    Although endometrial cancer is a common malignancy in women, rare histological subtypes can pose diagnostic challenges. Primary endometrial intestinal-type mucinous carcinoma is a newly recognized subtype of endometrial cancer that differs from Müllerian-type endometrial mucinous carcinoma. The present case report documents a rare case of intestinal-type mucinous carcinoma of the endometrium showing a polypoidal exophytic form. The patient, an 80-year-old female, was incidentally diagnosed with a uterine tumor during a follow-up for vulvar Paget's disease. Clinical and imaging examinations revealed a localized mass within the uterine cavity. Hysteroscopy and subsequent histological examination confirmed the presence of intestinal-type mucinous carcinoma of the endometrium. Microscopically, the tumor displayed adenocarcinoma containing an intestinal-type glandular epithelium with mild nuclear atypia. It stained positive for the gastrointestinal markers mucin 2 and caudal type homeobox 2, and stained negatively for estrogen receptor α. The patient underwent surgery and adjuvant chemotherapy, with no evidence of recurrence at the latest follow-up 6 months after surgery. Endometrial intestinal-type mucinous carcinoma is a rare histological subtype of endometrial cancer. Differential diagnoses include Müllerian-type endometrial mucinous carcinoma, endocervical adenocarcinoma, metastasis from gastrointestinal tract adenocarcinoma and non-neoplastic gastric/intestinal metaplasia. However, the prognosis of endometrial intestinal-type mucinous carcinoma remains unclear due to limited reported cases. Existing evidence suggests a poorer prognosis compared with classical mucinous carcinomas of the endometrium. The present case, which is characterized by a polypoidal exophytic tumor without myometrial invasion, showed a favorable outcome. Further documentation and characterization of the aforementioned rare malignancy are necessary to enhance the understanding of its clinical physiology and outcomes. The present case report highlights the diagnostic challenges associated with intestinal-type mucinous endometrial carcinoma. The inclusion of this type of malignancy in the latest World Health Organization classification emphasizes the need for further comprehensive studies and case reports to expand the current knowledge on this rare histological subtype.

    According to the National Comprehensive Cancer Network clinical practice guidelines of cervical cancer, concurrent chemoradiotherapy or radiotherapy is suggested for patients who receive radical hysterectomy and have intermediate- and high-risk cervical cancer. However, adjuvant chemotherapy has been increasingly chosen given the adverse events associated with chemoradiotherapy or radiotherapy and the increase in evidence regarding the efficacy of adjuvant chemotherapy. Given that adjuvant chemotherapy is not a standard treatment at present, if recurrence after adjuvant chemotherapy could be predicted, it would assist the decision of gynecological oncologists selecting which adjuvant therapy (chemotherapy or radiation therapy) to use. Cleft lip and palate transmembrane protein 1-like protein (CLPTM1L; also known as cisplatin resistance-related protein 9) is associated with apoptotic mechanisms and is related to the proliferation of the tumor cells and resistance against chemotherapy. In the present study, the association between CLPTM1L expression and recurrence of intermediate- and high-risk stage IB-IIB cervical cancer in patients undergoing radical hysterectomy followed by treatment with cisplatin and paclitaxel (TP) as adjuvant chemotherapy was determined. Patients were divided into two groups: Recurrence group and no-recurrence group. CLPTM1L expression was examined using immunohistochemistry in paraffin-embedded sections using weighted scores. Regarding the characteristics of the patients, a histology of non-squamous cell carcinoma, lymph node metastasis and parametrium invasion were more common in the recurrence group compared with the non-recurrence group. In the recurrence group, CLPTM1L expression was significantly higher than that in the no-recurrence group. Next, patients were divided into low and high-expression groups based on the weighted score with a cut-off value of 6. In the high expression group, patients exhibited a higher rate of recurrence (37.5 vs. 5.1%) and had worse overall survival. Multivariate analysis revealed that high CLPTM1L expression was independently related to recurrence. In in vitro analysis, small interfering RNA-mediated knockdown of CLPTM1L enhanced the sensitivity of cervical cancer cells to cisplatin. In conclusion, the present study revealed that CLPTM1L expression may be a predictive biomarker of recurrence of intermediate- and high-risk stage IB-IIB cervical cancer in patients undergoing radical hysterectomy followed by TP as adjuvant chemotherapy.

    BACKGROUND/AIM: The efficacy and safety of bevacizumab for ovarian cancer have been reported in randomized phase III clinical trials. It is important to gather experience and data in a real-world setting. The objective of the present study was to evaluate the efficacy and safety of bevacizumab for patients with ovarian cancer in a real-world setting. PATIENTS AND METHODS: For front-line settings, patients with FIGO stage III-IV ovarian cancer treated using bevacizumab and chemotherapy after debulking surgery (Chemo + Bev group, n=79), in addition to those treated with only chemotherapy after debulking surgery (Chemo group; n=66), at our institute were reviewed retrospectively. For recurrent settings, patients with recurrent ovarian cancers treated with bevacizumab and any chemotherapy were reviewed retrospectively (n=65). RESULTS: In the front-line setting, the disease-free survival was significantly longer in the Chemo + Bev group compared with that in the Chemo group (p=0.021). Hypertension and proteinuria were found to be statistically more frequent in the Chemo + Bev group compared with that in the Chemo group (p=0.002 and p=0.004). In the recurrent setting, in platinum-sensitive patients, the response rate (RR) and the disease control ratio (DCR) were 78.4 and 94.1%, respectively. In platinum-resistant patients, the RR and the DCR were 28.6 and 57.1% respectively. The median progression-free survival was 18.3 and 7.1 months for platinum-sensitive recurrence and platinum-resistant recurrence, respectively. The major ≥ grade 3 adverse event was neutropenia. CONCLUSION: The present study provided encouraging real-world evidence of the efficacy and safety of bevacizumab for ovarian cancer in real-world.

    2014年4月1日から2020年12月31日までにベバシズマブを投与した卵巣癌、腹膜癌、卵管癌患者計71例(Bev投与群)とベバシズマブを使用していない時期(2008年1月1日から2014年3月31日まで)に治療した76例(Bev非投与群)を対象に、治療効果、予後および有害事象をBev投与群(TC+Bev群)とBev非投与群(TC群)で比較した。対象はTC+Bev群71例、TC群76例で、患者背景に有意差はなかった。最良効果判定では、奏効率および病勢制御率でTC+Bev群で有意に良好であった。無病再発期間では、全観察期間において有意差はなかったが、Bevによる維持療法中は、TC+Bev群で有意に良好であった。全生存期間については、全観察期間および維持療法期間で有意差はなかった。Grade3以上の有害事象の発症率は両群間で有意差はなかった。TC+Bev群におけるベバシズマブに特徴的な有害事象は、尿蛋白5例、高血圧8例、消化管穿孔3例などを認めた。これらの有害事象でベバシズマブ投与を中止した患者は71例中9例であった。

    当院における75歳以上の高齢子宮体癌患者に対する治療について、2015年1月〜2019年12月に当院で入院加療を行った当該患者44例(75歳〜92歳)を対象に検討した。その結果、44例中40例に手術を施行し、全例に単純子宮全摘出術、両側付属器摘出術を行い、再発中リスク群16例のうち9例では術後化学療法を施行しなかった。術後化学療法を施行しなかった9例中再発例は2例(22%)で、75歳未満の中リスク群で術後化学療法をしなかった5例における再発率(20%)と大差なく、75歳以上の子宮体癌中リスク群においても、術後化学療法を施行しない選択肢も考慮してよいと考えられた。

    抗NMDA受容体脳炎は2007年にDalmauによって提唱された若年女性に好発する自己免疫性辺縁系脳炎であり、腫瘍、とくに卵巣奇形腫に随伴することが多い。今回、明らかな卵巣腫大は認めなかったが、抗NMDA受容体抗体が陽性であり、画像検査にて右卵巣にわずかに脂肪成分を疑う部位を認めたため腹腔鏡手術を施行し、卵巣奇形腫の診断に至った症例を経験したので報告する。19歳1妊未産の女性。当院に入院する11日前から感冒症状が出現。入院4日前の夕に異常行動と全身痙攣を認め前医救急搬送となった。頭部MRIに異常所見は認められなかったが、意識障害の進行と不随意運動を認め脳炎が疑われたため当院神経内科へ紹介となった。その後、ICUへ入室し人工呼吸器管理となった。頭部造影MRIで大脳辺縁系にT2WI高信号域を認め、辺縁系脳炎と考えられた。入院後7日目に抗NMDA受容体抗体が検出され、骨盤部MRIとCTを撮像も明らかな卵巣の腫大は認めなかったが、微小な奇形腫の可能性も考慮し当科紹介となった。当科にて再度MRIと造影CTを撮影したところ、右卵巣に明確ではないがごくわずかに脂肪成分を疑う部位を偶発的に認めたため、腹腔鏡下右付属器摘出術を施行し右卵巣内に割を入れたところ、ごくわずかな脂肪組織を含む腫瘍(大きさ3mm)を認め、摘出標本の病理検査結果は未熟奇形腫grade 1であった。その後、人工呼吸器を離脱し徐々に意識状態は改善した。今回われわれは、微小な卵巣未熟奇形腫に伴う抗NMDA受容体脳炎に対して画像検査を重ねることで微小な病変を疑い、腹腔鏡手術に踏み切り診断と回復に至った1例を経験した。抗NMDA受容体脳炎は意識障害や呼吸障害など重篤な臨床経過をたどることが多く、画像上検出が困難な微小な腫瘍でも本疾患を発症する可能性があることを念頭において、複数回の画像検査実施あるいは撮影条件を変更して画像検査を再検討する必要があると考えられた。(著者抄録)

    症例は71歳3妊3産で、画像所見より左卵巣成熟奇形腫を疑って腹腔鏡下両側付属器摘出術を開始したところ、漿膜下に子宮腫瘍および腹膜に癒着した左卵巣を認めた。術中所見およびMRI所見より子宮脂肪平滑筋腫と判断し、腹腔鏡下単純子宮全摘術および両側付属器摘出術を行った。

    <p>  Ovarian mature teratomas present various risks, such as acute abdomen, and hence surgery is the usual protocol. On the other hand, uterine lipoleiomyoma is a variant of uterine leiomyoma, and surgery is not always performed in postmenopausal cases. We report a case of postmenopausal uterine lipoleiomyoma, preoperatively diagnosed as ovarian mature teratoma, and treated surgically. A 71-year-old woman presented a 5.5-cm mass on the left dorsal side of the uterus that was fat suppressed on magnetic resonance imaging. The preoperative diagnosis was mature teratoma of the left ovary.</p><p>  Laparoscopic bilateral salpingo-oophorectomy was planned, but we changed the operative procedure to laparoscopic hysterectomy and bilateral salpingo-oophorectomy. This was because the mass was identified as a uterine mass as no swelling was observed in either ovary during surgery. Pathological examination of the excised specimen revealed lipoleiomyoma. It may be difficult to distinguish uterine lipoleiomyomas from ovarian mature teratomas when the lipoleiomyomas are subserosal. In this case, the tumor was presumed to have originated from the uterus based on the findings of the pelvic examination, and we were able to discuss the operative procedure options in advance. One of the merits of laparoscopic surgery in this case was that the burden of changing the surgical procedure was less than that with laparotomy; however, at the same time, preoperative exclusion of malignant disease was considered important. In the diagnosis of tumors containing fat, the differential diagnosis of lipoleiomyoma should be considered.</p><p></p>

    <p>Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis is an autoimmune limbic encephalitis that is prevalent in young women with ovarian teratoma. In the present study, we report a case of symptomatic improvement after laparoscopic right salpingo-oophorectomy for anti-NMDA receptor antibody encephalitis associated with a micro ovarian immature teratoma without obvious ovarian enlargement on imaging. A 19-year-old woman was admitted due to seizures to our hospital. She was transferred to our neurology department due to the progression of disorientation and involuntary movements, and encephalitis was suspected. Seventh day of hospitalization, anti-NMDA receptor antibodies were found to be positive. MRI and CT showed an area in the right ovary that was undefined but very faintly suspicious of fatty components, suggesting the presence of a micro ovarian teratoma. The patient underwent a laparoscopic right salpingo-oophorectomy. Pathological examination was immature teratoma, Grade1. Macroscopically, a right ovarian tumor (three mm in size) containing a small amount of fatty tissue was observed. The patient was weaned off the respirator on the fourth postoperative day. Her conscious level gradually improved and she was able to communicate. Careful scrutiny is necessary, bearing in mind that even microscopic teratomas that are difficult to detect on imaging can cause the disease. [Adv Obstet Gynecol, 73 (1) : 68-72, 2021 (R3.2)]</p>

    ベセスダシステムではAGC(異型腺細胞)の判定は特定の前癌病変を指すのではなく、癌へのリスクの高さを示す診断カテゴリーとされるが、基準が定まらず管理に苦慮することが多い。当院におけるAGCの管理(精査・フォローアップの方針)は、コルポ下生検と頸管内膜生検に加え体部内膜生検も積極的に行い、AGCを反復する症例で生検を複数回行っても診断がつかない場合には診断的円錐切除術を考慮する。今回、当院がベセスダシステムに完全移行した2015年から2017年までにAGCと判定された症例の転帰と臨床的取り扱いについて後方視的に検討した結果、当院のAGC管理は妥当と考えられた。

    症例は64歳女性で、高血圧、糖尿病、慢性腎不全の病歴があり、両側卵巣腫瘍のため当院に入院となった。身体検査で眼瞼結膜貧血、下肢浮腫、下腹部に圧痛を伴う腫瘍が認められ、MRIで右卵巣に13cm、左卵巣に6cmの嚢胞性腫瘤が検出された。胸部-骨盤部の単純CTまたは骨盤MRIでは転移は認められなかったが、診断および病期確定目的で開腹術を施行した際の迅速病理診断で右卵巣の粘液癌が明らかになった。両側卵管卵巣摘出術、単純子宮摘出術、骨盤・傍大動脈リンパ節郭清、大網部分切除術を施行した。転移や血管/リンパ管浸潤はみられず、ステージIA卵巣癌と診断した。術後1ヵ月時に背部痛が出現し、腫瘍マーカーが再度上昇したためシンチグラフィーを行ったところ、骨転移を検出した。食道胃十二指腸内視鏡検査で胃内に頂点部が潰瘍化した10mmの隆起病変が認められ、生検検体の免疫染色により本病変は卵巣癌の転移病変であることが示された。化学療法を追加したが、3ヵ月後に死亡した。

    症例は64歳女性で、高血圧、糖尿病、慢性腎不全の病歴があり、両側卵巣腫瘍のため当院に入院となった。身体検査で眼瞼結膜貧血、下肢浮腫、下腹部に圧痛を伴う腫瘍が認められ、MRIで右卵巣に13cm、左卵巣に6cmの嚢胞性腫瘤が検出された。胸部-骨盤部の単純CTまたは骨盤MRIでは転移は認められなかったが、診断および病期確定目的で開腹術を施行した際の迅速病理診断で右卵巣の粘液癌が明らかになった。両側卵管卵巣摘出術、単純子宮摘出術、骨盤・傍大動脈リンパ節郭清、大網部分切除術を施行した。転移や血管/リンパ管浸潤はみられず、ステージIA卵巣癌と診断した。術後1ヵ月時に背部痛が出現し、腫瘍マーカーが再度上昇したためシンチグラフィーを行ったところ、骨転移を検出した。食道胃十二指腸内視鏡検査で胃内に頂点部が潰瘍化した10mmの隆起病変が認められ、生検検体の免疫染色により本病変は卵巣癌の転移病変であることが示された。化学療法を追加したが、3ヵ月後

    <p>Gastric metastasis from ovarian cancer has rarely been reported. We herein report the case of a 64-year-old woman with gastric metastasis from ovarian cancer that was diagnosed as surgical stage IA. Diagnostic and staging laparotomy showed mucinous carcinoma of the right ovary. At one month after surgery, bone metastasis was detected via scintigraphy. On esophagogastroduodenoscopy, a 10-mm elevated lesion with ulceration on the top was seen in the stomach. The immunohistochemical analysis of biopsy specimens showed that these metastases arose from ovarian cancer. We recommend that physicians remain aware of the possibility of gastric metastasis in patients with ovarian cancer. </p>

    当院で後腹膜リンパ節生検または郭清を含む婦人科悪性腫瘍手術を施行した95例(年齢中央値55歳)を対象に、リンパ嚢胞発生のリスクファクターについて検討した。その結果、リンパ嚢胞発生率は35%、症候性リンパ嚢胞の発生率は4%であった。リンパ嚢胞発生のリスクファクターとしては年齢、BMI、摘出リンパ節数、鼠径上リンパ節摘出の有無、傍大動脈リンパ節摘出の有無、リンパ節転移の有無が明らかになった。リンパ浮腫のリスク因子はリンパ嚢胞のリスク因子と重複しており、リンパ浮腫を管理すると同時にリンパ嚢胞の有無を確認し、症候性リンパ嚢胞に対しては速やかに適切に対応することで、QOLを損ねず、がん治療がスムーズに継続できると考えられた。

    Background: Krukenberg tumors mostly occur after 40 years. Metastatic ovarian tumors in young age are very rare. Case: A 18-year-old female presented with colon cancer which was accompanied by Krukenberg tumor. The present case was a very rare case of metastatic ovarian tumor in very young age. The present patient presented with abdominal pain. On examination, colon tumor was detected and bilateral ovary were almost normal with only slight swelling. During the operation for colon tumor, biopsy of bilateral ovary was performed for histopathological evaluation. Although there were no specific findings in bilateral ovary, microscopic examination revealed poorly differentiated adenocarcinoma, diffusely invading the ovarian parenchyma. Diagnosis of colon cancer was made postoperatively and ovarian Krukenberg tumor was confirmed. Conclusion: In case of suspecting colon cancer even in very young patient with normal ovary, biopsy of ovary should be considered for the diagnosis of Krukenberg tumor.

    Nongestational choriocarcinoma is very rare and carries a poor prognosis in female patients. In this report; the authors present a case of nongestational choriocarcinoma with brain metastasis in a female. A 58-year-old female with intermittent back pain was referred to a private hospital. On examination, a mediastinal tumor and a pancreatic tumor were detected. Endoscopic ultrasound-guided fine needle aspiration biopsy of the tumor was performed for histological evaluation. Pathological diagnosis was difficult because only a small amount of tissue was collected. Head MRI showed multiple metastatic tumors in the brain. The patient was diagnosed with primary mediastinal choriocarcinoma with brain metastasis. She was treated with one course of an etoposide, methotrexate, dactinomycin, cyclophosphamide, and vincristine regimen, but her general condition gradually deteriorated, and she died on day 41. Nongestational choriocarcinoma is drug resistant, whereas gestational choriocarcinoma has better chemotherapeutic sensitivity.

    Objectives: Chemotherapy-induced thrombocytopenia seems to be a relevant problem and the risk of clinical bleeding in patients with gynecologic malignancy is reported to be higher than other malignancy. In this study, the authors investigated chemotherapy-induced thrombocytopenia recently performed in all patients with gynecologic malignancy. Materials and Methods: Between January 2009 and December 2011, the authors examined reported chemotherapy-induced thrombocytopenia using the Common Terminology Criteria for Adverse Events (CTCAE) v.4.0. They analyzed the incidence and clinical features of chemotherapy-induced thrombocytopenia in patients with gynecologic malignancy. Results: During this period they administered over 1,614 infusions (29 regimens) to 291 patients. Chemotherapy-induced thrombocytopenia occurred in 43 (14.8%) patients over 56(3.5%) chemotherapy cycles. Bleeding occurred in 13 (4.5%) patients over 14 (0.9%) cycles. Platelet transfusions were administered for eight (2.7%) patients over eight (0.5%) cycles. Median platelet count at platelet transfusions was 17,000 /mu l Chemotherapy-induced thrombocytopenia was associated with more than five previous chemotherapy cycles, previous radiotherapy, disseminated disease, distant metastatic disease, poor performance status, and taxane-including regimens. Clinical bleeding was associated with previous radiotherapy, distant metastatic disease, poor performance status, and taxane-including regimens. Conclusions: Estimating bleeding risk factor such as previous radiotherapy, distant metastatic disease, poor performance status, and taxane-including regimens seem to be important for safe management of chemotherapy-induced thrombocytopenia.

The authors investigated and compared clinical findings, treatments, and outcomes of patients with PPS and those with epithelial ovarian cancer (EOC) patients. Materials and Methods: The authors retrospectively reviewed data from 14 patients with PPC and 219 patients with EOC treated at the present hospital from January 2005 to December 2012, including demographic data, pathologic findings, treatments, and outcomes. Results: Patients with PPC were significantly older (62.6 +/- 8.4 years) than those with EOC (56.3 +/- 11.3 years) (p = 0.045). There was no significant difference in serum CA-125 levels. The five-year survival rates did not differ significantly between patients with PPC (61.1%) and those with EOC (60.3%; p = 0.78); nor between patients with PPC and those with Stage III serous EOC (43.8%; p = 0.40). Conclusions: Treatment strategies for EOC applied to PPC apparently led to similar survival patterns among the two patient groups. Cytoreductive surgery combined with pre/postoperative platinum-containing chemotherapy may be effective for PPC patients.

    Background: Anti-N-methyl D-aspartate (anti-NMDA) receptor encephalitis is often accompanied by ovarian teratoma. Early tumor resection is reported to be effective as a treatment. Case: A 21-year-old woman presented with anti-NMDA receptor encephalitis which was accompanied by ovarian teratoma. The present case was a very rare case of an early stage of anti-NMDA receptor encephalitis receiving operative treatment before confirming the presence of anti-NMDA receptor antibody. The diagnosis was established postoperatively by identifying anti-NMDA receptor antibody.     腹式単純子宮全摘出術後に頻尿や尿意切迫感を訴える患者が存在し活過動膀胱(OAB)と診断されることがある。そこで、腹式単純子宮全摘出術が排尿に与える影響を調査し、術後にOABが出現する頻度を検討する。2011年1月から2012年1月までに当施設で腹式単純子宮全摘術を施行した46例を対象とし、術前、術後3日目、術後7日目にそれぞれ1日の尿量、尿回数、飲水量、尿失禁の回数、尿失禁量を測定し、またOAB症状質問票(OABSS)を用いてOABの有無を診断した。術前、術後3日目、術後7日目のそれぞれの平均一日尿量は1538ml、1280ml、1537ml、平均尿回数は8.4回、7.7回、8.5回、平均飲水量は1338ml、1122ml、1261mlと術後3日目で有意に低値となった。術前、術後3日目、術後7日目のそれぞれのOABSS合計点数の平均は1.5、1.8、1.6で有意差を認めなかったが、41例中2例(4.9%)で術後にOABと診断された。頻度は低いが腹式単純子宮全摘術の合併症としてOABの存在を認識する必要があることが示唆された。(著者抄録)

    当科で手術を行った卵巣上皮悪性腫瘍119例を対象に、組織型別の術中腹水細胞診陽性率および予後について検討した。その結果、全進行期における細胞診陽性率は、漿液性腺癌で66.7%、明細胞腺癌で57.6%と高く、類内膜腺癌では29.0%と漿液性腺癌および明細胞腺癌と比べ、有意に低い結果となった。I・II期とIII・IV期に分けて検討した場合、類内膜腺癌では進行例でも陽性率43.8%と低く、漿液性腺癌および明細胞腺癌の陽性率と有意差を認めた。粘液性腺癌では陽性率と進行期との間に強い正の相関関係が示された。細胞診陽性率と予後との検討では、明細胞腺癌において陽性例は有意に予後不良であることが確認された。

    育児ストレスを背景とした情緒不安定な女性10症例(年齢27〜43歳、平均年齢36.3歳)に対して第一選択薬として四逆散エキスを用い、その有用性について検討した。漢方医学的所見では対象は四逆散の使用目標である腹力中等度を全例が満たしていた。その他、手掌発汗を60%、胸脇苦満を50%、心下痞硬を40%、腹直筋緊張を50%で認められた。1)四逆散エキス製剤7.5g/日の2週間投与による主訴の効果判定では著効2例、有効4例、やや有効3例、無効1例、悪化0例であり、やや有効以上が90%であった。2)残尿管や便秘、腹部膨満感や軟便などの便通異常のある8例中5例が改善傾向が認められたほか、肩こりと不眠が各7例中6例で軽減されていた。3)有害事象は1例で服用後、脱力感を感じたが、数時間で消失した。

    28歳女性(看護師)。月経は初潮時(11歳)より28歳まで2〜3ヵ月に1回であったが、無月経が3ヵ月以上持続したため前医を受診、基礎体温(BBT)は一相性、月経3日目で経腟超音波所見では両側卵巣は直径約3cm大で多嚢胞性卵巣(PCO)であった。テルグリド(1mg/日)の処方により6ヵ月後、プロラクチンは56.4ng/mlから10.4ng/mlに低下したが無月経は持続、当帰芍薬散を併用したが月経異常とPCOは改善せず来院となった。初診時、主な訴えは足のむくみ、冷え、便秘、肥満、不眠であり、当帰芍薬散(7.5g/日)+加工附子末(1.5g/日)+防風通聖散(1.7g/日)+サフラン(0.3g/日)を処方、その後、肥満改善のため防風通聖散を1.7gから3.3g/日に増量し、無理のないダイエットと軽い運動を指導した。その結果、6ヵ月後体重は10kg減となり月経が再開、初診から1年後体重は17kg減となり月経周期はほぼ30日に1回と定期的になり、経腟超音波上のPCOは消失してBBTは二相性になった。目下、初診より7年経過で体重コントロールとほぼ30日に1回の規則的な月経が維持されている。以上、本症例の臨床経過からも治療が困難とされる多嚢胞性症候群に対し、抗肥満作用のある防風通聖散は有用と考えられた。

    育児ストレスを背景とした情緒不安定な女性10症例(年齢27〜43歳、平均年齢36.3歳)に対して第一選択薬として四逆散エキスを用い、その有用性について検討した。漢方医学的所見では対象は四逆散の使用目標である腹力中等度を全例が満たしていた。その他、手掌発汗を60%、胸脇苦満を50%、心下痞硬を40%、腹直筋緊張を50%で認められた。1)四逆散エキス製剤7.5g/日の2週間投与による主訴の効果判定では著効2例、有効4例、やや有効3例、無効1例、悪化0例であり、やや有効以上が90%であった。2)残尿管や便秘、腹部膨満感や軟便などの便通異常のある8例中5例が改善傾向が認められたほか、肩こりと不眠が各7例中6例で軽減されていた。3)有害事象は1例で服用後、脱力感を感じたが、数時間で消失した。

    子宮頸部と体部に腫瘍が存在し、原発部位が頸部か体部かの判断が困難な場合、腫瘍が扁平上皮癌であれば頸癌、腺癌なら体癌と診断することが多い。今回著者等は、頸部と体部に腫瘍を認め、頸部腫瘍生検の結果が扁平上皮癌であったため頸癌の診断で手術を行ったところ、摘出標本検査の結果は体部腺扁平上皮癌であった症例を経験した。本例からの教訓として、頸部生検の結果が扁平上皮癌であっても、体部の生検を行う必要があったと考えられた。

    Claudin-4は細胞間接着装置であるtight junctionの主要な構成分子であり、卵巣癌の発癌・進展に重要な役割を担っていると報告されているが、抗癌剤耐性機序への関与については不明である。そこで今回claudin-4と卵巣癌のcisplatin耐性との関連について検討した。卵巣癌細胞株のclaudin-4発現をsiRNAにて抑制し、cisplatinへの感受性変化を検討した。さらにclaudin-4発現とcisplatinの細胞内蓄積量の相関についても検討した。またinformed consentを得た卵巣癌症例の手術切除標本を用いて、claudin-4発現とプラチナ製剤を中心とした化学療法の効果および患者予後との関連を免疫組織学的に検討した。Claudin-4発現抑制により、卵巣癌細胞株のcisplatin感受性が増大し、またcisplatinの細胞内蓄積量の増加がみられた。一方、卵巣癌臨床検体におけるclaudin-4発現を化学療法奏効例と無効例において比較したところ、無効例で有意にその発現が増大していた。またclaudin-4発現陽性症例は陰性症例と比較して有意に予後不良であった。以上よりclaudin-4は卵巣癌のcisplatin耐性機序に関与しており、また予後因子となる可能性が示唆された。(著者抄録)

    Purpose of investigation: Chemotherapy-related hypersensitivity reaction seems to be problematic in the safe management of chemotherapy. In this study we investigated chemotherapy-related hypersensitivity reaction in patients with gynecologic malignancy. Methods: Between January 2009 and December 2010, we examined hypersensitivity reaction (&gt;= grade2) using the Common Terminology Criteria for Adverse Events (CTCAE) v.4.0. We analyzed the incidence, clinical features, management, and outcome. Results: We administered over 1,057 infusions (24 regimens) to 205 patients. We found a total of four hypersensitivity reactions (a grade 2) cases (carboplatin: 2; nedaplatin: 1; docetaxel: 1). Signs and symptoms were varied. In two cases, the same regimen was rechallenged by using anti-allergic drugs. The docetaxel case was successful. The carboplatin case was not successful. Conclusion: Chemotherapy-related hypersensitivity reaction (&gt;= grade2) does not occur frequently. In the case of platinum, especially, carboplatin, re-administering after hypersensitivity reaction should be done carefully though platinum is a key drug in patients with gynecologic malignancies.

    70歳未満の子宮頸癌III期に対し、主治療前化学療法(NAC)を施行した72例(平均50.5歳:N群)と、同時化学放射線治療を主体とした39例(平均56.1歳:R群)の治療成績を比較した。年齢には両群間で有意差を認めたが、進行期、組織型に有意差はなく、全生存率にも有意差は認めなかった。次に、N群をNACにより腫瘍が縮小し手術施行・術後放射線治療を追加した46例(平均49.3歳:NAC+OP+R群)と、NAC後も手術施行できず放射線治療のみ行った26例(平均52.7歳:NAC+R群)に分け、R群と比較した。年齢には3群間で有意差を認めたが、進行期、組織型に有意差はなく、全生存率はNAC+OP+R群がNAC+R群に比較して有意に良好であった。進行子宮頸癌に対するNACは、手術を施行できれば予後改善につながる可能性があることが示唆された。

    月経前の情緒不安定な症状に対し甘麦大棗湯が有効であった14例について検討した。年齢は20〜40歳代で、いずれも月経前症候群(PMS)に典型的な症状を有していた。甘麦大棗湯の処方目標である「欠伸」は、11例で月経前に訴えが多かった。漢方医学的所見では、全例に舌診で水滞・お血が疑われ、腹力はいずれも中等度、臍上の動悸は11例、腹直筋緊張は6例、胃部振水音は12例、胸脇苦満は9例、右下腹部圧痛は4例、左下腹部圧痛は7例で認めた。甘麦大棗湯の内服により、月経開始前の焦燥感・不安感・易怒・流涙・自己嫌悪・情緒不安定などの症状は軽減され、不眠・過食に効果を示した症例もあった。

    左下腹部圧痛と便秘の他、諸症状を有する30〜50歳代の女性に対し桃核承気湯を処方し、有効であった10例について検討した。主訴は便秘3例、全身倦怠感3例、月経困難症1例、月経前症候群1例、めまい1例、頭痛1例で、合併する愁訴は情緒不安定、動悸、耳鳴り、肩こり、不眠などであった。漢方所見では、左下腹部圧痛、水滞、お血の所見を全例で認めた。1/3〜1日量の服用で、全例で便通状態の改善を認め、更にそれに伴いおよび不定愁訴も全て改善した。

    ＜文献概要＞卵巣小細胞癌は高カルシウム血症型小細胞癌と肺型小細胞癌に分類され,それぞれ発生の異なるまったく別の腫瘍と考えられている。高カルシウム血症型小細胞癌に関してはSMARCA4遺伝子の変異が同定され分子生物学的特性の解明の糸口となっているが,いずれも非常に稀な腫瘍であるため,まだまだ不明な点が多い腫瘍である。治療法としては手術に引き続き様々な化学療法が行われるが症例数が少ないために臨床試験を行うことは困難であり,シスプラチンやエトポシドを用いた化学療法が行われ有効なこともあるが,まだ有効性の高い治療法は確立されていない。

    女性性機能障害の管理にあたり,まず性歴,既往歴,内服薬,手術歴,社会的背景などを含む問診により性機能障害の分類をする.性交疼痛障害に対しては,腟の潤い不足が原因の際は潤滑ゼリーの使用,エストロゲン欠乏が原因の際はエストロゲン投与を考慮する.性交疼痛障害以外の性機能障害の治療には専門的な知識,および十分な経験が必要である場合が多く,管理が困難と判断された際には,専門医受診を勧める.(著者抄録)
