paired height measurements at ages ranging from 1 to 69 years. Birth length was available for 19,881 complete twin pairs, with a total of 72,692 paired height measurements. The association between birth size and later height was analyzed at both the individual and within-pair level by linear regression analyses.

RESULTS: Within twin pairs, regression coefficients showed that a 1-kg increase in birth weight and a 1-cm increase in birth length were associated with 1.14-4.25 cm and 0.18-0.90 cm taller height, respectively. The magnitude of the associations was generally greater within dizygotic than within monozygotic twin pairs, and this difference between zygosities was more pronounced for birth length.

CONCLUSION: Both genetic and individual-specific environmental factors play a role in the association between birth size and later height from infancy to adulthood, with a larger role for genetics in the association with birth length than with birth weight.

保健師の医療的ケアを要する避難行動要支援者に対する個別避難計画の作成意思と、その関連要因を明らかにすることを目的として、全国の指定都市及び中核市で医療的ケアを要する避難行動要支援者を担当する保健師を対象に、郵送自記式質問紙調査を行った。個別避難計画の作成意思の高低を目的変数とし、所属自治体の災害への準備状況、回答者の基本情報、知識、経験、スキル、個別避難計画作成に対する態度や認識等との関連について分析を行い、有意水準5%未満で差が認められた項目についてロジスティック回帰分析を行った。有効回答率は38.1%(256件)であった。避難行動要支援者に対する個別避難計画の作成意思と有意に関連していた項目は、個別避難計画に対する避難行動要支援者の生命を守る意義感の高さ、保健師役割としての認識の高さ、作成手順に関する知識の低さ、様式に関する知識の低さ、および作成時間の無さに対する認識の低さであった。(著者抄録)

目的 母子保健情報の利活用は,日常の母子保健活動における個別支援に貢献できると期待される。2020年6月より,乳幼児健康診査(以下,乳幼児健診)等の情報の電子化が各市区町村に義務付けられたが,その実施状況は明らかではない。2020年度厚生労働科学研究費補助金(成育疾患克服等次世代育成基盤研究事業(健やか次世代育成総合研究事業))「母子の健康改善のための母子保健情報利活用に関する研究」研究班(研究代表者:山梨大学山縣然太朗)は,情報利活用における課題を明らかにすることを目的として,市区町村における実態調査を行った。方法 2020年12月1日から2021年1月29日に,全1,741市区町村(指定都市,中核市,保健所設置市,特別区を含む。)の母子保健主管部(局)担当課を対象に,乳幼児健診等の情報の電子化,ならびに情報連携の実施等を問う自記式調査票による調査を行った。結果 全国1,741市区町村のうち985の市区町村から回答を得た(回答率56.6%)。乳幼児健診における最低限の項目の電子化をしていたのは,3-4ヵ月,1歳6ヵ月,3歳児健診で931(94.5%),936(95.0%),936(95.0%)であった。副本登録については,164(16.7%)の市区町村では情報の再入力,42(4.3%)では,何らかの追加作業が必要であり,追加作業が必要な市区町村では,必要のない市区町村に比べて有意に負担を感じていた。住民の転出入等に伴い乳幼児健診等の情報連携を実施した市区町村は,130(13.2%),実施しなかったのは756(76.8%),転出・転入なしが90(9.1%)であった。情報連携の実施率は指定都市・中核市・特別区で有意に高かった。情報連携を行わなかった理由として,指定都市・中核市・特別区では,「転入者を随時把握して情報照会することが負担」,その他の市町村では,「従来の方法で情報共有ができており,情報連携を行う必要性を感じない」がより多い傾向にあった。結論 最低限電子化すべき項目は9割以上の市区町村で電子化されていたが,情報連携実施は一割程度であった。都市部および大都市周辺の市区では情報連携の作業負担が大きく,その他の市町村では情報連携の利点を感じていない傾向が示された。電子化項目の整備,即時で簡便な副本登録による作業負荷軽減など多面的な取組が必要と考えられた。(著者抄録)

<p><b>Objectives</b>　This study targeted public health nurses in a Japanese municipality that had newly adopted the core components of the Finnish system to provide continuous support to families with children until preschool. It sought to analyze changes in their perceptions for activities in maternal and child health care before and after the adoption.</p><p><b>Methods</b>　Relevant data were collected through semi-structured focus group interviews, following an interview guide, conducted from September to October 2020. The study protocol was approved by the Ethics Committee of Osaka City University in Japan.</p><p><b>Results</b>　A total of 12 public health nurses participated. Prior to the adoption of the system modelled on Finnish practice, these nurses were dedicated to addressing the risks regarding families with children (especially those at high risk) through continuous engagement. However, they were hesitant to proactively engage with families at lower risk as non-continuous, one-off engagements resulted in a patchwork response. After the system was adopted, the nurses became aware of the trust cultivated with families under their care, which included those at lower risk, that enabled them to respond to the changing needs flexibly. The nurses recognized that they had acquired the capability to notice the subtle signs of changes, engage more proactively with the families under their care, and deliver the necessary preventive interventions at an early stage. They also demonstrated joy and satisfaction derived from the growth of children and mothers under their care, as well as keen awareness of the need to improve their professional skills, even though they were busy.</p><p><b>Conclusion</b>　These findings indicate that the continuous support system modelled on the core components from Finland enables public health nurses to proactively engage with families at lower risk and deliver preventive interventions at an early stage. The system also motivated Japanese public health nurses who found greater pleasure in their work.</p>

目的 本研究では,フィンランドの基盤のシステムである就学前のすべての子どもをもつ家族を担当保健師が継続して支援するシステムを導入した自治体において,システム導入前と導入後の保健師の母子保健活動に対する認識の変化について分析することを目的とした。方法 データ収集期間は,2020年9月から10月であった。データ収集は,インタビューガイドに基づいた半構造化面接によるフォーカスグループインタビューを実施した。本研究は,大阪市立大学大学院看護学研究科倫理審査委員会の承認を得て実施した。結果 研究参加者は,12人であった。担当保健師が継続して支援するシステムを導入する前の保健師の母子保健活動に関する認識として4つのカテゴリーが抽出され,導入後の認識としては8つのカテゴリーを抽出した。担当保健師による継続支援システムを導入する前から,保健師は【ハイリスクケースを中心とした継続的な対応】を行い,【対象者のリスクに注力】しながら活動していたものの,【ハイリスクケース以外の対象者への点での関りによるその場しのぎの対応】にならざるを得ない状況で,【積極的に対象者に介入することに躊躇】していた。しかしながら,担当保健師による継続支援システムの導入後,担当保健師としてハイリスクケース以外の家族に対しても【継続支援による信頼関係から生じる対象者の変化に応じた対応】ができるようになり,【対象者の些細な変化への気づき】もできるようになっていた。そのため,【担当保健師として積極的に対応】し,【早期からの継続的な予防的介入】が可能になったと認識していた。保健師は,【対象者への直接的な支援の増加による忙しさ】を感じつつも,【児の成長や育児スキルの上達への喜びを母親と共感】し,【保健師として喜びとやりがい】を感じながら,【保健師(専門職)としてのスキルアップ】の必要性も強く認識していることが明らかとなった。結論 本研究結果より,担当保健師による継続支援システムを導入することにより,保健師がハイリスクケース以外の家族に対しても積極的に関わることができ,早期からの予防的介入ができる可能性が高いことが示された。また,保健師としての喜びややりがいを高めることができることも示され,保健師としてのスキルアップの必要性も強く認識していることが明らかとなった。(著者抄録)

<p><b>Objectives</b>　This study aimed to clarify the relationship between economic disparity and eating habits of young children to explore the support that should be provided to families with young children.</p><p><b>Methods</b>　We conducted an anonymous self-administered questionnaire survey of parents who brought their children to the 3-year-old health checkup in four wards in City A. Questionnaire items examined the children's eating habits and the childrearing environment, such as the parents' socioeconomic status. Of 1,150 parents asked to participate in the survey, 616 responded (response rate: 53.6%). Invalid questionnaires (e.g., with missing answers) were eliminated. The remaining 498 parents (effective response rate: 80.8%) were classified into two groups to examine the association of household financial status with their 3-year-old eating habits: the relative poverty group and the non-relative poverty group. The groups were divided according to the poverty line of the Comprehensive Survey of Living Conditions. Statistical analysis was performed using Fisher's exact test and the Mann-Whitney <i>U</i> test. Eating habits that were identified to be associated with relative poverty were further tested using logistic regression analysis.</p><p><b>Results</b>　Analysis of the children's eating habits in the two groups revealed that, compared with the non-relative poverty group, there was a higher proportion of 3-year-olds who ate vegetables less than 6 days a week (<i>P</i>=0.003) and who consumed snacks 6 days or more per week (<i>P</i>=0.034) in the relative poverty group. The associations of relative poverty with vegetable intake less than 6 days a week and snack consumption 6 days or more a week remained significant after adjusting for parents' age, highest educational attainment, and subjective views on the everyday economic situation. Regarding the childrearing environment, the relative poverty group had a higher proportion of parents who were younger than 30 years of age (<i>P</i><0.001) and in single-parent households (<i>P</i>=0.007). The relative poverty group had a higher proportion of parents whose highest education level was high school (<i>P</i><0.001). Furthermore, the subjective view of the everyday economic situation was more negative in the relative poverty group (<i>P</i><0.001).</p><p><b>Conclusion</b>　Economic disparity was related to the eating habits of 3-year-old children. The study results indicate that identifying families with financial difficulties during pregnancy and childbirth is necessary to support them early on so that young children can acquire healthy eating habits.</p>

目的 本研究は、経済格差と幼児の食生活習慣との関連を明らかにし、今後の幼児をもつ家庭への支援のあり方を検討することを目的とした。方法 A市内4区の3歳児健康診査に来所した保護者を対象に、幼児の食生活習慣の状況、保護者の社会経済的地位を含む養育環境を問う無記名自記式質問紙調査を実施した。1,150人の保護者に調査を依頼し、616人から回答を得た(回収率53.6%)。このうち必要な項目等が欠損していた者を除外し、498人(有効回答率80.8%)を分析対象とした。本研究では、国民生活基礎調査において相対的貧困率の算出に用いられる貧困線を参考に、相対的貧困群と非相対的貧困群に分類し、幼児の食生活習慣との関連を分析した。統計学的分析方法は、Fisherの正確確率検定、Mann-WhitneyのU検定を実施後、相対的貧困と関連が認められた食生活習慣について、ロジスティック回帰分析を実施した。結果 相対的貧困群と非相対的貧困群における幼児の食生活習慣を分析した結果、相対的貧困群の幼児は、非相対的貧困群の幼児と比較して、週6日未満の野菜の摂取の割合が高く(P=0.003)、かつ週6日以上のスナック菓子の摂取の割合も高かった(P=0.034)。週6日未満の野菜の摂取と週6日以上のスナック菓子の摂取については、保護者の年齢や学歴、主観的経済観を調整しても相対的貧困と有意な関連が認められた。相対的貧困群の養育環境の特徴では、非相対的貧困群と比較して30歳未満の保護者の割合が高く(P<0.001)、ひとり親世帯の割合が高かった(P=0.007)。加えて、保護者の最終学歴が高校までの割合が有意に高かった(P<0.001)。さらに、相対的貧困群の保護者は、非相対的貧困群の保護者に比べて主観的経済観でもより生活が苦しいと感じていた(P<0.001)。結論 本研究結果から、経済格差が3歳児の食生活習慣と関連していることが明らかになった。今後、妊娠・出産期から経済的困難を抱える家庭を把握し、子どもが健康的な食生活習慣を身につけられるよう早期から支援していく必要性が示された。(著者抄録)

【目的】本研究は、国内における高年初産婦への支援に関する研究を「身体・心理面」「母乳育児」の2つの観点から概観し、地域母子保健における高年初産婦の育児支援への示唆を得ることを目的とした。【方法】医学中央雑誌刊行会から「高年初産」and「育児」をキーワードに、2000年以降2019年11月まで検索を行い、32件を採用した。【結果】身体面・心理面に関する報告では、高年初産婦は産後の睡眠時間が短く、育児不安やストレスが経時的に増強しやすいことが報告されていた。母乳については、34歳以下の初産婦に比べ、母乳栄養の確立は困難である場合が多いが、母乳育児では差がないことが報告されていた。【結論】今回概観した文献は、産後早期～2ヵ月頃、および病院でデータを収集した研究がほとんどであった。産後の不調へ早期支援が望まれる。(著者抄録)

【目的】本研究は、国内で報告された自治体での公衆衛生対策として示されている災害時の備えに関する研究を概観し、有効な災害時の備えと、その課題を提示することを目的とした。【方法】文献として医学中央雑誌刊行会発行の医中誌Webで"災害or風水害"and"準備or備え"and"自治体or保健所"をキーワードとして検索し、48件を採用した。【結果】公衆衛生対策の災害時に有効な備えとして、マニュアルの整備、水や食料の備蓄、関係機関との連携や協定の締結、研修や訓練、災害時要配慮者のリストアップなどが報告されていた。相互応援協定締結率の低さ、市町村における準備の遅れが課題となっていた。【結論】災害時の公衆衛生対策として自治体組織の備え、災害対応力の向上、災害時要配慮者支援の準備が有効とされ、市町村での準備の遅れなどが課題となっていた。今後、自治体における受援準備や要配慮者対策などの推進が期待される。(著者抄録)

<p><b>Objectives:</b> The objectives of this study were to explore the experience of relatives of patients with Huntington’s disease and to develop a foundation for discussing nursing professional’s roles in the care of patients with this disease.</p><p><b>Methods:</b> Data collected through semi-structured interviews were analyzed by a qualitative descriptive approach.</p><p><b>Results:</b> Participants (mean age = 53.6; SD = ±28.5) were 10 first-degree blood relatives of patients with Huntington’s disease. Results of our analysis generated 7 categories and 39 subcategories describing participants’ experience about their families and the disease. Many appeared to have a “sense of alienation toward affected individuals” and only a “vague awareness of their kin’s disease,” given the existence of family members with similar symptoms. They also often reported they felt “constrained by their hereditary status,” and were “hesitant to tell others about their family history.” In addition, participants who spent time with patients often “imagined themselves contracting the disease,” and reported that “changes in their relative’s personality interferred with their lives.” Also participants felt that “patients and their families stood alone from the communities”.</p><p><b>Conclusion:</b> These first-degree relatives of patients with Huntington’s disease appeared to have complicated feelings toward their own hereditary disease status as well as that of other family members and the affected relative.</p>

目的 妊娠間隔12ヵ月未満で出産した母親の育児負担感の実態とその関連要因を明らかにすることを目的とした。方法 A市3ヶ所の保健福祉センターにおける3ヵ月児健康診査に来所した母親を対象に、妊娠間隔やEPDS、育児感情尺度等を問う無記名自記式質問紙調査を実施した。回答が得られた757人のうち、欠損項目を有する72人を除く685人(有効回答率90.4%)を分析対象とした。妊娠間隔は、妊娠間隔12ヵ月未満群、妊娠間隔12ヵ月以上24ヵ月未満群、妊娠間隔24ヵ月以上群、およびきょうだい児なし群に分類し、育児負担感との関連を分析した。統計学的分析方法は、χ2検定、一元配置分散分析またはKruskal-Walisの検定、および重回帰分析を実施した。結果 妊娠間隔12ヵ月未満群が35人(5.1%)、妊娠間隔12ヵ月以上24ヵ月未満群が114人(16.6%)、妊娠間隔24ヵ月以上群が194人(28.3%)、きょうだい児なし群が342人(49.9%)であった。育児感情尺度の3つの下位項目に関連する要因を重回帰分析した結果、育児への束縛による負担感では、妊娠間隔(P=.032)、家族構成(P=.014)、睡眠時間(P=.010)および夜間起床回数(P=.001)と有意な関連が認められた。子どもの態度や行為への負担感では、妊娠間隔(P<.001)、母親の年齢(P=.003)、睡眠時間(P=.009)および夜間起床回数(P=.002)と有意な関連が認められた。育ちへの不安感では、妊娠間隔(P<.001)、母親の年齢(P=.016)および在胎週数(P<.001)と有意な関連が認められた。加えて、妊娠間隔12ヵ月未満群は他の妊娠間隔群に比べ、ひとり親世帯(P=.005)、未婚(P=.007)、最終学歴が中学卒業(P=.0027)、24歳以下の若年(P<.001)が有意に多かった。結論 妊娠間隔は、育児感情尺度の育児への束縛による負担感、子どもの態度や行為への負担感および育ちへの不安感と有意に関連していた。さらに、育児への束縛による負担感、ならびに子どもの態度や行為への負担感は、妊娠間隔が短くなるほど母親の負担感が増大する可能性があることが示された。妊娠間隔12ヵ月未満の母親は、ひとり親世帯、未婚者、低学歴、若年の割合が有意に高いことが明らかとなり、支援の必要性が示された。(著者抄録)

OBJECTIVE: The objective of this study was to analyze how parental education modifies the genetic and environmental variances of BMI from infancy to old age in three geographic-cultural regions. METHODS: A pooled sample of 29 cohorts including 143,499 twin individuals with information on parental education and BMI from age 1 to 79 years (299,201 BMI measures) was analyzed by genetic twin modeling. RESULTS: Until 4 years of age, parental education was not consistently associated with BMI. Thereafter, higher parental education level was associated with lower BMI in males and females. Total and additive genetic variances of BMI were smaller in the offspring of highly educated parents than in those whose parents had low education levels. Especially in North American and Australian children, environmental factors shared by co-twins also contributed to the higher BMI variation in the low education level category. In Europe and East Asia, the associations of parental education with mean BMI and BMI variance were weaker than in North America and Australia. CONCLUSIONS: Lower parental education level is associated with higher mean BMI and larger genetic variance of BMI after early childhood, especially in the obesogenic macro-environment. The interplay among genetic predisposition, childhood social environment, and macro-social context is important for socioeconomic differences in BMI.

【目的】本研究は、国内におけるこれまでの経済格差と子どもの健康に関する研究を概観し、公衆衛生活動における基礎資料とすることを目的とした。【方法】文献には、医学中央雑誌刊行会から「貧困or社会階層or経済状況or経済的理由」and「子ども」をキーワードに、2008年以降2018年7月まで検索を行い、30件を採用した。【結果】経済格差による、新生児の医学的特徴、子どものQOL、食生活、運動時間、成人後の肥満、母親の喫煙、保護者の養育態度、感染症の問題が報告されていた。そのほか、妊婦健診未受診妊婦、児童虐待等との関連が報告されていた。【結論】経済格差による子どもの健康への影響が明らかにされつつある。経済格差が子どもの健康に影響を及ぼすという視点を持って、子どもや妊婦を包括的に支援していく必要がある。今後、経済格差による子どもの健康への影響を低減させるための支援体制や施策の検討が望まれる。(著者抄録)

<p><b>Objectives</b>　The aims of this study were to investigate perceived childcare burdens among mothers with an inter-pregnancy interval of less than 12 months, and to identify the factors associated with those perceptions.</p><p><b>Methods</b>　Anonymous, self-administered questionnaires on inter-pregnancy intervals, the Edinburgh postnatal depression scale (EPDS), and the childcare emotion scale were distributed to mothers who visited one of three health and welfare centers in City A for three-month well-baby checkups. Six hundred eighty-five subjects were classified into four groups according to their inter-pregnancy interval (<12 months apart; ≥12 months but <24 months apart; ≥24 months apart; and a group with no siblings), and they was used to analyze associations between the timing of pregnancy and the mothers’ perceptions of their childcare burdens. Chi-squared tests, a one-way analysis of variance, the Kruskal-Wallis test, and linear regression analyses were used for analysis.</p><p><b>Results</b>　The inter-pregnancy interval was <12 months in 35 of the 685 respondents, ≥12 months but <24 months in 114 respondents, and ≥24 months in 194 respondents. There were 342 respondents in the no-siblings group. Linear regression analyses disclosed some factors associated with the subordinate items of the childcare emotion scale. “Feeling restricted by childcare duties” is significantly correlated to inter-pregnancy interval (<i>P</i>=.032), family composition (<i>P</i>=.014), time of sleeping (<i>P</i>=.010), and night wake-up time (<i>P</i>=.001). “Feeling of burden because of the child's attitude and behavior” was significantly related to inter-pregnancy interval (<i>P</i><.001), mother's age (<i>P</i>=.003), time of sleeping (<i>P</i>=.009), and night wake-up time (<i>P</i>=.002). “Anxiety about the child's growth” was significantly related to inter-pregnancy interval (<i>P</i><.001), mother's age (<i>P</i>=.016), and gestational age (<i>P</i><.001). In addition, the group with gestation intervals of less than 12 months had significantly higher numbers of women in single-parent households (<i>P</i>=.005), who were unmarried (<i>P</i>=.007), had only a junior high school education (<i>P</i>=.0027), and were 24 years old or younger (<i>P</i><.001).</p><p><b>Conclusion</b>　The pregnancy interval is significantly related to the “Feeling restricted by childcare duties,” “Feeling of burden because of the child's attitude and behavior,” and “Anxiety about the child's growth,” even after adjusting the influence of other factors. In addition, the results of this study demonstrated that mothers in the <12 months group were more likely to be in a single-parent household, unmarried, with low educational attainment, and were 24 years old or younger. The results suggest that greater support is needed for these mothers.</p>

母乳中物質が母および児の健康とどのような関係があるか、母親のストレス緩和のひとつとして乳房マッサージ前後の変化を検証した。B助産院で桶谷式乳房管理法を受けることを希望した授乳中の母親の中から、研究に同意が得られた41名(平均年齢33.6歳)を対象とした。母乳中のCoQ10濃度と関連があった対象者の属性項目は、就業状況、出生順位であり、児との関係では在胎週数がCoQ10濃度と出生体重がコルチゾール濃度と関連があった。乳房マッサージ施術は自覚的なストレス・不安・イライラ・緊張・倦怠感の低下に効果があり、かつ心地よさの上昇、睡眠不足感覚の解消につながっていた。また、乳房マッサージ施術は母乳中のCoQ10濃度の上昇やコルチゾール濃度の低下と関連があることが示唆された。妊娠前から就業していない対象者では、還元型Co10濃度および総CoQ10濃度が高く、抗酸化力が他の対象者よりも高い可能性が示唆された。

Background: There is evidence that birth weight is positively associated with education, but it remains unclear whether this association is explained by familial environmental factors, genetic factors or the intrauterine environment. We analysed the association between birth weight and educational years within twin pairs, which controls for genetic factors and the environment shared between co-twins. Methods: The data were derived from nine twin cohorts in eight countries including 6116 complete twin pairs. The association between birth weight and educational attainment was analysed both between individuals and within pairs using linear regression analyses. Results: In between-individual analyses, birth weight was not associated with educational years. Within-pairs analyses revealed positive but modest associations for some sex, zygosity and birth year groups. The greatest association was found in dizygotic (DZ) men (0.65 educational years/kg birth weight, p=0.006)
smaller effects of 0.3 educational years/kg birth weight were found within monozygotic (MZ) twins of both sexes and opposite-sex DZ twins. The magnitude of the associations differed by birth year in MZ women and opposite-sex DZ twins, showing a positive association in the 1915-1959 birth cohort but no association in the 1960-1984 birth cohort. Conclusion: Although associations are weak and somewhat inconsistent, our results suggest that intrauterine environment may play a role when explaining the association between birth weight and educational attainment.

Background: The genetic architecture of birth size may differ geographically and over time. We examined differences in the genetic and environmental contributions to birthweight, length and ponderal index (PI) across geographical-cultural regions (Europe, North America and Australia, and East Asia) and across birth cohorts, and how gestational age modifies these effects.

Methods: Data from 26 twin cohorts in 16 countries including 57 613 monozygotic and dizygotic twin pairs were pooled. Genetic and environmental variations of birth size were estimated using genetic structural equation modelling.

Results: The variance of birthweight and length was predominantly explained by shared environmental factors, whereas the variance of PI was explained both by shared and unique environmental factors. Genetic variance contributing to birth size was small. Adjusting for gestational age decreased the proportions of shared environmental variance and increased the propositions of unique environmental variance. Genetic variance was similar in the geographical-cultural regions, but shared environmental variance was smaller in East Asia than in Europe and North America and Australia. The total variance and shared environmental variance of birth length and PI were greater from the birth cohort 1990-99 onwards compared with the birth cohorts from 1970-79 to 1980-89.

Conclusions: The contribution of genetic factors to birth size is smaller than that of shared environmental factors, which is partly explained by gestational age. Shared environmental variances of birth length and PI were greater in the latest birth cohorts and differed also across geographical-cultural regions. Shared environmental factors are important when explaining differences in the variation of birth size globally and over time.

Abstract
The mechanisms behind handedness formation in humans are still poorly understood. Very low birthweight is associated with higher odds of left-handedness, but whether this is due to low birthweight itself or premature birth is unknown. Handedness has also been linked to development, but the role of birthweight behind this association is unclear. Knowing that birthweight is lower in multiple births, triplets being about 1.5 kg lighter in comparison with singletons, and that multiples have a higher prevalence of left-handedness than singletons, we studied the association between birthweight and handedness in two large samples consisting exclusively of triplets from Japan (n = 1,305) and the Netherlands (n = 947). In both samples, left-handers had significantly lower birthweight (Japanese mean = 1,599 g [95% confidence interval (CI): 1,526-1,672 g]; Dutch mean = 1,794 g [95% CI: 1,709-1,879 g]) compared with right-handers (Japanese mean = 1,727 g [95% CI: 1,699-1,755 g]; Dutch mean = 1,903 g [95% CI: 1,867-1,938 g]). Within-family and between-family analyses both suggested that left-handedness is associated with lower birthweight, also when fully controlling for gestational age. Left-handers also had significantly delayed motor development and smaller infant head circumference compared with right-handers, but these associations diluted and became nonsignificant when controlling for birthweight. Our study in triplets provides evidence for the link between low birthweight and left-handedness. Our results also suggest that developmental differences between left- and right-handers are due to a shared etiology associated with low birthweight.

Background: There is evidence that birth size is positively associated with height in later life, but it remains unclear whether this is explained by genetic factors or the intrauterine environment. Aim: To analyze the associations of birth weight, length and ponderal index with height from infancy through adulthood within mono- and dizygotic twin pairs, which provides insights into the role of genetic and environmental individual-specific factors. Methods: This study is based on the data from 28 twin cohorts in 17 countries. The pooled data included 41,852 complete twin pairs (55% monozygotic and 45% same-sex dizygotic) with information on birth weight and a total of 112,409 paired height measurements at ages ranging from 1 to 69 years. Birth length was available for 19,881 complete twin pairs, with a total of 72,692 paired height measurements. The association between birth size and later height was analyzed at both the individual and within-pair level by linear regression analyses. Results: Within twin pairs, regression coefficients showed that a 1-kg increase in birth weight and a 1-cm increase in birth length were associated with 1.14–4.25 cm and 0.18–0.90 cm taller height, respectively. The magnitude of the associations was generally greater within dizygotic than within monozygotic twin pairs, and this difference between zygosities was more pronounced for birth length. Conclusion: Both genetic and individual-specific environmental factors play a role in the association between birth size and later height from infancy to adulthood, with a larger role for genetics in the association with birth length than with birth weight.

It is well established that boys are born heavier and longer than girls, but it remains unclear whether birth size in twins is affected by the sex of their co-twin. We conducted an individual-based pooled analysis of 21 twin cohorts in 15 countries derived from the COllaborative project of Development of Anthropometrical measures in Twins (CODATwins), including 67,850 dizygotic twin individuals. Linear regression analyses showed that boys having a co-twin sister were, on average, 31 g (95% CI 18 to 45) heavier and 0.16 cm (95% CI 0.045 to 0.274) longer than those with a co-twin brother. In girls, birth size was not associated (5 g birth weight
95% CI -8 to -18 and -0.089 cm birth length
95% CI -0.202 to 0.025) with the sex of the co-twin. Gestational age was slightly shorter in boy-boy pairs than in boy-girl and girl-girl pairs. When birth size was standardized by gestational age, the magnitude of the associations was attenuated in boys, particularly for birth weight. In conclusion, boys with a co-twin sister are heavier and longer at birth than those with a co-twin brother. However, these differences are modest and partly explained by a longer gestation in the presence of a co-twin sister.

本研究は、これまでの結核におけるDOTS対策に関する文献学的考察を行い、今後のDOTS対策推進に寄与することを目的とした。引用文献の検索には、医学中央雑誌web版を用いて原著論文の検索を行った。検索された54件の文献を、日本版DOTS戦略に基づき4つの視点で整理し、法的改正等の動向も交えた。結果、院内DOTSは、患者教育、服薬支援、保健所との連携が重要であり、医療連携は、結核専門機関から一般医療機関へ、医療と服薬支援が的確に繋がるしくみの必要性が指摘されていた。地域DOTSは、患者をアセスメントし、中断リスクや患者のニーズから患者に合うDOTS実施が重要であり、かつ、これらの活動には、患者と信頼関係を築きながら服薬支援する重要性が報告されていた。DOTS推進のためのツールやしくみは構築されつつあるが、一方で支援者に必要とされてきた姿勢を蔑ろにする状況も報告されていた。今後DOTS実施者が、患者中心の服薬支援を行う重要性を的確に継承するための研究が求められる。

Background: There is evidence that birthweight is positively associated with body mass index (BMI) in later life, but it remains unclear whether this is explained by genetic factors or the intrauterine environment. We analysed the association between birthweight and BMI from infancy to adulthood within twin pairs, which provides insights into the role of genetic and environmental individual-specific factors.
Methods: This study is based on the data from 27 twin cohorts in 17 countries. The pooled data included 78 642 twin individuals (20 635 monozygotic and 18 686 same-sex dizygotic twin pairs) with information on birthweight and a total of 214 930 BMI measurements at ages ranging from 1 to 49 years. The association between birthweight and BMI was analysed at both the individual and within-pair levels using linear regression analyses.
Results: At the individual level, a 1-kg increase in birthweight was linearly associated with up to 0.9 kg/m(2) higher BMI (P &lt; 0.001). Within twin pairs, regression coefficients were generally greater (up to 1.2 kg/m(2) per kg birthweight, P &lt; 0.001) than those from the individual-level analyses. Intra-pair associations between birthweight and later BMI were similar in both zygosity groups and sexes and were lower in adulthood.
Conclusions: These findings indicate that environmental factors unique to each individual have an important role in the positive association between birthweight and later BMI, at least until young adulthood.

Whether monozygotic (MZ) and dizygotic (DZ) twins differ from each other in a variety of phenotypes is important for genetic twin modeling and for inferences made from twin studies in general. We analyzed whether there were differences in individual, maternal and paternal education between MZ and DZ twins in a large pooled dataset. Information was gathered on individual education for 218,362 adult twins from 27 twin cohorts (53% females; 39% MZ twins), and on maternal and paternal education for 147,315 and 143,056 twins respectively, from 28 twin cohorts (52% females; 38% MZ twins). Together, we had information on individual or parental education from 42 twin cohorts representing 19 countries. The original education classifications were transformed to education years and analyzed using linear regression models. Overall, MZ males had 0.26 (95% CI [0.21, 0.31]) years and MZ females 0.17 (95% CI [0.12, 0.21]) years longer education than DZ twins. The zygosity difference became smaller in more recent birth cohorts for both males and females. Parental education was somewhat longer for fathers of DZ twins in cohorts born in 1990-1999 (0.16 years, 95% CI [0.08, 0.25]) and 2000 or later (0.11 years, 95% CI [0.00, 0.22]), compared with fathers of MZ twins. The results show that the years of both individual and parental education are largely similar in MZ and DZ twins. We suggest that the socio-economic differences between MZ and DZ twins are so small that inferences based upon genetic modeling of twin data are not affected.

Background: Genes and the environment contribute to variation in adult body mass index [BMI (in kg/m(2))], but factors modifying these variance components are poorly understood.
Objective: We analyzed genetic and environmental variation in BMI between men and women from young adulthood to old age from the 1940s to the 2000s and between cultural-geographic regions representing high (North America and Australia), moderate (Europe), and low (East Asia) prevalence of obesity.
Design: We used genetic structural equation modeling to analyze BMI in twins &gt;= 20 y of age from 40 cohorts representing 20 countries (140,379 complete twin pairs).
Results: The heritability of BMI decreased from 0.77 (95% CI: 0.77, 0.78) and 0.75 (95% CI: 0.74, 0.75) in men and women 2029 y of age to 0.57 (95% CI: 0.54, 0.60) and 0.59 (95% CI: 0.53, 0.65) in men 70-79 y of age and women 80 y of age, respectively. The relative influence of unique environmental factors correspondingly increased. Differences in the sets of genes affecting BMI in men and women increased from 20-29 to 60-69 y of age. Mean BMI and variances in BMI increased from the 1940s to the 2000s and were greatest in North America and Australia, followed by Europe and East Asia. However, heritability estimates were largely similar over measurement years and between regions. There was no evidence of environmental factors shared by co-twins affecting BMI.
Conclusions: The heritability of BMI decreased and differences in the sets of genes affecting BMI in men and women increased from young adulthood to old age. The heritability of BMI was largely similar between cultural-geographic regions and measurement years, despite large differences in mean BMI and variances in BMI. Our results show a strong influence of genetic factors on BMI, especially in early adulthood, regardless of the obesity level in the population.

2011年12月～2012年7月に人口約48万人のA市で、3および4ヵ月乳幼児健診の際に質問票調査を実施し、stepfamiliesと母親の精神保健の特徴を非複合家族と比較した。2258名(回答率75.1%)から回答を得、シングルマザーを除いた2246名、うちstepfamilies47名(2.1%)とnon-stepfamilies 2199名(97.9%)の回答を用いた。stepfamiliesではnon-stepfamiliesに比し、親の学歴が高校以下、子どもが3名以上の割合が有意に高かった。stepfamiliesの母親は、経済資源欠如の感情、出産教育クラスへの不参加、妊娠中の喫煙および計画外妊娠の割合が有意に高く、また、鬱および親の役割への自信の欠如の割合も有意に高かった。母親の鬱は、母親の年齢、主観的健康感、ストレスレベル、授乳への自信、親の役割への自信および子どもの数などの因子と関連した。以上、stepfamiliesは社会的不利および地域社会に基づくヘルスケアの問題と関連した多くの特徴を示した。

【目的】乳児と3歳児をもつ母親の母性意識における肯定的感情と否定的感情について比較分析し、その関連要因を明らかにすることを目的とした。【方法】A市の4ヵ月児健康診査および3歳児健康診査の対象児をもつ母親に、自記式質問紙を郵送し、回答を得た。【結果】母性意識の肯定的感情得点は、乳児の母親と3歳児の母親では差は認められなかった。しかし、否定的感情得点は、乳児の母親に比べ、3歳児の母親の方が有意に高かった。重回帰分析の結果、ストレス得点は乳児の母親、3歳児の母親ともに肯定的感情得点および否定的感情得点の両方と関連を認めた。さらに、妊娠中から育児のイメージができた母親は、乳児および3歳児をもつ母親ともに、母性意識の肯定的感情得点が高かった。【結論】母性意識の健全な育成には、母親のストレス軽減のための長期的なサポートが重要であり、かつ妊娠中から育児のイメージができるような支援が必要であることが示された。(著者抄録)

However, mothers with toddlers aged three years scored significantly higher for negative emotions than did mothers with infants. Stress scores were related to both positive and negative emotion scores for mothers with infants and for mothers toddlers aged three years. Additionally, mothers who had established a mental image of childcare during their pregnancy exhibited higher scores for positive emotions regardless of their child's age.Conclusions: These findings suggest that for the healthy development of perceptions of motherhood, it is important to provide long-term support to mothers by helping them reduce stress levels and establish a mental image of childcare during pregnancy.

Human height variation is determined by genetic and environmental factors, but it remains unclear whether their influences differ across birth-year cohorts. We conducted an individual-based pooled analysis of 40 twin cohorts including 143,390 complete twin pairs born 18861994. Although genetic variance showed a generally increasing trend across the birth-year cohorts, heritability estimates (0.69-0.84 in men and 0.53-0.78 in women) did not present any clear pattern of secular changes. Comparing geographic-cultural regions (Europe, North America and Australia, and East Asia), total height variance was greatest in North America and Australia and lowest in East Asia, but no clear pattern in the heritability estimates across the birth-year cohorts emerged. Our findings do not support the hypothesis that heritability of height is lower in populations with low living standards than in affluent populations, nor that heritability of height will increase within a population as living standards improve.

A看護師養成所3年次学生84名を対象に、リフレクティブサイクルを用いた自習を受けた学生の自己教育力と特性的自己効力感の関連について分析し、実態を把握するための質問紙調査を実施した。80名から回答が得られた。分析の結果、特性的自己効力感が高い学生は、自己教育力も高い傾向があることが明らかになった。自己教育力の「自信・プライド・安定性」は、特性的自己効力感との相関が最も高かった。さらに、「自信・プライド・安定性」は自己教育力の他の三つを支える土台となり、その有無が主体的であることを決定づけると先行研究で述べられており、「自信・プライド・安定性」の側面を強化するような働きかけが重要であることが示唆された。階層的クラスター分析を行ったところ、自己教育力の高い群、低い群ともに「振り返る」という単語が出現していた。高い群は振り返ることで、目標を定めて次のステップに活用できているが、低い群は現状理解にとどまっていると考えられた。

研究目的は、ポートフォリオを用いた臨地実習における成長報告書の自由記述を内容分析し、学生の内省傾向を明らかにすることである。看護学生72名の自由記述データを計量テキスト分析の手法を用いて、共起ネットワークの作成と対応分析を行った。共起ネットワークの構造分析の結果、「成長ベスト3」「学んだこと」「獲得した力」それぞれの項目で3～5群のカテゴリーが抽出された。振り返りの大切さに気づいた学生が、より高い看護実践能力の向上を目指して努力する内省が読み取れた。対応分析による抽出語の全体的布置において、「獲得した力」が第1成分の寄与率の高いプラス側第1象限に位置しており、「成長ベスト3」と「学んだこと」を区別する基準軸になっていると考えられた。第1成分のプラス側には、「活かす」「力」「今後」の語が並んでおり、学生は応用力を獲得していると考えられる。しかし、それを「成長」としては捉えておらず、「学んだ知識や技術」を「自己の成長」と捉える傾向がある事が示唆された。(著者抄録)

Height variation is known to be determined by both genetic and environmental factors, but a systematic description of how their influences differ by sex, age and global regions is lacking. We conducted an individual-based pooled analysis of 45 twin cohorts from 20 countries, including 180,520 paired measurements at ages 1-19 years. The proportion of height variation explained by shared environmental factors was greatest in early childhood, but these effects remained present until early adulthood. Accordingly, the relative genetic contribution increased with age and was greatest in adolescence (up to 0.83 in boys and 0.76 in girls). Comparing geographic-cultural regions (Europe, North-America and Australia, and East-Asia), genetic variance was greatest in North-America and Australia and lowest in East-Asia, but the relative proportion of genetic variation was roughly similar across these regions. Our findings provide further insights into height variation during childhood and adolescence in populations representing different ethnicities and exposed to different environments.

We analyzed birth order differences in means and variances of height and body mass index (BMI) in monozygotic (MZ) and dizygotic (DZ) twins from infancy to old age. The data were derived from the international CODATwins database. The total number of height and BMI measures from 0.5 to 79.5 years of age was 397,466. As expected, first-born twins had greater birth weight than second-born twins. With respect to height, first-born twins were slightly taller than second-born twins in childhood. After adjusting the results for birth weight, the birth order differences decreased and were no longer statistically significant. First-born twins had greater BMI than the second-born twins over childhood and adolescence. After adjusting the results for birth weight, birth order was still associated with BMI until 12 years of age. No interaction effect between birth order and zygosity was found. Only limited evidence was found that birth order influenced variances of height or BMI. The results were similar among boys and girls and also in MZ and DZ twins. Overall, the differences in height and BMI between first-and second-born twins were modest even in early childhood, while adjustment for birth weight reduced the birth order differences but did not remove them for BMI.

BACKGROUND: The occurrence of multiple births has been recognized as a risk factor for child maltreatment. However, few population-based studies have examined the relationship between multiple births and child maltreatment. This study aimed to evaluate the degree of risk of child maltreatment among singletons and multiple births in Japan and to identify factors associated with increased risk.
METHODS: Using population-based data, we analyzed the database of records on child maltreatment and medical checkups for infants aged 1.5 years filed at Nishinomiya City Public Health Center between April 2007 and March 2011. To protect personal information, the data were transferred to anonymized electronic files for analysis.
RESULTS: After adjusting by logistic regression for each associated factor and gestation number, multiples themselves were not associated with the risk of child maltreatment. However, compared with singletons, multiples had a significantly higher rate of risk factors for child maltreatment, including low birth weight and neural abnormality. Moreover, compared with mothers of singleton, mothers of twins had a significantly higher rate of poor health, which is a risk factor of child maltreatment.
CONCLUSION: Multiples were not associated with the risk of child maltreatment. However, compared with singletons, multiples and their mothers had a significantly higher rate of risk factors of child maltreatment.

Similarly, a greater mean BMI of up to 0.3 kg/m2 in childhood and adolescence and up to 0.2 kg/m2 in adulthood was observed in DZ twins, although the pattern was less consistent. DZ twins presented up to 1.7% greater height and 1.9% greater BMI than MZ twins; these percentage differences were largest in middle and late childhood and decreased with age in both sexes. The variance of height was similar in MZ and DZ twins at most ages. In contrast, the variance of BMI was significantly higher in DZ than in MZ twins, particularly in childhood. 父親の育児ニーズが高いとされる1歳6ヵ月児をもつ父親の育児行動の実態と、それらの関係要因を明らかにすることを目的とした。無記名自記式質問紙を郵送し、回答を得た329人(有効回答率29.2%)を分析対象者とした。育児行動は、「あやす」、「遊び相手」、「おむつ交換」、「お風呂」、「食事の世話」、「寝かしつけ」とした。6つの育児行動は、通勤時間を含めた労働時間が「12時間未満の者」が「12時間以上の者」に比べて有意に週3回以上行っていた。「食事の世話」は、「育児休業の取得者もしくは取得希望者」、「有給休暇をとりやすい者」の方がそうでない者に比べて週3回以上行っており、「おむつ交換」は「平等的性役割分業意識である者」が「伝統的性役割分業意識」に比べて、「育児講座や相談の利用がある者」が「利用なしの者」に比べて有意に週3回以上行っていた。(著者抄録)

For over 100 years, the genetics of human anthropometric traits has attracted scientific interest. In particular, height and body mass index (BMI, calculated as kg/m(2)) have been under intensive genetic research. However, it is still largely unknown whether and how heritability estimates vary between human populations. Opportunities to address this question have increased recently because of the establishment of many new twin cohorts and the increasing accumulation of data in established twin cohorts. We started a new research project to analyze systematically (1) the variation of heritability estimates of height, BMI and their trajectories over the life course between birth cohorts, ethnicities and countries, and (2) to study the effects of birth-related factors, education and smoking on these anthropometric traits and whether these effects vary between twin cohorts. We identified 67 twin projects, including both monozygotic (MZ) and dizygotic (DZ) twins, using various sources. We asked for individual level data on height and weight including repeated measurements, birth related traits, background variables, education and smoking. By the end of 2014, 48 projects participated. Together, we have 893,458 height and weight measures (52% females) from 434,723 twin individuals, including 201,192 complete twin pairs (40% monozygotic, 40% same-sex dizygotic and 20% opposite-sex dizygotic) representing 22 countries. This project demonstrates that large-scale international twin studies are feasible and can promote the use of existing data for novel research purposes.

本研究では、大阪市のA区に居住する10代母親のニーズを把握し、今後の母子保健施策の基礎資料とすることを目的とした。大阪市A区保健福祉センター管内で、2009年度から2012年度までに妊娠届けを提出した妊娠届時10代の妊娠48名。うち、同意が得られた5名の母親にインタビュー調査を実施した。3ヵ月児健康診査のデータ分析では、10代妊婦の高い喫煙率、10代母親の低い母乳栄養率、生活の困りごとや経済不安が多いことが明らかとなった。また、インタビュー調査により、【育児リテラシーの不足】、【生活・社会的基盤の脆弱性】、【ソーシャルサポートの不足】、【育児をする上での強み】、【母子保健制度・システムの活用不足】という5つのカテゴリーが抽出された。10代の妊娠・出産の背景にある社会状況と育児状況の可視化、保健福祉サービス提供者であり、母親がいつでも相談できるパートナーとしての保健師の周知、10代の母親が活用しやすいサービス提供が求められよう。(著者抄録)

本研究は、これまで報告されてきた母乳育児と愛着形成に関連した研究に関して文献的考察をすることにより、地域母子保健において母乳育児を推進するための基礎資料とすることを目的とした。引用文献の検索には、医学中央雑誌web版を用い、2000年以降2013年までの収載誌からキーワードを「母乳育児」と「愛着形成」として検索し、185件がヒットした。さらに原著論文で絞り込み34件を採用した。結果、出産直後の授乳やタッチケア等の母子接触をおこなうことは、母子相互の愛着形成を促進することが、報告されていた。また、母子同室等や家族のサポートが授乳を確立し継続させるとともに児への愛着形成を促すうえで重要であることが指摘されていた。授乳期にある母親の長期にわたる継続的な母乳育児の支援が必要とされており、今後医療機関等の施設のみでなく、地域母子保健における介入研究の成果が期待される。(著者抄録)

早期からの父親の虐待的子育てに関連する要因を明らかにすることを目的に、4ヵ月児健診の機会を利用し、両親への質問紙調査を実施した。分析対象は、乳児から小学生の子どもをもつ父親1,563人であった。その結果、虐待的子育てを行うことがある父親は439人(全体の28.1%)であった。父親の虐待的子育ては、子どもの人数の多さ、父親の学歴、育児ストレス、親役割充足感の低さ、および母親の虐待的子育てとの関連が認められた。これらの結果から、特に第2子以降の出産に伴う年長児への対応について、父親への教育や相談等の支援体制を整備する必要性が示唆された。

父親の育児ニーズが高いとされる1歳6ヵ月児をもつ父親の育児行動の実態と、それらの関係要因を明らかにすることを目的とした。無記名自記式質問紙を郵送し、回答を得た329人(有効回答率29.2%)を分析対象者とした。育児行動は、「あやす」、「遊び相手」、「おむつ交換」、「お風呂」、「食事の世話」、「寝かしつけ」とした。6つの育児行動は、通勤時間を含めた労働時間が「12時間未満の者」が「12時間以上の者」に比べて有意に週3回以上行っていた。「食事の世話」は、「育児休業の取得者もしくは取得希望者」、「有給休暇をとりやすい者」の方がそうでない者に比べて週3回以上行っており、「おむつ交換」は「平等的性役割分業意識である者」が「伝統的性役割分業意識」に比べて、「育児講座や相談の利用がある者」が「利用なしの者」に比べて有意に週3回以上行っていた。

ObjectiveThis study aimed to elucidate the actual activities conducted by public health nurses during their dispatch and their health status during and after dispatch to the three prefectures most severely affected by the Great East Japan Earthquake.
SampleA survey request was sent to a total of 2,237 facilities. Of these, 778 facilities returned questionnaires from dispatched public health nurses.
ResultsThe participants of this study were 1,570 dispatched health nurses who participated in activities mostly at evacuation centers, followed by evacuees' homes. After dispatch, an earlier postdisaster phase at the start of dispatch was independently associated with poor subjective well-being, low mood, worsened sleep state, and intense fatigue. Work hours per day were associated with poor subjective well-being and intense fatigue after dispatch.
ConclusionResults suggest that the factor that most strongly affected the postdispatch health of the nurses was the phase that they began their dispatch.

目的:母親の育児に関する相談事について3ヵ月児健康診査のデータから実態を明らかにし、地域母子保健における効果的な支援のあり方を検討した。方法:大阪市B区の3ヵ月児健康診査を受診した児の健診データのうち、個人情報を除外したものを用いた。結果:分析対象者は2,552人であった。相談事を記載していた母親は1,213人(47.6%)で、相談内容は、「皮膚の手当て」が最も多く、「授乳」、「便」等が多かった。相談あり群は、第1子、集合住宅居住者、生活の困りごとのある者、気分や体調が悪い者、自分の時間がなく苦痛と感じている者に有意に多かった。一方、なんとなく不安と回答しながら、相談事を記載していない母親が36.1%認められた。結論:相談内容は現時点の問題が多く、相談事の有無には母親の背景要因が影響していた。不安があるが相談事を記載していない母親もおり、相談事記載の有無や項目数のみでは、育児不安や育児負担感を測れないことが示された。(著者抄録)

Length and weight in infancy are associated with neurodevelopment, but less is known about growth in other anthropometric measures. In this study we analyzed how the development in length, weight, head circumference, and chest circumference over infancy is associated with motor development in early childhood, using a twin study design. Information on physical development over infancy and the age at achievement of eight developmental milestones over early childhood was collected for 370 Japanese twin pairs. Linear mixed models were used to analyze how physical development is associated with motor development between individual twins, as well as within twin pairs, adjusting the results for shared maternal and postnatal environmental factors. Delayed motor development was associated with smaller body size over infancy, and we also found some suggestive evidence that it was associated with catch-up growth as well. When studying the associations within twin pairs discordant for motor development, similar associations were found. However, chest circumference showed the most robust association within discordant twin pairs. Smaller body size and rapid catch-up growth are associated with delayed motor development. When studying these associations within twin pairs and thus adjusting the results for gestational age as well as many other maternal and postnatal environmental factors, chest circumference showed the most robust association. Chest circumference, rarely used in developed countries, can offer additional information on prenatal conditions relevant for further motor development not achieved by more traditional anthropometric measures.

行政3週間実習(行政実習)、行政1週間と産業2週間の計3週間実習(産業実習)、行政1週間と学校2週間の計3週間実習(学校実習)という実習形態で学生の到達度に違いがあるか否かを検討した。2012-2013年度に大阪市立大学医学部看護学科の4年次生として在籍し、地域看護学実習を履修した計124名に調査票を配布、104名(回収率:83.9%)から回答を得た。調査は「保健師に求められる実践能力と卒業時の到達目標と到達度」(厚生労働省、2010)の項目を用いた。一元配置分散分析では、個人・家族を対象とした「訪問・相談による支援を行う」において、行政実習を経験した学生が学校実習や産業実習を経験した学生と比較して有意に到達度が高く、「健康教育による支援を行う」において、行政実習を経験した学生が産業実習を経験した学生と比較して有意に到達度が高かった。考察:実習場の特性により経験しにくい項目については、実習運営のさらなる工夫が必要であろう。(著者抄録)

本研究の目的は、就学前の双子をもつ母親の母性意識を、単胎児をもつ母親の母性意識との比較から分析し、さらに双子をもつ母親の母性意識に関連する要因を明らかにすることである。対象は、関西地区のA市とB市内の2区に在住する就学前の双子をもつ母親278人と、比較対照群として、同じ関西地区に在住する双子の年齢構成と性別をマッチさせた単胎児の母親290人である。調査の結果、乳幼児の双子をもつ母親は、単胎児をもつ母親と比較して母性意識における肯定的感情が低く、かつ否定的感情が高いことが明らかとなった。また、母親が感じているストレス、出産後の気分の落ち込み、および抑うつ状態は、双子をもつ母親の母性意識の肯定的感情と否定的感情の両面に関連していることが明らかになった。これらのことから、乳幼児の双子をもつ母親の母性意識の肯定的感情を高め、否定的感情を低減させるには、配偶者が積極的に双子の育児に関わることが重要であり、さらに、母親の精神面の支援は、産後からの長期的な視点を持った関わりが必要であることが示された。(著者抄録)

本研究では,4ヵ月児健康診査のデータベースの分析から,母親の喫煙が児の出生時の身体計測値(体重,身長,頭囲,胸囲)ならびに出生後の発育にどのように影響するかを明らかにすることを目的とした.方法:本研究で用いたデータベースは,4ヵ月児健康診査を受診した児の健診データのうち,個人情報を全て除外し,連結不可能匿名化したデータファイルを用いた.統計解析には,母親の喫煙と児の身体計測値を重回帰分析等を用いて分析した.結果:3494人の単胎児のデータを分析対象とした.妊娠中に喫煙をしていた母親は全体の2.9%であった.妊娠中の母親の喫煙は,他の要因の影響を調整しても,出生体重,出生身長,出生頭囲と有意な関連が認められた.さらに,4ヵ月児健康診査時の身長および頭囲においても関連が認められ,喫煙していた母親から出生した児は,喫煙していなかった母親から出生した児よりも有意に身長および頭囲が小さく,他の要因を調整しても有意であった.結論:妊娠中の母親の喫煙は,出生時の身体計測値のみならず,4ヵ月児健康診査時の身長や頭囲にも影響していることが明らかとなり,禁煙支援のための対策をさらに強化する必要性が示された.

Objectives: This study aimed to determine the issues mothers face in regards to child rearing by analyzing data obtained from mothers at health check-up 3 months after delivery. The factors that constitute effective support provided by local maternal and child health institutions were then examined.<br/>Methods: Health examination data, excluding personal data, and self-reported questionnaire responses about child rearing concerns were analyzed for mothers who attended the health check-up 3 months after delivery in Ward B in Osaka City.<br/>Results: Study subjects comprised 2,552 mothers, 1,213 (47.6%) of whom asked for advice about child rearing-related issues. The most common concern was “skin treatment”, followed by “breast-feeding” and “stools.” The common factors among the mothers who asked for advice were as follows: “first-time mothers”; “living in an apartment”; “having problems in life”; “feeling mentally and/or physically ill”; and “the feeling of a lack of time for themselves”. The influence of these factors was statistically significant. On the other hand, 36.1% of the mothers did not ask for advice, even though they had reported being anxious.<br/>Conclusion: Many of the mothers’ concerns pertained to the current situation, and it was found that their backgrounds influenced whether they asked for advice. Some mothers had concerns but neither voiced them or sought advice. These findings suggest that mothers’ concerns and feelings of burden related to child rearing cannot be evaluated based on whether they ask for advice or by the number of concerns they report.

<b>Objectives:</b> To investigate the effect of maternal smoking on anthropometric measurements (body weight, height, head circumference, and chest circumference) of newborns and their postnatal growth, with the aim of obtaining fundamental data for future considerations on maternal and child health.<br><b>Methods:</b> A database containing the results of health checkups for four-month-old infants in A City was analyzed by removing personal information.<br><b>Results:</b> A total of 3,494 infants were analyzed. The rate of maternal smoking during pregnancy was 2.9%, while that of paternal smoking was 34.9%. Maternal smoking during pregnancy affected all anthropometric measurements of infants at birth: newborns of smoking mothers were significantly lighter (<i>P</i>=0.013) and shorter (<i>P</i><0.001) and had a significantly smaller head circumference (<i>P</i>=0.004) and a significantly smaller chest circumference (<i>P</i>=0.045) than those of non-smoking mothers. Differences in newborn size without chest circumference between the two groups remained significant after adjusting for factors influencing the above measurements. Maternal smoking during pregnancy affected height and head circumference measured at the four-month-old checkup: infants of smoking mothers were significantly shorter (<i>P</i>=0.004) and had a significantly smaller head circumference (<i>P</i>=0.023). Differences in infant size between the two groups remained significant after adjusting for factors influencing the above measurements.<br><b>Conclusion:</b> Maternal smoking during pregnancy affects body weight, height, head circumference, and chest circumference of newborns, as well as height and head circumference of infants at four months old. These results demonstrate the necessity for stronger guidance on smoking cessation during pregnancy.

We aimed to analyze the characteristics of development of relative weight in Japanese triplets from birth until 12 years of age. Data were collected through a mailed questionnaire sent to mothers of triplets asking for information recorded in medical records. Altogether we had information on 1,061 triplet children of 354 mothers born between 1978 and 2006. For these births, data on triplets' height and weight growth, gestational age, sex, parity, and maternal age at delivery were obtained from records in the maternal and child health handbooks and records from the schools where children receive health check-ups. In addition, information on maternal height and weight was obtained. Triplets have a lower ponderal index at birth and lower body mass index (BMI) compared with the general population until 12 years of age, except for the period during 1 and 3 years of age. Moreover, birth weight had the strongest contribution to BMI of triplets until 6 years of age. After 9 years of age, maternal BMI was a significant factor affecting BMI of triplets.

Background Twins and triplets fall behind singletons in cognitive and physical development. We analysed whether these differences are modified by family social position. Methods IQ, height, body mass index (BMI) and elbow flexion, handgrip and knee extension strength were measured in Swedish men born between 1951 and 1976 during the conscription examination at 17 to 20 years of age. Information from Swedish registers on all siblings and parental socio-economic position were linked to these measures. Together, the measures were available for 1140329 singletons, 19755 twins and 207 triplets. The data were analysed using regression analyses and fixed-effect models which compare twins and triplets with their singleton brothers. Results Twins and triplets had slightly lower IQ, height, BMI and muscle strength than did singletons both at the population level as well as when compared with their singleton brothers. Although the differences between twins and triplets were small, they were largest in families with lower socio-economic position. We also found larger differences in IQ among the sons of mothers born before 1940. Conclusions Twins and triplets fall slightly behind singletons in cognitive and physical development, even in early adulthood, but twins and triplets show no major differences. These differences are largest in families in a low socio-economic position. Our results underscore the importance of the postnatal environment in the development of twins and triplets and suggest that their parents, especially those in a low socio-economic position, need special support.

The West Japan Twins and Higher Order Multiple Births Registry was established in the 1990s by recruiting young twins and multiples and through referrals from public health centers. To date, over 7,000 twins and 4,300 higher order multiple births and their families have been registered in the survey, and it includes one of the largest triplet samples in the world. Follow-up questionnaires are also mailed every 2 to 3 years as part of longitudinal survey studies. This article describes the goals of the registry, the recruitment of multiples, and the overall focus of the study. The goals of the registry are not only to provide data for research on human genetics and maternal and child health, but also to provide appropriate information for families with multiples.

Background: Chest circumference (CC) is suggested to be a good indicator of early life nutrition, but little is known on the heritability of CC. Thus we analysed the effects of genetic and environmental factors on the development of CC in Japanese infants. Methods: CC was measured longitudinally from birth until 1 year of age in a cohort of 211 monozygotic and 160 dizygotic complete Japanese twin pairs born in 19892002. The data were analysed using applications of structural linear equation modelling for twin data. Results: No sex-specific differences in the variance components were found. Environmental factors unique to each twin explained the major part of the variation of CC at birth whereas environmental factors shared by co-twins were more important at 13 months of age. From 3 months of age, the effect of genetic factors become steadily stronger and they explained the majority of the variation at 1 year of age. Strong genetic continuity in the development of CC was found, but also new sets of genes were activated during the first year of life. The origin of the environmental part of the variation could be tracked before 3 months of age. A substantial part of common and specific environmental factors affecting CC affected also birthweight. Conclusions: CC is sensitive to intrauterine environmental factors, but these effects diminish during the first year of life, at least if postnatal environment is good. CC can be a useful indicator when identifying newborns who have suffered suboptimal pre-natal conditions.

目的 本研究では、出生人口に基づいた乳児健康診査のデータベースの分析から、授乳期の栄養方法とその関連要因を分析し、かつ栄養方法の違いによる母親の育児への思いを明らかにすることで、今後の地域母子保健における効果的な支援のあり方を検討する基礎的資料とすることを目的とした。方法 本研究で用いたデータベースは、大阪市福島区において2005年4月から2009年12月までに3ヵ月児健康診査を受診した児の健診データのうち、個人情報をすべて除外したデータファイルである。分析に用いたデータは、乳児に関する要因として、受診時の栄養方法(母乳栄養、混合栄養、人工栄養)、在胎週数、胎児数、出生体重、保育器の使用状況、出生年等を用いた。さらに、母親に関する要因として、計画妊娠の有無、妊娠中の喫煙状況と飲酒状況、婚姻状況、妊娠高血圧症候群の有無、出産時の母親の年齢、就労状況、経済不安の有無、育児協力者の有無、育児相談者の有無、母親の子どもとの生活への思い、ならびに子育て中の母親の気分を使用した。結果 2005年4月から2009年12月までに、2,552人の乳児が3ヵ月児健康診査を受診した。このうち、受診時の栄養方法について不明な者61人、および出生後6ヵ月以降に受診した者15人を除く、2,476人のデータを分析対象とした。3ヵ月児健康診査を受診した児の栄養方法を分析すると、全体の56.8%が母乳栄養による授乳、28.7%が混合栄養による授乳、14.5%が人工栄養による授乳であった。授乳期の栄養方法は、出生年と有意な関連が認められ、出生年が近年になるほど、人工栄養の割合が低くなっていた。また、これらの栄養方法は、双胎出生、出生体重、母親の妊娠中の喫煙状況、ならびに母親の出産時の年齢と有意な関連が認められた。さらに、混合栄養または人工栄養による授乳を行っている母親では育児が楽しいと回答した者が88.4%であったのに対し、母乳栄養による授乳を行っている母親では93.4%と、母乳栄養による授乳を行っている母親に育児が楽しいと回答する者が有意に多かった。結論 本調査結果から、出生後3ヵ月から5ヵ月における乳児の授乳はおよそ57%が母乳栄養による授乳、15%が人工栄養による授乳であり、さらに人工栄養による授乳は年々減少していることが明らかとなった。母乳栄養による授乳を行っている母親は、育児が楽しいと回答する者が有意に多いことが判明した。一方、これらの栄養方法は、双胎出生、出生体重、母親の妊娠中の喫煙状況、ならびに母親の年齢との関連が認められ、授乳指導を効果的に行うためにはこれらの要因を考慮した上で指導する必要があることが示された。(著者抄録)

目的 本研究では、大阪市あいりん地域における生活支援つき簡易宿所転用共同住宅に入居する日雇い労働者であった単身高齢男性の健康関連行動を、性と年齢を一致させた一般市民との比較から分析することにより、今後の効果的な地域保健活動を検討するための基礎的資料とすることを目的とした。方法 本研究では、生活支援つき簡易宿所転用共同住宅に入居している単身高齢男性のうち、調査の趣旨説明に同意の得られた162人を対象者とした。さらに、比較対照群として、性と年齢を一致させた大阪市に在住する一般市民360人を得た。分析にあたっては、一般市民を単身高齢男性および家族と同居する高齢男性に分類して分析を行った。調査項目は、主観的健康感、睡眠の充足度、アルコール摂取、喫煙、既往歴、健康診査の受診状況等である。結果 主観的健康感については有意(P=0.036)な差異が認められ、一般の単身高齢男性は、簡易宿所転用共同住宅の単身高齢男性や家族と同居している高齢男性に比べて、健康でないと回答した者の割合が高かった。睡眠の充足度についても、一般の単身高齢男性は、簡易宿所転用共同住宅の単身高齢男性や家族と同居している高齢男性に比べ、睡眠不足であると回答した者が有意(P=0.002)に多かった。アルコールの摂取頻度は、毎日摂取している者が簡易宿所転用共同住宅の単身高齢男性では22.2%、一般の単身高齢男性では26.3%、家族と同居している高齢男性が42.0%と、毎日アルコール摂取している者の割合が家族と同居している高齢男性で有意(P<0.001)に多くなっていた。喫煙状況では、簡易宿所転用共同住宅の単身高齢男性が、一般の単身高齢男性、および家族と同居している高齢男性に比べ有意(P<0.001)に多くなっていた。簡易宿所転用共同住宅の単身高齢男性の既往歴に関しては、46.9%の者に何らかの既往歴が認められた。そのうち最も多い疾患は、結核が10.5%であったが、82.1%の者が結核健診を受診していた。結核健診の受診理由を尋ねると、入居施設からの受診勧奨による者が42.8%を占めていた。(著者抄録)

We analyzed the characteristics associated with the growth in height of Japanese triplets from birth to 12 years of age. The study included 376 mothers and their 1,128 triplet children, who were born between 1978 and 2006. Data were collected through a mailed questionnaire sent to the mothers asking for information recorded in medical records. For these births, data on triplets' length and height growth, gestational age, sex, parity, maternal age at delivery, and maternal height were obtained from records in the Maternal and Child Health Handbooks and records in the school which children receive health check-ups. The height deficit of the triplets compared to the general population of Japan remained between 2% and 5% until 12 years of age. Moreover, at 12 years of age, the differences of height between the general population and triplets were approximately -3.6 cm for male and -4.4 cm for female. Maternal height showed the strongest contribution to height of triplets from 6 to 12 years of age. In conclusion, triplets remain shorter than singletons until 12 years of age.

社会のニーズに対応できる高度な専門性と実践力を備えた保健師を養成するために,公衆衛生看護学実習モデルを作成した。研究方法は,地域看護学実習の臨地実習施設および教育機関へのインタビュー結果をもとに,行政職員および教員からなる研究班員により検討した。養成したい保健師像は,公衆衛生看護の理論にもとづいた実践を志向し,自立して状況の判断と行動ができる高度専門職業人である。教育の前提として,看護師教育課程において地域看護学の基礎的な講義と実習を履修していることとし,それに積み上げる形で大学院修士課程の看護実践コースにおいて保健師教育を行うこととした。実習の目的は「人々の健康と生活に着目し,誰もが健康で文化的な生活を送れるよう社会的公正の実現をめざし,実践をとおして公衆衛生看護の専門的技術を習得する」こととした。実習は公衆衛生看護学実習I～IVの合計10単位で設定し,実践における課題解決能力を備えるために公衆衛生看護学特定課題研究6単位を教育課程に位置づけた。対人支援技術に加えて保健福祉事業の企画から実施・評価,組織化やシステム構築・施策化など,看護管理を含めた実践者養成に主眼を置き,目標ごとの具体的な体験項目やコアとなる能力の到達度も示した。(著者抄録)

Objectives: Previous studies have shown strong genetic influence to head circumference (HC), but still little is known on the development of genetic etiology of HC in infancy, especially in non-Caucasian populations. Thus, we decided to analyze the genetics of HC growth in Japanese infants. Methods: Longitudinal measures of HC were available from birth to 13 months of age in 206 monozygotic and 156 dizygotic complete twin pairs. Genetic modeling for twin data was used. Results: We found only little evidence for sex-specific differences in the genetics of HC and thus analyzed boys and girls together. After 5 months of age the heritability of HC was high, but before that age also a substantial common environmental component was present. Not only strong genetic persistence for HC was found but also a new genetic variation emerged. New environmental variation shared by co-twins affecting HC was found until 3 months of age, and this effect was further transmitted until 1 year of age. Conclusions: HC and its growth are strongly genetically regulated. Largely, the same genetic factors affect the variation of HC at different ages, and new genetic variation emerged during the first year of life. Knowledge on the genetic component in the variation of HC may help to design tools for defining abnormal growth of HC in population-based screenings for related disorders. Am. J. Hum. Background: Genetic factors explain a major part of the variation of adult stature, but little is still known on the genetics of growth, especially in non-Caucasian populations.
Aim: To analyse the quantitative genetics of pre-pubertal growth in Japanese children.
Subjects and methods: Data from birth until 11 years of age were collected on 349 complete twin pairs based on previously recorded height measures. The data were analysed using two different multivariate models by the Mx statistical package.
Results: No major sex differences were found and thus boys and girls were analysed together. Since 1 year of age, genetic factors explained from 42-71% and environmental factors shared by co-twins from 14-33% of the variation of height. Genetic continuity of height was high and 75% of the genetic variance was shared since 1 year of age. Environmental factors affecting height showed weaker correlations between early and late childhood than genetic factors.
Conclusion: Growth from early to late childhood is largely regulated by the same set of genes. However, also environmental factors shared by co-twins are important for growth. Identifying specific environmental factors affecting growth has potentially important public health implications, even in an affluent society such as Japan.

We analyzed whether motor development in early life is different between singletons and triplets in Japan. The motor development was reported by mothers by postal questionnaire for 1,121 triplet children and in regular health check-ups for 13,906 singleton children. Children who were suspected of having neurological abnormality or disability were excluded from the analysis. The ages of milestone achievements were significantly higher in triplets for each outcome compared to singletons. Further, after adjustment for gestational age, birthweight, and birth length, the differences were significant for maintaining head, sitting alone and standing holding on. In children with birthweight of 2 kg or more, the ages of milestone achievements were significantly higher in triplets for each outcome compared to singletons, except walking holding on. Moreover, after adjustment for the confounding factors, the differences were significant for sitting alone and walking independently. On the contrary, singletons attained motor development facilitating crawling, walking holding on, and walking independently slower than triplets among those children with birthweight of 2 kg or less after adjustment for gestational age. In conclusion, triplets are overall at higher risk for the delay of gross motor milestones as compared to singletons independently of birth-related factors. In contrast, among children with a birthweight of less than 2 kg, singletons showed slower motor development than triplets after adjusting for gestational age. There is an obvious need to apply developmental standards that consider at least both multiple birth status (singleton, twin or triplet) and birthweight.

We analyzed the genetic architecture of prepubertal development of relative weight to height in 216 monozygotic and 159 dizygotic complete Japanese twin pairs (52% girls). Ponderal index at birth (kg/m(3)) and body mass index (BMI, kg/m(2)) from 1 to 11 years of age were used. Additive genetic factors explained the major proportion (52-74%) of the variation of BMI from 1 to 11 years of age. Environmental factors common to both co-twins also showed some effect (7-28%), but at most ages this was not statistically significant. Strong genetic tracking was found for BMI from 1 to 11 years of age, but there was also evidence for a persistent effect of common environmental factors. Our results suggest that the genetic architecture of BMI development in the Japanese population is generally similar to that found in previous twin studies in Caucasian populations.

目的 本研究では、子どもを虐待していると思うことがあるとの認識を虐待認識として捉え、4ヵ月児健診を受診し、かつ第1子が小学生までの子どもをもつ母親を対象に、虐待認識の実態とその関連要因について明らかにすることにより、虐待予防対策を検討するための基礎的資料とすることを目的とした。方法 対象者は、西宮市の4ヵ月児健診を受診した母親のうち、無作為抽出した3,000人の母親に自記式質問紙を郵送し、1,725人(回収率57.5%)から回答を得た。このうち、第1子が12歳以下である母親1,471人を本研究の対象者とした。本研究では、子どもを虐待しているのではないかと思うことがあるか否かを二件法で問い、その内容について調査した。分析に使用したデータは、子どもの年齢、子どもの数、母親の体調、ストレス解消法の有無、自由時間の有無、睡眠状態、育児協力者の有無、母親の不安状態、抑うつ状態、可愛がりにくい子どもがいるか否か等である。結果 調査時点で、虐待認識のある母親は333人(全体の22.6%)であった。虐待認識の内容は、各年齢を通じて感情的な言葉が最も多く、続いて叩くなどの行為が挙げられていた。母親の虐待認識は子どもの年齢階級で差異が認められ、1歳以下の子どもをもつ母親では虐待認識のある者の割合が13.8%と、他の年齢の子どもをもつ母親よりも有意(P<0.001)に低かった。ロジスティック回帰分析の結果、母親の虐待認識には可愛がりにくい子どもがいること、子どもが2人以上いること、STAIにおける特性不安が高不安であること、母親の体調が悪いもしくは治療中であること、および障がい児をかかえていることと関連が認められた。可愛がりにくい子どもがいる理由で最も多く挙げられていたものは、下の子どもがいる場合の上の子どもへの対応の難しさであった。結論 本研究結果から、対象者全体の22.6%の母親に虐待認識が認められた。これらの母親は、子どもに対して感情的な言葉、叩くなどの行為を認知していることが確認された。また、母親の虐待認識は、子どもの年齢と関連しており、2歳以上の子どもをもつ母親で有意に多くなっていた。さらに、子どもが複数いる母親に、虐待認識のある者が多いことが示され、初任産婦が優先されやすい現在の母子保健サービスのあり方や優先順位を虐待予防の視点から再度検討する必要性が示唆された。(著者抄録)

<b>Objective</b>&emsp;This research was conducted to determine the prevalence of recognition of child maltreatment among mothers with children aged 12 or under, and to identify associated factors in order to prevent child maltreatment.<br/><b>Methods</b>&emsp;The subjects of this study were 3,000 women extracted by systematic random sampling of mothers of 6,790 children who had had four-month health check-ups in Nishinomiya city. The response rate was 57.5%. After excluding mothers with children aged 13 years and over, the study sample included 1,471 mothers with children aged 12 or under. A questionnaire survey was conducted by mail. Recognition of child maltreatment by mothers was assessed with a question that asked the mother if she was sometimes aware that she had potentially abused her child, and, if yes, what kind of acts had she performed.<br/><b>Results</b>&emsp;There were 333 mothers (22.6%) who answered &ldquo;yes&rdquo; to the question &ldquo;Are you sometimes aware that you have potentially abused your child?&rdquo; These mothers reported emotional or physical aggression toward their children. Results of logistic regression showed that recognition of child maltreatment was associated with existence of a child whom the mother felt difficulty in cherishing, number of children, maternal poor health, higher scores of STAI trait anxiety and disabled children. Mothers reported difficulties in child-rearing for more than one child as the main reason behind existence of a child whom the mother felt difficulty in cherishing.<br/><b>Conclusion</b>&emsp;These findings suggested that mothers with more than one child need more support in order to prevent child maltreatment.

社会のニーズに対応できる高度な専門性と実践力を備えた保健師を養成するために,公衆衛生看護学実習モデルを作成した。研究方法は,地域看護学実習の臨地実習施設および教育機関へのインタビュー結果をもとに,行政職員および教員からなる研究班員により検討した。養成したい保健師像は,公衆衛生看護の理論にもとづいた実践を志向し,自立して状況の判断と行動ができる高度専門職業人である。教育の前提として,看護師教育課程において地域看護学の基礎的な講義と実習を履修していることとし,それに積み上げる形で大学院修士課程の看護実践コースにおいて保健師教育を行うこととした。実習の目的は「人々の健康と生活に着目し,誰もが健康で文化的な生活を送れるよう社会的公正の実現をめざし,実践をとおして公衆衛生看護の専門的技術を習得する」こととした。実習は公衆衛生看護学実習I～IVの合計10単位で設定し,実践における課題解決能力を備えるために公衆衛生看護学特定課題研究6単位を教育課程に位置づけた。対人支援技術に加えて保健福祉事業の企画から実施・評価,組織化やシステム構築・施策化など,看護管理を含めた実践者養成に主眼を置き,目標ごとの具体的な体験項目やコアとなる能力の到達度も示した。

<b>Objective</b> This research was conducted to determine the prevalence of recognition of child maltreatment among mothers with children aged 12 or under, and to identify associated factors in order to prevent child maltreatment.<br/><b>Methods</b> The subjects of this study were 3,000 women extracted by systematic random sampling of mothers of 6,790 children who had had four-month health check-ups in Nishinomiya city. The response rate was 57.5%. After excluding mothers with children aged 13 years and over, the study sample included 1,471 mothers with children aged 12 or under. A questionnaire survey was conducted by mail. Recognition of child maltreatment by mothers was assessed with a question that asked the mother if she was sometimes aware that she had potentially abused her child, and, if yes, what kind of acts had she performed.<br/><b>Results</b> There were 333 mothers (22.6%) who answered “yes” to the question “Are you sometimes aware that you have potentially abused your child?” These mothers reported emotional or physical aggression toward their children. Results of logistic regression showed that recognition of child maltreatment was associated with existence of a child whom the mother felt difficulty in cherishing, number of children, maternal poor health, higher scores of STAI trait anxiety and disabled children. Mothers reported difficulties in child-rearing for more than one child as the main reason behind existence of a child whom the mother felt difficulty in cherishing.<br/><b>Conclusion</b> These findings suggested that mothers with more than one child need more support in order to prevent child maltreatment.

<b>Object</b>: To demonstrate the efficacy of using laughter in clinical and nursing practice, the present study was designed to overview the effects of laughter previously reported in Japan regarding both physical and mental aspects through a literature review.<br><b>Method</b>: Literature search was performed of the Igaku Chuo Zasshi Publishing database using “laughter” or “humor” as the keyword for publications during the period of 1980 to May 2009, and 29 articles were retrieved. For international journals, PubMed was used with the keywords “laughter” and “effect” and the target period of 2000 to May 2009, and 20 papers by Japanese authors were identified.<br><b>Results</b>: As the physical effects of laughter, many researchers reported enhancement of the NK cell activity and its effects on the immune system. There were also studies on the alleviation of pain, improvement of various symptoms in allergic patients, reduction of the increase in blood glucose after eating, and improvement of cerebral activity. As the mental effects of laughter, there were a lot of studies on efficacy in coping with stress. In addition, humor have benefits in relation to interpersonal skills, and a relaxation effect.<br><b>Conclusion</b>: While there have been many experiments involving healthy people or specific disease, the efficacy of laughter in patients with acute/chronic disease as well as its preventive effects are expected to be explored in future research on laughter as a means of nursing care in clinical practice.

入院患児のあそび環境に関する研究動向を明らかにすることを目的に、医学中央雑誌Web版を用いて1988～2008年の文献を対象に検索を行い、得られた25件の原著論文を分析した。その結果、ほとんどの病院の小児病棟にはプレイルームが設置されており、多くの医療者が入院患児のあそびの重要性を認識しているが、業務の多忙さから満足のいくあそびが提供できておらず、専門スタッフ導入についても、認知の低さや経済面での問題から十分な導入が進んでいないことが明らかになった。

We analyzed the characteristics of weight growth and present the weight growth charts from birth to 6 years of age in Japanese triplets. The study included 366 mothers and their 1098 triplet children, who were born between 1978 and 2006. Data were collected through a mailed questionnaire sent to the mothers asking for information recorded in medical records. For these births, data on triplets&apos; weight growth, gestational age, sex, parity, and maternal age at delivery were obtained from records in the Maternal and Child Health Handbooks, which is provided by the authorities after a report of pregnancy. Birthweight proved to be the strongest contribution on weight of triplets from 1 to 6 years of age. In addition, gestational age was also a significant contributing factor to weight from birth to 6 years of age. Moreover, males had a higher weight from birth to 6 years of age than females. Compared to the 50th percentile of the growth standard for the general population of Japan, the weight deficit of the triplets was more than 40% at birth (male, -1.28 kg; female, -1.28 kg), decreased within the first 1 year of age, and fluctuated between 4% and 9% until 6 years of age (male, -1.82 kg; female, -1.78 kg). In conclusion, triplets have lower birth weight than singletons and in spite of the rapid catch-up growth during first year of life they are behind singletons even in mid-childhood. This study provides growth curves for use in triplets.

Maintaining the peak bone mass at a higher level acts to prevent fracture due to osteoporosis in the future. To elucidate whether health training for the prevention of osteoporosis that included bone density measurement would help trainees understand preventive measures and change their behavior, we evaluated the effects of training by comparing the subjects' level of understanding concerning exercise and diet and their behavior before training with those 3 months after training. The subjects were 45 healthy female college students. They had their bone density measured and were informed of the results, and then received health training to prevent osteoporosis. A comparison of their understanding/behavior scores before training with those 3 months after training demonstrated the development of their understanding regarding 2 items (habit of exercise, and consumption of green and yellow vegetables) and an improvement in their behavior for 2 items (consumption of fish and salt). These results suggest that health training for osteoporosis prevention has the effect of promoting trainees' understanding of a healthy lifestyle and improving their diets toward the prevention of osteoporosis.

目的：本研究では、地域看護学実習終了時における学生の地域保健活動への関心の程度(関心度)とその関連要因を明らかにし、より効果的な実習を行うための基礎的資料とすることを目的とした。
方法：対象者は、A県内の看護系大学で、平成17年4月～7月に地域看護学実習を実施した2校に在籍する133人の4回生(年生)である。地域看護学実習終了時に調査を実施し、郵送にて回収した。調査内容は、学生の基本情報と地域保健活動への関心度、実習施設や実習経験、現場保健師の実習指導と教員との連携等である。学生の地域保健活動に関する関心度については、意味的微分法(SD法)を用いて測定し、得点化(関心度得点)した。統計学的分析方法については、質的変数の独立性の検定にはχ2検定を、地域保健活動に関する関心度得点における2群間の比較にはMann-WhitneyU検定を、多群間の比較にはKruskal-Wallis検定を用いた。結果　編入生を除く有効回答者は78人(有効回答率58.6%)であった。入学前に希望していた職種は保健師15.4%、助産師7.7%、看護師64.1%、わからない12.8%であった。本調査結果から、講義・演習をとおして保健師に興味をもった学生、ならびに実習をとおして保健師に興味をもった学生は、実習終了時の地域保健活動への関心度得点が有意に高かった。また、現場保健師の実習指導では、学生の気づきを大切にした指導、具体的でわかりやすい説明、学生が困っているときの声かけ、および現場保健師による学生への過度な期待がなかった場合に地域保健活動への関心度得点が有意に高かった。結論　本研究結果より、大学の講義・演習あるいは実習経験により保健師に興味をもった学生は地域保健活動への関心度得点が高く、さらに、現場保健師による4つの実習指導手法が学生の地域保健活動への関心度を高めることが明らかとなった。今後、学生の地域保健活動への関心を高めるような講義・演習内容の検討、ならびに実習指導の充実が望まれる。

<b>Purpose</b> The purpose of this survey was to study anxiety and associated factors in the mothers of twins or triplets as compared with the mothers of singleton children.<br/><b>Methods</b> The subjects were 130 mothers of twins or triplets aged 3 or under and 860 mothers of similarly aged singleton children. The Japanese version of State-Trait Anxiety Inventory (STAI) was used to evaluate their anxiety states.<br/><b>Results</b> 1. Mothers of twins or triplets showed significantly higher STAI state anxiety scores than those of singleton children. However, there was no significant difference in STAI trait anxiety between mothers with twins or triplets and those with singleton children.<br/>2. Mothers of twins or triplets showed greater anxiety during pregnancy than those of singleton children. There were also higher rates of cases where stress could not be alleviated in mothers of twins or triplets than in those with singleton children. STAI state anxiety of mothers was associated with anxiety during pregnancy, anxiety for future child-rearing, problems with stress alleviation, maternal health conditions, poor sleeping conditions and having siblings.<br/><b>Conclusion</b> This study indicated a tendency for mothers of twins or triplets to show greater anxiety as compared with those having singleton children. It is important to improve the child-rearing environment to reduce anxiety felt by mothers of twins or triplets.

乳幼児・学童期のBody Mass Index(BMI)に対する遺伝要因と環境要因の影響を検討した.対象は,同性双生児201組(一卵性男男81組,一卵性女女81組,二卵性男男16組,二卵性女女23組)で,1歳時,3歳時,6歳時,8歳時の縦断データを統計遺伝学的に解析した.その結果,遺伝要因の寄与は1歳時には30%程度で,3歳時までに急速に上昇して90%程度になり,以後8歳時まで継続した.共有環境要因の影響は1歳時には見られるが,3歳時までに消失した.年齢間のクロス相関は一卵性が二卵性よりも大きく,異なる年齢に共通に関与する遺伝要因の存在が示唆された.以上のことからも,小児のBMIには遺伝要因が寄与することが確認され,小児肥満予防においても遺伝要因を視野に入れた対策が必要と考えられた．

多胎児虐待症例では，一方の児のみが虐待される危険が高く，その背景に多胎児に対する親の愛着感情の偏りが共通して存在する。本研究では，地域に在住する双子の母親126名，三つ子の母親96名，四つ子の母親7名，五つ子の母親27名を対象に，多胎児に対する母親の愛着感情の偏りが発生する状況とその関連要因について分析した。

Twin studies using information on monozygotic (MZ) and dizygotic (DZ) twins have importantly increased our understanding of the effect of genetic and environmental factors on human growth and anthropometrics. Twin pregnancies differ in many ways from singleton pregnancies, most notably because of their earlier gestational age and lower birth weight. The differences between twins and singletons, however, largely disappear during the few first years of life, and thus in later life twins well represent the general population. Twin studies have shown that growth is under strong genetic regulation, and from 70 to 80% of the variation in adult height is explained by genetic differences between individuals. About half of the genes affecting height are the same during the growth period after 4 years of age, and genetic factors are also important when regulating the timing of pubertal growth. For body mass index the proportion of the trait variation explained by genetic differences is lower than for height, i.e., from 60 to 80%, and genetic correlations during the childhood and adolescence are lower than for growth in height. This is probably because body mass index is a more heterogeneous trait than height. Lower genetic variation for body mass index is found in physically active persons as compared to sedentary persons indicating that physical activity can suppress the function of genes predisposing to obesity. New molecular genetic methods have also recently improved opportunities in twin studies. An example is a study which found differences in gene expression in fat tissue within MZ pairs discordant for obesity. The nearly perfect natural experiment of twins will almost certainly create new opportunities in several areas of research in the post-genomic era.

＜文献概要＞はじめに　フィンランドの母子保健は,国際的にも高い評価を受けており,優れたシステムを有している。このフィンランドにおける母子保健活動の地域拠点が「ネウボラ」である。ネウボラの中で必須と言えるシステムは,(1)子どもを持つ家族への継続的な支援により構築される担当保健師との信頼関係と,(2)父親を含めた家族全員の支援なのである。このシステムがあるからこそ,保健師は家族との信頼関係を築きやすく,家族の抱える繊細な問題を早期に発見し,支援につなげることができている。一方,日本の母子保健を担ってきた保健師は,地区担当制,業務分担制,地区担当と業務担当が相互に連携しながら活動する重層型の体制で活動してきた。しかし,家族に対してさまざまなサービスが用意されているにもかかわらず,個人の状況に応じたサービスにつながらない,妊娠出産に関する悩みの相談先が分かりにくい・相談体制がないという課題が指摘されてきた。これは,母子保健サービスが単発のサービス提供に終わっており,担当保健師に気軽に相談でき,顔の見える関係づくりが多くの場合できていなかったことが原因の1つと考えられる。他方,2017(平成29)年4月から,母子保健法の改正により,子育て世代包括支援センターを市町村に設置することが努力義務となった。同センターでは以下のことが求められている。・妊娠期から子育て期にわたり,地域の特性に応じ,切れ目なく支援する・妊産婦,子育て家庭の個別ニーズを把握し,必要なサービスを円滑に利用できるようきめ細かく支援する・地域のさまざまな関係機関とのネットワークを構築し,必要に応じ社会資源の開発などを行う 全妊婦に対し妊婦面接を行う過程を通じ,家族の養育力を高める取り組みなどが進められている。しかしながら,具体的な運用については市町村の創意工夫が求められており,戸惑いの声も聞かれている。本稿では,フィンランドにおけるネウボラの必須のシステムの基本を取り入れた大阪市版ネウボラ具体的運用について解説し,皆さまの母子保健再構築の一助となることを願う。
