    This paper investigates the patterns of violent crime in Kuala Lumpur and Putrajaya between 2015 and 2020 using advanced spatial analysis techniques, including Spatial Autocorrelation, Standard Deviational Ellipse (SDE), and Mean Centre (MC). The study analyses the spatial distribution and temporal dynamics of violent crimes, tracks shift in crime hotspots and examines the influence of socio-economic factors on these patterns. Findings reveal that violent crimes predominantly occurred in the late afternoon and night, with peak incidents at 3:00 PM (1,830 cases, 8.28%). The study highlights notable shifts in crime hotspots, initially concentrated in commercial districts and gradually expanding to transportation networks and emerging urban areas. The Moran Index values indicate a transition from a near-random pattern in 2015 (-0.002668) to a mild clustering in 2018 (0.032962). Additionally, socio-economic factors, including population density, economic conditions, and the COVID-19 pandemic, significantly impacted crime patterns, leading to changes in crime rates and hotspot locations. These findings are crucial for law enforcement and urban planning, emphasizing the need for adaptive crime prevention strategies that respond to evolving urban challenges, and demonstrating the importance of integrating socio-economic data into spatial crime analysis for comprehensive crime mitigation.

    ABSTRACT

    This study explores the correlation between property crime and demographic factors in Kuala Lumpur and Putrajaya using spatial autocorrelation (SA) and ordinary least squares (OLS) regression from 2015 to 2020. The 2016 SA analysis shows a significant increase in Moran's I index (0.012905), with a positive z‐score of 2.020088 and a p value of 0.043374, indicating spatial clustering of crime. The study examines how factors like total population, household areas, residential areas, male populations and female populations influence number of property crime cases, revealing varying relationships year by year. By highlighting fluctuations in R‐squared and F‐statistic values, this research challenges static crime models, advocating for adaptable, data‐driven strategies in crime prevention. These findings emphasize the importance of continuous policy adjustments to address the evolving socio‐economic dynamics of urban areas.

    In an era where urbanization is rapidly transforming the landscape of cities, this study provides a crucial overview of how demographic shifts influence property crime in metropolitan areas. The paper delves into the complex interplay between property crime rates and demographic characteristics in the urban settings of Kuala Lumpur and Putrajaya, Malaysia, over six years from 2015 to 2020. Employing linear regression analysis, the study meticulously examines the relationship between property crime and various demographic factors, including total population, male and female populations, and residential and household densities. The findings indicate a consistent positive correlation between total population and property crime, emphasizing urban density's role in crime propensity. Notably, the male population shows a stronger correlation with property crime than females. The study also highlights how residential and household densities influence property crime in these urban settings. These insights are invaluable for policymakers and urban planners, guiding targeted strategies to reduce property crime in growing cities.

    Abstract

    Extensive research over the years has illuminated how urban land use and development intricately influence property crime dynamics. Yet, there remains a notable paucity of simultaneous explorations of spatial–temporal patterns and their correlation with urban geography. This study aims to bridge this gap by employing advanced spatial analytical techniques, providing a nuanced understanding of how property crime is distributed and how it evolves within the urban fabric of Malaysia. This research rigorously investigates the spatial–temporal patterns of property crime within the urban context of Kuala Lumpur and Selangor in Malaysia, utilizing a novel combination of mean center and standard deviational ellipse methodologies. Concentrating on the regions of Selangor, the Federal Territory of Kuala Lumpur, and the Federal Territory of Putrajaya, the study meticulously analyzes property crime data spanning from 2015 to 2020. This analysis uncovers pivotal insights into the distribution and trends of criminal activities. The integration of these spatial analytical techniques not only facilitates the identification of prevalent hotspots and directional tendencies in property crimes but also probes into their association with urban land use and developmental patterns. The outcomes of this study shed light on the intricate interplay between the incidence of crime and urban spatial configurations, thereby yielding significant theoretical and practical ramifications. The research makes a substantial contribution to the disciplines of criminological geography and urban planning, underscoring the indispensable role of advanced spatial analysis in devising robust crime prevention mechanisms and bolstering the safety of urban environments.

    This study examines property crime trends in Selangor, Kuala Lumpur, and Putrajaya from 2015 to 2020, utilizing Geographic Information Systems (GIS) and comprehensive crime data from the Royal Malaysia Police. It focuses on the relationship between property crime rates and the delineation of police station jurisdictions across Selangor State, Kuala Lumpur Federal Territory (KLFT), and Putrajaya Federal Territory (PFT). The objective is to understand the dynamics of property crime, its evolution, and the influence of the National Transformation Program on crime prevention strategies. By assessing the spatial distribution of property crimes, this research aims to identify underlying trends and the efficacy of hot spot policing. The analysis is underpinned by an extensive review of secondary data from the Intelligence/Operations/Records Division of the Royal Malaysia Police Headquarters, enabling a deep dive into the spatial characteristics of property crime within these regions. The findings contribute to the broader understanding of crime prevention in Malaysia, underscoring the importance of GIS technology and the need for strategic interventions tailored to specific crime patterns. This study provides critical insights for policymakers and law enforcement to formulate evidence-based strategies, enhance public safety, and inform future crime prevention efforts.

    AIM: Studies have not fully examined whether the medical care system would be able to manage the high number of casualties due to a Nankai Trough earthquake, whose probability of recurrence in the next 30 years is ~70%. This study assessed the demand-supply balance of the disaster medical care system in Osaka city by integrating the data on damage estimation and disaster coping hospitals using a geographic information system. METHODS: We obtained data on the distribution of casualties in two cases, high and low rates of evacuation from the tsunami, and available beds in Osaka city calculated from operating data of each disaster coping hospital. We expanded these data on a geographic information system and investigated the balance of medical care. RESULTS: The total number of available beds in the disaster medical care facilities was 5,559, and the shortage with evacuation rates being either low or high, would be 47,631 and 1,487, respectively. With a low evacuation rate, bed shortage is a common occurrence in coastal areas. With a high evacuation rate, bed shortage decreases, and problems with medical care arise in the eastern area of Osaka city. DISCUSSION: In the case of a low evacuation rate, greater bed shortage was found along the coast of Osaka Bay where the probabilities of flooding are high. In the case of a high evacuation rate, however, the number of casualties was much lower. A shortage of medical care did not occur along the coast, but in the eastern part of Osaka city.

    保健医療計画では,二次医療圏を単位として医療提供体制の整備が行なわれてきた。しかし,住民の受療行動は,必ずしも二次医療圏内で完結しないことから,実際の行動に基づく検討が必要である。本研究では,住民の受療行動を明らかにするため,島根県雲南市大東町・加茂町と邑智郡邑南町で実施した健康調査対象者のうち高血圧症515名,脂質異常症253名,糖尿病104名の治療者について,二次医療圏外の医療施設の選択状況とその要因について自家用車利用群・非利用群に分けて解析した。その結果,雲南市,邑南町の両地域では,糖尿病治療者で高血圧症治療者,脂質異常症治療者に比べて二次医療圏外の医療施設を選択している者の割合が高かった。また,高血圧症治療者について二次医療圏外の医療施設を選択する確率は,雲南市の女性自家用車利用群と頸動脈プラークスコア高値者,邑南町の女性自家用車利用群で有意に低下した。糖尿病治療者では,二次医療圏外の医療施設を選択する確率は,雲南市のバス停までの距離が長い自家用車利用群,邑南町の女性自家用車利用群,BMI高値者で有意に低下した。なお,自家用車非利用群では,各選択要因について有意な関係を認めなかった。以上より,中山間地域の医療提供体制の検討では,隣接医療圏の医療資源や住民の生活様式を踏まえた議論が必要であることが考えられた。(著者抄録)

    Health care plan has been established in each secondary medical area. Although health care plan should be focused on needs of people live in such units, studies with a view from residents’ perspective have not always been considered adequately. The aim of this study was to examine the factors that effect on utilization of health care facilities located outside of the secondary medical area in a rural region. The data included 515 patients with hypertension, 253 patients with hyperlipidemia, and 104 patients with diabetes. Factors relating utilization of health care facilities were assessed by age, sex, employment, number of diseases, body mass index (BMI), carotid plaque score, HbA1c, distance to the nearest clinic, and distance to the nearest bus stop. We conducted a logistic regression analyses of that data divided into two groups; car use and non car use. The result showed that sex, BMI, carotid plaque score and distance to the nearest bus stop were statistically significantly associated with the utilization of health care facilities located outside of the secondary medical area in patients with car use. Whereas, there were no statistically significantly associations in patients with non car use. Our findings suggested that it is important to consider health utilization of patients with lifestyle related diseases for making sustainable health care plan.

      As is often the case with rural communities in Japan, a dearth of health resources poses a serious problem in a mountainous region of Shimane Prefecture. Although a medical care plan focusing on needs of people should be devised with a view to building up a sustainable health care system, studies have not always adequately been pursued. The aim of this study was to assess the data on the patients with lifestyle-related health problems to and from health care facilities. A Geographic Information System was used for measuring the travel time and road distance. The data for 255 patients with hypertension, 114 patients with hyperlipidemia, and 42 patients with diabetes were amassed. The results showed that the average travel time and distance were much longer in patients with diabetes than those with hypertension. Statistically no significant difference was observed in severity of diabetes (HbA1c, BMI and LDL-c) and blood pressure levels between patients who received regular treatment at clinics in the two towns and those who went all the way to clinics outside of the towns. In addition, compared with the patients with diabetes aged 75 years or over, the average travel time and road distance for aged of 74 years or younger were much longer. Similarly, compared with the patients with hypertension aged 75 years or over, the average travel time and road distance for aged of 74 yeras or younger were also much longer. These results indicated that the diabetics and the elderly age 74 or younger tended to go to health facilities beyond the secondary-medical care zone. These findings also suggested that a closer cooperation between facilities in the neighboring secondary-medical care zones to improve the quality of medical services and support general practitioners to review the existing disease management program would make it possible to cope with the present situation.

    中山間地域の医療体制は,経営の不採算性や医師不足などを背景に診療科や病棟の縮小,閉鎖を余儀なくされている。このため,地域医療を維持していくために住民ニーズに合わせた二次医療圏の地域医療計画が必要であるが,住民の受療行動に基づく実証研究は少ない。そこで,個人承諾を得た島根コホート研究により,地理情報システムを活用して高血圧症,脂質異常症,糖尿病の受療行動を医療機関への移動距離と移動時間に基づき解析した。対象者は,2009年に島根県雲南市大東町,加茂町で実施した健康調査で,現病歴,通院医療機関,職業,移動手段について有効回答を得た高血圧症255名,脂質異常症114名,糖尿病42名である。糖尿病治療者の医療機関への移動距離,移動時間は,高血圧症治療者に比べて有意に長く,糖尿病での市外への通院先は糖尿病専門医による治療や透析治療が可能であった。ただし,糖尿病治療者の雲南市外と市内の対象者間でBMIや耐糖能には,有意な差がなかった。また,年齢別の解析では,74歳以下の治療者が75歳以上の治療者に比べて糖尿病と高血圧症で移動距離と移動時間が有意に長かった。以上より,中山間地域の糖尿病患者や前期高齢者は,二次医療圏を越えて通院する傾向が示された。ただし,隣接二次医療圏間の連携強化や治療の質の標準化,さらには患者の自己管理を総合的に支援するディジーズマネジメントプログラムの検討により二次医療圏内の既存の医療資源で対応できる可能性が推察された。(著者抄録)

    〈緒言〉　中山間地域の医療体制は，医療経営における不採算性，臨床研修制度に起因する医師不足などを背景に診療科や病棟の縮小，閉鎖が余儀なくされている。このため住民ニーズに基づいた地域医療計画の確立は，地域医療を維持していくうえの緊急の課題であり，そのためには住民の受療行動を実証的に明らかにする必要がある。そこで，地理情報システム（GIS）を活用して住民の高血圧症，脂質異常症，糖尿病に関する受療行動を明らかにした。<BR>〈対象と方法〉　2009年に島根県雲南市大東町，加茂町の特定健診で島根大学の健康調査に同意した935名のなかで，高血圧症255名，脂質異常症114名，糖尿病42名を分析対象とした。受療行動は，自宅住所地と受診医療機関所在地との推定移動距離（移動距離）と移動時間（移動時間）をGISにより算出した。なお解析には，ESRIジャパン株式会社ArcGISコレクションプレミアムシリーズ全国道路網（島根県版）を活用した。<BR>〈結果〉　糖尿病の受療者は，平均移動距離が9.6±10.8km，平均移動時間が22.0±21.9分であり，高血圧症に比べて移動距離と移動時間が有意に長かった。さらに，糖尿病治療者の雲南市外の通院先である一般診療所についてみると糖尿病専門医による治療，または透析治療が可能であるものの，雲南市外と市内の対象者間でHbA1c，BMI，LDL-c，収縮期血圧，拡張期血圧に有意な差はなかった。<BR>〈考察〉　今回の対象地域では，高血圧症や脂質異常症に比べて合併庄や血糖コントロールなど専門的な技術や設備が求められる糖尿病治療の特質が住民の行動に影響を及ぼし，住民が二次医療圏を越えて通院していることが推察された。しかし，雲南市外と市内の一般診療所の受療者間で，BMIや耐糖能に有意な差がないことから，合併症の少ない糖尿病については圏域内のかかりつけ医による対応が可能であることが考えられた。

    　　The establishment of a sustainable healthcare system based on residents' needs is an urgent issue in a rural region. For this purpose, considering the structure of a healthcare system, such as the numbers of beds, doctors, and nurses is important. In addition, the study of healthcare utilization using the date gathered through geographic information systems (GIS) would be very helpful in building such a healthcare system. In Japan, however, there are few published studies available on how health services are utilized. The aim of this paper was to review articles of GIS research on healthcare utilization. We conducted a systematic search of published peer-reviewed literature on PubMed. We found 38 articles that satisfied our inclusions criteria for review. Of them, 12 articles had a map for understanding health needs or demands, 23 articles carried analyses of distance and time for health utilizations, and seven articles had a buffer or database for analyses of healthcare utilization. Most of these articles have dealt with pediatric care and emergency care. Given the aging population in a rural region, we concluded that a more evidence-based approach should be taken to rural health focussing on lifestyle-related diseases.
