    気質は就労者のワークライフバランスに影響を及ぼしていることから、就労者の気質と、仕事と家庭のスピルオーバーとの関連を調べた。2020年12月に、日本在住で20歳以上65歳未満の就労者を対象にオンラインでアンケート調査を行い、完全回答が得られた839名の回答を分析した。仕事と家庭のスピルオーバーの評価には日本語版The Survey Work-Home Interaction NijmeGen(SWING)を用い、気質の評価には日本語版The Temperament Evaluation of Memphis, Pisa, Paris and San Diego-auto questionnaire(TEMPS-A)を用いた。その結果、仕事から家庭への負のスピルオーバーは循環、焦燥、不安気質で有意に高かった。家庭から仕事への負のスピルオーバーは、循環気質と焦燥気質が有意に高く、抑うつ気質が有意に低かった。仕事から家庭および家庭から仕事への正のスピルオーバーは発揚気質で有意に高く、焦燥気質で有意に低かった。

    <p>Sickness absences are a significant public health and economic problem worldwide. However, sickness absence diagnoses and trends have not been reported in much detail in Japan. This study was a retrospective cohort study. We examined data on certified diagnoses and the durations of sickness absence lasting over 90 days (long-term sickness absence) from 2009–2018 among city public servants in Japan. We found that 1) “Mental and behavioral disorders” (495.0–780.6 per 100,000 employees) was the most prevalent reason for long-term sickness absence, and “Mood disorders” (318.6–584.3 per 100,000 employees) was the most prevalent mental disorders diagnosis in each study year; 2) the prevalence of long-term sickness absence for mental disorders showed decreasing trends (781/100,000 in 2009 to 622/100,000 in 2018; [p=0.005, for the trend test]); 3) the trends differed by gender (p<0.05) and age (p<0.001); and 4) the duration of long-term sickness absence related to mental disorders (13.2 ± 9.0 months) was longer than long-term sickness absence resulting from all physical disorders except for diseases of the circulatory system (15.1 ± 11.6 months). Increased focus on significant depressive and neurotic disorders is needed when promoting mental health in the workplace. </p>

    市の公務員で、90日以上続いた病気欠勤の診断名とその期間のデータを分析した。その結果、精神および行動障害(495.0～780.6人/100000人)が長期病欠の最も多い理由で、精神疾患の診断名では気分障害(318.6～584.3人/100000人)がそれぞれの年度で最も多かった。精神疾患による長期病欠の有病率は減少傾向(2009年の781人/100000人から2018年の622人/100000人)を示した。性別と年齢では異なる傾向があった。精神疾患に関連する長期病欠の期間(13.2ヵ月)は循環器系の疾患(15.1ヵ月)を除いて、すべての身体疾患による長期病欠よりも長かった。

    症例は72歳女性で、うつ症状を呈しており、夫からの強い勧めにより2020年5月中旬に当院を受診した。精神疾患の既往歴や家族歴はなかった。COVID-19パンデミック前は、地域活動への参加、散歩や旅行、ガーデニング、刺繍などの趣味を持っていた。しかし3月中旬には、地域活動や夫との旅行の予定がすべてキャンセルされ、地域社会から孤立していた。ミニメンタルステート検査スコアは26/30であった。脳MRIにより、脳萎縮や血管疾患の兆候は認められなかった。また、血液生化学検査で異常は認められなかった。神経認知障害の可能性は除外された。DSM-5基準により、臨床的に大うつ病性障害と診断され、抗うつ薬が処方された。

    BACKGROUND: The work required to assist individuals in improving their mental health is stressful and known to be associated with burnout. In Japanese companies, non-medical occupational health (OH) staff often take the role of maintaining and improving workers' mental health. However, few studies have examined burnout in this population. AIMS: To assess the relationship between burnout and occupational stressors among non-medical OH staff. METHODS: We conducted a cross-sectional study of OH staff who had participated in mental health seminars between 2016 and 2018. Occupational stressors were assessed using the Japanese version of the Job Content Questionnaire. Burnout was assessed using the Japanese version of the Maslach Burnout Inventory. RESULTS: We administered the survey to 230 non-medical OH staff, of which 188 completed the questionnaire. According to a hierarchical multiple linear regression analysis, high job demands were associated with greater emotional exhaustion, depersonalization and personal accomplishment. Greater job control was associated only with higher personal accomplishment. Lower job support was associated with greater emotional exhaustion and depersonalization. CONCLUSIONS: The present study found relationships between occupational stressors and burnout dimensions among OH staff. To avoid burnout among non-medical OH staff, it is important to take measures against occupational stressors, especially job demands and low levels of job support.

    森田神経質者にみられる強いとらわれやこだわりは、自閉症スペクトラム(ASD)の特徴である柔軟性の乏しい行動や予測困難性などとの関連が疑われる。そこで森田神経質傾向とASD傾向について森田神経質調査票とASD指数日本語版(AQ-J)を用いその関連を調査した。当院外来通院患者39名(男性22名、女性17名;44.4歳)を対象とした。AQ-J総得点と森田神経質調査票の得点には正の相関を認めた。森田神経質調査票の下位尺度の中では弱力的傾向のみがAQ-J総得点と正の相関を示した。森田神経質調査票の得点とAQ-Jの下位尺度である社会的スキル、注意の切り替え、コミュニケーション、想像力との間に正の相関を認めた。森田神経質の弱力性のみがAQ-Jの下位尺度と正の相関を示した。本研究の結果よりASD傾向と森田神経質の弱力性との関連がある可能性が認められた。生来のASD傾向が森田神経質の弱力性の一部を形作り、森田神経質の中でも独立した要因である可能性が示唆された。(著者抄録)

    うつ病と適応障害の就労者の、森田神経質傾向、気質特性(循環気質、発揚気質)との関連を明らかにすることを目的に、当院の当該外来患者82名(男性46名、女性36名、平均年齢41.8±10.6歳)を対象に、森田神経質調査票、気質を検出する質問紙であるTEMPS-Aから循環気質(気分やエネルギーのムラが大きいタイプ)と発揚気質(楽観的でエネルギッシュなタイプ)の42項目を用いた質問紙調査を行った。うつ病群と適応障害群で比較検討した結果、うつ病と適応障害では森田神経質や循環気質、発揚気質などの素因に差は認められなかった。一方、うつ病患者群では森田神経質傾向と循環気質傾向に正の相関を認めたが、適応障害患者群では認めず、両者の成因に違いがあることが推測された。

    Background: Stigma related to mental illness can be an obstacle affecting the quality of life of people with mental illness. Although mental illness in the workplace is a public problem globally, few studies have investigated the effect of stigma on job-related problems such as burnout. Aim: This study aimed to clarify the association between mental-illness-related stigma and burnout among nonprofessional occupational mental health staff. Methods: In this cross-sectional study, nonprofessional occupational mental health staff's perceived mental-illness-related stigma was assessed using Link's Devaluation-Discrimination Scale, and their burnout was assessed using the Maslach Burnout Inventory. The association between stigma and burnout was analyzed by multiple linear regression analysis. Results: In total, 282 participants completed the questionnaire (response rate: 91.3%). We excluded 54 nurses from the analysis to examine strictly nonprofessional occupational mental health staff. Finally, 228 eligible respondents were surveyed. Multiple linear regression analysis revealed that mental-illness-related stigma was significantly associated with a high degree of depersonalization, which was one of the burnout dimensions. However, the impact of stigma over the depersonalization domain of burnout was minor. Conclusion: The results suggest that higher perceived mental-illness-related stigma is associated with more severe burnout. It is important to take measures against mental-illness-related stigma to avoid burnout among occupational mental health staff.

    精神障害による長期休職者(LTSA-MD)と非LTSA-MDにおける職業性ストレス、ストレス反応、社会的サポートを比較した。2011年に職業性ストレス簡易調査票(BJSQ)に回答した公務員のうち、精神障害のため90日以上の長期休職に至ったLTSA-MD 191名と、傾向スコアマッチング法により選定した非LTSA-MD 191名を対象とした。ウィルコクソンの符号順位検定を用いて、BJSQの各カテゴリーのスコアを両群間で比較した。2011年のBJSQでは、LTSA-MD群は非LTSA-MD群に比べ、仕事のストレス要因では高い質的労働負荷、低い技能活用度を、ストレス反応では活気の低さ、イライラ感の高さ、疲労感、不安感、抑うつ感、身体愁訴を、社会的サポートでは同僚からの支援の少なさを認めた。仕事と生活の満足度では低い結果を認めた。

    自閉性スペクトラム傾向(ASD)と森田神経質傾向の関連を自己記入式調査票によって調査し、共通である要素について検討した。当院外来通院患者のうち、本研究に同意が得られた39名(男性22名、女性17名、平均44.4±13.2歳)を対象とした。診断名は気分障害圏が19名、神経症圏が14名、その他6名であった。AQ-JからASDと考えられたものは10名(25.6%)、森田神経質調査票から森田神経質と考えられたものは13名(33.3%)であった。AQ-J合計得点と森田神経質調査票の総得点には正の相関を認め、森田神経質調査票の下位尺度の中では弱力的傾向のみがAQ-J総得点と正の相関を示していた。また、森田神経質調査票の総得点とAQ-Jの下位尺度の社会的スキル、注意の切り替え、コミュニケーション、想像力との間に正の相関を認めた。

    <p>After workers take long-term sickness absence due to mental disorders (LTSA-MD), the occupational stress of the coworkers in the same work unit might be affected. The aims of this study were to evaluate the effect of the incident of LTSA-MD on the coworkers' occupational stress. A retrospective cohort study of 16,032 public servants was conducted. The Brief Job Stress Questionnaire (BJSQ) was used, which was administered in 2011 and 2012. To analyze the amount of change in occupational stress, the difference between the scores of the BJSQ scales in 2011 and 2012 was calculated. After adjusting for the baseline BJSQ scales, sex, age, total number of workers, and social support, analysis of covariance of the difference between the BJSQ scales' scores showed that job stressors and stress responses worsened among the coworkers after the incident of LTSA-MD. Social support did not change among the coworkers. This study indicates that an incident of LTSA-MD in the same work unit adversely affects the coworkers' occupational stress. Focusing on the coworkers' mental state after an incident of LTSA-MD in the same work unit and an early intervention strategy are needed to prevent secondary mental illness and sickness absence in the coworkers.</p>

    精神障害による長期病欠(LTSA-MD)の発生が同僚の職業性ストレスに及ぼす影響について検討した。2011〜2012年に地方公務員16032名(男性11178名、女性4854名、平均42.0±9.5歳)を対象とする後向きコホート研究を行った。職業性ストレスは、職業性ストレス簡易調査票(BJSQ)により評価した。調査期間中に108名が90日を超えるLTSA-MDを取得した。ベースラインのBJSQスコア、性別、年齢、総従業員数、社会的サポートを調整後、LTSA-MD発生後の同僚では職務ストレッサーおよびストレス反応が悪化していた。同僚における社会的サポートの変化は認めなかった。以上より、同一部署のLTSA-MD発生は、同僚の職業性ストレスに悪影響を及ぼすことが示された。同僚における二次的な精神障害や病欠を予防するには、早期の介入的戦略が必要であると考えられた。

    <p>After workers take long-term sickness absence due to mental disorders (LTSA-MD), the occupational stress of the coworkers in the same work unit might be affected. The aims of this study were to evaluate the effect of the incident of LTSA-MD on the coworkers' occupational stress. A retrospective cohort study of 16,032 public servants was conducted. The Brief Job Stress Questionnaire (BJSQ) was used, which was administered in 2011 and 2012. To analyze the amount of change in occupational stress, the difference between the scores of the BJSQ scales in 2011 and 2012 was calculated. After adjusting for the baseline BJSQ scales, sex, age, total number of workers, and social support, analysis of covariance of the difference between the BJSQ scales' scores showed that job stressors and stress responses worsened among the coworkers after the incident of LTSA-MD. Social support did not change among the coworkers. This study indicates that an incident of LTSA-MD in the same work unit adversely affects the coworkers' occupational stress. Focusing on the coworkers' mental state after an incident of LTSA-MD in the same work unit and an early intervention strategy are needed to prevent secondary mental illness and sickness absence in the coworkers.</p>

    Most teachers have a high risk of work-related stress and mental disorders. Drunken driving and hazardous alcohol consumption (HAC) among teachers are social problems. Gender differences among teachers in burnout, occupational stress, self-efficacy and job satisfaction were reported. This study aimed to clarify gender differences in the relationships between perceived individual-level occupational stress and HAC among Japanese teachers. A cross-sectional study was conducted in 2013 and a total of 723 male and 476 female teachers remained after excluding non-drinkers. Perceived individual-level occupational stress was assessed using the Generic Job Stress Questionnaire. HAC was defined as ethanol consumption greater than or equal to 280 g in 1 week for male teachers, and greater than or equal to 210 g for female teachers. Multiple logistic regression analyses were conducted. HAC was identified in 16.6% of male and 12.4% of female teachers. The average ages (± standard deviation: SD) of male and female teachers were 46.9 ± 10.9 years and 39.9 ± 12.3 years, respectively. Schoolteacher was the most common position classification among male (48.7%) and female teachers (86.3%). For those with a moderate level of stress, "social support from supervisors" was associated with HAC among males (odds ratio [OR] = 0.43, 95% confidence interval [CI] = 0.23-0.8), whereas for female teachers with a high level of stress, "variance in workload" was associated with HAC (OR = 2.09, 95% CI = 1.04-4.24), using an adjusted model. This study showed that moderate social support from supervisors was negatively related to HAC among male teachers, and high variance in workload was positively related to HAC among female teachers. Gender differences need to be considered when developing HAC prevention strategies for teachers.

    セロトニン症候群とは、脳内セロトニン機能の異常亢進によって、中枢神経症状、自律神経症状などを呈する症候群である。今回、我々は抑うつ状態に対し加療中にセロトニン症候群を呈した1例を経験した。症例は68歳男性。抑うつ状態に対しparoxetine漸増10日後と遅い時期に錯乱状態となり、さらに3日後より焦燥や不安などの精神症状、意識障害、振戦、発汗、腱反射の亢進が出現し寡動緘黙状態で入院となった。薬剤中止後は徐々に症状の改善を認め、発症22日目に全症状が消失した。抗うつ薬増量後は、24時間以上経過した場合でもセロトニン症候群を念頭に神経学的症状に留意する必要がある。(著者抄録)

    2011年の簡易職場ストレス質問票データを用いて、部門の精神疾患による長期病欠発生率と、部門の従業員が経験した職場ストレスの関連性を後向きに検討した。対象は公務員12356名(男性8766名、女性3590名、平均43.1±10.6歳)で、管理職者は4339名、従業員は8017名であった。2011〜2013年に精神疾患による長期病欠を取得したのは135名であった。精神疾患による長期病欠リスク上昇と関連する管理職者の因子は、質的に高い過重労働、劣悪な物理的環境、技術の活用が十分でないことであった。精神疾患による長期病欠リスク上昇と関連する従業員の因子は、身体的負荷が高いこと、対人関係の衝突が少ないこと、劣悪な物理的環境、職務コントロールが高いこと、職務の報酬が低いことであった。精神疾患による長期病欠リスクを軽減するためには、職場ストレス因子の軽減(特に心理的および物理的職場環境の改善)が重要であることが示された。

    Insomnia among workers reduces the quality of life, contributes toward the economic burden of healthcare costs and losses in work performance. The relationship between occupational stress and insomnia has been reported in previous studies, but there has been little attention to temperament in occupational safety and health research. The aim of this study was to clarify the relationships between temperament, occupational stress, and insomnia. The subjects were 133 Japanese daytime local government employees. Temperament was assessed using the Temperament Evaluation of Memphis, Pisa, Paris, and San Diego-Auto questionnaire (TEMPS-A). Occupational stress was assessed using the Generic Job Stress Questionnaire (GJSQ). Insomnia was assessed using the Athens Insomnia Scale (AIS). Stepwise multiple logistic regression analyses were conducted. In a stepwise multivariate logistic regression analysis, it was found that the higher subdivided stress group by "role conflict" (OR = 5.29, 95% CI, 1.61-17.32) and anxious temperament score (OR = 1.33; 95% CI, 1.19-1.49) was associated with the presence of insomnia using an adjusted model, whereas other factors were excluded from the model. The study limitations were the sample size and the fact that only Japanese local government employees were surveyed. This study demonstrated the relationships between workers' anxious temperament, role conflict, and insomnia. Recognizing one's own anxious temperament would lead to self-insight, and the recognition of anxious temperament and reduction of role conflict by their supervisors or coworkers would reduce the prevalence of insomnia among workers in the workplace.

    Japanese teachers are mentally and physically burdened with various work stressors. This cross-sectional study examined the relationship between depressive symptoms and perceived individual level occupational stress including role problems among Japanese schoolteachers. This study included 1,006 teachers working in public schools in a Japanese city. The Japanese version of Zung's Self-Rating Depression Scale (SDS) was used to evaluate depressive symptoms, and the Generic Job Stress Questionnaire was used to evaluate occupational stress and three measures of social support. Subjects with SDS scores of more than 50 were categorized into the "depressive group." We examined the relationship between depressive symptoms and perceived individual level occupational stress using multiple logistic regression analyses. A total of 202 (20.1%) teachers belonged to the depressive group. We found that high role ambiguity, high role conflict, high quantitative workload, and low social support from family or friends were significantly related to depressive symptoms. To moderate role ambiguity and role conflict experienced by teachers, it is necessary to clarify the priority order of teachers' work. Furthermore, it is necessary to reduce workload by focusing on the content of teachers' work and the setting of education itself. Focusing on these elements will reduce teachers' depressive symptoms.

    The relationship between temperaments and mental disorders has been reported in previous studies, but there has been little attention to temperaments in the occupational safety and health research. The aim of this study was to clarify the effects of temperaments on occupational stress among local government employees. The subjects were 145 Japanese daytime workers in local government. Temperaments were assessed by the Temperament Evaluation of Memphis, Pisa, Paris, and San Diego-Auto questionnaire (TEMPS-A). Occupational stress was assessed using the Generic Job Stress Questionnaire (GJSQ). Hierarchical multiple linear regression analysis was used. Hyperthymic temperament predicted a higher level of job control, and a lower level of role ambiguity and job future ambiguity. Irritable temperament predicted a lower level of social support from supervisors and a higher level of role conflict, variance in workload and intragroup conflict. Anxious temperament predicted a lower level of social support from coworkers and a higher level of job future ambiguity. The sample size was small. Only Japanese local government employees were surveyed. Hyperthymic temperament played a protective role, and irritable, anxious temperament played a vulnerable role against one's own occupational stress and recognizing the roles they play in work life would lead to self-insight. Additionally, recognition of the temperaments and temperament-related stressors by one's supervisors or coworkers would facilitate provision of social support.

    幼少時期のストレスフルな出来事は摂食障害発症と関係していると言われている。また、雌ラットにおいては性周期により摂餌行動が影響を受けることはよく知られている。そこで我々は、性周期を同調させた雌ラットを用いて、幼少時期のストレスが成長後の食行動について影響するかどうかを調べた。心理的ストレスとして母仔分離ストレスを用い、幼少時期に親と1日6時間分離されたラット群と母仔分離を行わない群のラットを比較した。腟インピーダンスを測定することにより性周期を測定し、性周期1サイクル中のそれぞれの群において1日摂餌、飲水量を測定した。非母仔分離群では性周期により摂餌、飲水にて周期的な変化を示したが、母仔分離を受けた群では、性周期による変化が失われていた。発情期では母仔分離群は非母子分離群に比して摂餌減少と飲水増加と逆の傾向が見られた。このことから、雌ラットにおいて幼少時期のストレスは性周期における摂餌量の周期的変化を阻害し、幼少時期のストレスにより成長後においても摂餌行動に対して持続する影響が及ぼされることが示唆された。(著者抄録)

    Traumatic and stressful events during childhood are associated with the development of eating disorders. We conducted an animal study to test if association stress in childhood affects ingestive behavior later in life by using female rats that have an adjusted estrous cycle. Food and water intake in maternal separated and non-separated rats was measured in each estrous phase. First, electrical impedance of the vagina was conducted to test estrous cycle adjustment. Second, the effects of 6 h per day maternal separation from birth to weaning, which models a psychologically stressful experience in childhood, was used to test feeding behavior during an ovarian cycle in female adult rats with matched estrous cycles. Non-separated rats showed periodical changes, but maternal separated rats showed no significant changes in food and water intake during an estrous cycle. An opposing tendency for food and water intake was seen between maternal separated and non-separated rats. These observations suggest that electrical impedance of the vagina showed the highest value in the estrous phase of rats housed in a reversed light-dark cycle, and maternal separation was found to disturb changes in feeding behavior during the estrous cycle.

    長年、大うつ病性障害として治療し、倦怠感、意欲低下や食欲低下などの遷延と認知機能低下が生じ加療目的で入院となり、精神運動抑制を含むこれらの症状について大うつ病エピソードの再発、アパシーなどとの鑑別に苦慮し、不整脈の出現を契機にジギタリス中毒の診断に至ったアルツハイマー型認知症(DAT)の75歳の男性の症例を報告した。入院時主訴は、本人は気分がふさぐ、家族からは3分前のことを忘れてしまう、というものであった。入院時診断はDATの疑い、大うつ病エピソード再発の疑いであった。第42病日頃より連日40回台/分の徐脈傾向、散在性の心室性期外収縮を認め、物が二重に見えると訴えた。ドネペジルを中止したが変化無く経過し、第49病日には36回/分の不正な徐脈、2から3連発の心室性期外収縮を認め、ジゴキシン血中濃度が3.7と高い値を示した。ジギタリス中毒を疑い、すべての内服を中止、補液などの対応となった。ジゴキシン中止後速やかに改善した。

    外来での森田療法的アプローチが有効であった社交不安障害(SAD)を併存するアルコール症(Al症)者の一例を報告した。症例はAl症専門外来を受診、断酒プログラム開始となった45歳女性。社交不安が強くグループミーティング出席や発言に多大な苦痛を伴うことが判明、SADと診断し外来での森田療法的アプローチを開始した。対人不安や社交不安を回避するために飲酒し、自己評価が低下し不安になる、などの悪循環を打破するべく、発言せずともグループミーティングに出席する、抗酒剤内服、1日断酒、など断酒のためにできることを続けるよう促した。次第に社交不安は軽減、グループミーティングで発言できたことや1年以上の断酒が自信に繋がり、患者自ら院外のAAや断酒会、社会活動への参加を希望するようになった。社会生活に復帰したいという欲求に対し目的本位の行動ができるようになったと考えられ、社交不安障害を併存するAl症者に対して有効と考えられた。(著者抄録)

    身体化症状に対する強いとらわれと、元来の生の欲望に基づくと考えられる理想化された過去への強いとらわれが見られる身体表現性障害(転換性障害)の症例に対し、外来にて森田療法的アプローチを試みたので若干の考察を加えて報告した。症例は結婚して一子をもつ主婦であるが、現状の生活の行き詰まりがあり、そのことがかえって女性、主婦、母親の理想像への希求を強め、「自分は価値のない人間である」「無能な人間である」という事への恐怖とその裏返しの「人に認められたい」という生の欲望を肥大させていった。その結果、現在の人生を選択したという過去の決断を、理想の実現を妨げたものとしてとらわれていると考えられた。理想への欲求は当然のものと普遍化しつつも、その過剰さを指摘し、その欲求を現実的に対処することに向けていくことを促していくことで、過去への判断へのとらわれも軽減していったものと思われる。また、過去へのとらわれは元来の生の欲望や自己愛性を表していることから、それを治療者が把握しておくことで、行動の促しを支持する際の一助となると考えられた。(著者抄録)

    52歳女。甲状腺機能低下症でレボチロキシン50μg/日を服用していた。8年前に抑うつ気分、不眠、意識低下を主訴とした大うつ病エピソードで発症した。数種類の抗うつ薬を試みたが効果がなく、3度の大うつ病エピソードを経験していた。3年前にパロキセチン20mg/日から40mg/日への増量を試みたところ、軽躁状態となり、2年前に双極性障害と診断された。その後、炭酸リチウム、クエアチピン、バルプロ酸などが効果がなく、急速交代化した。しかし、ラモトリギンとバルプロ酸の併用にて1年4ヵ月の寛解状態が維持できた。ラモトリギンは25mg/日をバルプロ酸400mg/日に加えて、隔月投与から開始して、2週間後から連続投与とした。5週目よりはラモトリギンを50mg/日、7週間後には75mg/日へ増量したが、副作用を訴えたため、50mg/日へと減量した。その後1年4ヵ月は再燃がなかった。

    疼痛性障害と診断された症例に対して森田療法的アプローチを用いて症状が改善した3例を報告した。3例に共通する要素として、当科受診前に多数の医療機関を受診し解決を求めていたが、期待するような結果は得られず、かえって症状を取り去ることに執着し、痛みが生活、考えの中心に常に居座るようになり、生活を規定し、痛みを取り去らないと生活が出来ないと考えるようになっていた。そのことが痛みへの感覚を鋭敏にし、そのような痛みとの関わりの中で、徐々に疲弊し、自分で対処することを避けるようになり、他者に解決を依存する状態になっていく。治療枠組みとして原因追及、鎮痛剤投与などは行わない約束をし、支持的対応とともに森田療法的アプローチを行った。痛みの症状は軽快したが、他の症状に推移することを認め、適切な生の欲望の発揮と、生活の中でのとらわれの姿勢そのものの治療が重要であると考えられた。(著者抄録)

    Work of psychiatrists in general hospitals covers such diverse tasks as visiting patients in other departments. The situation and roles are different between psychiatrists in the same general hospital because demand for education and research, the role in psychiatric emergency systems, and involvement in management vary with each hospital category and its bed presence. Occupational stress is often described as "demand-control-support imbalance" and "effort reward imbalance," and psychiatrists in public general hospitals with beds seem to suffer more occupational stress. In consideration of the wide range of diversity of general hospital psychiatrists, individual support to psychiatrists should be considered from every aspect, such as self-help, mutual help, and public aid. For example, many general hospital psychiatrists find it worthwhile to treat many interesting cases such as symptomatic psychoses, therefore enhancement of such an advantage is related to self-help. To upgrade the support system of human resources from universities as needed (as mutual help), and to hire a medical secretary and to increase economic support (as public aid) are preferable.

    The patients of eating disorders like anorexia nervosa or bulimia nervosa are increasing in number especially among young female. However, the pathology of eating disorders has not been clarified because so many factors as socio-cultural factors, psychological factors, and biological factors are indicated to be concerned. In addition, the methods of treatment have not been established yet. To investigate the biological brain mechanisms of eating disorders, many types of animal models have been proposed. According to the clinical feature of the patients, anorexic status in adolescent female rats was set by food restriction. Psychological and physiological stress changed the feeding behavior in the anorexic status, and the relations of the behavioral changes and the brain neurotransmitters were examined. Those results suggested that abnormalities in many brain neurotransmitters are concerned in anorexic behavior, binge-eating behavior, and anxiety. We can speculate some part of biological bases of human eating disorders, and effective prevention and treatment through such animal models.

However, the pathology of eating disorders has not been clarified because so many factors as socio-cultural factors, psychological factors, and biological factors are indicated to be concerned. In addition, the methods of treatment have not been established yet. To investigate the biological brain mechanisms of eating disorders, many types of animal models have been proposed. According to the clinical feature of the patients, anorexic status in adolescent female rats was set by food restriction. Psychological and physiological stress changed the feeding behavior in the anorexic status, and the relations of the behavioral changes and the brain neurotransmitters were examined. Those results suggested that abnormalities in many brain neurotransmitters are concerned in anorexic behavior, binge-eating behavior, and anxiety. We can speculate some part of biological bases of human eating disorders, and effective prevention and treatment through such animal models.

    Corticotropin-releasing factor (CRF) plays an important role in stress responses and is mediated through two subtypes of receptors, CRF receptor type 1 (CRFR1) and CRF receptor type 2 (CRFR2). Each CRF receptor might have a different function through several neurotransmitter systems; however, the mechanism remains unclear. To clarify the role of each receptor in dopamine (DA) metabolism, we measured the change of extracellular concentrations of DA and the metabolites in the ventromedial hypothalamus (VMH) that played important roles in the stress response of freely moving female rats in response to the direct administration of comparative CRFR1 selective agonist, CRF, or CRFR2 selective agonist, Urocortin II (Ucn II), into the brain region. Administration of 10 mu g CRF increased extracellular concentrations of DA compared with 2 mu g CRF immediately after injection, and this effect was not observed after 60 min of 10 mu g CRF injection. On the other hand, this change did not always occur after Ucn II administration. These results suggest that the activation of CRFR1, but not CRFR2, modulates the release of DA in VMH.     Although neurochemical changes have been reported in the brain in animal models of binge eating, biochemical changes of specific proteins in the brain are unknown. Our aim was to elucidate brain proteins altered in rats during enhanced rebound hyperphargia. Rats were deprived of food for 22 h/day for 6 days, then allowed free access to food for 24 h in normal cages (rebound hyperphargia) or in space-restricted cages (enhanced rebound hyperphargia). Proteins extracted from the rat brain were separated by two-dimensional gel electrophoresis, and compared with those from control rats freely fed for 7 days in normal cages. Proteins expressed differently from controls were identified by N-terminal amino acid sequencing and mass fingerprinting using a MALDI-TOF mass spectrometer. Among proteins in the corpus striatum, frontal lobe, hippocampus and thalamus/hypothalamus, ubiquitin C-terminal hydrolase L1 and peroxiredoxin 2 decreased in the hippocampus and phosphatidylethanolamine-binding protein increased in the thalamus/hypothalamus of rats with the enhanced rebound hyperphargia induced by space-restriction. In this study, we first demonstrated that three brain proteins changed in rats during enhanced rebound hyperphagia. These proteins might have pathophysiologic relevance to binge eating.

    神経性やせ症や神経性過食症などの摂食障害患者は若い女性を中心に増加している。職域においても職業性ストレスが過食症状等の食行動異常に関連しており,食行動異常は心理的要因,生物学的要因,社会文化的要因などが複雑に相互に絡み合って生じると推測されるが原因は明らかではなく,対応方法も確立されていない。このような状況で,動物モデルを用いて摂食行動異常の脳内機序の解明が進められている。近年は臨床的知見を考慮し,思春期の雌ラットで摂餌を制限してダイエット類似状態をつくり出したうえで,心理的・身体的ストレスを加えることによる摂餌行動の変化と脳内神経伝達物質との関連などが検討されるようになった。そこから食行動異常としての拒食や過食,不安症状などにおける多くの神経伝達物質や受容体の役割が推定されている。これら知見を総合的に解釈していくことは過食症状を含めた食行動異常の生物学的基盤の解明と,合理的で有効性の高い予防法および対応方法の確立に結びつくものと考える。(著者抄録)

    ＜ポイント＞妄想性障害患者は病識が乏しく、自ら進んで受診することはまれである。同僚が妄想の対象となると人間関係への影響は非常に大きく、職場で事例化すれば対応は難しい。妄想性障害では被害型がもっとも多い。妄想性障害に対する早期の気づき、早期対応のためには、事例性に基づいた変化への気づき、具体的には、事実に反した内容のことを繰り返し話すなどの変化を見逃さないことや、気づいたら複数で対応すること、情報を共有すること、証拠を残すことが重要である。産業保健スタッフは、二次的に出現しうる不眠、不安などの精神症状や頭痛、嘔気などの身体的不調を相談する目的で精神科を受診するよう促すのがポイントである。(著者抄録)

    急激な社会経済状況の変化に伴い,職域でうつ病を含めたメンタルヘルス不調が増えている.産業医であれば内科医であってもメンタルヘルス不調に対応する必要があり,その場合,安全配慮義務と危機管理を念頭に事例性に注目する.メンタルヘルス不調への一次〜三次予防として国は,「事業場における労働者の心の健康づくりのための指針」,「心の健康問題により休業した労働者の職場復帰支援の手引き」など,さまざまな指針や手引きを示している.うつ病を含めたメンタルヘルス不調に対して,産業医は国の示した指針や手引きなどを理解し,精神科主治医との立場の違いを理解し,連携を行うことが求められる.(著者抄録)

    睡眠障害が職場のメンタルヘルスや社会経済に及ぼす影響が大きくなっている。睡眠をめぐる問題は職場内での対応や産業医への相談だけでは解決が困難な場合が多く専門機関での治療を要することがある。近年認知療法の普及に伴い、不眠症に対する認知行動療法(Cognitive Behavioral Therapy for Insomnia)がわが国でも注目されるようになってきた。これは従来睡眠障害に対して行われていた行動療法的アプローチであるものを発展的に認知療法と融合させたものと考えられる。刺激制御法や睡眠制限法などの従来の行動療法的アプローチとCBTIの概略を簡単に紹介する。(著者抄録)
