    To evaluate the antibody response following the initial four doses of mRNA vaccines (BNT162b2 or mRNA-1273) in SARS-CoV-2-naïve healthy adults and investigate factors influencing antibody titer increases, this prospective cohort study was conducted in Japan from March 2021. The study included participants who received either the 1st and 2nd doses (n = 467), 3rd dose (n = 157), or 4th dose (n = 89). Blood samples were collected before and up to 6 months after each dose, and anti-receptor-binding domain antibody levels were measured. Multivariate analysis (usin multiple linear regression or linear mixed models) revealed several factors significantly associated with higher post-vaccination antibody levels, including mRNA-1273 vaccine (after the 1st and 2nd dose), male gender (after the 3rd and 4th doses), younger age (after the 1st and 2nd dose), non-smoking status (after the 2nd dose), non-use of immunosuppressive agents (after the 1st dose), higher pre-vaccination antibody titers (after the 2nd, 3rd, and 4th doses), and higher post-vaccination fever (after the 2nd and 4th doses). Furthermore, longer intervals since the last dose were significantly associated with higher antibody levels after the 3rd and 4th doses. These findings provide valuable insights for optimizing vaccination strategies.

    <p>Latent tuberculosis infection (LTBI) with fibrotic lesions (FL) can progress to active tuberculosis (TB). Most previous studies have used tuberculin skin tests, which have lower specificity than interferon-gamma release assays (IGRAs), for LTBI diagnosis. This study evaluated the incidence of active TB among individuals with LTBI (diagnosed using IGRAs) and FL in Nishinari District, Osaka City. In total, 54 men (mean age: 68.7 years) were enrolled, of whom 10 (18.5%) were homeless, and 36 (66.7%) were welfare recipients. The median observation period was 1,084 days (range: 64–2,907 days). The incidence rate of active TB among individuals with LTBI and FL was 1.18 (95% confidence interval: 0.32–4.29) cases per 100 person-years. Among the 19 participants who had not been treated with anti-TB therapy, one (5.3%) progressed to active TB, and among the 30 participants who had completed anti-TB treatment, one (3.3%) progressed to active TB. The other 5 participants did not have TB. This study revealed the incidence of active TB among individuals with LTBI, diagnosed using IGRAs, and FL in a vulnerable urban population. The higher incidence than that reported in previous studies reinforces the importance of improved LTBI management strategies, including chest radiography screening, and LTBI treatment.</p>

    BACKGROUND: The association between inactivated influenza vaccination and viral load in young children remains unclear. METHODS: During the 2013/2014 to 2017/2018 influenza seasons in Japan, children under 6 years of age with pre-defined influenza-like illness and influenza-positive status by real-time RT-PCR were recruited at pediatric clinics for this observational study. Influenza viral load was measured for the most predominant subtype/lineage in each season. Using median dichotomized viral load as an outcome, a multilevel logistic regression model was applied to estimate the multivariable adjusted odds ratio (MOR) and 95% confidence interval (CI) for higher viral load. RESULTS: A total of 1,185 influenza-positive children were analyzed. The median log10 viral load copy number (copies per milliliter) was 5.5 (interquartile range, 4.6 to 6.1) and did not differ by vaccination status: 5.5 for unvaccinated, 5.7 for one dose, and 5.5 for two doses (p = 0.67). The MOR of vaccinated (one or two doses) versus unvaccinated children was 1.19 (95% CI: 0.86-1.64). Other factors showing significant associations with higher viral load were positive results for A(H1N1)pdm09 and A(H3N2) in comparison with B/Yamagata. The respective MORs were 3.25 (95% CI: 2.28-4.64) and 1.81 (95% CI: 1.32-2.49). Significantly elevated MORs against higher viral load were also observed for higher body temperature at influenza diagnosis and shorter duration from fever onset to specimen collection. CONCLUSION: No association was observed between inactivated-influenza vaccination and viral load at influenza-positive diagnosis. Influenza subtype/lineage, body temperature, and time elapsed since fever onset were significantly associated with viral load.

    BACKGROUND: Although several assays are used to measure anti-receptor-binding domain (RBD) antibodies induced after severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) vaccination, the assays are not fully comparable in practice. This study evaluated the immunogenicity of the BNT162b2 mRNA vaccine in healthy adults using two immunoassays. METHODS: This prospective cohort study included SARS-CoV-2-naïve adults, predominantly healthcare workers, aged 20-64 years, who received two BNT162b2 vaccine doses between March and May 2021. Blood samples were collected before the first vaccination (S0), before the second vaccination (S1), 4 weeks after the second vaccination (S2), and 6 months after the second vaccination (S3). anti-RBD antibodies were measured using the Architect SARS-CoV-2 IgG II Quant (Abbott Laboratory) and Elecsys anti-SARS-CoV-2 S (Roche Diagnostics) assays. RESULTS: Among the 385 participants, the geometric mean antibody titers (GMTs) on the Architect assay (AU/mL) were 7.5, 693, 7007, and 1030 for S0, S1, S2, and S3, respectively. The corresponding GMTs on the Elecsys assay (U/mL) were 0.40, 24, 928, and 659, respectively. The GMT ratio (S3/S2) was 0.15 on the Architect and 0.71 on the Elecsys assay. The correlation between antibody titers measured with the two assays were strong at all time points after vaccination (Spearman's correlation coefficient: 0.74 to 0.86, P < 0.01 for all). GMT was significantly lower in the older age group after vaccination (P < 0.01), with no significant differences according to sex. Seroprotection (≥5458 AU/mL on the Architect assay and ≥ 753 U/mL on the Elecsys) at each time point was 0 %, 1 %, 67 %, and 1 % on the Architect assay and 0 %, 1 %, 62 %, and 43 % on the Elecsys, respectively. CONCLUSIONS: Two BNT162b2 vaccine doses resulted in adequate anti-RBD antibody response, which varied by age. As the two assays showed different kinetics, the results of single immunoassays should be interpreted with caution.

  • Epidemiological study of osteonecrosis of the femoral head using the national registry of designated intractable diseases in Japan.

    OBJECTIVES: We aimed to investigate the epidemiological characteristics of osteonecrosis of the femoral head (ONFH) using the designated intractable diseases (DID) database in Japan. METHODS: Data on patients who had received public subsidies for medical costs due to ONFH from 2012 to 2013 were extracted from the DID database. The incidence and prevalence of ONFH, distribution of gender, age, and the prevalence of associated risk factors were assessed. These epidemiological characteristics were compared with those of another nationwide ONFH survey conducted during a similar period. RESULTS: Data on 3264 newly diagnosed patients (incident cases) and 20,042 patients registered until 2013 (prevalent cases) were evaluated. The corrected annual incidence and prevalence of ONFH per 100,000 were 3.0 and 18.2-19.2, respectively. The ratio of males to females was 1.4 in 2012 and 1.2 in 2013, respectively. Peak distribution was observed at ages 40s and 60s in males and females, respectively. The prevalence of the risk factors were steroid-associated: 39%, alcohol-associated: 30%, both: 4%, and none: 27%. CONCLUSIONS: The DID database data showed a similar distribution of gender and age to that in other nationwide surveys but lower incidence and prevalence of ONFH. Sampling bias may affect the epidemiological characteristics of ONFH.

    <p><b>Background:</b> Since June 2013, Japan has suspended proactive recommendation of human papillomavirus (HPV) vaccination due to self-reported diverse symptoms, including pain and motor dysfunction, as possible serious adverse events following immunization. Although these symptoms may be seen in adolescents without HPV vaccination, their frequency, taking into account disease severity, has not been examined.</p><p><b>Methods:</b> A two-stage, descriptive, nationwide epidemiological survey was conducted in 2016, with a 6-month target period from July 1 to December 31, 2015, to estimate the prevalence and incidence of diverse symptoms among Japanese adolescents without HPV vaccination. Participants were 11,037 medical departments in hospitals selected nationwide by stratified random sampling. Eligible patients had to satisfy four criteria: (1) aged 12–18 years upon visiting hospital; (2) having at least one of four symptoms/disorders (pain or sensory dysfunction, motor dysfunction, autonomic dysfunction, or cognitive impairment); (3) symptoms/disorders persisting for at least 3 months; and (4) both criteria (2) and (3) influence attendance at school or work. We then extracted data of patients with diverse symptoms similar to those after HPV vaccination while considering opinions of doctors in charge.</p><p><b>Results:</b> Estimated 6-month period prevalence of diverse symptoms among girls aged 12–18 years without HPV vaccination was 20.2 per 100,000. Annual incidence was estimated to be 7.3 per 100,000.</p><p><b>Conclusion:</b> Adolescent Japanese girls without HPV vaccination also visited hospitals with diverse symptoms similar to those following HPV vaccination. Our findings predict the medical demands for coincident diverse symptoms, which are temporally associated with but not caused by HPV vaccination of Japanese adolescents.</p>

  • Influence of Prior Influenza Vaccination on Current Influenza Vaccine Effectiveness in Children Aged 1 to 5 Years

    Background: Although annual influenza vaccination is an important strategy used to prevent influenza-related morbidity and mortality, some studies have reported the negative influence of prior vaccination on vaccine effectiveness (VE) for current seasons. Currently, the influence of prior vaccination is not conclusive, especially in children. Methods: We evaluated the association between current-season VE and prior season vaccination using a test-negative design in children aged 1–5 years presenting at nine outpatient clinics in Japan during the 2016/17 and 2017/18 influenza seasons. Children with influenza-like illness were enrolled prospectively and tested for influenza using real-time RT-PCR. Their recent vaccination history was categorized into six groups according to current vaccination doses (0/1/2) and prior vaccination status (unvaccinated = 0 doses/vaccinated = 1 dose or 2 doses): (1) 0 doses in the current season and unvaccinated in prior seasons (reference group); (2) 0 doses in the current season and vaccinated in a prior season; (3) 1 dose in the current season and unvaccinated in a prior season; (4) 1 dose in the current season and vaccinated in a prior season; (5) 2 doses in the current season and unvaccinated in a prior season, and (6) 2 doses in the current season and vaccinated in a prior season. Results: A total of 799 cases and 1196 controls were analyzed. The median age of the subjects was 3 years, and the proportion of males was 54%. Overall, the vaccination rates (any vaccination in the current season) in the cases and controls were 36% and 53%, respectively. The VEs of the groups were: (2) 29% (95% confidence interval: −25% to 59%); (3) 53% (6% to 76%); (4) 70% (45% to 83%); (5) 56% (32% to 72%), and (6) 61% (42% to 73%). The one- and two-dose VEs of the current season were significant regardless of prior vaccination status. The results did not differ when stratified by influenza subtype/lineage. Conclusion: Prior vaccination did not attenuate the current-season VE in children aged 1 to 5 years, supporting the annual vaccination strategy.

    <p><b>Background:</b> Cronkhite–Canada syndrome (CCS), chronic enteropathy associated with <i>SLCO2A1</i> gene (CEAS), and intestinal Behçet's disease (BD) are classified as intractable intestinal disorders in Japan. However, the national prevalence of these diseases remains unknown. We performed a nationwide survey to estimate the patient numbers and prevalence rates of these diseases throughout Japan in 2017.</p><p><b>Methods:</b> We conducted a mail-based survey targeting hospitals across Japan to estimate the annual numbers of patients with CCS, CEAS, and intestinal BD in 2017. Using a stratified random sampling method, we selected 2979 hospital departments and asked them to report the number of patients who met specific diagnostic criteria. The total number of patients for each disease was estimated by multiplying the reported numbers by the reciprocal of the sampling rate and response rate. The corresponding prevalence rates per 1,000,000 population were calculated based on the mid-year population of Japan in 2017.</p><p><b>Results:</b> The overall survey response rate was 68.1% (2029 departments). The estimated numbers of patients with CCS, CEAS, and intestinal BD were 473 (95% confidence interval: 357-589), 388 (289-486), and 3139 (2749-3529), respectively; the prevalence rates per 1,000,000 population were 3.7 (male: 4.0; female: 3.5), 3.1 (male: 3.0; female: 3.1), and 24.8 (male: 24.5; female: 25.0), respectively. The male-to-female ratios were 1.10, 0.94, and 0.93 for patients with CCS, CEAS, and intestinal BD, respectively.</p><p><b>Conclusions:</b> Estimates of the national prevalence of CCS, CEAS, and intestinal BD in Japan were generated and found to be higher than those previously reported.</p>

    <p>Station staff may be at high risk for influenza due to high frequency of contacts with other persons. We examined the risk of influenza by occupational group in a railway company. A retrospective observational study was conducted among employees at a branch office of a railway company in eastern Japan, located in a metropolitan area, for each of the 2012/13, 2013/14, and 2014/15 influenza seasons. The study population was employees who had received an influenza vaccination for the season in question and the previous season. Outcome was defined as self-reported influenza illness during the relevant season, identified through the vaccine screening questionnaire in the next season. Study participants were employees from whom we were able to obtain outcome information. Standardized morbidity ratios (SMRs) by occupational group (station staff, engineers, train crew) for each season were calculated. For the 2012/13, 2013/14, and 2014/15 seasons, attack rates were 4.7% (19/403), 5.2% (21/407), and 7.8% (31/397), respectively. Compared with all participants, SMRs of station staff were lower in the 2012/13 (SMR=57; 95%CI=18–133) and 2014/15 (SMR=75; 95%CI=36–138) seasons and similar in the 2013/14 season. Enhanced countermeasures, regardless of occupational group, may be effective in preventing the spread of influenza infection.</p>

    65歳以上の高齢者における市中肺炎の発症と肺炎球菌ワクチンの予防効果を明らかにするため、院内症例対照研究を行った。福岡県の4病院が経験した市中肺炎の新規発症患者70名(男性49名、年齢65〜98歳)(症例群)と、患者ごとに性別と年齢をマッチさせて選出した肺炎以外の外来受診患者146名(男性97名、65〜97歳)(対照群)を研究対象とした。年齢、性別、受診病院で調整した場合、低アルブミン血症は市中肺炎罹患のリスク増大と有意に関連し(調整済みオッズ比(OR)4.15、95%信頼区間(CI)1.76〜9.80)、日常生活活動度は市中肺炎リスクと負に関連していた(「自立可能」対「障害あり」の比較における調整済みOR 0.45、95%CI 0.22〜0.93)。さらにサブグループ解析で低アルブミン血症がみられた集団に限定した場合、ここ5年以内にワクチン接種を受けたことは市中肺炎リスク低減と関連していることが無調整の場合には有意に示されたが(粗オッズ比0.22、95%CI 0.06〜0.86)、先の3因子で調整後には有意なものとしては検出されなかった。高齢者において低アルブミン血症がみられること、および日常生活活動に障害があることは市中肺炎リスクを増大させている可能性が示された。また肺炎球菌ワクチンは低アルブミン血症の高齢者に対し市中肺炎を予防できると考えられた。

    スタチンは動物実験においてステロイド関連骨壊死の発生を抑制することが報告されている。しかし、臨床例において大腿骨頭壊死症(osteonecrosis of the femoral head;ONFH)の発生を抑制できるかは明らかになっていない。本研究の目的はアトルバスタチンがステロイド関連ONFHの発生を抑制するかについてランダム化比較試験を行うことである。初回ステロイド投与を受ける全身性エリテマトーデス患者を対象とし、アトルバスタチン投与を行う治療群(23例)と投与を行わない対照群(24例)に無作為に振り分けた。ONFH発生の有無はステロイド投与から6ヵ月後のMRIで判定した。ONFHは両群ともに6例に発生し、発生率に有意な差はなかった。多変量解析の結果、アトルバスタチンのONFHに対する抑制効果は明らかでなかった。本ランダム化比較試験において、アトルバスタチンは全身性エリテマトーデス患者におけるステロイド関連ONFHの発生を抑制しなかった。(著者抄録)

    <p><b>Objective </b>To retrospectively investigate the clinical manifestations of systemic amyloid light-chain (AL) amyloidosis in Japanese patients and the treatment strategy for the condition. </p><p><b>Methods </b>We conducted a survey of Japanese AL amyloidosis patients, who were treated between January 1, 2012, and December 31, 2014. </p><p><b>Results </b>A total of 741 AL amyloidosis patients were included in this study (436 men and 305 women; median age: 65 years old, range: 31-93). The most frequently affected organ was the kidneys (n=542), followed by the heart (n=252), gastrointestinal (GI) tract (n=164), autonomic nervous system (n=131), liver (n=71), and peripheral nervous system (n=71). Diagnostic findings were most commonly detected in the GI tract (upper GI tract: 350 cases, lower GI tract: 167 cases), followed by the bone marrow and kidneys. An abdominal fat-pad biopsy was only conducted in 128 patients. Autologous stem cell transplants (ASCTs) and bortezomib were used to treat 126 and 276 patients, respectively. </p><p><b>Conclusion </b>The clinical features of Japanese patients with systemic AL amyloidosis are similar to those reported previously for cases in the US and Europe. Regarding treatment, a significant number of ASCTs were performed in Japan as well as in Western countries. Surprisingly, a marked number of patients received bortezomib as a treatment for AL amyloidosis. </p>

    当院で2014年9月から2016年8月までに直接作用型抗ウイルス薬治療を施行したC型慢性肝疾患患者616例及び2007年5月以降に経験したC型急性肝炎患者8例に対し、感染経路の実態を調査し再感染の可能性について検討した。616例のうち感染経路を推定しえた症例は372例(60.4%)であり、その内訳は輸血歴/手術歴/薬物使用歴/家族歴/刺青:189/279/30/18/24例(51.3%/75.8%/8.2%/4.9%/6.5%)であった(重複回答含む)。薬物使用歴、刺青を有する患者の特徴として若年、男性、genotype 2があげられた。また、外来治療を自己中断した症例の特徴も同様に若年、男性、genotype 2であった。観察期間内に再感染例はみられなかったが、今後再感染を予防する上で、これらに対する慎重な経過観察と患者教育が重要な課題である。(著者抄録)

    65歳以上の肺炎患者を症例とし、1症例に対し性と年齢(10歳以内)をマッチさせた、65歳以上の同じ医療施設の非肺炎患者1例から3例を対照とする症例対照研究を行った。データセットに含まれる肺炎患者50例(男性32名、女性18名、平均78歳)と非肺炎患者110例(男性66名、女性44名、平均77歳)全員を肺炎球菌ワクチン接種者32例と非接種者128例に分けた。肺炎球菌ワクチン接種者は非接種者に比べ、75歳以上の後期高齢者、老人ホーム入所者、寝たきり、インフルエンザワクチン接種者の割合が多く、ADL自立の割合が少なかった。肺炎球菌ワクチン接種者は非接種者に比べ、低アルブミン血症在宅酸素療法、肺気腫、慢性気管支炎の割合が高く、心臓病の割合が少なかった。

    目的 第二期がん対策推進計画の柱として、がんの早期発見とたばこ対策の推進がある。しかし、我が国のがん検診受診率は総じて低く、また喫煙率も高い。本研究は、がん死亡リスクの高い喫煙者のがん検診受診状況を把握するため、喫煙習慣と肺・胃・大腸がん検診受診の関連、さらに1日の喫煙本数およびブリンクマン指数(BI)と各がん検診受診の関連を検討した。方法 2011年に大阪市住民(25〜64歳の男女)を対象とした社会生活と健康に関する横断調査を実施した(有効回収率:52.4%)。40〜64歳の2,016人(男性966人、女性1,050人)について、喫煙習慣、喫煙本数およびBIと肺・胃・大腸がん検診受診の関連について、男女別に多変量調整ロジスティック回帰分析を行った。結果 現在喫煙者の肺がん検診受診率は男女ともに50.0%を超えていたものの、各がん検診において、現在喫煙者の受診率は非喫煙者と比較して低かった。喫煙習慣、喫煙本数およびBIと各がん検診受診の関連を検討した結果、男性の現在喫煙者は、非喫煙者と比較して胃および大腸がん検診を有意に受診しておらず、オッズ比はそれぞれ0.71(P=0.036)、0.67(P=0.012)であった。さらに男性では、1日の喫煙本数20本以上およびBI600以上の現在喫煙者は、3種すべてのがん検診を有意に受診しておらず、非喫煙者を基準として20本以上の現在喫煙者のオッズ比は肺0.61(P=0.009)、胃0.61(P=0.009)、大腸0.59(P=0.004)、BI600以上の現在喫煙者は肺0.55(P=0.006)、胃0.62(P=0.028)、大腸0.56(P=0.006)であった。女性では、大腸がん検診受診との間に有意な関連が得られ、非喫煙者に対する1日の喫煙本数20本以上の現在喫煙者のオッズ比は0.39(P=0.004)、BI400以上の現在喫煙者のオッズ比は0.51(P=0.020)であった。結論 喫煙者は非喫煙者に比べてがん検診を受診しておらず、とくに男性においては、喫煙本数が多い者、BIが高い者でその傾向が強かった。喫煙者は検診を受診しないといった意味でもがん死亡リスクが高いと言える。がん死亡リスクの高い者が多く存在する検診未受診者に対してがん検診受診を促進すると同時に、健康やがんについて考える機会である「検診の場」を利用して、喫煙者を対象とした禁煙指導をあわせて実施・推進するべきだと考えられた。(著者抄録)

    目的 大阪府における2014年の麻疹の流行状況を分析し、府内の今後の麻疹対策について検討を行う。方法 2014年に大阪府内で麻疹と報告された46例に府内集団発生事例で感染者の居住地が他府県であった1例を加えた47例について年齢分布、週別患者発生状況、推定感染経路、渡航歴、麻疹含有ワクチン歴、ウイルス検出状況、発症から届出までに要した日数について分析を行った。結果 患者は青年層成人(20〜39歳)が24例(51%)と半数以上を占めていた。患者報告数は2週から27週にかけてピークを形成し、47週に終息した。主な感染経路としては、感染源不明の国内感染が47例中16例(34%)と最多で、次いで家族内感染(26%)、渡航や海外からの輸入事例(21%)の順であった。また患者の83%が接種歴なし、または不明であった。検出ウイルスはB3、H1、D8とすべて海外由来株であった。15歳以上群は15歳未満群に比べ、発症から届出までの日数が有意に長かった(P=0.001)。結論 府内の麻疹を制圧するためには発症から届け出の遅れを最小限にすることが求められる。医療機関、とくに成人を診療する医療機関への啓発が必要とされる。またすべての感受性者に対する麻疹含有ワクチン接種が必要である。(著者抄録)

    Osteonecrosis is a condition of bone death leading to joint pain, bone destruction, and loss of function. Nontraumatic osteonecrosis, a disease entity in which the underlying etiology and pathogenesis have not been fully elucidated, commonly involves the femoral heads. Although a wide spectrum of conditions, including systemic steroid administration, that are potentially related to nontraumatic osteonecrosis of the femoral head (ONFH) have been reported so far, alcohol consumption continues to be important as an environmental factor. In this chapter, we present a summary of the descriptive and analytic epidemiology regarding alcohol-induced ONFH, as well as related issues including potential mechanisms for disease etiology and influences of alcohol-associated factors.

    糖尿病患者はインフルエンザに罹患すると重篤化する懸念がある。当該患者集団でのワクチンの免疫原性の検討は重要である。2009/2010シーズンに、外来通院中糖尿病患者49人にA(H1N1)pdm09ワクチンを2回接種し、解析は48人で行った。免疫原性の指標として幾何平均抗体価(GMT)、平均上昇倍数(MFR)、抗体保有率(SP)、抗体応答率(SR)、抗体陽転率(SC)を算出した。抗体応答との関連因子として接種前HI価、年齢、HbA1cなどについて検討した。ワクチン接種後、いずれのカテゴリーにおいてもGMTの上昇は有意であり、全体の1回目接種後(S1)MFRは5.3倍であった。全例解析でSPはS1で52%、2回目接種後(S2)で54%、SRはS1で69%、S2で77%、SCはS1で46%、S2で48%であった。MFRとSCの結果から、本対象での抗体誘導は欧州医薬品審査庁の基準を満たした。接種前HI価とSPに正の関連、年齢とSRに負の関連を認めたが、糖尿病関連因子と抗体応答との間には有意な関連を認めなかった。本結果は、糖尿病の進行が直ちにワクチンの免疫原性の低下に関与するとは言えないことを示唆し、接種禁忌でない糖尿病患者にワクチン接種を推奨することには合理性があると考えられた。(著者抄録)

    目的 基本健康診査による健診情報を活用し、都市部住民における喫煙習慣と生活習慣病の関連を検討する。方法 複数年度の健診情報を連結した縦断研究である。2001年度に大阪市全24区の保健センター(現:保健福祉センター)で基本健康診査を受診した63,704人のうち、年齢40〜74歳であり、医療機関で疾病治療中でない者を抽出した。さらに、当該年度の健診情報に基づき、以下のいずれかの項目に該当する者を除外した。(1)Body Mass Index≧25kg/m2、(2)空腹時血糖≧126mg/dlまたは随時血糖≧200mg/dl、(3)HDLコレステロール<40mg/dlまたはnon-HDLコレステロール≧170mg/dl、(4)収縮期血圧≧140mmHgまたは拡張期血圧≧90mmHg、(5)問診票の既往歴欄で、「糖尿病・脂質異常症・高血圧・脳卒中・心筋梗塞・狭心症・その他の心臓病」のいずれかについて既往「あり」と回答。曝露要因は、2001年度の健診受診時の喫煙状況、1日の喫煙本数、Brinkman Indexである。結果指標は、2004年度あるいは2005年度の基本健康診査情報(保健福祉センター実施分)に基づき、新規に確認された(a)肥満、(b)糖尿病、(c)脂質異常症、(d)高血圧、とした。解析には多重ロジスティック回帰モデルを用いた。結果 2001年度の健診情報に基づき定義した対象者15,639人のうち、2004年度あるいは2005年度に健診を受診し、各結果指標の有無を追跡できた者は、肥満:9,327人、糖尿病:9,273人、脂質異常症:9,273人、高血圧:9,323人であった(追跡率:約60%)。(a)肥満:喫煙状況、1日の喫煙本数、Brinkman Indexについて有意な正の関連を認めた。(b)糖尿病:男性でのみ、喫煙状況とBrinkman Indexについて有意な正の関連を認めた。(c)脂質異常症:喫煙状況、1日の喫煙本数、Brinkman Indexについて有意な正の関連を認めた。(d)高血圧:喫煙との有意な関連を認めなかった。結論 都市部の基本健康診査受診者において、喫煙が肥満、脂質異常症のリスクを高めるという結果を得た。また、男性で喫煙による糖尿病のリスク上昇が示された。本結果は、今後の特定健康診査において、喫煙に関する保健指導の基礎資料となると期待される。(著者抄録)

    <b>Purpose</b>&emsp;To summarize data on associations between inactivated influenza vaccines and the Guillain-Barr&eacute; syndrome.<br/><b>Methods</b>&emsp;The association between inactivated influenza vaccines and the Guillain-Barr&eacute; syndrome was described and discussed, mainly on the basis of previous studies cited in &ldquo;Prevention and Control of Influenza: Recommendations of the Advisory Committee on Immunization Practices&rdquo;.<br/><b>Results</b>&emsp;The A/New Jersey/76 swine influenza vaccine has been reported to be associated with occurrence of the Guillain-Barr&eacute; syndrome. However, it is unclear whether seasonal inactivated influenza vaccines are associated with the occurrence of the syndrome. According to a study from the United States, which reported a statistically significant association between seasonal inactivated influenza vaccines and Guillain-Barr&eacute; syndrome, the best estimate of maximum attributable risk, after allowing for study limitations, is 1.6 cases per million vaccinations.<br/><b>Discussion</b>&emsp;We could not draw any definite conclusions about any causal relationship between seasonal influenza vaccination and the Guillain-Barr&eacute; syndrome. Therefore we considering the estimated benefits of seasonal influenza vaccination to outweigh the reported risk of subsequent development of the Guillain-Barr&eacute; syndrome.

    目的 不活化インフルエンザワクチン接種とギラン・バレー症候群(GBS)の関連について文献的に考察する。方法 米国予防接種諮問委員会(The US Advisory Committee on Immunization Practices:US-ACIP)の勧告に引用されている文献を中心に、不活化インフルエンザワクチンとGBSの関連についてこれまでの報告を要約するとともに、考察を加える。結果 1976年、米国において接種キャンペーンが実施されたA/New Jersey/76インフルエンザワクチンについてはGBSとの因果関係が明らかであった。その後の季節性インフルエンザワクチンとGBSについては、一貫した論拠は得られなかった。統計学的に有意な関連を報告した文献では、研究の限界を考慮した寄与危険は最大で100万接種あたり1.6例と推定されていた。考察 通常の季節性インフルエンザワクチンとGBSの因果関係について、結論は得られなかった。しかし、これまで報告されているインフルエンザの疾病負担およびワクチン有効性と対比すると、インフルエンザワクチン接種が疾病負担を軽減する有益性は、観察されている季節性ワクチン接種後のGBSのリスクを大きく上まわると考察された。(著者抄録)

    <b>Purpose</b>&emsp;To assess target groups for influenza vaccination.<br/><b>Methods</b>&emsp;The target groups for influenza vaccination specified in the recommendations of the US Advisory Committee on Immunization Practices (ACIP) were summarized and compared between 1997 and 2006.<br/><b>Results</b>&emsp;Principal changes after the 1997's recommendation are as follows: 1) all children aged 6-59 months became included into the high risk group for active vaccination; 2) recommendation for vaccination timing of pregnant women was extended from those at the second trimester or after to all women who are pregnant during the influenza season; 3) persons with neuro- or muscular-disorders and therefore prone to development of respiratory systems or aspiration pneumonia became included into the high risk group; 4) a category with increased risk to visit a clinic, hospital or emergency department due to influenza-related symptoms was newly established, covering children aged 24-59 months and persons aged 50-64 years; 5) healthy household contacts and caregivers of children aged 0-59 months were included into the group who can transmit influenza to persons in the high risk group; 6) people likely to transmit influenza to persons in the high risk group were classified into 3 categories (health-care providers, household contacts and caregivers, and those in close contact with children aged 0-59 months).<br/><b>Conclusion</b>&emsp;The ACIP has gradually expanded the target populations for routine influenza vaccination. The most notable change during past 10 years seems to be the recommendation for inclusion of all children aged 6-59 months and people in close contact with babies for active vaccination.

    インフルエンザワクチンは使用実績が豊富なワクチンであると同時に,その有効性について,長い間議論されてきたワクチンでもある.有効性研究の方法論の歴史もまた長く,ヒトを対象にさまざまな研究デザインによる評価が実施されてきた.2000年代以降は,症例対照研究の一種であるtest-negative designが圧倒的に主流となった.この流れは,インフルエンザと類似症状を呈する新型コロナウイルス感染症のワクチン有効性評価にも引き継がれている.本稿では,test-negative designによるインフルエンザワクチン有効性評価について,その原理の概要や,広く適用されることになった経緯に加え,可能な限り「適切に」有効性を評価するための注意点などについて紹介する.あわせて,test-negative designによるワクチン有効性評価の今後の見通しについて,コロナ禍の影響もふまえて述べる.(著者抄録)

    厚生労働省研究班「子宮頸がんワクチンの有効性と安全性の評価に関する疫学研究」(研究代表者:祖父江友孝)は2016年に全国疫学調査を実施し、「HPVワクチン接種歴のない青少年においても、接種後の"多様な症状"と同様の症状を有する者が一定数存在する」ことを示した。本調査は、「接種者と非接種者の比較はしなかった」という点が大きな特徴である。本稿では、わが国で社会問題となった「多様な症状」について比較を行うことがいかに難しいかについても触れながら、調査の概要について述べる。(著者抄録)

    ＜文献概要＞はじめに　疫学(epidemiology)は,「人集団における健康関連事象(主に疾病)について,頻度・分布と規定要因を明らかにし,その結果を対策に応用する学問」と定義されている.疫学で用いる研究デザインは,おおむね図1のように分類される.すなわち,本項で扱う「記述疫学(descriptive epidemiology)」は,エビデンスレベル(研究結果から得られる根拠の質)がもっとも低い研究デザインに該当する.より質の高い医学研究への興味が高まるなか,特に若手の研究者で,記述疫学研究を軽視する傾向があるかもしれない.しかしながら,記述疫学は,疫学の定義に照らし合わせると「疾病の"頻度・分布"の解明」を担うものである.ある疾病について,患者数などの頻度や,性・年齢などの特性の分布がわからなければ,医療供給体制や行政サービスがどの程度必要か予測できないだけでなく,エビデンスレベルがさらに高い医学研究の計画も立てようがない.すなわち,記述疫学とは,疫学研究デザインの中でも基本の「き」に相当するものであることをまずご理解いただきたい.

    近年、インフルエンザワクチンの有効性は、test-negative design(test-negative case-control design)により、複数シーズンにわたって継続的に効果を評価する方法が主流になっている。わが国でも当該手法による研究結果の報告が増えつつあるが、比較的新しい手法のため、その原理が十分理解されているとはいい難い。本稿では、test-negative designによるインフルエンザワクチン有効性評価の手法について、古典的な疫学研究デザインと対比させながら解説する。インフルエンザワクチン有効性の論拠に無用の混乱が生じることのないよう、研究を実施する側も結果を解釈する側も、test-negative designの原理および関連する疫学の基本について、十分理解する必要がある。(著者抄録)

    現在わが国で市販されている不活化インフルエンザワクチンの「検査確定インフルエンザ」に対する予防効果は,ワクチン株と流行株が良好に合致しているシーズンで有効率50〜60%程度と報告されており,接種により発病リスクを1/2〜2/3程度に下げると考えられる。この数値は「有意」な(=統計学的に意味がある)有効率であるものの,国民が期待する水準を満たしているとは言い難く,批判にさらされやすい。本稿では,インフルエンザワクチンの有効性について,現状と課題を整理する。(著者抄録)

    ＜Key Points＞インフルエンザワクチンは、「効くけれども、効果が分かりにくい」ために「効かない」と批判されがちなワクチンである。ウイルス学的・免疫学的な背景に加えて、「ワクチン有効率」という疫学指標が正確に理解されていないことも一因と考える。わが国における不活化インフルエンザワクチンの有効率は、発病予防を結果指標とした場合、小児で20〜60%、成人で50〜60%と報告されている。有効率の算出原理からみると、「非接種者でも抗体を有していること」の影響は大であり、効果を検出しにくい理由となっていることが分かる。ワクチンの効果を医療者の立場から正しく情報提供していくためにも、疫学指標を適切に解釈する知識が望まれる。(著者抄録)

    インフルエンザワクチンの有効性評価は、例え無作為化比較対照試験の結果であっても普遍性を有しない。従って良質の観察研究を積み上げることが重要であるが、妥当性のある結果を得るためには交絡因子を考慮することが必須である。非接種者にも抗体保護者が多数含まれるという研究環境を克服するために、antibody efficacyを求める方法が考案された。近年、有効性を統一的手法により継続的にモニタリングするため、testnegative designによる症例対照研究が行われるようになった。インフルエンザワクチンの有効性研究手法は、未だ発展途上にある。インフルエンザワクチンの適用に関し、WHOは妊婦を最優先とするよう提示したが(2012)、本邦ではその要否を判断するために調査が進行中である。(著者抄録)

    近年、炎症性腸疾患の有病率および罹患率が上昇しており、その背景には生活習慣の関与も示唆されている。「潰瘍性大腸炎」に対しては、「虫垂切除歴」によるリスク低下、「禁煙」によるリスク上昇などの報告がある。しかし、「飲酒」や「食習慣」に関しては、報告数が少なく、必ずしも明確な結論が得られていない。「Crohn病」に対しては、「虫垂切除歴」や「喫煙」によるリスク上昇などの報告がある。しかし、「虫垂切除歴」に関しては、「Crohn病」の初期症状としての腹痛により「虫垂切除」を受けたことが「Crohn病」との関連を示したにすぎない(reverse causality)という可能性もある。また、「飲酒」や「食習慣」に関しては、いまだに報告数が少ない。このように、炎症性腸疾患の原因は、いまだ不明な部分が多い。しかし、新たな発症者を可能な限り予防するため、公衆衛生上の観点からも、疾患のリスク因子を早急に解明することが急務である。(著者抄録)

    2009年4月にメキシコとアメリカで感染者が確認された新型インフルエンザであるA/H1N1pdmは瞬く間に世界中に広がり、本邦でも同年5月には患者が確認された。インフルエンザワクチンの有効性評価のための疫学研究の原理と方法について述べ、著者等が2009年のパンデミック時に経験した新型インフルエンザワクチンの免疫原性と有効性の調査研究について報告した。本稿で詳述した研究全てにおいて、接種前抗体価が高いほど、ワクチン接種後の免疫原性が低いという関連が認められた。発症予防効果を主要評価項目とするワクチンの有効性評価の際には、アウトカムの精度および正確度が重要である。特に、後方視的調査ではバイアスを伴いやすいので十分な注意が必要である。

    特発性大腿骨頭壊死症(ION)の記述疫学に関する直近の全国調査として、2004年1年間の受療患者について調査した「全国疫学調査」の結果を概説する。一次調査の結果、2004年1年間の受療患者数は11,400人(95%信頼区間:10,100-12,800)、新患数は2,200人と推定され、ともに10年間で約1.5倍に増加していた。二次調査の結果、確定診断時年齢のピークは40歳代であり、40歳未満が39%を占めていた。誘因の分布は、「ステロイド全身投与歴あり(ステロイド性)」51%、「アルコール愛飲歴あり(アルコール性)」31%、「両方あり」3%、「両方なし」15%であり、40歳未満に限るとステロイド性は60%であった。ION関節のうち60%に手術が施行されており、術式の内訳は、人工物置換術(人工骨頭・人工関節置換術)が65%、骨切り術は25%であった。40歳未満に限った場合も、骨切り術は38%にとどまり、人工物置換術が45%を占めていた。(著者抄録)

    潰瘍性大腸炎は原因不明の難治性炎症性腸管障害であり,経過中に寛解と再燃を繰り返す者が多い.診断後1年間の再燃率は約30〜50%,5年では約60〜70%と報告されている.しかし,再燃率は診断時の病態により影響を受けるため,報告により若干のばらつきがある.再燃に影響する因子として,診断時の年齢,病変部位,重症度などがある.しかし,その他の因子については再燃因子というには論拠が希薄である.また,家族歴,喫煙,虫垂切除歴など潰瘍性大腸炎の発生に関連する因子は,必ずしも再燃に強く影響しているわけではなさそうである.再燃に影響する因子の解明は,患者の寛解維持効果を向上させるためにも重要な検討課題である.(著者抄録)

    "A Dictionary of Epidemiology"によると、疫学は、「特定の集団における健康関連事象について、その分布あるいは規定要因を調査し、健康問題の対策に応用すること」と定義されている。現在、わが国の死因の約6割は、がん、心疾患、脳血管疾患などの生活習慣病が占めるものの、結核をはじめとする感染症は依然として私たちの健康を脅かしている。また、2009年4月以降、世界的に流行している新型インフルエンザにより、感染症に対する社会的な関心が高まっている。感染症制御のための対策は、三大要素である「感染源(病原体)、感染経路、感受性宿主」のいずれかに対して行うものであるが、「病原体が明らかであってこそ対策が可能」ととらえがちである。しかし、病原体が必ずしも明らかでなくとも、疫学手法の適用により公衆衛生の観点から対策を講じることができる。本稿では、過去の歴史的・代表的事例を取り上げ、感染症制御における疫学の役割に触れてみたい。(著者抄録)

    西太平洋地域において集団発生や重症例・死亡例が報告されている手足口病について、公衆衛生対策と臨床マネジメントに関するガイドラインの作成に参画し、感染症疫学の専門家として“Epidemiology”セクションのlead writerとcoordinatorを務めた。当該ガイドラインは2011（平成23）年に公表され、標準的な指針として世界で広く活用された。
