    症例は66歳,男性。既往歴,併存症なし。直腸癌,cT4aN3M0,cStage IIIcに対し,術前化学放射線療法後に腹腔鏡下直腸低位前方切除術(D3郭清,R0切除)を施行した。術後補助化学療法としてCAPOXを8コース施行した。術後1年目の腹部造影CT検査にて前立腺浸潤を伴う吻合部近傍の骨盤内再発を指摘され,術前化学療法としてFOLFIRI+panitumumabを12コース施行後,再発病変の縮小および他の遠隔再発が存在しないことを確認した上でロボット支援下腹会陰式直腸切断術,前立腺合併切除,尿道膀胱吻合術を施行した。術後経過は良好で,術後12日目に軽快退院となった。術後7ヵ月現在,再発なく経過しており,また手術直後に認めた尿失禁も徐々に改善し現在は排尿障害を認めていない。今回われわれは,前立腺浸潤を伴う直腸癌術後局所再発に対して,尿道再建を含めロボット支援下で安全に治療し得た1例を経験したので報告する。(著者抄録)

    症例は72歳,男性。腹部違和感を主訴に前医を受診し,腹部単純CT検査で12cm大の左副腎腫瘍を指摘され当院へ紹介となった。内分泌スクリーニング検査では主項目は正常値であり,副腎機能亢進は認めなかった。病変の大きさから悪性を疑い,質的診断目的に腹部造影CT検査を施行したところ,S状結腸に約6cmにわたる壁肥厚を認めた。下部消化管内視鏡検査を施行したところ,S状結腸に全周性の2型腫瘍を認めた。生検結果は中分化管状腺癌であった。腫瘍マーカーはCEA23.1ng/mL,CA19-9 962U/mLであった。副腎腫瘍に関してはその大きさから悪性を疑ったが,画像検査では原発性,転移性の診断には至らなかった。S状結腸と副腎以外に病変は認めず,原発性副腎癌,転移性副腎癌のいずれにせよ完全切除が可能と判断し,診断と治療を兼ねてS状結腸切除術および左副腎合併切除術を施行した。病理組織学的検査にて副腎腫瘍の組織像が大腸癌の組織像に類似していたため,S状結腸癌の左副腎転移と診断した。(著者抄録)

    症例は82歳,男性。X-6年,肛門管癌,左鼠径リンパ節腫大に対して腹腔鏡下腹会陰式直腸切断術および左鼠径リンパ節郭清を含むD3リンパ節郭清術を施行した。左鼠径リンパ節に転移を認め,最終病理診断はpT1bN2aM0,pStage IIIaであった。術後補助化学療法としてCAPOX療法を8コース完遂した。術後5年間無再発にて経過し終診となった。X年,会陰部の皮下腫瘍を自覚し再発の可能性を疑われ,精査加療目的に当院を紹介受診した。針生検の結果,肛門管癌術後再発と診断し,会陰部皮下腫瘍切除術を施行した。今回われわれは,肛門管癌術後6年目に会陰部皮下腫瘍を契機に発見された肛門管癌術後晩期再発の1例を経験したため,若干の文献的考察を加えて報告する。(著者抄録)

    症例は75歳,男性。直腸RS癌(pT3N2aM0,Stage IIIb)術後,異時性肝転移切除術後。RAS野生型,BRAF野生型,MSI陰性,HER2陰性。残肝再発および肺転移に対し,FOLFIRI+PANI,mFOLFOX6,FTD/TPI,REG療法を行ってきたが,後方治療の最終ラインでPDと判定した。全身状態が安定しておりliquid biopsyでRAS野生型であったため,抗EGFR抗体薬リチャレンジ療法(CET+IRI)を導入した。4コース終了後に転移巣の著明な縮小を認め,また腫瘍マーカーも正常化した。現在も治療継続中である。抗EGFR抗体薬は一定の休薬期間を置くことで感受性の高い集団が再度増加し,再投与によって治療効果が得られる場合がある。自験例のような最終ライン不応例において,治療選択肢の一つとして有用であると考えられた。(著者抄録)

    症例は77歳男性で、前立腺癌手術後の経過観察中に行われたCT検査で縦隔腫瘍を指摘され、当院紹介となった。造影CTで、第8胸椎から第2腰椎レベルで後縦隔から後腹膜腔にかけて連続した多病巣性嚢胞性病変が認められた。腫瘍の大きさは最大約20mmで、全体的に蛇行していたが、直線距離は約180mmで、内部に脂肪成分が多かった。MRIのT2強調像で高信号域が認められ、放射線学的に脂肪腫または高分化脂肪肉腫と診断した。腹臥位で行ったCTでは、重力によって胸腔が広がり、胸腔から腫瘍の尾側まで切除可能であったため、腹臥位で両側胸腔鏡下手術を行った。手術時間は4時間30分で、出血は少なかった。摘出標本は約20×180mmのsegmental tumorで、手術所見から脂肪腫ではなく胸管嚢胞が示唆された。病理組織学的検査では、線維性壁を持つ嚢胞構造と、嚢胞構造周囲にα-平滑筋アクチンで染色された平滑筋が認められ、胸管嚢胞と診断した。術後経過は良好で、術後4日目で退院した。術後13ヵ月時点のCTでは再発はみられなかった。

    ＜文献概要＞好酸球性食道炎(以下,EoE)は食道上皮への好酸球浸潤を特徴とする食道運動障害や器質的狭窄を引き起こす疾患である.今回,内科的治療に難渋したEoEに対して手術加療を行った1例を経験したため報告する.症例は67歳,男性.2021年4月に脱水・嘔吐を主訴に前医を受診した.上部消化管内視鏡検査で下部食道に縦走潰瘍および白斑を認めた.生検の結果,60個/HPFの好酸球浸潤を認め,EoEの診断となった.薬物治療を含む各種内科的治療に抵抗性であったため当院紹介となり,腹腔鏡下下部食道・噴門側胃切除を施行した.術後,食事摂取良好であった.内科的治療が奏効しない場合,外科的治療が有効である可能性がある.

  • A Case of Gastrointestinal Stromal Tumor of the Greater Omentum with Cyst Formation

    <p>A 56-year-old female complaining of right lateral abdominal pain was referred to our department. Contrast-enhanced CT revealed a 10-cm mass with a cystic part at the greater omentum or mesentery and a solid part that was enhanced in the arterial phase. Thus, we suspected a gastrointestinal stromal tumor (GIST) of the mesentery or greater omentum. We did not select a puncture biopsy examination because this had a risk of rupture, but instead performed tumor resection for both definitive diagnosis and treatment. Intraoperative findings showed a tumor of size 10×7.5 cm. The cystic part was well-defined and the solid part was adherent to the greater omentum. There was no invasion into the mesentery of the transverse colon. Therefore, we considered that the mass had developed from the greater omentum. The tumor was resected without damage to the cystic lesion. The patient was subsequently diagnosed with GIST of the greater omentum by a pathohistological examination. The incidence of GIST is 1–2 per 100,000 in the general population and GIST arising from the greater omentum is extremely rare. Past reports have shown that more than 30% of GISTs of the greater omentum are accompanied by a cyst. Thus, the possibility of GIST should be kept in mind when a hypervascular tumor with a cyst is found at the greater omentum or mesentery.</p>

  • 嚢胞形成を伴う大網原発消化管間質腫瘍の1例

    症例は56歳の女性で,主訴は右季肋部痛であった.造影CTで嚢胞部分を伴う10cm大の大網あるいは腸間膜の腫瘍を認めた.充実性部分が早期濃染を示したため消化管間質腫瘍(gastrointestinal stromal tumor;以下,GISTと略記)の可能性が示唆された.嚢胞成分が多く穿刺による組織診断が不可能であり,確定診断および治療目的に腫瘤切除術を施行した.開腹すると10cm×7.5cmの腫瘤を認め,嚢胞部分は周囲と境界明瞭,充実性部分は大網と癒着,横行結腸間膜前葉への浸潤なく大網由来の腫瘤と考えた.嚢胞損傷なきよう腫瘤を摘出し,病理組織学的検査でGISTと診断した.GISTの頻度は10万人に1～2人とされ,なかでも大網原発GISTは極めてまれである.過去の報告例では3割以上が嚢胞を伴い,大網や腸間膜に存在する嚢胞を伴う多血性腫瘤はGISTを念頭に置く必要がある.(著者抄録)

    反回神経リンパ節(No.106rec)における腫瘍常在記憶(TRM)細胞を対象に、食道扁平上皮癌の腫瘍排出リンパ節内の抗腫瘍免疫応答が予後に寄与するか検討した。2011年から2017年までに当院で胸部食道癌に対する根治的食道切除術を受けた患者462例について106recリンパ節の臨床的意義とTRM細胞を調査し、No.106recリンパ節転移を示した84例(60歳以上:63例)を対象とした。免疫組織化学検査によりTRM細胞を解析し、フローサイトメトリーによりTRM細胞の機能を調査した。その結果、TRM細胞は転移リンパ節と原発腫瘍の両方で観察された。TRM細胞が豊富な標本では、TRM細胞が乏しい標本と比較して、リンパ管侵襲とリンパ節転移が減少し、生存期間が延長していた。転移リンパ節におけるTRM細胞は、原発腫瘍におけるTRM細胞よりも生存期間の延長と有意に関連していた。TRM細胞は細胞傷害性マーカーとしてグランザイムBを高レベルで発現していた。以上より、転移リンパ節におけるTRM細胞の高い浸潤が生存を改善することが示唆された。

    ロボット支援下胃切除術後の周術期に,腹腔内出血・消化管出血を契機に診断された後天性血友病Aの1例を経験したので報告する.症例は79歳の男性で,胃癌に対してロボット支援下幽門側胃切除術を施行された.手術時間は6時間8分,術中出血量は10mlであった.術後に貧血を認め,術後2日目にCT,上部消化管内視鏡検査を行い,胃内に血腫を認めた.活動性出血はなく輸血を行うも創部,腹腔内,消化管内の微小出血による貧血が遷延し,血液検査にてAPTT 63.9秒と凝固異常を認めた.精査の結果,第VIII因子6%,第VIII凝固因子インヒビター27BU/mlであり後天性血友病の診断に至った.第VII第X凝固因子製剤によるバイパス療法とステロイド投与による治療を開始し,治療開始後約2週間で凝固障害の改善傾向を認め,術後23日に軽快退院となった.担癌患者に原因不明の凝固異常を呈した場合,本疾患も鑑別として想起すべきと考えられた.(著者抄録)

    症例は74歳男性。庭で剪定作業を行っている際に梯子から転落し、アザレアの枝が左側の胸鎖乳突筋に刺さる事故を起こした。直ちに近医へ搬送され、当日のうちにその除去手術を受けた。以後は無症状であったが、4ヵ月後に嗄声を自覚し始めたため再受診した。精査の結果、刺さった枝が残存していることが疑われたため、上縦隔内の異物の外科的管理を目的に当科へ紹介された。上部消化管内視鏡検査では胸部上部食道の前壁に隆起病変を認め、造影CTでは上縦隔の気管と食道の間に当たる位置に直線形状で約10×45mm大の異物が描出された。異物除去手術を実施し、圧を5～8cmH2Oかける気縦隔下でビデオ縦隔鏡を挿入した。その結果、広い術視野が得られ、瘢痕組織を切開して4cm長のアザレアの枝を注意深く安全に除去することに成功した。処置中に他の縦隔臓器を損傷することは無かった。手術時間は166分、出血量は45mlであった。手術から5日目に合併症のみられない状態で退院した。以降、4ヵ月毎に行った経過観察では創傷治癒が良好であることが確認されたが患者は嗄声を依然として自覚しており、そのため音声リハビリテーションが実施された。

    症例は65歳、女性。約2ヵ月前からの立位困難感を主訴に、近医を受診した。腹部所見で右下腹部に腫瘤を指摘され、精査加療目的に当科紹介となった。来院時に著明な炎症反応上昇およびCT検査にて後腹膜から右腸腰筋内に連続する膿瘍形成を認めた。また、内視鏡検査でBauhin弁背側に潰瘍を認めた。術前診断は大腸穿通による腸腰筋膿瘍で治療難治性のため、大腸癌合併を考慮し手術を施行した。術中所見では盲腸背側に腫瘍性病変を認め腸腰筋と一塊となり、同部位に膿瘍を認めた。回盲部切除、D3リンパ節郭清を施行した。病理検査結果では盲腸粘液嚢胞腺癌で最終診断はpT4b、N0、M0、pStage IIcであった。術後経過は良好で、退院後に補助化学療法としてFOLFOX6療法を8サイクル施行した。術後1年8ヵ月現在、無再発生存している。腸腰筋膿瘍を合併する大腸癌は非常にまれな病態であり、若干の文献的考察を加え報告する。(著者抄録)

    症例は80歳、男性。直腸S状部癌および腹膜播種に対し腹腔鏡下直腸高位前方切除術を施行した。直腸S状部癌、muc、pT4a、N3(14/15)、M1c、P1、pStage IVc、RAS/BRAF野生型の診断にて、入院の上mFOLFOX6+panitumumab(Pmab)療法を開始した。化学療法開始時の血液検査ではCr 1.45mg/dLと軽度腎機能障害を認めた。化学療法開始3日目朝より徐々に意識レベルの低下を認めた(JCSIII-200)。血液検査にて血漿アンモニア濃度338μg/dLと高値を認め、5-FUに伴う高アンモニア血症と診断し、5-FU持続静注の中止および分枝鎖アミノ酸製剤の投与を行った。意識レベルは翌朝には改善を認め、血漿アンモニア濃度も78μg/dLまで低下し、その後も意識障害は認めず退院となった。second-lineとしてCPT-11+Pmab療法へ変更し化学療法を開始したが、患者希望に1コースで投与終了とし化学療法を終了とした。今回われわれは、5-FUに起因する高アンモニア血症というまれな症例を経験したため若干の文献的考察を加えて報告する。(著者抄録)

    <p><b>Purpose:</b> The effects of gastrectomy on body weight, blood glucose, and lipids were investigated in patients with gastric cancer. <b>Materials and Methods:</b> Data from 299 patients with gastric cancer who received curative gastrectomy in our department between January 2014 and July 2018 were analyzed retrospectively. The body weight reduction ratio (BWRR) one year after surgery was calculated and changes of fasting blood sugar (FBS) and HbA1c in 58 cases with diabetes mellitus and those of total cholesterol and triglycerides in 74 cases with dyslipidemia were examined. Regarding above mentioned factors, patients with improvement of diabetes mellitus and dyslipidemia were compared with those without improvement. <b>Result:</b> The median BWRR was 9.45% and there was a significant difference in surgical procedures (total gastrectomy vs distal or proximal gastrectomy: 14.5% vs 8.9%; <i>P</i>=0.0005). In patients with diabetes mellitus, the average FBS and HbA1c improved from 150 mg/dl to 128 mg/dl and from 7.06% to 6.51%, respectively. In patients with dyslipidemia, triglycerides significantly decreased from 189 mg/dl to 107 mg/dl. The changes of FBS, HbA1c, total cholesterol, and triglycerides were not significantly influenced by the surgical procedures. No significant factors were found that affected improvement of diabetes mellitus. However, patients with improvement of diabetes mellitus tended to have undergone Roux-en-Y reconstruction. Dyslipidemia tended to improve in patients with a high BWRR. <b>Conclusion:</b> Preoperative diabetes mellitus and dyslipidemia both improved after gastric cancer surgery. Roux-en-Y reconstruction may be preferable, especially in patients with diabetes mellitus.</p>

    目的:胃癌患者に対する胃切除が,体重や血糖,脂質にどのような影響を及ぼしているか検討した.方法:当科で2014年1月から2018年7月までに根治手術を施行した胃癌症例299例を対象とし,術後1年の体重減少率を算出した.また,糖尿病群58例における空腹時血糖値およびHbA1c値の変化,脂質異常症群74例における総コレステロール値および中性脂肪値の変化を検討した.さらに,糖尿病改善群と脂質異常改善群を定義し,それぞれ非改善群との比較を行った.結果:体重減少率中央値は9.45%であり,術式で有意差を認めた(胃全摘術vs幽門側/噴門側胃切除;14.5% vs 8.9%;P=0.0005).糖尿病群の平均空腹時血糖値およびHbA1c値は,150mg/dlから128mg/dl,7.06%から6.51%に改善していた.脂質異常症群において中性脂肪値は189mg/dlから107mg/dlへと著明な低下を認めた.術式別の変化を比較したが,低下の程度はともに差を認めなかった.糖尿病改善に影響する有意な因子は認めなかったが,Roux-en-Y再建が多い傾向にあった.また,脂質異常症は体重減少率高値の症例で改善している傾向にあった.結語:胃癌術後において,体重減少だけでなく,術前糖尿病や脂質異常症が改善し,特に糖尿病症例ではRoux-en-Y再建がよい可能性が示唆された.(著者抄録)

    胃癌に浸潤したCD103(インテグリン接着分子αEβ7)を発現するT細胞と三次リンパ構造(TLS)との関連性と臨床的意義について検討した。CD103+T細胞は、CD8+T細胞を伴って胃癌の腫瘍上皮に観察され、胃癌の予後改善と関連していた。また、CD103+T細胞はTLSの周辺に存在し、CD103が強陽性の患者はTLSが豊富であった。CD103強陽性T細胞と豊富なTLSの両方を有する患者は、CD103弱陽性T細胞と乏しいTLSを有する患者よりも予後が良好であった。さらに、PD-1遮断療法を受けた患者では、CD103強陽性と豊富なTLSが良好な反応を予測した。CD103+CD8+T細胞の特徴として、CD103-CD8+T細胞に比べて、PD-1、グランザイムB、インターフェロン-γを高発現していた。以上より、胃癌に浸潤したCD103+CD8+細胞はTLSと相関しており、その結果としてCD103+CD8+細胞の胃癌に対する抗腫瘍免疫応答が促進することが示唆された。

    症例は56歳男性で、健診時の胸部X線異常陰影を指摘され、前医でCTを施行され縦隔腫瘍と診断され当院呼吸器外科紹介となった。検査結果および腫瘍サイズが大きいことより、臨床的には食道の全周性に局在する高分化型脂肪肉腫と術前診断した。左第7肋間中腋窩線に12mmポート、左第5肋間中腋窩線に5mmポート、左第9肋間前腋窩線に12mmポートを留置し、炭酸ガス陽圧気胸下に左胸腔鏡手術を行った。さらに、右第5、7肋間中腋窩線に12mmポート、右第8肋間後腋窩線に5mmポート、右第9肋間前縦窩線に5mmポートを留置し、左側と同様に陽圧気胸下に右胸腔鏡手術を行った。HE染色での診断では異型の乏しい脂肪細胞の増殖がみられ、形態での高分化型脂肪肉腫との鑑別は困難であるが、脂肪腫との診断となった。術後に軽度の胃排出遅延を認めたが早期に改善し、術18日後に軽快退院した。

    症例は50代、女性。主訴は肛門痛で、画像検査にて9.8cmの骨盤内巨大腫瘤を指摘された。生検ではgastrointestinal stromal tumor(GIST)と診断された。腫瘍が大きく肛門温存は困難と判断し、メチル酸イマチニブ(IM)での術前療法を開始した。治療開始6ヵ月後のMRI検査で75%の腫瘍縮小を得た。腫瘍縮小に伴い肛門温存可能と判断し、腹腔鏡下超低位前方切除術を施行した。術後はIMによる補助治療を2年6ヵ月行い、切除5年目の現在、無再発生存中である。術前療法としてのIM投与は巨大直腸GISTにおける肛門温存に対して有用であり、安全な切除を可能にすると考えられた。(著者抄録)

    症例は63歳、女性。前医の検診で高LDH、CK血症を指摘された。精査の結果、多発肝腫瘍・左肺腫瘍および直腸Rbに1/4周性の2型腫瘍を認めた。病理診断は腺癌であり、直腸Rb癌、多発肝転移、肺転移、cT3N1aM1b(H3、PUL1)、cStage IV bと診断した。CAPOX+bevacizumab併用療法を8コース施行した。直腸原発巣および多発肝腫瘍の著明な縮小を認めたため切除可能と判断し、腹腔鏡下腹会陰式直腸切断術および肝部分切除術を施行した。最終病理診断はypT2N0M1b(H2、PUL1)、Stage IV bであった。術後にUFT+LV療法を2コース施行後に、残存している転移性肺腫瘍に対し左肺上葉切除術を施行した。その後は術後補助化学療法を施行することなく、原発巣手術より1年6ヵ月経過後も再発・新規病変なく外来通院継続中である。今回、遠隔転移を有する切除不能進行直腸癌に対して抗VEGF薬併用療法が著効し、R0切除可能となった1例を経験したため報告する。(著者抄録)

    胸腔鏡下食道癌手術時の反回神経麻痺に対する術中神経モニタリング(IONM)の効果について検討した。その結果、CD分類1以上の術後反回神経麻痺はIONM導入前が30.9%であったのに対し、IONM施行例では13.3%と減少がみられた。また、両側の反回神経麻痺はIONM導入前が6.7%であったのに対し、IONM施行例では1例も認められなかった。今回の検討より、胸腔鏡下食道癌手術時のIONMは反回神経麻痺の発生回避に有用と思われたが、間欠法でのIONMにおける陽性的中率は55%程度であり、反応が消失しても丁寧な操作を連続することが重要であると考えられた。

    症例は66歳、男性。下血、便の狭小化を主訴に下部消化管内視鏡検査を施行し、直腸下部に全周性の腫瘍を認めた。生検ではGroup 5であった。術前検査にて直腸癌および直腸周囲リンパ節転移、傍大動脈リンパ節転移、左鎖骨上窩リンパ節転移、多発骨転移を認め、cT4N3M1b(OSS、LYM)、cStage IVbであった。また、左側下大静脈を伴っていた。腫瘍出血のコントロール目的に原発巣切除の方針とした。腹腔鏡下腹会陰式直腸切断術を施行した。左側下大静脈は左尿管および左精巣動静脈よりさらに背側に位置しており、術中に操作の障害となることはなく定型どおりに手術可能であった。左側下大静脈を認める患者においては、手術の際に下大静脈を損傷しないよう術前に十分な解剖学的認識を得ておく必要がある。今回、比較的まれである左側下大静脈を伴った直腸癌に対して腹腔鏡下腹会陰式直腸切断術を施行した症例を経験したため報告する。(著者抄録)

    症例は78歳男性.主訴は腹部膨隆.前医にて胃癌,胆石症に対し開腹下手術歴があり,腹壁瘢痕ヘルニアのため定期診察されていた.ヘルニア門辺縁の腹壁に単純CTで3cmの充実性腫瘤を指摘されたが,経過観察となっていた.しかし,1年後の単純CTで腫瘤径が11cmと増大したため,当科紹介となった.術前に施行した18F-fluorodeoxy glucose positron emission tomography(以下,FDG-PET)で腹壁腫瘤のstandard uptake value maxは1時間値4.6,2時間値6.2と異常集積を認め,急速増大を示していたことからも小腸GISTや胃癌の腹壁播種などの悪性腫瘍が鑑別に挙げられた.悪性を念頭に置いた腫瘍摘出術およびメッシュによる腹壁瘢痕ヘルニア修復術を施行した.病理組織診断では腹壁デスモイド腫瘍の診断であった.本例では,腫瘍の急速増大,FDG-PETでの大きい集積増加率が見られたことから,術前診断において悪性腫瘍との鑑別が困難であった.(著者抄録)

    下行大動脈の位置と縦隔の幅に焦点を合わせて、胸腔鏡下食道切除術の手順に影響を及ぼす術前予測因子について後向きに検討した。食道癌に対する胸腔鏡下食道切除術を受けた患者を対象とした。手術時間と胸部の出血量が中央値を上回った30例は困難群(男性24名、女性6名、年齢中央値68歳)に分類した。残りの65例は通常群(男性52名、女性13名、年齢中央値66歳)に分類した。下行大動脈の位置評価時、左下肺静脈レベルで大動脈-脊椎骨角度を測定した。縦隔幅の評価時に、気管分岐部のレベルで胸骨-脊椎骨距離を測定した。変数増加ステップワイズ法によるロジスティック回帰分析の結果、胸腔鏡下食道切除術の技術的困難に対する独立した予測因子は、大動脈-脊椎骨角度(≧30°)、胸骨-脊椎骨距離(<100mm)、臨床T病期(T3)であった。

    食道癌に対する胸腔鏡下食道切除術(TSE)と開胸食道切除術(OE)の長期成績を比較検討した。1995年5月～2014年12月に胸部食道癌に対してTSEを施行し治癒切除となった454例と、同時期にOEを施行し治癒切除となった201例を対象とした。傾向スコアマッチングにより各群174例を選定し、成績を比較検討した。術後5年生存率はTSEが55.6%、OE群が55.0%で、有意差は見られなかった。リンパ節転移を有する患者でも、術後5年生存率はTSE群が41.5%、OE群が43.6%と有意差は見られなかった。リンパ節転移を認めない患者の術後5年生存率は、TSE群が76.3%、OE群が62.5%であった。食道癌に対するTSEとOEの長期成績は同等であり、胸壁破壊の軽減により術後呼吸機能低下および呼吸器合併症が軽減され、整容性も維持されるTSEは胸部食道癌に対する手術として有用であることが示唆された。

    補助化学療法が不完全であった食道扁平上皮癌患者の周術期治療成績と長期予後を後ろ向きに調査した。ネオアジュバント化学療法は、2回施行を完全、1回施行を不完全とした。ネオアジュバント化学療法を2回施行した患者96例(NAC2群、年齢中央値65歳)と1回しか施行しなかった患者37例(NAC1群、年齢中央値66歳)を対象とした。追跡期間中央値は39ヵ月であった。補助療法前のクレアチニン値はNAC2群が高かった。主な有害事象はNAC1が腎障害、NAC2が胃腸毒性であった。根治的治療(手術ないし根治的化学放射線療法)直後の腎機能障害はNAC2が多かった。TMN分類のypT4はNAC2群が多かった。両群のTMNステージは補助化学療法で改善した。追跡期間中の再発頻度、再発パターン、無病生存期間、全生存期間は両群間で有意差はなかった。

    Purpose: We evaluated the effectiveness of neoadjuvant chemoradiotherapy (CRT) followed by esophagectomy for cT4 esophageal cancer or lymph node metastases (LNM) invading adjacent structures. Materials and methods: We retrospectively evaluated 42 consecutive patients with thoracic esophageal cancer who underwent CRT followed by esophagectomy between 2008 and 2013. All were initially considered to be unresectable because of cT4 (n = 32) disease or LN invasion (n = 10). Radiotherapy was administered at 41.4 Gy/23 fr with concurrent chemotherapy. At completion of CRT, restaging was performed using computed tomography (CT). Results: All cT4 tumors were downstaged, LNM invading to adjacent structures were considered to be released, and subtotal esophagectomy was performed. The median follow-up period was 42 months. The curative resection (R0) rate was 94% in cT4 group and 70% in LN invasion group. The 3-year overall survival (OS) and 3-year locoregional control (LRC) rates were 65–80% in the cT4 group and 50–67% in LN invasion group, respectively. Conclusions: The cT4 group showed good rates of R0, OS, and LRC. Surgical resection should be an effective option when downstaging is achieved by CRT for patients with initially inoperable thoracic esophageal cancer.

    臨床ステージT4(cT4)食道癌患者または隣接臓器に浸潤しているリンパ節転移(LNM)を伴う食道癌患者に対し、ネオアジュバント化学放射線療法(CRT)に続く食道切除術の治療効果について後向きに解析した。本検討では、食道切除術後にネオアジュバントCRTを受けたcT4胸部食道癌患者42例を対象とした結果、全例にcT4腫瘍がダウンステージされたが、隣接臓器に浸潤しているLNMは遊離が考えられ、食道切除術が施行されていた。また、平均経過観察期間は42ヵ月で、cT4群では根治的切除(R0)率は94%、3年間での全生存率(OS)または3年局所領域制御(LRC)率は65～80%と良好な治療転帰が得られたが、LN浸潤群のR0では70%、OSおよびLRCでは50～67%であった。本検討より、外科的切除はCRTによりダウンステージされた場合には、効果的な選択肢となることが示唆された。

    2007年4月-2015年12月にリンパ節郭清を伴う胸腔鏡下食道切除術を受けた連続症例を対象に後向き研究を行った。臨床病期がIB、IIA、IIB、IIIAまたはIIIBの食道癌(EC)で、活動性悪性腫瘍を併発していない患者を対象とした。ネオアジュバント化学療法(NAC)を受けた患者(NAC群:男性109名、女性30名、年齢中央値65歳)と受けなかった患者(非NAC群:男性65名、女性21名、年齢中央値65歳)の間で、術中転帰および術後の罹病率・死亡率を比較した。術前の臨床検査値から、NAC群の方が非NAC群より貧血、血小板減少症および腎機能障害が多かったことが認められた。手術時間、失血量、郭清リンパ節数、全合併症発症率または術後の入院期間に群間差はなかった。

    食道切除患者を対象に、術後12ヵ月における体重回復に寄与する栄養状態の経時的変化について調べた。対象は食道切除を施行した食道癌患者80例とした。80例中、12ヵ月の追跡調査を完了した40例を解析対象とした。術前および術後1ヵ月、3ヵ月、6ヵ月、12ヵ月の臨床データ、身体計測データ、検査値、栄養状態について評価を実施した。食道癌患者の7.2%が罹患前の標準体重(UBW)と比較して術前期における体重減少を認めた。患者の9.3%が術前期から術後12ヵ月までに体重が減少した。術後期の体重増加は有意ではなかった。術後の栄養状態における経時的な変化には時間差が認められた。多変量解析の結果、UBWと比較して術前10%以上の体重減少は術後12ヵ月における85%以上の体重回復に否定的に寄与する独立因子であることが示された。以上から、食道切除施行患者における十分な栄養状態のためには、術前の体重減少を最小限に抑える必要があることが示された。

    傍気管嚢胞(paratracheal air cyst)は,第1～第3胸椎レベルの気管右側に好発する,比較的まれな良性の嚢胞性疾患である.われわれは術前CTで傍気管嚢胞を診断した3例の食道癌手術症例を経験した.いずれの症例も嚢胞が右反回神経周囲リンパ節を郭清する過程で同定され,郭清手技に注意を要した.傍気管嚢胞は術前にその存在を認識していなければ,術中にリンパ節と判別することが難しい形状やサイズであり,認識なく術中に遭遇すれば嚢胞損傷の危険性が高いと考えられた.胸部食道癌手術において左右反回神経周囲リンパ節郭清は重要であり,食道癌の術前評価では傍気管嚢胞も念頭に置くことは重要であると考えられた.(著者抄録)

    A 73-year-old man was diagnosed with essential thrombocythemia and treated with acetylsalicylic acid and hydroxyurea for 15 years. He was referred to our hospital for treatment of esophageal cancer detected by endoscopy. The cancer stage was T1N1M0, cStage IIB according to the classification of the Union for International Cancer Control, 7th edition. After administering two courses of preoperative chemotherapy (5-fluorouracil + cisplatin), the patient underwent video-assisted thoracoscopic radical esophagectomy. Hydroxyurea administration was discontinued during anticancer therapy and restarted to control the platelet count. Heparin was continuously infused during the perioperative period, and hydroxyurea was restarted through a jejunostomy tube on the postoperative day 2. No thrombosis or hemorrhage developed, and the patient was discharged from the hospital on the postoperative day 33. This is the first case report of video-assisted thoracoscopic radical esophagectomy for esophageal cancer in a patient with essential thrombocythemia.

    症例は73歳男性で、出血性胃潰瘍と後縦靱帯骨化症の病歴があり、本態性血小板血症に対して約15年間にわたりアセチルサリチル酸とヒドロキシウレアが処方されていた。心窩部痛を訴え近医を受診したところ、食道十二指腸鏡検査(EGD)で胸部食道癌を指摘され、精査目的で当院に来院した。EGD、造影CT、18F-FDG-PETの所見から、T1N1M0、cStage IIB胸部食道癌と診断した。術前化学療法としてシスプラチンおよび5-フルオロウラシルを2コース投与した。抗癌剤治療中はヒドロキシウレアの投与を中止したが、治療後に血小板数が116.0×10^4/mm3に増加したため投与を再開した。また、術前7日間は出血性合併症を回避するためアセチルサリチル酸投与も中止したが、代替としてヘパリンを周術期に持続的に投与した。さらに深部静脈血栓症予防のため弾性ストッキングを下肢に着用させた。手術は胸腔鏡下根治的食道切除、拡大リンパ節郭清、後縦隔経路再建、空腸瘻造設術を行い、組織学的に低分化扁平上皮癌と診断した。血栓症および出血は認められず、術後33日目に退院した。

    頸胸境界部に転移リンパ節を認めることは稀ではなく、また、頸部・頸胸境界部の内頸静脈、総頸動脈、鎖骨下動静脈にリンパ節転移巣が浸潤することも稀ではない。このような症例に対しては、まず化学放射線療法が選択されることが多いが、必ずしも病変が消失するとは限らず、集学的治療としての救済手術が必要となる場合もある。著者等は、頸胸境界部での血管浸潤が強く疑われる転移リンパ節を有する食道癌患者においては、術後の肩関節機能の温存および安全な術野の確保が可能なtransmanubrial approachが有用と考え、2014年6月から導入した。これまで左右1例ずつに施行し、両例とも良好な術野のもと安全に手術を遂行できたので、その詳細を報告した。

    大腸癌化学療法中に発症したニューモシスチス肺炎(pneumocystis pneumonia:PCP)の1例を経験した。症例は、70歳、男性。排便時出血を主訴に受診。進行S状結腸癌(T2N2aM1b、Stage IV B)と診断し、一次治療としてmFOLFOX6+panitumumabを3サイクル、二次治療としてFOLFIRI+bevacizumabを行った。ステロイド剤は制吐薬適正使用ガイドラインに準じた。二次治療2サイクル目より38℃台の発熱を認めた。胸部CT検査で両側肺野にすりガラス影を認めた。炎症反応の上昇を認めたため、抗生剤投与を開始した。β-D-glucan値の上昇(148pg/mL)を認め、PCPと診断した。ST合剤を併用投与したところ、速やかに炎症反応は低下し、すりガラス影は消失した。ステロイド長期投与や放射線治療の治療歴がなくても、担癌状態、抗癌剤投与や加齢はPCP発症例の危険因子となり得る。抗癌治療中に発熱や呼吸器症状がみられる場合には、本症例も念頭に置いた諸検査が必要と考える。(著者抄録)

    症例は64歳男性で、癌のため根治的食道切除術を行い、同時に後縦隔経路による胃管再建術を行った。食道癌の病理学的診断では中程度分化型扁平上皮癌であった。5-フルオロウラシルとシスプラチンによるアジュバント化学療法を2コース施行した。28ヵ月後の18-FDG-PETで縦隔リンパ節に癌の再発を検出したため、5-フルオロウラシルとシスプラチンによる化学放射線療法と、ドセタキセル、シスプラチン、5-フルオロウラシルの追加化学療法を行った。化学療法終了後に好中球減少症と上腹部痛が出現した。内視鏡検査により、胃管の小彎に多発性潰瘍が明らかになった。プロトンポンプ阻害剤を1ヵ月間経口投与したが潰瘍には効果がなかった。内視鏡検査時の生検検体と血清学的試験でサイトメガロウイルス陽性が示されたため、追加化学療法を延期してホスカルネットナトリウム投与を2週間行った。投与後の上部消化管内視鏡検査では潰瘍の治癒が明らかになった。その後、ドセタキセルによる緩和的化学療法を6コース終了したが、肺と肝臓に転移し、術後51ヵ月で死亡した。

    症例は51歳男性で、食道癌のため入院し、胸部上部食道の扁平上皮癌T1bN1M0、ステージIIBと診断した。根治的治療として胸腔鏡下根治的食道切除術と局所的リンパ節郭清を施行した。手術は右肺収縮下の左外側臥位で開始した。胸腔鏡下手術は第5肋間腔において4ポートで囲んで5cm小開胸経由で施行した。気管分岐部リンパ節の郭清中、後方から中間右気管支へと横断する太い静脈を検出し、静脈を右後分節から左心房へ排出する右肺静脈の異常と考えられた。気管分岐部リンパ節を慎重に郭清し、異常静脈を保存した。根治的食道切除術後に後縦隔経路による胃管再建術を施行した。異常静脈は術前CTでも明確に描出されていた。術後経過は良好で、術後22日目に退院となった。

    Here, we report on an anomaly of the right pulmonary vein in the subcarinal area and emphasis the importance of its focus in subcarinal lymph node dissection. A 51-year-old Japanese man underwent thoracoscopic radical esophagectomy with regional lymph node dissection for esophageal carcinoma T1bN1M0, stage IIB. While dissecting the subcarinal lymph node, we encountered a thick vein crossing posterior to the intermediate right bronchus. We recognized the anomalous right pulmonary vein (anomalous V2) that drained into the left atrium from the right posterior segment (S2). We cautiously dissected the subcarinal lymph node and were able to preserve the anomalous vein. This anomalous pulmonary vein can cause serious complications. If the anomalous V2 had been injured or ligated, congestion of the right S2 or cardiac tamponade might have develop. Since anomalous pulmonary veins are easily identified by enhanced computed tomography, careful observation is essential for avoiding their unnecessary injury.

    A 64-year-old man underwent radical esophagectomy for cancer and simultaneous reconstruction using the gastric conduit through the posterior mediastinum. Two courses of adjuvant chemotherapy were performed. Twenty-eight months postoperatively, recurrence of the cancer was detected in the mediastinal lymph nodes, and he underwent concurrent chemoradiotherapy and boost chemotherapy. Endoscopy was then performed to investigate the cause of epigastralgia, and multiple ulcerations were found in the lesser curvature of the gastric conduit. Although a proton-pump inhibitor was orally administered, the ulceration was intractable. Re-examination of the original biopsy specimens and serological testing revealed positivity for cytomegalovirus. The ulcers began to heal after administration of foscarnet sodium. After the treatment, no signs of exacerbation associated with reinstitution of chemotherapy were observed.

    外科感染症分離菌とその薬剤感受性について2011年度分離菌を中心に検討した。調査対象として検体が採取された症例は259例で、204例(78.8%)から754株の細菌と31株の真菌を分離した。年平均検体数は30年間で198.8検体、最近の5年間では226.2検体であった。年平均分離細菌総株数は30年間で392.2株、最近の5年間では678.4株であった。一次感染全体では、162例のうち120例(74.1%)から細菌を分離した。全204検体から真菌を除き754株の細菌を分離し、1検体平均で3.7株を分離した。一次感染では、嫌気性グラム陰性菌の分離頻度が高く、次いで好気性グラム陰性菌、術野感染では、好気性グラム陽性菌の分離頻度が高く、次いで嫌気性グラム陰性菌であった。バンコマイシン耐性のMRSAやEnterococcus spp.及び多剤耐性緑膿菌(MDRP)は認めなかった。

    Bacteria isolated from surgical infections during the period from April 2011 to March 2012 were investigated in a multicenter study in Japan, and the following results were obtained. In this series, 785 strains including 31 strains of Candida spp. were isolated from 204 (78.8%) of 259 patients with surgical infections. Five hundred and twenty three strains were isolated from primary infections, and 231 strains were isolated from surgical site infection. From primary infections, anaerobic Gram-negative bacteria were predominant, followed by aerobic Gram-negative bacteria, while from surgical site infection aerobic Gram-positive bacteria were predominant, followed by anaerobic Gram-negative bacteria. Among aerobic Gram-positive bacteria, the isolation rate of Enterococcus spp. was highest, followed by Streptococcus spp. and Staphylococcus spp., in this order, from primary infections, while Enterococcus spp. was highest, followed by Staphylococcus spp. from surgical site infection. Among aerobic Gram-negative bacteria, Escherichia coli was the most predominantly isolated from primary infections, followed by Klebsiella pneumoniae, Pseudomonas aeruginosa and Enterobacter cloacae, in this order, and from surgical site infection, E. coli was most predominantly isolated, followed by P. aeruginosa, K. pneumoniae, and E. cloacae. Among anaerobic Gram-positive bacteria, the isolation rate of Eggerthella lenta was the highest from primary infections, followed by Parvimonas micra, Collinsella aerofaciens, Lactobacillus acidophilus and Finegoldia magna, and from surgical site infection, E. lenta was most predominantly isolated, followed by P micra and L. acidophilus, in this order. Among anaerobic Gram-negative bacteria, the isolation rate of Bacteroidesfragilis was the highest from primary infections, followed by Bilophila wadsworthia, Bacteroides thetaiotaomicron, Bacteroides uniformis and Bacteroides vulgatus, and from surgical site infection, B. fragilis was most predominantly isolated, followed by Bacteroides caccae, B. thetaiotaomicron, Bacteroides ovatus and B. wadsworthia, in this order. In this series, vancomycin-resistant MRSA (methicillin-resistant Staphylococcus aureus), vancomycin-resistant Enterococcus spp. and multidrug-resistant P. aeruginosa were not observed. We should carefully follow up B. wadsworthia which was resistant to various antimicrobial agents, and also Bacteroides spp. which was resistant to many β-lactams.

    2010年度の外科感染症分離菌と薬剤感受性についての多施設共同研究の結果を報告した。対象208例のうち170例から細菌606株と真菌25株が分離され、細菌は一次感染422株(A群)、術野感染184株(B群)であった。A群では嫌気性グラム陰性菌の分離頻度が高く、次いで好気性グラム陰性菌、B群では好気性グラム陽性菌に次いで嫌気性グラム陰性菌が多かった。好気性グラム陽性菌は両群ともEnterococcus spp.の分離頻度が高く、次いでStaphylococcus spp.で、陰性菌はA群はEscherichia coli、B群はE.coliとPseudomonas aeruginosaが多かった。嫌気性グラム陽性菌はB群からParvimonas micraなどが分離され、陰性菌はA群でBilophila wadsworhia、B群でBacteroides fragilisが多かった。バンコマイシン耐性のメチシリン耐性黄色ブドウ球菌やEnterococcus spp.および多剤耐性緑膿菌は認めなかった。

    Bacteria isolated from surgical infections during the period from April 2010 to March 2011 were investigated in a multicenter study in Japan, and the following results were obtained. In this series, 631 strains including 25 strains of Candida spp. were isolated from 170 (81.7%) of 208 patients with surgical infections. Four hundred and twenty two strains were isolated from primary infections, and 184 strains were isolated from surgical site infection. From primary infections, anaerobic Gram-negative bacteria were predominant, followed by aerobic Gram-negative bacteria, while from surgical site infection aerobic Gram-positive bacteria were predominant, followed by anaerobic Gram-negative bacteria. Among aerobic Gram-positive bacteria, the isolation rate of Enterococcus spp. such as Enterococcus faecalis, Enterococcus faecium, and Enterococcus avium was highest, followed by Streptococcus spp. such as Streptococcus anginosus and Staphylococcus spp. such as Staphylococcus aureus, in this order, from primary infections, while Enterococcus spp. such as E. faecalis and E. faecium was highest, followed by Staphylococcus spp. such as S. aureus from surgical site infection. Among aerobic Gram-negative bacteria, Escherichia coli was the most predominantly isolated from primary infections, followed by Klebsiella pneumoniae, Klebsiella oxytoca, Enterobacter cloacae, and Pseudomonas aeruginosa in this order, and from surgical site infection, E. coli and R aeruginosa were most predominantly isolated, followed by E. cloacae and K. pneumoniae. Among anaerobic Gram-positive bacteria, the isolation rates of Parvimonas micra, Eggerthella lenta, Streptococcus constellatus, Gemella morbillorum, and Collinsella aerofaciens were the highest from primary infections, and the isolation rate from surgical site infection was generally low. Among anaerobic Gram-negative bacteria, the isolation rate of Bilophila wadsworthia was the highest from primary infections, followed by, Bacteroides fragilis and Bacteroides ovatus, and from surgical site infection, B. fragilis was most predominantly isolated, followed by Bacteroides thetaiotaomnicron, in this order. In this series, vancomycin-resistant MRSA (methicillin-resistant S. aureus), vancomycin-resistant Enterococcus spp. and multidrug-resistant P. aeruginosa were not observed.

    (目的)これまでにわれわれが行ってきた胸腔鏡下食道癌切除における術後感染、特に術後肺炎、術後創感染に関して通常開胸手術と比較検討した。(対象および方法)1997年9月から2013年12月にかけて施行した。導入初期30例を除く胸腔鏡下食道癌切除術389例と、同時期に行った通常開胸下食道癌切除術181例を対象とし、術後肺炎および術後開胸創感染の発生に及ぼす影響を多変量ロジスティック回帰分析により評価した。(結果)患者年齢と胸腔鏡下手術はそれぞれが肺炎の発生を増加/減少させる独立した有意な因子であった。また術後開胸創感染の発生において、胸腔鏡下手術がこれを減少させる独立した有意な因子であった。(結語)胸腔鏡下食道切除は、通常開胸手術に比較して、術後肺炎と開胸創感染の発生を減少させる。(著者抄録)

    小腸が線維性癒着によって腹腔内で切離された症例を報告する.症例は51歳男性.3日前より持続する腹部膨満感と嘔吐を主訴に外来を受診.腹部X線写真上,小腸の拡張と鏡面形成を認め,42歳時に胃全摘術の手術既往があることから術後癒着性イレウスの診断にて緊急入院となった.イレウス管を留置して保存的治療を行ったが,入院7日目に突然の高熱と炎症反応の増悪を認め,腹部CT検査で腹腔内に液体貯留が疑われたため消化管穿孔と判断し緊急手術を施行した.右上腹部の線維性癒着により,小腸が腸間膜を残して完全に切離されていた.切除標本を検討したところ,切離されていた部位の腸管粘膜や近傍の腸間膜には壊死像や虚血所見を認めず,絞扼壊死から穿孔に至ったのではなく,線維性癒着により物理的に直接切離されたものと考えられた.(著者抄録)

    We report a case of ileal dissection caused by a fibrous adhesion. A 51-year-old man visited the hospital complaining of abdominal fullness and vomiting over the previous 3 days. He was diagnosed as having adhesive intestinal obstruction based on the examination findings and his previous history of total gastrectomy. Seven days after conservative treatment by insertion of a long tube, an emergency operation was performed. Laparotomy revealed that the small intestine was completely dissected by a fibrous adhesion in the right upper part of the abdomen. There was no evidence of ischemia or necrosis in the mucosal surface of the resected specimen.

    大阪府・奈良・和歌山県下の大学、一般病院、診療所に勤務する医師(2003年435名、2007年409名、2010年449名)を対象に、胃食道逆流症(GERD)と非びらん性GERD(NERD)治療の考え方についてアンケート調査を行った。2003年/2007年でGERDの病態を正しく理解は56.1%/44.4%、NERDの病態を正しく理解は39.8%/69.9%、NERDとGERD、逆流性食道炎を区別していたのは10.1%/13.6%であった。GERD診断に内視鏡検査が「必要」は53%/51%、必要な理由は「悪性疾患を除外するため」が54.8%/48.1%、「GERDの重症度を知るため」が46.1%/33.5%、「逆流性食道炎かどうか分らないので」が28.5%/35.0%であった。2003年/2007年/2010年の逆流性食道炎の初期治療は「PPI常用量」が16.3%/52.2%/71.3%、NERDの初期治療は「PPI常用量」が24.4%/25.4%/42.1%、逆流性食道炎の維持療法では「PPI半量」が57.0%/57.7%/64.2%、NERDの維持療法では「PPI半量」が47.6%/53.9%/59.8%であった。

    Background: Aberrant hypermethylation of specific genes is present in esophageal squamous cell carcinomas (ESCCs). Such hypermethylation is also present in normal-appearing esophageal mucosae of ESCC patients and is considered to contribute to the formation of a field for cancerization. On the other hand, the presence of global hypomethylation in ESCCs or in their background esophageal mucosae is unknown. Method: We collected 184 samples of esophageal mucosae (95 normal mucosae from healthy subjects, and 89 non-cancerous background mucosae from ESCC patients) and 93 samples of ESCCs. Methylation levels of repetitive elements (Alu, LINE1) and cancer/testis antigen genes (NY-ESO-1, MAGE-C1) were measured by bisulfite pyrosequencing and quantitative methylation-specific PCR, respectively. Results: Methylation levels of Alu, LINE1, NY-ESO-1, and MAGE-C1 were significantly lower in ESCCs than in their background and normal mucosae. Also, in the background mucosae, a significant decrease of the Alu methylation level compared with the normal mucosae was present. In ESCCs, methylation levels of the two repetitive elements and the two cancer/testis antigen genes were correlated with each other. Conclusion: This is the first study to show the presence of global hypomethylation in ESCCs, and even in their non-cancerous background mucosae. Alu hypomethylation might reflect the severity of an epigenetic field for cancerization. © 2012 Springer Science+Business Media B.V.

    症例は咳と嚥下障害を主訴とする51歳男性で、上部消化管検査では切歯から18～22cmはなれた位置に潰瘍病巣(2型)と胃前庭部に幾分隆起した表在性病巣(0-IIa型)の局在が認められた。根治的化学放射線療法に続いて実施した頸部食道癌に対するサルベージ手術後に気管腕頭動脈瘻から2度にわたって大量出血した。最初の出血エピソード中に腕頭動脈の損傷部を非解剖学的バイパス(右femoro-axillo動脈バイパス移植)で横断し、脳と遊離空腸移植への動脈血流を維持した。第二次の出血エピソード中には腕頭動脈の遠位断端を再閉鎖した。両出血エピソード中には腕頭動脈の断端を密封して死腔を充たすために筋皮弁を効果的に採用した。患者は2度の大量出血を克服し、最初の手術から315日後に退院した。

    Massive bleeding from large vessels is a serious and life-threatening complication after radical esophagectomy. Here, we present the rare case of patient survival following two episodes of massive bleeding from a tracheo-brachiocephalic artery fistula after salvage surgery for cervical esophageal cancer following definitive chemoradiation. During the first episode of bleeding, the damaged part of the brachiocephalic artery was transected with an extra-anatomical arterial bypass (right femoro-axillo artery bypass graft) to maintain the arterial blood flow to the brain and the free jejunal graft. During the second bleeding, the distal stump of the brachiocephalic artery was reclosed. During both episodes of bleeding, the musculocutaneous flap was effectively employed to seal the stumps of the brachiocephalic artery and fill the dead space. Finally, the patient was discharged 351 days after the first surgery. (132/150). © 2012 The Japan Esophageal Society and Springer.

    62歳男性。右下腹部の圧痛を主訴に造影CTにて上行結腸に径6cmの腫瘤性病変、採血にて炎症反応の上昇を認め、精査入院となった。小腸造影や大腸内視鏡、FDG-PETを行うも確定診断には至らなかったが、経過中に高度の炎症と腹壁膿瘍の形成がみられ、更に入院12日目の腹部CTでは腫瘤は8cmへと増大傾向を認めた。以上より、結腸あるいは小腸の消化管間質腫瘍(GIST)が最も疑われたが、経皮的生検は播種を危惧し施行せず、入院25日目よりイマチニブを試験的に投与したところ、30日目の腹部CTでは腫瘍の縮小がみられたものの、腹壁への浸潤が確認された。そこで、翌日に経皮的膿瘍ドレナージを行い、イマチニブは入院52日目に内服完了とし、入院76日目に腫瘍を含む腸間膜を回腸ごと切除し、端々吻合した。その結果、本症例は病理組織学的に小腸間膜脂肪織炎で、患者は術後9日目に軽快退院となった。

    2009年度における外科感染症分離菌の動向とその薬剤感受性成績を中心に検討した。検体が採取された症例は220例で、174例から642株の細菌と16株の真菌が分離された。平均検体数は28年間で196.4検体、最近の5年間では223.8検体であった。総分離細菌株数は、1990年代半ばから増加し、更に最近の数年間では著増した。平均分離細菌総株数は28年間で371.7株、最近の5年間では569.8株で、検体あたりの分離菌数の増加がみられた。一次感染症から411株、術野感染から244株の細菌を分離した。一次感染症では、嫌気性グラム陰性菌の分離頻度が高く、次いで好気性グラム陰性菌で、術野感染では、好気性グラム陽性菌の分離頻度が高く、次いで嫌気性グラム陰性菌であった。バンコマイシン耐性のメチシリン耐性黄色ブドウ球菌(MRSA)やEnterococcus spp.および多剤耐性緑膿菌は認めなかった。

    Bacteria isolated from surgical infections during the period from April 2009 to March 2010 were investigated in a multicenter study in Japan, and the following results were obtained. In this series, 671 strains including 16 strains of Candida spp. were isolated from 174 (79.1%) of 220 patients with surgical infections. Four hundred and eleven strains were isolated from primary infections, and 244 strains were isolated from surgical site infection. From primary infections, anaerobic Gram-negative bacteria were predominant, followed by aerobic Gram-negative bacteria, while from surgical site infection aerobic Gram-positive bacteria were predominant, followed by anaerobic Gram-negative bacteria. Among aerobic Gram-positive bacteria, the isolation rate of Enterococcus spp. was highest, followed by Streptococcus spp., and Staphylococcus spp. in this order, from primary infections, while Enterococcus spp. was highest, followed by Staphylococcus spp. from surgical site infection. Among aerobic Gram-negative bacteria, Escherichia coli was the most predominantly isolated from primary infections, followed by Klebsiella pneumoniae, Enterobacter cloacae and Pseudomonas aeruginosa, in this order, and from surgical site infection, E. coli was most predominantly isolated, followed by P. aeruginosa and E. cloacae. Among anaerobic Gram-positive bacteria, the isolation rate of Eggerthella lenta was the highest from primary infections, followed by Parvimonas micra, Streptococcus constellatus and Finegoldia magna, and from surgical site infection, E. lenta was most predominantly isolated. Among anaerobic Gram-negative bacteria, the isolation rate of Bilophila wadsworthia was the highest from primary infections, followed by Bacteroides fragilis, Bacteroides ovatus and Bacteroides thetaiotaomicron, and from surgical site infection, B. fragilis was most predominantly isolated, followed by B. ovatus, B. wadsworthia and B. thetaiotaomicron, in this order. In this series, we noticed no vancomycin-resistant Gram-positive cocci, nor multidrug-resistant P. aeruginosa. We should carefully follow up B. wadsworthia which was resistant to various antibiotics, and also Bacteroides spp. which was resistant to many beta-lactam antibiotics.

    Background: The presence of lymph node metastasis in esophageal squamous cell carcinoma (ESCC) patients is a critical factor for decision of treatment strategy. However, there have been no molecular markers to assess lymph node metastasis. In this study, we aimed to identify CpG islands (CGIs) whose DNA methylation statuses are associated with the presence of lymph node metastasis. Materials and Methods: A total of 96 ESCCs were divided into a screening set (n = 48) and a validation set (n = 48). Genome-wide methylation analysis was performed by methylated DNA immunoprecipitation-CGI microarray analysis. Methylation levels were analyzed by quantitative methylation-specific PCR (qMSP). Results: Genome-wide methylation analysis identified 25 CGIs differentially methylated between 8 ESCCs with lymph node metastasis and 4 without. In the screening set, 7 CGIs had significantly different methylation levels (P&lt
    0.05) between the ESCCs with and without lymph node metastasis, and cut-off methylation levels for these CGIs were determined. The validation set was analyzed with the prefixed cut-offs, and methylation statuses of 2 CGIs in the vicinities of PAX6 and ENST00000363328 were validated to be associated with the presence of lymph node metastasis. Using these 2 markers, the presence was predicted with a sensitivity of 93% and specificity of 57%. In addition, the methylation statuses of the 2 CGIs were significantly associated with disease-free survival (P = 0.006). Conclusions: Methylation statuses of these 2 CGIs were significantly associated with the presence of lymph node metastasis of ESCCs. These CGIs are promising markers to predict the presence of lymph node metastases. © 2010 Society of Surgical Oncology.

    麻酔導入後に用いた薬剤が原因と考えられるアナフィラキシーショックから心停止に至った症例を経験した。患者は55歳、男性で、食道癌根治術予定であった。麻酔導入後に抗菌薬とステロイドを投与した際に、アナフィラキシーショックを起こし血圧が低下した。心肺蘇生を行い約24分後に心拍が再開し、経皮的心肺補助装置(percutaneous cardiopulmonary support、PCPS)、大動脈内バルーンパンピング(intra-aortic balloon pumping、IABP)などを用いた全身管理の結果、第4病日にPCPSより離脱し、第8病日に抜管でき後遺症なく救命できた。心拍再開にアドレナリンは無効であり、バゾプレッシン投与が有効であった。麻酔中の心停止であったため、心拍再開まで絶え間ない心肺蘇生を行えたこと、心拍再開後に直ちにPCPSを開始できたこと、などが後遺症なく救命できた要因と考えられた。(著者抄録)

    患者は77歳、男性。cT3N3M0、Stage IIIの胸部食道癌に対しnedaplatin 80mg/m2、day 1、5-fluorouracil 800mg/m2、day 1～5の投与による術前化学療法を施行した。治療5日目に意識障害が出現し、この時点で血中Na値は116mEq/Lと著明に低下していた。明らかな脱水所見はなく、血漿浸透圧低下を伴った低Na血症と尿浸透圧の上昇、尿中Na排泄の持続を認め、原因となる基礎疾患を認めないことから、抗腫瘍薬投与に起因するSIADH(抗利尿ホルモン不適合分泌症候群)であると診断した。Na補充と水分制限および利尿剤投与により意識障害、電解質異常は改善した。その後症状の再燃なく、食道癌根治切除再建術を施行することができた。最終病理診断はpoorly differentiated squamous cell carcinoma、Mt、Ch-1p+3型、pT2(MP)、infβ、ie(-)、ly0、v0、pN0、pPM0、pDM0、pRM0、M0、fStage IIであった。現在まで食道癌の再発は認めず、経過観察中である。(著者抄録)

    大腸癌76例を対象として、正常部と癌部のDPD活性・OPRT活性を測定し、臨床病理組織学的因子との関係を検討した。DPD活性は正常部と癌部で差がなかった。OPRT活性は癌部で有意に高かった。低分化型腺癌はDPD活性が有意に高く、OPRT活性は有意に低かった。さらに、リンパ節転移陽性例でOPRT活性は有意に低かった。DPD・OPRT活性からみると、低分化型腺癌は5-FU抵抗性であり、S-1などのDPD阻害剤を含む抗癌剤が有効であると思われる。(著者抄録)

    食道癌48例を対象として癌組織と正常組織のOPRT・DPD活性を測定し、臨床病理学的因子との関係を検討した。DPD活性は癌組織と正常組織に差がなく、臨床病理学的因子との関連もなかった。OPRT活性は、癌組織で有意に高値であった。リンパ節転移陽性、リンパ管侵襲陽性例で有意に低値で、Stage I、IIの症例がIII、IVの症例に比べ高値であった。OPRT/DPD比は進行度、生存率との関係を認めた。食道癌においてOPRT活性は生物学的な悪性度に関係する可能性が示唆された。(著者抄録)

    71歳男。患者は嚥下困難を主訴とした。前医で食道癌と診断され、著者らの施設へ紹介となった。受診時、CTで肺転移、食道癌の気管浸潤が疑われ、食道癌に対し化学放射線療法が行われたが、気管分岐直上に径2cmに及ぶ食道気管瘻を併発していた。瘻孔が大きく、原発巣の縮小により狭窄部が消失して食道ステントでの瘻孔閉鎖が困難である可能性が高かった。また、化学放射線療法の奏功例で比較的長期予後が望めること、および化学放射線療法後の食道・気管のダブルステントではステント間組織の圧迫壊死により瘻孔が拡大することもあることからバイパス術が治療の適応と判断された。だが、汎血球減少症、肺炎を来していたため、その改善を待って食道バイパス術は施行され、あわせて術後は化学療法が行われた。その結果、患者は手術から40日経過で退院後、1年経過で肺転移巣の増大傾向がみられ、抗癌剤が変更されたが、目下も経口摂取は可能で在宅生存中である。

    消化器外科領域の感染症患者或いは感染を合併した入院患者を対象に、2008年4月～2009年3月における分離菌の動向と、その薬剤感受性成績を中心に検討した。2008年度の調査対象として検体が採取された症例は215例であった。このうち173例(80.5%)から694株の細菌と18株の真菌が分離されたが、残る42例からは細菌、真菌いずれも分離されなかった。一次感染症からは357株、術後感染症からは337株の細菌が分離された。一次感染症においては嫌気性グラム陰性菌が多く分離され、次いで好気性グラム陰性菌が分離された。一方、術後感染症においては好気性グラム陽性菌の頻度が高く、次いで嫌気性グラム陰性菌が分離された。好気性グラム陽性菌は一次感染症ではEnterococcus faecalis、Enterococcus faecium、Enterococcus aviumなどの頻度が高く、Streptococcus anginosus、Staphylococcus aureusなどがこれに次いだ。術後感染症ではEnterococcusの分離頻度が高かった。好気性グラム陰性菌では一次感染症ではEscherichia coliの分離頻度が高く、Klebsiella pneumoniae、Pseudomonas aeruginosaがこれに次ぎ、術後感染症ではP.aeruginosa、E.coli、Enterobacter cloacae、K.pneumoniaeの分離頻度が高かった。嫌気性グラム陽性菌では一次感染症からEggerthella lenta、Parvimonas micraなど、術後感染症でもE.lenta、P.micraなどの頻度が高くかった。嫌気性グラム陰性菌では一次感染症ではBacteroides fragilisの頻度が高く、術後感染症でもB.fragilisの頻度が高かった。バンコマイシン耐性のメチシリン耐性黄色ブドウ球菌(MRSA)や多剤耐性緑膿菌などは認められなかった。

    77歳女。以前より食道裂孔ヘルニアを指摘されており、今回、食後の心窩部不快感、呼吸困難を強く自覚するようになった。上部消化管造影で胃は大彎側が翻転する形で縦隔内に存在し、胸腹部CTでは大網、横行結腸も縦隔内に脱出していた。経胸壁心臓超音波では、左房がヘルニア嚢とその内容物により背側から圧排されていた。胃軸捻転を伴った混合型食道裂孔ヘルニアと診断し、開腹術を施行した。全胃と横行結腸が縦隔内へ脱出していたが、ヘルニア嚢との癒着はなく、容易に還納できた。ヘルニア嚢を切除した後、プレジェット付きの非吸収糸で横隔膜脚を縫合し、食道裂孔を縫縮した。噴門部右側と胃穹窿部は横隔膜脚に縫合固定した。術後呼吸困難は改善し、画像検査で問題はなく、術後11日目に退院となった。術後20日目の呼吸機能検査では、肺活量が術前1.35Lから1.94Lに、%肺活量が65.2%から96.0%に、1秒量が0.9Lから1.27Lに、1秒率が66.7%から65.5%に改善していた。現在も症状の再燃はない。

    Bacteria isolated from infections in abdominal surgery during the period from April 2007 to March 2008 were investigated in a multicenter study in Japan, and the following results were obtained. In this series, 707 strains including 24 strains of Candida spp. were isolated from 181 (79.0%) of 229 patients with surgical infections. Three hundred and ninety-five strains were isolated from primary infections, and 288 strains were isolated from postoperative infections. From primary infections, anaerobic Gram-negative bacteria were predominant, followed by aerobic Gram-negative bacteria, while from postoperative infections aerobic Gram-positive bacteria were predominant, followed by anaerobic Gram-negative bacteria. Among aerobic Gram-positive bacteria, the isolation rate of Enterococcus spp. was highest, followed by Streptococcus spp., and Staphylococcus spp. in this order, from primary infections, while Enterococcus spp. was highest, followed by Staphylococcus spp. from postoperative infections. Among aerobic Gram-negative bacteria, Escherichia coli was the most predominantly isolated from primary infections, followed by Klebsiella pneumoniae, Pseudomonas aeruginosa and Enterobacter cloacae, in this order, and from postoperative infections, P. aeruginosa was most predominantly isolated, followed by E. cloacae, E. coli and K. pneumoniae. Among anaerobic Gram-positive bacteria, the isolation rate of Parvimonas micra was the highest from primary infections, followed by Streptococcus constellatus and Gemella morbillorum, and from postoperative infections, Anaerococcus prevotii was most predominantly isolated. Among anaerobic Gram-negative bacteria, the isolation rate of both Bacteroides fragilis and Bilophila wadsworthia were the highest from primary infections, followed by Bacteroides thetaiotaomicron and Campylobacter gracilis, and from postoperative infections, B. thetaiotaomicron was most predominately isolated, followed by B. fragilis, Bacteroides caccae and B. wadsworthia in this order. In this series, we noticed no vancomycin-resistant Gram-positive cocci, nor multidrug-resistant P aeruginosa. There were nine strains of coagulase-negative Staphylococci which show higher MIC against teicoplanin more than 4 gg/mL, but all of them had good susceptibilities against various anti-MRSA antibiotics. We should carefully follow up B. wadsworthia which was resistant to various antibiotics, and also Bacteroides spp. which was resistant to many beta-lactam antibiotics.

    Background/Aims: Many patients who undergo distal gastrectomy develop remnant gastritis. The diagnosis of remnant gastritis is commonly established endoscopically. This report describes the correlation between endoscopic and histological findings of inflammatory changes in the remnant gastric mucosa.
    Methodology: Ninety-seven patients who underwent curative distal gastrectomy for gastric cancer, with radical lymphadenectomy, were studied. Endoscopy was performed 12 weeks after surgery. Patients with marked redness, erosion, or edema of the gastric remnant, as seen by endoscopy, were judged to have remnant gastritis, and were divided into group P (positive) and group N (negative). Histological remnant gastritis was evaluated with the neutrophil infiltration score based on the updated Sydney System for endoscopically taken biopsies. Cyclooxygenase (COX)-2 expression was evaluated immunohistochemically.
    Results: There was no difference in neutrophil infiltration or COX-2 expression scores between the patients in groups P and N. There was a significant correlation between neutrophil infiltration score and COX-2 expression score (p&lt;0.001).
    Conclusions: Endoscopic diagnosis of remnant gastritis was not correlated with histological findings. Histological remnant gastritis had significant correlation with COX-2 expression score, which was induced by inflammation. Therefore, proper diagnosis of remnant gastritis should be based on histological examination.

    Background/Aims: Helicobacter pylori is a carcinogen for gastric cancer. There have been few reports on carcinogenesis about H. pylori infection in the gastric remnant after distal gastrectomy. The relationship between carcinogenesis and H. pylori infection was studied by means of inflammation and oxidative stress.
    Methodology: Ninety-seven patients who had undergone curative distal gastrectomy for gastric cancer were studied. All patients underwent endoscopic examination 3 months after surgery. The presence of H. pylori was determined using urease rapid test, hematoxylin-eosin staining, and immunohistochemical staining. Fifty-one patients were positive (group A) and 46 patients were negative (group B). The grades of remnant gastritis were scored by updated Sydney System. 8-Hydroxydeoxyguanosine (8-OHdG) as an oxidative stress marker, was analyzed immunohistochemically, and graded in 4 grades.
    Results: Both the neutrophil infiltration score and 8-OHdG expression score was higher in group A than group B (p=0.03 and 0.05). The correlation between the 8-OHdG expression score and the neutrophil infiltration score was significant (p=0.02).
    Conclusions: As gastritis is related to oxidative stress, H. pylori infection is suspected to play an important role in carcinogenesis in the gastric remnant.

    Tendency of isolated bacteria from infections in abdominal surgery during the period from April 2006 to March 2007 were investigated in a multicenter study in Japan, and the following results were obtained. In this series, 474 strains including 23 strains of Candida spp. were isolated from 170 (75.2%) of 226 patients with surgical infections. Two hundred and twenty-six strains were isolated from primary infections, and 224 strains were isolated from postoperative infections. From primary infections, anaerobic Gram-negative bacteria were predominant, followed by aerobic Gram-negative bacteria, while from postoperative infections aerobic Gram-positive bacteria were predominant, followed by anaerobic Gram-negative bacteria. Among aerobic Gram-positive bacteria, the isolation rate of Staphylococcus spp. was higher from postoperative infections, while Enterococcus spp. was higher from primary infections. Among aerobic Gram-negative bacteria, Escherichia coli was the most predominantly isolated from primary infections, followed by Klebsiella pneumoniae, Enterobacter cloacae and Pseudomonas aeruginosa, in this order, and from postoperative infections, P. aeruginosa was the most predominantly isolated, followed by E. coli and E. cloacae. Among anaerobic Gram-negative bacteria, the isolation rate of Bilophila wadsworthia was the highest from primary infections, followed by Bacteroides fragilis and from postoperative infections, B. fragilis was most predominately isolated, followed by Bacteroides caccae, Bacteroides thetaiotaomicron and B. wadsworthia in this order. In this series, we noticed no methicillin-resistant Staphylococcus aureus, nor multidrug-resistant P. aeruginosa. There were three strains of methicillin-resistant coagulase-negative Staphylococcus aureus, but all of them had good susceptibilities against various anti-MRSA antibiotics. We should carefully follow up B. wadsworthia.

    In July 2005, an 82-year-oldman noted a right lower back protrusion. He was diagnosed as having an upper lumber hernia based on abdominal computed tomography findings, including retroperitoneal fat herniated between the quadratus lumborum and the internal oblique mauscle. A hen's egg-sized soft mass was palpable under the inferior margin of the right 12th rib. In March, 2006, under epidural anesthesia a hernioplasty was performed with the patient in the left lateral position. The muscles around the orifice were strong, so that the orifice could be securely repaired using a three-layered, simple closure. The patient was discharged 13 days after surgery, and remains well without any evidence of relapse. Lumbar hernia is rare. Thus, we report this case of lumbar hernia, which was repaired using a simple closure.

    A 51-year-old woman, with the diagnosis of pseudomyxoma peritonei, was referred to us for further evaluation of intestinal obstruction. Barium swallow showed a stricture of the distal stomach with complete obstruction. Endscopically no tumor invaded the mucosa on the stricture. Computed tomography of abdomen revealed that stomach was compressed by tumor and there was no obstruction in the small intestine and cob-rectum. The Stent was inserted as palliation for obstruction. She was alive for 7 years after stent insertion. Stent insersion was effective for the certain period for pyloric stenosis caused by extra-compression of pseudomyx-oma peritonei.

    It is known that a hypercoagulable state develops after radical esophagectomy for cancer. To prevent hypercoagulability, anti-inflammatory agents, such as steroids, are administrated. We studied the effect of neutrophil elastase inhibitor (Sivelestat sodium hydrate, SN) on coagulability in patients after radical esophagectomy. A total of 41 patients who had undergone radical esophagectomy together with reconstruction were stratified into two groups. In the SN+group (<i>n</i>=21), SN was administrated continuously, at 0.2mg/kg/hr, from just after surgery until the 5th post-operative day. In the other group no administration took place. All patients received an injection of methylpredonisolone of 500 mg/body just before surgery. The platelet count, level of fibrinogen (FBG), FDP, D-Dimer, AT3, TAT, activated partial thrombin time (APTT), protein C and alfa-2 plasmin inhibitor (A2PI) were measured before surgery and on the 1st to 7th days after operation. Differences in platelet count, and in levels of FBG, FDP, D-Dimer and TAT were not significant between the two groups. In the SN+group, the level of AT3 and protein C were higher than the SN- group on the 2nd and 3rd post-operative days, and α2PI on the 3rd post-operative day. In the SN- group, the level of APTT was higher on the 3rd postoperative day. In conclusion, continuous infusion of SN is effective to prevent hypercoagulability after radical esophagectomy.

    <B>Aim</B>: The physical condition and quality of life of the patients who had distal subtotal gastrectomy were well studied, however the condition of psychological health of these patients is remained to be evaluated.<B>Patients and methods</B>: The General Health Questionnaire 28 (GHQ28) was used to interview 67 patients without signs of recurrence 12 months or more after distal subtotal gastrectomy and D2 lymph node dissection for gastric cancer. Higher GHQ scores mean poorer psychological health. Information about gastrointestinal symptoms was also obtained.<B>Results</B>: The GHQ score of the patients who underwent Billroth II reconstruction (n=21) was 7.2±5.0 and significantly higher than the scores of the patients who underwent Roux-Y (3.2±3.2, n=27, p<0.01) or Billroth I (3.8±4.6, n=19, p<0.05) reconstruction. At least one gastrointestinal symptom was present in 86% of the patients who underwent Billroth II reconstruction. The incidence of symptoms was significantly higher after Roux-Y (41%, p<0.01). The incidences of bitter reflux, nausea, and diarrhea were significantly higher in the Billroth II group than in the Roux-Y group.<B>Conclusion</B>: In preserving psychological health and reducing postoperative symptoms, Billroth II reconstruction should be avoided, when compared with Roux-Y or Billroth I reconstruction.

  • SUCCESSFUL PLACEMENT OF SELF-EXPANDABLE METALLIC STENT AS PALLIATION FOR RECTAL STENOSIS CAUSED BY SCHNITZLER'S METASTASIS

    A 70-year-old Japanese man with pyloric stenosis caused by unresectable gastric cancer had undergone Roux en Y bypass surgery at the age of 69. Oral anticancer agents had been administered on an outpatient basis. Rectal stenosis caused by Schnitzler's metastasis developed 13 months after the bypass surgery. We inserted a self-expandable metallic stent (EMS) with the flared part located on the oral side in order to prevent migration. The patient was able to maintain bowel activity without stent dislocation for 6 months until the small intestine became obstructed. Despite a laparotomy, conventional colostomy tended to be effective palliation only for a short period, because multiple obstructions occurred later on. EMS placement, which is less invasive compared to colostomy and makes spontaneous bowel movement possible, promised to relieve mental and physical stress of patients in the terminal stage.

  • Three Cases of Surgical Repair of Benign Esophagotracheal Fistula Caused by Different Etiologies

    Three cases of surgical repair of benign esophagotracheal fistula caused by different etiologies were reported. Case 1 was a 63-year-old male who presented with cough and chest pain 1 year after 60 Gy external irradiation and 10 Gy intraluminal irradiation for superficial esophageal cancer. Endoscopy revealed a fistula between the midesophagus and left main bronchus, and biopsy showed no tumor involvement at the fistula. After failure of packing with intraesophageal stenting, an operation was performed through right thoracotomy. The fistula was closed using the adjacent esophageal wall, and subtotal esophagectomy and lymphadenectomy were performed. Because no tumor was found histologically, the fistula was caused by radiation damage. Case 2 was a 56-year-old male referred to us for further evaluation of repeated pneumonia of the left lower lobe after cholecystectomy, which required endotracheal intubation for anesthesia. Contrast study of the esophagus revealed a fistula between the bottom of the diverticulum, which had been known to be associated, and the left main bronchus. Under left thoracotomy, diverticulectomy was performed together with left lower lobectomy, because of carnification of the lobe due to repeated inflammation. Histological evaluation established a diagnosis of an esophagobronchial fistula classified as Braimbridge type I. Case 3 was a 31-year-old male referred to us for evaluation of cough on ingestion after a traffic-induced chest injury. Endoscopic observation revealed an esophagotracheal fistula. Under right thoracotomy, the fistula was closed using the adjacent esophageal wall. The defect of the remnant esophagus was closed by Gambee’s suture. The etiology of the fistula was thought to be traumatic. In cases of benign esophagotracheal fistula, intraesophageal stenting seldom packs the fistula. Operative sectioning is most useful for treating benign esophagotracheal fistulas.

    A 60-year-old female with an abnormal mass on the right lower abdominal wall was diagnosed to have metastatic adenocarcinoma with excisional biopsy. Gastrofiberscopy, colonofiberscopy and jejuno-graphy could not detect the original lesion, however, PET-CT revealed the lesion in the left middle abdomen. Small intestinal endoscopy indicated the tumor with ulceration and severe stenosis in the jejunum about 15cm from the Treitz ligament. The biopsy specimen revealed moderately differentiated adenocarcinoma histologically. Partial resection of the jejunum was performed, and the postoperative course was uneventful, and chemotherapy was adapted from 17 postoperative days. PET-CT and small intestinal endoscopy were effective in detecting and diagnosing jejunal cancer.
