<p>The Hashimoto Research Group for Comprehensive Research of Gene Mutation-related Rare and Intractable Diseases of the Skin is a contributor to the Project for Research on Intractable Diseases of the Ministry of Health, Labor, and Welfare (MHLW) of Japan. Our research group performs clinical research on 23 rare intractable genetic skin diseases that are classified into eight disease groups. Among the 23 diseases, 17 are mainly studied by our research group, and 6 diseases are studied in collaboration with other research groups. Cockayne syndrome and familial chronic and benign pemphigus (also known as Hailey-Hailey disease) are the designated intractable diseases that are mainly studied by our research group. This review summarizes the activities of our research group for these 23 intractable hereditary skin diseases, including the MHLW tasks for designated intractable diseases, epidemiological studies using nationwide surveys, preparation of patient registries, creation of repositories, development and publication of clinical practice guidelines, clinical trials for novel treatments in collaboration with the Japanese Agency for Medical Research and Development, help with genetic diagnosis, applications for the listing of new designated intractable diseases, communication of information to academic societies, medical professionals and patients, spreading awareness of our activities to the public, supporting patient societies, and presentation and publication of achievements. These studies are performed in collaboration with the relevant academic societies, mainly the Japanese Dermatological Association.</p>

＜文献概要＞76歳,男性。3年前から左上肢の腫脹,1ヵ月前から左環指,小指の疼痛を伴う紫紅色斑,腫脹を自覚した。病理組織学的に,真皮から脂肪織に小型血管が増加していた。CD34は血管内皮細胞で陽性,D2-40,LANA-1は陰性だった。CT angiographyでは,動脈相で静脈が濃染され,左肘部から環指,小指に結節状の濃染がみられた。これらより,左上肢動静脈奇形に伴う偽カポジ肉腫と診断した。マイクロシャントを多数認め,病変が広範囲であったため血管内治療,外科的切除術は困難と判断した。初診5ヵ月後に病変から多量に出血し,左上肢切断術を施行した。動静脈奇形に伴う偽カポジ肉腫では,血管内治療や外科的切除術が困難な場合,切断術が必要となることがある。

80歳男性。左下腿瘢痕癌、左鼠径リンパ節に対して瘢痕癌切除と左鼠径・左骨盤リンパ節郭清術が行われたが、術後2ヵ月で局所再発、in-transit転移、郭清巣の再発を生じた。放射線治療を行う方針であったが、経過中に敗血性ショックを生じ、入院23日目に死亡となった。病理解剖所見から左鼠径部の嚢腫状再発に感染が生じ、敗血症をきたしたと考えられた。

＜文献概要＞55歳,男性。顔面を初発とし,体幹にも拡大する紅斑を主訴に当科を紹介受診した。顔面全体の紅斑とGottron徴候がみられ,体幹は紅皮症へと進展していた。筋逸脱酵素の上昇,筋電図やMRI所見より皮膚筋炎が疑われたが,皮膚筋炎特異抗体はいずれも陰性であった。左上腕伸側の紅斑部からの皮膚生検では,表皮内に個細胞壊死,真皮乳頭層に著明な浮腫がみられた。筋生検組織の免疫組織化学染色で筋線維にミクソウイルス耐性蛋白質Aが発現していたことより皮膚筋炎と診断し,抗SAE抗体陽性が判明した。皮膚筋炎特異抗体陰性例では,筋生検の施行や抗SAE抗体を含めた自己抗体の探索が確定診断に有用となる。

＜文献概要＞・3歳時に小児汎発性膿疱性乾癬(generalized pustular psoriasis:GPP)と診断され,エトレチナート,シクロスポリンによる治療,さらに2度のバイオスイッチを要し,幼少期より長期にわたり治療を継続している1例を報告する.・本邦では2022年に小児の尋常性乾癬,関節症性乾癬,膿疱性乾癬に対してIL-17阻害薬のセクキヌマブが6歳以上の小児に対しても保険適用となった.生物学的製剤を含めた小児膿疱性乾癬の治療について,有効性,安全性を含めて考察する.(「症例のポイント」より)

<p>Photodynamic therapy (PDT) is a promising technique for the treatment of skin ulcers infected with multidrug-resistant bacteria because of its ability to prevent the emergence of new multidrug-resistant bacteria. We aim to use portable and disposable organic light-emitting diodes (OLEDs) as a PDT light source for home treatment of infected skin ulcers. OLEDs, as flexible surface light sources, provide uniform and widespread light delivery by adhering to the skin. However, OLEDs emit lower light intensity than traditional, relatively larger PDT light sources, which requires evaluation of differences in treatment effects due to variations in irradiation conditions. In this paper, we review the bactericidal and antitumor effects of low-intensity and prolonged illumination PDT and compare these effects with those of traditional PDT. In addition, we discuss the trend in the development of optical devices and introduce 5-aminolevulinic acid-mediated PDT using OLED light with low-intensity and prolonged illumination against methicillin-resistant <i>Staphylococcus aureus</i> and <i>Pseudomonas aeruginosa</i>.</p>

光線力学治療(photodynamic therapy:PDT)は,多剤耐性菌を生じないモダリティとして,多剤耐性菌感染皮膚潰瘍の治療への応用が期待されている.我々は,在宅での感染皮膚潰瘍のPDTに,ポータブルでディスポーザブルな有機発光ダイオード(organic light-emitting diode:OLED)を光源として使用することを目的に研究を行っている.OLEDは,フレキシブルな面発光源であるため,皮膚に貼り付けて広範囲を均一に光照射できる利点がある.しかし,OLEDは従来の比較的大型なPDT光源より発光強度が低いため,照射条件の違いによる効果の差異を評価する必要があった.本稿では,低輝度・長時間照射によるPDTの殺菌効果と抗腫瘍効果を従来のPDTと比較して解説する.さらに,照射デバイスの開発動向について議論し,5-アミノレブリン酸を用いたメチシリン耐性黄色ブドウ球菌と緑膿菌に対するOLEDを用いた低輝度・長時間照射のPDTを紹介する.(著者抄録)

78歳,男性。初診の数日前から出現した右側腹部と両鼠径部のそう痒を伴う紅斑を主訴に受診した。貨幣状湿疹による自家感作性皮膚炎と考え,ステロイド外用で治療を開始したが,鼠径部に紅斑と一部に新生水疱とびらんを生じたため,自己免疫性水疱症を疑い精査を行った。病理組織像では表皮下水疱を認め,蛍光抗体直接法ではIgGとIgAが表皮基底膜部に線状にわずかながらに沈着しており,暫定的に線状IgA/IgG水疱性皮膚症と判断し,ジアフェニルスルホン単剤で診断的治療を開始した。蛍光抗体間接法を含む血清解析では水疱性類天疱瘡,後天性表皮水疱症は否定された。ジアフェニルスルホン単剤(25mg/day)にて皮疹に奏効し,ジアフェニルスルホンを漸減,中止していたが,症状が再燃し,初期量に戻すもコントロールが得られず,再度精査を行った。病理組織学的所見は前回と同様であったが,蛍光抗体直接法でIgGとIgAが表皮基底膜部に明瞭に線状に沈着していたことから,線状IgA/IgG水疱性皮膚症の診断に至り,増悪時にはジアフェニルスルホン単剤(50～75mg/day)を増量することでほぼ皮疹の無い状態を維持することができている。また,自験例では膀胱癌を併発しているように,当疾患は悪性腫瘍を合併することが多いとされ,積極的な悪性腫瘍検索が望まれる。(著者抄録)

日本人の白斑またはロドデンドロール誘発性白斑の診断がある患者35例と年齢・性別をマッチさせた健康対照19例の皮膚におけるミクロフローラの相違を分析した。頻繁に影響される前額部と背中に焦点を置いた。健康対照に比べて白斑患者の非病変領域と病変領域で細菌叢のα多様性が増加した。白斑患者群内で非病変領域と病変領域の間で有意な相違はなかった。健康対照に比べて白斑患者の両方の領域でGammaproteobacteriaの存在量が上昇し、白斑患者の背中の非病変領域でCorynebacterium spp.が有意に増加した。状態の安定期にEnterococcus spp.が有意に減少し、進行期にさらに減少した。改善を示した症例でStaphylococcus spp.の割合は有意に増加した。白斑病変は健康対照に比べて真菌叢のα多様性がわずかに高かった。Malassezia sympodialisは両方の部位で安定期に有意に減少し、背中では安定期に比べて進行期にM.globosaの存在量が増加した。以上より、白斑患者において特有のミクロフローラの存在が示唆された。

＜文献概要＞症例のポイント　・病理組織学的に表皮下水疱を認め,1M食塩水剥離皮膚を用いた蛍光抗体間接法でIgGが真皮側に反応し,免疫ブロット法の結果から抗ラミニン332型類天疱瘡と考えたが,病初期に粘膜病変がないことが非典型的であった.・病初期に粘膜病変がなく,水疱とびらんが頭頸部と体幹上部に限局しているという特徴的な臨床像からBrunsting-Perry型類天疱瘡と最終診断した.ラミニン332に反応する症例は比較的まれである.

INTRODUCTION: Atopic dermatitis (AD) is a common and relapsing skin disease characterized by skin barrier dysfunction, inflammation, and chronic pruritus. Both cutaneous barrier dysfunction and immune dysregulation are critical etiologies of the pathology of AD. Although various anti-inflammatory pharmacological agents, including cytokine inhibitors and signaling pathway blockers, have been developed recently, keeping the skin clean is of utmost importance in maintaining physiological cutaneous barrier function and avoiding an AD flare. Ultrafine bubbles (UFBs) are less than 1 μm in diameter and usually used to clean medical equipment. A UFB shower is expected to keep skin clean with attention to the temperature and strength of the shower. METHODS: We examined the effects of a UFB shower on two mouse models of AD: Dermatophagoides farinae body (Dfb)- induced AD in NC/Nga mice and interleukin (IL)-33 transgenic (tg) mice. Each model comprised three groups: UFB shower-treated, normal shower-treated, and untreated. We evaluated the mice using a dermatitis score, scratching counts, histology, and the expression of inflammatory cytokines and skin barrier-related proteins. RESULTS: In the Dfb-induced AD mouse model, clinical features improved markedly in the UFB shower-treated mice compared to other groups. IL-4 and IL-13 levels decreased in the skin of normal and UFB shower-treated mice. In addition, in the skin of UFB shower-treated mice, the expression levels of skin barrier-related proteins were increased compared to normal showertreated mice. However, we found no significant differences in IL33tg mice. DISCUSSION: These results suggest that UFB shower can recover the skin barrier function and improve skin inflammation, especially in conditions such as extrinsic AD.

<p>A 46－year－old man with a 10－year history of hidradenitis suppurativa (HS) presented with a 5 cm wide, ulcerated, and granulomatous tumor in the perineum. Histopathology of the skin biopsy revealed squamous cell carcinoma (SCC). The patient underwent an en bloc resection of the perineal tumor and HS lesions, followed by skin flap reconstruction and split－skin grafting. The surgical margins were negative.</p><p>Three and a half years after the surgery was performed, the patient developed recurrent SCC in the perineum. The tumor was resected once more and the pelvic floor muscles were partially resected. The defect was reconstructed with split－skin grafting.</p><p>HS－associated SCCs are usually large and have a high recurrence rate. This case highlights the importance of long－term follow－up for patients with HS－associated SCC, even after surgical resection with negative margins.[<i>Skin Cancer (Japan)</i> 2024 ; 39 : 157－162]</p>

＜文献概要＞86歳,女性。初診1ヵ月前より右頬部に紅色丘疹を自覚した。初診時,右頬部に8×5cm大の暗紫色調の腫瘤と,周囲の紫斑および結節を認めた。病理組織標本では,真皮全層と脂肪織に小型のリンパ球様腫瘍細胞が密に集簇していた。初診3週間後,右頬部腫瘤は15×15cm大と急速に拡大し,体幹,四肢にも暗紫色斑が散在していた。右上腕の暗紫色斑より生検を施行した。HE標本では右頬部腫瘤と同様の所見であり,免疫組織化学でCD56+,CD123+であったため,自験例を芽球性形質細胞様樹状細胞腫瘍と診断した。本疾患は予後不良として知られるが,自験例のように顔面,頭部原発例は特に進行が早いと考えられた。

84歳,女性。初診2ヵ月前より口腔内にびらんが出現し,約1ヵ月前より四肢・体幹を中心に小紅斑・水疱が出現した。左大腿水疱部の病理組織像では表皮下水疱,水疱内と真皮浅層血管周囲に好酸球の浸潤を認めた。CLEIA法による抗Cは陰性であった。蛍光抗体直接法では表皮基底膜部にIgGとC3の線状沈着を認め,正常ヒト皮膚を用いた蛍光抗体間接法ではIgG抗基底膜部抗体を認め,1M食塩水剥離皮膚の表皮側に反応した。免疫ブロット法では患者血清のIgGがBP180NC16aとLAD-1に反応,IgAがLAD-1に弱く反応した。以上より抗LAD-1抗体陽性の粘膜類天疱瘡と診断した。抗LAD-1抗体陽性のMMPの報告例の皮膚症状は中等症が多く,約半数の症例ではMMPの高リスク群に相当する治療が必要であった。本症例でもプレドニゾロン60mg/日投与で治療を開始し,粘膜皮膚症状は著明に改善した。粘膜類天疱瘡の皮膚症状の発症には主として抗BP180NC16a抗体が関与し,粘膜疹の発症にはBP180C末端とLAD-1に対する抗体が関与していると考えられている。自験例のようにBP180NC16aとLAD-1抗体の両方に陽性の症例群は,MMPとBPの両者の性質を持った疾患スペクトラムにあることが示唆された。(著者抄録)

＜文献概要＞58歳,女性。幼少時より手指背側に無症候性の小丘疹を自覚していた。40歳代から小丘疹が増加傾向のため当科を受診した。初診時,手指背側や拇指球の手掌側と手背側の接合部,足側面に1～2mm大の常色～黄色の小丘疹が散在していた。病変部の病理組織学的所見では,表皮のクレーター状の陥凹を覆う限局性の角質増殖と,顆粒層の肥厚がみられたが,弾性線維の変性は明らかではなかった。病理所見と臨床所見よりfocal acral hyperkeratosisと診断した。アダパレン外用で加療しているが,著変なく経過している。本邦での報告数は少ないが,臨床所見は疣贅に類似しており,日常診療でも念頭に置くべき疾患と考える。

＜文献概要＞53歳,女性。受診2ヵ月前より左頬部に皮下硬結を自覚した。副腎皮質ステロイド薬外用で加療されるも改善せず,当科を紹介受診した。初診時,左頬部に15×7cm大の下床に硬結を伴う褐色～淡紅色の局面を認めた。病理組織検査では,真皮浅層から中層にかけて毛包周囲性,血管周囲性に好酸球,好中球,リンパ球などの多彩な炎症細胞の浸潤と膿瘍形成を認めた。また,毛包上皮内にムチンの沈着を認めた。臨床所見と併せ,毛包にムチン沈着を伴う好酸球性膿疱性毛包炎,古典型と診断した。本疾患の臨床的特徴,病理組織学的特徴に加え,本邦でのムチン沈着をきたす好酸球性膿疱性毛包炎の報告例,毛包性ムチン沈着症との鑑別点をまとめ,考察した。

＜文献概要＞症例1:76歳,男性.爪甲脱落と両手指色素斑にて当科を受診した.下痢と味覚異常があり,消化管内視鏡検査で大腸に多発ポリープを認めた.症例2:50歳,女性.爪甲脱落,びまん性脱毛,両手掌と爪周囲の色素沈着を主訴に受診した.味覚異常があり,消化管内視鏡検査で胃・大腸に多発ポリープを認めた.いずれもCronkhite-Canada症候群と診断した.症例1は,対症療法にて症状は軽快した.症例2は,プレドニゾロン50mg/日より投与開始となり,腸管症状,味覚異常,皮膚症状が改善したため,治療開始5ヵ月後には内服を終了した.いずれも現在まで症状再燃なく経過している.症例1はIgG4関連自己免疫性膵炎の既往があり,症例2は副腎機能低下を合併していた.Cronkhite-Canada症候群の原因は未だに不明であるが,IgG4関連疾患などの免疫異常が存在する可能性があり,合併症にも注意する必要がある.

＜文献概要＞19歳,女性。初診の1年前から舌の左側に色素斑がみられ,当科を受診した。初診時,舌の左側縁に5mm大の灰褐色斑と点状の黒褐色色素沈着の集簇がみられた。家族歴や既往歴に特記すべき事項はなかった。病理組織学的所見では,表皮は肥厚し,粘膜固有層に滴落したメラニンとメラニンを貪食したメラノファージを認め,軽度の炎症細胞浸潤がみられた。Fontana-Masson染色では,基底層と粘膜固有層のメラニン色素が染色された。以上より,pigmented fungiform papillae of the tongueと診断した。病理組織学的所見では,自験例を含めた基底層にメラニン沈着を認めた全例で炎症細胞浸潤を認めた。炎症後色素沈着としての一面が推察できるが,症例数が少ないため,今後の知見の集積を要する。

55歳,女性.初診2年前より外陰部にそう痒があり,2ヵ月前から外陰部,肛囲に腫瘤を自覚していた.近医で尖圭コンジローマと診断されイミキモドを外用するも改善せず,当科を紹介受診した.初診時は,外陰部から肛囲にかけて鶏冠状に隆起した灰白色の腫瘤を多数認め,肛門部腫瘤は9.5×3cm大であった.有棘細胞癌を鑑別疾患として,肛門部腫瘤より皮膚生検を施行した.病理組織学的所見では,表皮は過角化を伴って外方性,乳頭腫状に肥厚していた.錯角化があり,表皮上層の角化細胞にコイロサイトーシスを認めた.核の異型性は乏しく,悪性を示唆する所見はなかった.臨床所見,病理組織学的所見から巨大尖圭コンジローマ(giant condyloma acuminatum,GCA)と診断した.腫瘤の縮小を目的に亜鉛華デンプン外用を開始したところ,開始から2ヵ月で腫瘤はすべて消退した.その後,通院しなくなり,再発については確認できていない.GCAとはカリフラワー状の腫瘤を形成し,外見上は悪性腫瘍が示唆されるが,病理組織学的に良性を示す腫瘍性病変と定義されている.治療として外科的切除術が最も確実な治療法と考えられているが,外科的切除術後でも再発率は50%と高い.2010年にGCAに対して亜鉛華デンプンによる治療が有効であった2例が報告された.腫瘍サイズが大きいまま外科的に治療した場合,皮膚欠損範囲が大きく,治癒に時間を要する.まず亜鉛華デンプン外用で腫瘍の縮小を目指すことで,治療侵襲性を小さく出来る可能性がある.また,自験例のように完全消退する可能性もある.(著者抄録)

<p>Gorlin syndrome and Cowden syndrome are hereditary diseases that are characterized by multiple malignancies, cutaneous symptoms, and various other abnormalities. Both disorders are caused by a mutation of the gene that regulates cell proliferation and growth, resulting in tumorigenesis. Representative mutations are mutation in the patched 1 gene (<i>PTCH1</i>) in Gorlin syndrome and mutation in the phosphatase and tensin homolog deleted from chromosome 10 (<i>PTEN</i>) gene in Cowden syndrome. Making a diagnosis of these diseases in the early years of life is important because detection of malignancies at an early stage is linked to improved prognosis. Both Gorlin syndrome and Cowden syndrome have cutaneous findings in the early phase in childhood, and the role of dermatologists is therefore important. These diseases are generally diagnosed by clinical criteria, but some patients who do not meet the criteria need genetic examinations including a genetic diagnostic panel and next-generation sequencing. The most important treatment and management are detection and resection of malignancies in the early stage, and targeted therapies have recently been used for treatment of tumors and other symptoms in these diseases. Although evidence of the effectiveness of targeted therapies has been limited, they are promising therapeutic options and further clinical trials are needed in the future.</p>

A 55-year-old woman had been experiencing an itchy sensation in the vulva for two years prior to the initial visit, and a mass had been observed in the vulva and perianal area for two months. The patient was referred to our department after topical imiquimod failed to improve the lesion. From the vulva to the perianal area, a grayish-white mass, 9.5 cm × 3 cm in size, with a chicken-crown-shaped elevation was observed. Histopathologically, the epidermis was thickened in an externally directed, papillomatous shape with hyperkeratosis. There was parakeratosis and koilocytosis of keratinocytes in the epidermis. No findings suggestive of malignancy were found.
Based on the clinical and histopathological findings, a diagnosis of giant condyloma acuminatum (GCA) was made. The patient was treated with topical zinc oxide starch powder to reduce the size of the mass. Surprisingly, the masses disappeared in two months. We have not confirmed whether GCA has recurred since the patient stopped coming to our hospital.
GCA is defined as a cauliflower-shaped tumor that appears malignant but is histopathologically benign. Although surgical resection is considered the most reliable treatment, recurrence rates are as high as 50% even after surgical resection.
In 2010, two cases of GCA were reported in which treatment with zinc oxide starch powder was effective. If the tumor is large and is treated as it is, the resulting skin defect will also be large and healing will take time. The application of topical zinc oxide starch powder to the tumor may reduce its size, or there might be complete regression of the mass.

<p>An 84-year-old woman was referred to us because of erosions in the oral cavity for 2 months, and blisters on the extremities and trunk for one month. Anti-BP180-NC16a domain antibodies were negative in CLEIA. Histopathology of the blister on the left thigh showed subepidermal bulla. Direct immunouorescence revealed linear depositions of IgG and C3 in the epidermal basement membrane zone. Indirect immunouorescence detected IgG anti-basement membrane zone antibodies in the patient serum, which were positive on the epidermal side in 1M NaCl-split skin. Immunoblotting analysis showed IgG antibodies against BP180 NC16a domain and 120 kDa LAD-1,and IgA antibodies against LAD-1 in the patient sera. Based on these clinical and histopathological ndings, the patient was diagnosed as anti-LAD-1 antibody-positive mucous membrane pemphigoid (MMP) with skin lesions. Cases of anti-LAD-1 antibody-positive MMP were previously reported to have moderate skin manifestations, and about half of cases require treatment similar to high-risk MMP. Treatment was initiated with prednisone 60 mg/day, and mucocutaneous symptoms improved markedly. Anti-BP180 NC16a domain antibodies may be involved in the development of cutaneous lesions of MMP, whereas antibodies against BP180 C-terminal domain and LAD-1 may be involved in the development of mucosal lesions. 目的:天疱瘡・類天疱瘡患者が発症初期から現在に至るまでに体験した日常生活における困難感を明らかにする.方法:13名の患者を対象に半構造化インタビューを行い,質的記述的に分析した.結果:身体症状の強い時期は,【症状により日常生活行動に支障を来す】【患部の必要な処置に伴い痛みや負担を感じる】が,治療によりこれらの大部分は軽快し,【ステロイド療法の副作用により日常生活行動に制約がある】といった治療により軽快しない困難感を生じていた.また,【希少疾患であること,他者に理解されないことで不安,孤独を感じる】【病気や治療,再燃に対する不安,恐れを感じる】【症状や治療の副作用による影響で人付き合いが難しい】【病気により学業,就職,仕事が思うようにいかない】心理的・社会的な困難を感じていた.結論:天疱瘡・類天疱瘡患者は,症状や治療,疾患の希少性・難治性,症状の可視性や他者に理解されないことでの困難感を抱いていた.(著者抄録)

尋常性乾癬に対する光線療法単独とアプレミラスト+光線療法の有効性と安全性を比較する多施設共同無作為比較試験を行った。中等症以上の尋常性乾癬がみられる20～80歳の患者40例(男性28例、女性12例、平均61.0±14.6歳)を、併用群29例と単独群13例に無作為に割り付けた。全患者に狭帯域UV-B照射を含む光線療法を8週間施行し、併用群にはさらに10～60mgのアプレミラストを経口投与した。主要有効性評価項目は、治療開始から8週目までの乾癬面積および重症度指数(PASI)スコアに基づく改善率とし、副次有効性評価項目は、治療開始から4週および8週でのPASIスコアと改善率、有害事象等とした。単独群に比べて併用群では4週時および8週時のPASIスコアが有意に低く、8週で75%以上のPASIスコア改善を達成した患者が多かった。体表面積(BSA)の評価では両群とも有意な減少が見られたが、併用群ではBSA減少が多い傾向が見られた。軟便、下痢等の有害事象は単独群よりも併用群で多く見られ、重篤有害事象としイレウス、甲状腺腫、完全房室ブロックが併用群の2例で報告された。また有害薬物反応は併用群の14例で29件報告された。BSAが高い当該患者では、光線療法の単独療法は不十分で、アプレミラストの併用療法により転帰が改善した。

＜文献概要＞症例1:41歳,男性.1週間前に左胸部に有痛性紅色結節が生じ,急速に増大して壊死組織を付す紅色局面となった.症例2:1歳,男児.躯幹と右大腿に,有痛性の膿点を伴う紅色丘疹が多発し,周囲の発赤・腫脹を伴った.兄弟・母にPanton-Valentine leukocidin(PVL)陽性methicillin-resistant Staphylococcus aureus(MRSA)によるせつ腫症,蜂窩織炎の既往あり.症例3:70歳,男性.両下腿に潰瘍,痂皮や小膿疱を伴う紅色結節が多発した.3例とも,創部よりPVL陽性市中感染型MRSA(community-acquired MRSA:CA-MRSA)を検出した.症例1は癰,症例2,3はせつ腫症と診断した.症例1はミノサイクリン塩酸塩の投与,症例2はスルファメトキサゾール・トリメトプリムの投与,症例3はドキシサイクリン塩酸塩の投与にて治癒した.上記のような組織壊死傾向の強いせつ,癰,せつ腫症,皮下膿瘍などの皮膚軟部組織感染症では,PVL陽性CA-MRSAを念頭に置いて治療する必要がある.

61歳,男性。鼻腔・口腔粘膜のびらんと,体幹四肢の緊満性水疱を主訴に当科を受診した。病理組織学的に表皮下水疱であり,蛍光抗体直接法では基底膜部にIgG,C3の線状沈着を認めた。Split skinを用いた蛍光抗体間接法ではIgGが真皮側に陽性であった。真皮抽出液を用いた免疫ブロット法で患者血清は290kDaの抗原に反応し,ELISA法で抗VII型コラーゲンが検出された。以上より,抗VII型コラーゲン抗体による粘膜類天疱瘡と診断した。プレドニゾロン内服により皮膚病変は容易に改善したが粘膜病変は遷延し,血漿交換療法,免疫グロブリン大量静注療法,ジアフェニルスルホン,アザチオプリン,テトラサイクリン/ニコチン酸アミドの併用でようやく軽快した。重症で非常に難治性の粘膜症状を呈する粘膜類天疱瘡では,皮膚症状を主とする後天性表皮水疱症と抗VII型コラーゲン抗体の抗原エピトープが異なる可能性があると考えた。(著者抄録)

女性型脱毛症(FPHL)のマウスモデルについて検討した。3週齢と7～8週齢のC57BL/6雌マウスを2群に分け、1群には卵巣摘出(OVX)を行い、他の群には偽手術を行った。3週齢マウスでは7週齢目に背部皮膚を採取し、7～8週齢マウスでは12と24週齢目に採取した。前群では、休止期の毛包の孔径と毛幹の直径はOVXによって低下し、後群ではこれらの値が上昇した。特に真皮と皮下組織の厚さはOVXで有意に上昇した。OVXマウスがFPHLの理想的なマウスモデルとして使用できるかはさらに検討する必要があるが、皮膚の厚さに対するこれらの知見は、閉経関連皮膚状態のような非毛髪関連疾患の新規治療を確立するために使用できると考えられた。

症例は47歳男性で、mRNA-1273 COVID-19ワクチン(モデルナ社)の初回接種から19日後、下肢および足背の紫斑性発疹のため来院した。発疹は5日以内に自然軽快したが、2回目のワクチン接種から15日後に再び出現した。皮膚生検の病理所見は白血球破砕性血管炎に類似していたが、免疫蛍光染色で微小血管壁にIgA沈着を認めなかった。尿検査では高度の蛋白尿および潜血を認めた。腎生検を行ったところ、光学顕微鏡観察で軽度のメサンギウム増殖、細胞増多、管内細胞増多に細胞性・線維細胞性半月体の形成を伴い、免疫蛍光染色ではメサンギウム領域でIgAが顆粒状に陽性を示し、IgA血管炎と一致する所見を認めた。メチルプレドニゾロン静脈内投与に続いてプレドニゾロン経口投与を行ったところ、2週間で血清クレアチニン値および蛋白尿は改善した。

＜文献概要＞54歳,女性。7歳時に尋常性乾癬発症,45歳頃より関節症状が出現し,増悪してきたため,51歳時に当科を紹介受診した。乾癬性関節炎の診断でアダリムマブやインフリキシマブ,イキセキズマブ,グセルクマブを投与した。投与約4年後から鼻閉症状が出現し,耳鼻科にて慢性副鼻腔炎と診断された。内視鏡下副鼻腔手術を施行され,病理組織学的に右篩骨洞と右上顎洞の粘膜に,隔壁を有しY字に分枝する菌糸を認めたことから,副鼻腔真菌症と診断した。生物学的製剤を一時中止していたが,術後経過良好であったため,再開することができた。生物学的製剤を投与中に抗菌薬が無効の副鼻腔炎が生じた場合は副鼻腔真菌症を考慮すべきであると考えられた。

＜文献概要＞54歳,男性.初診約4ヵ月前より左頬部に無症候性の約10mm大の紅色結節が出現した.前医で腫瘍摘出され,腺様嚢胞癌(adenoid cystic carcinoma:ACC)の病理診断であったため,精査加療目的で当科を紹介受診した.病理組織学的に真皮内に管腔様構造,篩状構造を有する腫瘍胞巣を認め,画像検査にて他臓器,唾液腺の病変がないことから,皮膚原発の腺様嚢胞癌(primary cutaneous adenoid cystic carcinoma:PCACC)と診断した.皮膚原発の腺様嚢胞癌は非常に稀である.ACCは唾液腺に好発する悪性腫瘍の1つであり,その他涙腺,気管支,乳腺,子宮等にも発生することが知られている.PCACCにおいて画一的な治療方針はまだないが,深達度や病理組織所見での神経浸潤の有無で,個々の症例に応じて治療方針や術後経過観察期間を決定することが重要である.

77歳,男性。初診の1週間前から手足に紅斑と水疱を認めた。その後皮疹が全身に拡大し,粘膜にもびらんを認めたため,精査加療目的で当科に紹介された。初診時,体幹・四肢に紅斑と水疱が多発しており,口唇・口腔内・陰茎にびらんを認めた。抗BP180-NC16A部位抗体,抗デスモグレイン1,3抗体は陰性であった。病理組織にて好酸球浸潤を伴う表皮下水疱,蛍光抗体直接法で表皮基底膜部にC3の線状沈着を認めた。1M食塩水剥離皮膚を基質とした蛍光抗体間接法では,IgG抗体が真皮側に反応した。正常ヒト真皮抽出液を用いた免疫ブロット法にて患者血清IgG抗体は200kDaラミニンγ1(p200)に反応し,ラミニン332リコンビナント蛋白を用いた免疫ブロット法で165kDaラミニンα3に反応した。臨床症状と病理所見,蛍光抗体直接法,蛍光抗体間接法,免疫ブロット法の結果より,抗ラミニンγ1類天疱瘡と抗ラミニン332型粘膜類天疱瘡の合併と診断した。プレドニゾロン1mg/kg/日点滴を行ったところ,新生水疱を認めなくなり,紅斑は退色,びらんは上皮化した。その後,プレドニゾロンを漸減したが,皮膚・粘膜症状は再燃しなかった。自験例では皮膚・粘膜症状は重篤であったが,ステロイド全身投与が速やかに奏効した。(著者抄録)

84歳、男性。二ヵ月間持続する全身の浮腫性紅斑と緊満性水疱を主訴に当科を受診した。病理組織学的に表皮基底細胞に空胞変性と真皮浅層に好酸球浸潤を認め、蛍光抗体直接法で表皮基底膜部にIgG及びC3が線状に沈着していた。また、抗BP180抗体も4120U/mLと高値であり、水疱性類天疱瘡(BP)と診断した。Bullous Pemphigoid Disease Area Index(BPDAI)で中等症でありプレドニゾロン(PSL)を40mg/日(0.6mg/kg/日)より内服開始し皮疹は改善したが、25mg/日まで漸減した際に再燃した。比較的高用量のPSL内服中の再燃であり、併存疾患であるてんかんに対するカルバマゼピンとの相互作用によりPSLの効果が減弱したと考えた。PSLを40mg/日に増量することで皮疹は痂皮化、色素沈着化し、以後は慎重に漸減し、皮疹のコントロールを得ている。内服ステロイドの代謝に影響を及ぼす薬剤は複数あり、BP等の自己免疫性疾患に対して内服ステロイドを使用する際に、これらの薬剤の併用がないか、確認することが大切であると考える。(著者抄録)

＜文献概要＞80歳,女性。約3年前より左大腿に皮膚腫瘍が出現し,徐々に増大した。初診時,左大腿に約2cmの暗赤色結節を認めた。病理組織学的所見では真皮内に好酸性の胞体と円形核をもつ細胞が結節状に増殖し,胞体に粘液を有する細胞も少数みられたため,粘表皮癌と診断した。唾液腺と子宮頸部,そのほかに原発巣を疑う所見はなく,皮膚原発粘表皮癌と考えた。10mmマージンで切除し,術後10ヵ月現在,再発はない。体幹・四肢の粘表皮癌は報告例が少なく,大腿に粘表皮癌が生じた報告はこれまでにみられない。ダーモスコピー像でみられたmilia-like cystsに類似するwhite clodsは病理組織での好酸性壊死組織と対応しており,粘表皮癌の予後因子と関連している可能性があると考えられた。

＜文献概要＞80歳,男性。顔面日光角化症と有棘細胞癌の既往あり。初診数ヵ月前より,右鼻背部に黒褐色結節が出現し,徐々に増大した。初診時,6×7mm大の扁平な黒褐色結節があった。ダーモスコピーでは面皰様開孔や外方向性乳頭状増殖,不規則なfissures/ridgesなどがみられ,脂漏性角化症を疑った。病理所見では表皮の乳頭状増殖,表皮上層にhaloを伴う表皮角化細胞が存在しており,基底細胞様細胞の増殖がみられる部分があった。さらに,異型表皮角化細胞が増殖しており,脂漏性角化症と色素沈着型乳頭状Bowen病の合併の可能性を考えた。ダーモスコピーで脂漏性角化症を疑う所見であっても,悪性腫瘍鑑別のため生検を考慮する必要があると考える。

69歳,女性。初診の1ヵ月前より,両上肢,体幹に水疱,紅斑が出現し,近医にて伝染性膿痂疹が疑われ,抗菌薬の内服,外用で加療されたが軽快しないため,当科を受診した。初診時,背部,両上肢にびらん,痂疲,色素沈着をともなった軽微な浸潤性紅斑や緊満性水疱を認めた。病理組織学的所見では再生上皮を伴った表皮下水疱を認め,蛍光抗体直接法ではC3が基底膜に線状に沈着していた。また,CLEIA法による血清抗BP180NC16A抗体,抗デスモグレイン1抗体,抗デスモグレイン3抗体,ELISA法による血清抗BP230抗体はすべて陰性であった。ヒト皮膚表皮抽出液を用いた免疫ブロット法においてBP230が陽性であった。プレドニゾロン15mg内服投与を開始し,糖尿病治療のため2年半内服していたテネリグリプチンを入院9日目より中止した。以後皮疹は速やかに改善傾向を示し,プレドニゾロンも漸減,50日後に中止したが,皮疹の再燃はなかった。近年,経口血糖降下薬であるdipeptidyl peptidase-4(DPP-4)阻害薬内服後に発症した類天疱瘡が多く報告されており,自験例のようにELISA/CLEIA法にて抗BP180NC16A抗体は陰性で,ELISA法にて全長BP180抗体が陽性である症例が多いといわれている。また,自験例では免疫ブロット法でBP230抗体が陽性であった。近年の研究では抗BP230抗体が水疱性類天疱瘡における水疱形成に関連している可能性も示唆されており,今後その役割について検討していくことはDPP-4阻害薬関連類天疱瘡の発症機序を考えるうえで意義があると考える。(著者抄録)

症例1:70歳,男性。右上背部に腫瘤と疼痛を自覚し当科を受診した。中心部に壊死組織,周囲に多数の膿栓を伴う直径30cm大の暗赤色局面を認め,癰と診断した。アンピシリンナトリウム・スルバクタムナトリウム点滴を開始しデブリードマンの後,第20～30病日まで局所陰圧洗浄療法を行った。全層植皮術で再建し第80病日に退院した。症例2:74歳,男性。体動不能を主訴に当院救急外来受診。左上背部に,中央部に壊死組織,周囲に多数の膿栓を伴う直径23cm大の暗赤色局面を認めた。メロペネム点滴を開始し,症例1と同様に局所陰圧洗浄療法を行った。巨大癰はほぼ全例で糖尿病を合併しているため重症化しやすく,早期の治療介入を要する。近年,巨大癰のような広範囲の欠損創には陰圧閉鎖療法を行った報告が多い。一方,陰圧洗浄療法は創傷治癒と感染創の双方に速やかなアプローチが可能であり,病早期から使用できる利点がある。実際,自験例では短期間に良好な肉芽形成を得る事ができた。巨大癰に対し,陰圧洗浄療法は非常に良い適応と考えられた。(著者抄録)

＜文献概要＞43歳,男性。2007年頃に左腰部の色素斑が隆起してきた。悪性黒色腫(T4bN1aM0,Stage IIIA)の診断であり,2012年当科を受診した。腫瘍切除後の追加治療は希望されず,受診も自己中断していた。2015年,悪性黒色腫の肺転移が判明し,右上葉の胸水貯留と呼吸苦のため入院した。ベムラフェニブ投与25日後に症状は軽快し,一時退院することができた。以後は通院でベムラフェニブ内服を継続,60日後に再度胸水が貯留し,心外閉塞・拘束性ショックにより永眠された。分子標的薬や免疫チェックポイント阻害薬により,悪性黒色腫末期患者の生存期間の延長だけでなく,生活の質の向上が望めると考えられた。

皮膚T細胞リンパ腫(CTCL)の治療薬である選択的なレチノイドX受容体活性化薬のベキサロテンで誘発される中枢性甲状腺機能低下症(CH)の病態生理を検討した。24週間のベキサロテン治療中の甲状腺機能正常性のCTCL患者10例(年齢56～79歳)の血清TSHとフリーT4(F-T4)濃度を測定した。CH診断後にTRH刺激試験を実施し、F-T4を治療前のレベルに維持するようにレボチロキシン(LT4)補充量を調節した。ベキサロテン投与1週間後、F-T4低値または正常値、TSH低値または正常値の複合所見に基づき、10例全例のCHを確認した。1週間後の刺激試験後のTSH反応は30分後にピークに達した。しかし、その値は全例で4μIU/mL以下であり、TSH反応は誇張され遅延していなかったが鈍化していることが示された。24週間ベキサロテンを継続した8例のLT4補充量の中央値は125μg/day(範囲:75～150)であった。TRH刺激後30分、15分、60分、90分、120分のTSH値はLT4補充量と有意な相関があったが(ρ=-0.913、p=0.002)、1週間後のTSHおよびF-T4基礎レベルは相関がなかった。以上より、ベキサロテンによるCHは下垂体性甲状腺機能低下症が原因であった。

<p>Bexarotene-induced central hypothyroidism (CH), for which levothyroxine (LT4) replacement is recommended, has been shown to be caused by pituitary but not hypothalamic disorder experimentally, though the underlying mechanism in humans remains unclear. Here, the pathophysiology of bexarotene-induced CH was examined using a TRH stimulation test in cutaneous T-cell lymphoma (CTCL) patients. In this retrospective longitudinal observational study, serum TSH and free T4 (F-T4) levels were measured in 10 euthyroid patients with CTCL during 24 weeks of bexarotene treatment. TRH stimulation testing was performed following CH diagnosis, with LT4 replacement dosage adjusted to maintain F-T4 at the pre-treatment level. After one week of bexarotene administration, all 10 patients developed CH, based on combined findings of low or low-normal F-T4 with low or normal TSH levels. TSH peak response after a stimulation test at one week was reached at 30 minutes. However, that was <4 μIU/mL in all patients, indicating a blunted though not exaggerated and delayed TSH response. In eight who continued bexarotene for 24 weeks, median LT4 replacement dosage was 125 (range, 75–150) μg/day. TSH level at 30 as well as 15, 60, 90, and 120 minutes after TRH stimulation was significantly correlated with LT4 replacement dosage (<i>ρ</i> = –0.913, <i>p</i> = 0.002), whereas TSH and F-T4 basal levels at one week were not. These results suggest that pituitary hypothyroidism is responsible for bexarotene-induced CH, while TSH levels after TRH stimulation precisely reflect residual pituitary-thyroid function in patients receiving bexarotene.</p>

<p>A 69-year-old womanwas referred to us for erythema and bullae onthe upper extremities and the back that appeared a month before. Oral antibiotics prescribed by a local dermatologist under the putative diagnosis of infectious impetigo were ineffective. Physical examination revealed erosions,crusts, and pigmentation associated with faint erythema, and tense bullae on the back and upper extremities. Skinbiopsy revealed subepidermal bullae with regenerated epidermis. The direct immunofluorescence of the skin sample disclosed linear deposition of C3 at the dermo-epidermal junction. Chemiluminescent enzyme immunoassay (CLEIA) for BP180 NC16A, desmoglein-1, and desmoglein-3, and enzyme-linked immunosorbent assay (ELISA) for BP230 showed negative results ; however, ELISA for the full-length BP180 triplet was positive. Immunoblotting using human epidermal extracts as substrate uncovered positivity at 230 kDa BP230. The patient had been taking oral dipeptidyl peptidase-4 (DPP-4) inhibitors for 4.5 years (linagliptin for 2 years and then teneligliptin for 2.5 years). Oral prednisolone (15 mg/day) and minocycline (50 mg/day) were started, and teneligliptin was stopped nine days later. As the eruption showed remarkable improvement, the prednisolone was tapered off at day 50 after the first visit. No recurrence of bullous disorder was observed. The DPP-4 inhibitor-associated bullous pemphigoid (DPP-4i-BP) is reported to be relatively mild, often negative for anti-BP180 NC16A autoantibodies in CLEIA/ELISA,and sometimes positive for the full-length BP180 triplet. In addition, anti-BP230 antibodies were positive only by immunoblotting. <p>A 77-year-oldman was referredto us because of a one-week history of erosions on the lips, oral cavity, andpenis, andnumerous erythemas andblisters on the trunk andextremities. Chemiluminescent enzyme immunoassays did not detect anti-BP180-NC16A domain autoantibodies or anti-desmoglein 1 and 3 autoantibodies. Histopathological examination showed subepidermal bullae with infiltration of eosinophils andneutrophils. Direct immunofluorescence detected linear deposition of C3 at the basement membrane zone (BMZ). Indirect immunofluorescence using normal skin as substrate showed IgG anti-BMZ antibodies, which reacted with the dermal side of 1M NaCl-split skin. Immunoblotting with normal human dermal extract detected 200 kDa laminin <i>γ</i>1 (p200), andimmunoblotting with laminin 332 recombinant proteins detected165kDa laminin <i>α</i>3. We made a diagnosis of coexistence of anti-laminin <i>γ</i>1 pemphigoidandanti-laminin 332 mucous membrane pemphigoid. Intravenous administration of prednisolone led to quick improvement of the mucocutaneous lesions. <p>A 61-year-old man was referred to our hospital because of erosions on his nasal and oral mucosa and blisters on his trunk and extremities. Histopathological examination revealed subepidermal blister formation. Direct immunofluorescence showed linear deposition of IgG and C3 on the basement membrane zone. Indirect immunofluorescence using 1 M NaCl split skin showed the presence of IgG autoantibodies reactive to the dermal side. Western blot analysis using dermal extract showed autoantibody reactive to 290 kDa protein. ELISA showed the presence of autoantibodies for type VII collagen. Therefore, we diagnosed the patient as mucous membrane pemphigoid (MMP) with antibodies against type VII collagen. Although his cutaneous lesions were easily improved by oral prednisolone, mucous lesions were refractory, which finally improved by additional treatment with plasmapheresis, intravenous immunoglobulin, diaminodiphenylsulfone,azathioprine, tetracycline, and niacinamide. <p>Case 1 : A 70-year-old man presented with a painful nodule on his right shoulder. The lesion, 30 cm in diameter, was characterized by a dark red necrotic plaque that was surrounded by numerous pus-filled bumps. The patient was diagnosed with a carbuncle, admitted, and treated with infusion antibiotic therapy. On the 13th day after admission, the patient underwent tissue debridement. From the 20th to the 30th day post-admission, he received negative pressure wound therapy with instillation therapy and dwelling time (NPWT-id), followed by a full-thickness skin graft. Eighty days after admission, complete epithelialization had occurred. Case 2 : A 74-year-old man presented with a lesion that was similar to Case 1. A dark red necrotic carbuncle, 23 cm in diameter, was on his left shoulder and was surrounded by numerous pus-lled bumps. After admission, the patient underwent the same treatment plan : antibiotic infusion, debridement, NPWT-id, and a full-thickness skin graft. Eighty days after admission, epithelialization was complete. Previous studies reported that the majority of patients with large carbuncles also had severe diabetes. For this reason,patients with these lesions tend to experience severe symptoms and require early treatment. In recent years, NPWT has become the preferred treatment for large-sized carbuncles. However, the disadvantage of NPWT is that it cannot be performed until the infection is controlled. By including instillation therapy and dwelling time to NPWT, we were able to provide a rapid approach to both wound healing and infection treatment. Both patients experienced wound healing with good granulation tissue formation within a short period of time. <p><b>Purpose:</b> The study aim was to understand difficulties experienced in daily life from the early stage of onset to the present by patients with pemphigus and pemphigoid.</p><p><b>Methods:</b> We conducted semi-structured interviews with 13 patients with pemphigus or pemphigoid, and analyzed the data qualitatively and descriptively.</p><p><b>Results:</b> During periods when their physical symptoms were strong, patients experienced difficulties such as “symptoms interfering with daily activities”, and “feelings of pain and burden when the affected area required treatment”, even though these difficulties were alleviated by the treatment. Patients also experienced difficulties such as “restrictions in daily activities owing to side effects of steroid therapy”. Furthermore, patients reported psychological and social difficulties experienced “anxious and lonely because of their rare disease and lack of understanding by others”, “anxious and afraid because of the disease, treatments, and relapse”, or “difficulty in socializing with others owing to symptoms and treatment side effects”, and “disturbances in schoolwork, employment, and jobs owing to the disease”.</p><p><b>Conclusion:</b> Patients with pemphigus or pemphigoid experienced difficulties owing to the effects of symptoms and treatment, the rarity and intractability of the disease, the visibility of the symptoms, and the lack of understanding by others.</p>

57歳男性。既往歴:浸潤性胸腺腫,重症筋無力症。初診3ヵ月前より四肢の筋力低下,全身倦怠感,味覚障害が出現し,当院神経内科に入院となった。初診時,顔面,体幹,四肢に鱗屑を伴う角化性紅斑がびまん性にみられ,紅皮症を呈していた。病理組織像では,錯角化を伴う過角化,表皮基底層の液状変性,表皮内個細胞壊死およびリンパ球浸潤を認め,graft-versus-host disease(GVHD)-like erythrodermaを呈したthymoma-associated multiorgan autoimmunity(TAMA)と診断した。低γグロブリン血症を呈し,緑膿菌性肺炎を繰り返すことからGood症候群を併発していると考えられた。ステロイドパルス療法,抗菌薬投与およびγグロブリン大量療法(intravenous immunoglobulin:IVIG)施行後,全身状態は一時的に回復したが,その後播種性Mycobacterium abscessus感染症を併発し,初診10ヵ月後に永眠した。TAMAはGVHD様皮膚症状,肝障害,腸炎を発症する疾患であり,しばしば低γグロブリン血症やGood症候群を併発する。胸腺腫患者にGVHD-like erythrodermaが出現した場合,致死的で極めて予後不良の重篤な感染症,特に日和見感染の発症に十分留意する必要がある。(著者抄録)

＜文献概要＞33歳,男性.初診1年前から臀部,両下肢に米粒～粟粒大の黒褐色斑,結節が急激に多数出現した.採血検査,PET-CT検査では異常所見はなかった.異なるダーモスコピー所見((1)中央はhomogeneous pigmentationで辺縁は灰白色の背景を有するpigment network,reticular pattern,(2)dots and globules)を呈する病変2ヶ所から皮膚生検を施行し,病理組織学的にSpitz母斑の所見であったためeruptive disseminated Spitz nevi(EDSN)と診断した.EDSNはきわめて稀な疾患であり,本邦では報告がない.孤発性のSpitz母斑ではBRAF遺伝子変異,HRAS遺伝子変異の報告があり,EDSNの今後の病態解明が進むことが期待される.多発する黒褐色斑,小結節を診た場合はEDSNの可能性も念頭に置き診療にあたることが必要と考える.

症例は84歳、男性。既往歴に菌状息肉症があり、外用薬にて治療していた。両側精巣硬結を主訴に受診し、高位精巣摘除術の結果、精巣病変はびまん性大細胞型B細胞リンパ腫と判明した。R-miniCHOP療法および予防的髄腔内注射により完全寛解に到達し、診断から1年を経過した現在も再燃徴候なく経過している。菌状息肉症では健常人と比較して二次性悪性腫瘍の危険性が高まることが知られているが、診断確定のためには病理学的検索が重要である。(著者抄録)

＜文献概要＞89歳,女性.約10年前に左下腿に約15mmの黒色斑が生じた.病変は徐々に増大,隆起し出血や浸出液を伴うようになった.初診時,左下腿に65×60mm大のびらんを伴う有茎性紅色腫瘤が存在し臨床像から有棘細胞癌,無色素性悪性黒色腫を疑った.ダーモスコピーで腫瘍辺縁の一部にarborizing vessels, blue-gray globules, blue-white structuresを認め,生検の結果から基底細胞癌(basal cell carcinoma:BCC)と診断した.全切除標本の病理組織学的所見では,腫瘍は好塩基性の細胞で構成され,腫瘍胞巣辺縁では柵状配列を呈し,周囲の間質との間にムチン沈着を認めた.Melan-A,HMB45,Fontana Masson染色では腫瘍基部に陽性細胞を多数認めた.低色素性BCCでは,腫瘍の辺縁にBCCに特徴的なダーモスコピー所見がみられる可能性があり,腫瘍辺縁のダーモスコピーによる詳細な観察が重要と考える.

Rhododendrol(RD)誘発性脱色素斑の治療のため来院した患者における脱色素斑の重症化と白斑黒皮症の発症に関わるリスク因子を後ろ向きに調査した。RD含有化粧品を使用した後に脱色素斑を発症した女性患者101例(年齢中央値60歳)を対象とした。多変量ロジスティック回帰分析の結果、年齢、RDを含有する化粧品の使用数、BMI、抗核抗体および抗甲状腺抗体の使用は脱色素斑の重症度と有意な関連性はなかった。一方で、喫煙歴がある患者では喫煙歴がない患者と比較して初診時の脱色素斑の重症度が有意に高かった(p=0.012)。また、喫煙中の患者において白斑黒皮症の発症が高かった(p=0.006)が、他の因子は白斑黒皮症の発症と有意な関連性はなかった。Mann-Whitney U検定の結果では、喫煙歴がある患者(脱色素スコア中央値26)では喫煙歴がない患者(脱色素スコア中央値13)と比較して初診時の脱色素斑の重症度が有意に高かった(p=0.03)。以上の結果より、喫煙がRD誘発性の脱色素斑の重症化と白斑黒皮症発症のリスク因子であることが示された。

症例は75歳男性で、そう痒を伴う多発紅斑と水疱を呈していた。既往歴は脳出血後片側不全麻痺、慢性腎不全、疥癬であった。皮膚病変の発症1日前に、肺炎球菌ワクチンの接種歴があった。身体診察では、ワクチン注射部位の浮腫・腫脹と、頭部、頸部、四肢に複数の緊満性水疱、糜爛、紅斑が見られ、臨床検査では好酸球増加、腎不全、C反応性タンパク質上昇が認められた。病理組織学的には好酸球およびリンパ球浸潤を伴う表皮下水疱が示され、イムノブロット分析では正常な皮膚抽出物中のBP180 C末端ドメインおよびラミニン-y1(p200)組換えタンパク質とのIgG陽性反応が見られた。これらの所見から、BP180C末端ドメインおよびラミニン-γ1(p200)に対するIgG自己抗体を伴う類天疱瘡と診断した。15mg/日のプレドニゾロン経口投与で有意な改善が見られた。

<p>A 57-year-old man was referred to our department with scaly erythema over the face, trunk, and extremities. He had a history of metastatic thymoma and myasthenia gravis (MG). He underwent thymectomy 11 years prior and chemotherapy 9 years prior. He had been admitted to the department of neurology due to muscle weakness, fatigability, and dysgeusia. He presented with extensive erythema and scaling over his entire body, and was in a state of erythroderma. In addition, he had leg edema and white patches on the tongue. Skin biopsy from the thigh demonstrated hyperkeratosis with parakeratosis, epidermal liquefaction and spongiosis, apoptosis of keratotic cells in the epidermis, and lymphocytic infiltration into the epidermis and dermis. Based on the graft-versus-host disease (GVHD)-like skin symptoms, accompanied by thymoma and MG, we diagnosed the condition as thymoma-associated multi-organ autoimmunity (TAMA) complicated by MG. Topical corticosteroid treatment moderately improved the erythema. A 3 day course of intravenous methylprednisolone improved the dysgeusia. However, pneumonia occurred frequently due to <i>Pseudomonas aeruginosa </i>infections and we detected low immunoglobulin G (IgG) levels. He was promptly treated with antibiotics and supplemental intravenous immunoglobulin (5 g/day, three days). His general condition recovered temporarily, but he developed disseminated <i>Mycobacterium abscessus </i>infection and passed away 10 months after the first visit. TAMA was reported to affect the skin, liver, and colon, and it is clinicopathologically similar to GVHD. GVHDlike erythroderma is a cutaneous feature of TAMA. MG is a frequent autoimmune complication of TAMA, and TAMA can be accompanied by reduced serum IgG levels and Good syndrome. ＜文献概要＞63歳,男性.53歳時から尋常性乾癬あり.初診1ヵ月前より下肢を中心に緊満性水疱が多発するようになり当科を受診した.血清抗BP180抗体,抗デスモグレイン1および3抗体は陰性,血清TARC値は高値であった.病理組織学的に表皮下水疱を認め,蛍光抗体直接法で表皮真皮境界部にC3とIgGの線状沈着があり,蛍光抗体間接法でIgG抗表皮基底膜部抗体を認めた.1M食塩水剥離ヒト皮膚を基質とした蛍光抗体間接法では真皮側にIgGの線状沈着があり,正常ヒト真皮抽出液を用いた免疫ブロット法にて,患者血清中のIgGは200kDa蛋白質に陽性であった.以上から,抗ラミニンγ1類天疱瘡と診断した.治療はステロイド全身投与とメトトレキサートで行い,現在メトトレキサート単剤で抗ラミニンγ1類天疱瘡と尋常性乾癬の両疾患を寛解維持している.経過中の皮膚症状の増悪と血清TARC値の増減が一致しており,病勢との相関が示唆された.

症例:77歳、女性。現病歴:初診1ヵ月前から左頬粘膜に違和感が出現した。その後、頬部に皮膚結節が出現し急速に増大したため当科を受診した。初診時、左頬部に13cmの表面がカリフラワー状の腫瘤があり、壊死組織、悪臭を伴っていた。口腔内に頬部と連続する結節があり、中央には皮膚口腔瘻が存在した。また開口障害があった。皮膚病変の生検を行い扁平上皮癌と診断した。CT、MRI、FDG-PETで周囲筋組織への浸潤、左顎下部、両側頸部のリンパ節転移を認めたが、遠隔転移は認めなかった。皮膚原発か口腔原発かの判断が困難であったため、口腔扁平上皮癌に準じてcetuximab併用放射線療法を行い、腫瘍は著明に縮小し瘻孔も閉鎖した。その後、リンパ節病変の残存が疑われたためテガフール・ギメラシル・オテラシルカリウム配合錠を半年間内服した。治療開始後4年1ヵ月経過するも再発はない。治療開始時には周囲組織浸潤や瘻孔による口腔機能障害があったが、治療奏効により機能回復した。(著者抄録)

60歳,女性。初診1年前から左下腿の皮下結節を自覚した。皮下結節は徐々に増大し,右下腿にも出現したため当科を受診した。初診時,両下肢に下床と可動性良好な数cm大の皮膚皮下結節が多発していた。左下腿結節の病理組織学的所見では,真皮から皮下脂肪組織に淡紅色に染まる無構造物質の沈着があり,その周囲にリンパ球主体の炎症細胞浸潤が軽度みられた。無構造物質はCongo Red染色とDFS染色で赤橙色に染色され,アミロイド沈着と考えた。さらに,数年前から指摘されていた両肺の小結節の生検を行ったところ,アミロイドの沈着が確認された。その他の臓器には異常を認めなかった。以上より,全身性ALアミロイドーシスと診断した。両下腿の多発皮下結節を生じるアミロイドーシスは限局性アミロイドーシスの症例が多く,全身性アミロイドーシスの診断契機となったのはこれまでの報告で自験例のみである。全身性アミロイドーシスは病型や臓器病変によって治療法や予後が異なるため,早期に治療介入できるように他科と連携して確定診断および病型の特定を行っていく必要がある。(著者抄録)

日本人皮膚T細胞リンパ腫(CTCL)患者に対するベキサロテン+光線(化学)療法併用の有効性と安全性を検討した。CTCL患者25例(男性11例、女性14例、平均69.1±13.2歳)を対象にベキサロテン300mg/m2/日を経口投与した後、bath-ソラレン+紫外線A(PUVA)療法または狭域紫外線B照射を実施した。modified Severity-Weighted Assessment Tool(mSWAT)とPhysician Global Assessment of Clinical Condition(PGA)を用いて治療後24週までの有効性を評価すると共に、安全性評価を実施した。mSWATを用いた評価では、Full Analysis Set(FAS)解析対象25例の全奏効率(CR+PR)は80.0%、Per Protocol Set(PPS)解析対象の12例では75.0%であった。PGA評価では、全奏効率はFAS解析例が84.0%、PPS解析例が83.3%を示していた。FAS解析では奏効までの期間は20.7±9.62日、奏効持続期間は117.0±43.0日、増悪までの期間は163.6±28.8日であった。また、バイオマーカーとしてのTARC値は12週後と24週後においてベースライン時と比較して有意に低下していたが、LDHと可溶性IL-2受容体に関して有意な変化はみられなかった。なお、全例に何らかの有害事象が認められ、発現率の高いものとして高トリグリセリド血症(21例23件)、甲状腺機能低下(21例22件)、好中球減少(15例16件)、高コレステロール血症(14例15件)、貧血(8例8件)、鼻咽頭炎・胸焼け(5例6件)を認めた。大半の有害事象は軽度～中等度であった。日本人CTCL患者に対するベキサロテン+PUVA併用療法は、有効かつ安全であることが示された。

Dermokine is a chiefly skin-specific secreted glycoprotein localized in the upper epidermis, and its family consists of three splice variants in mice and five in humans. To investigate the pathophysiological impact of dermokine, we generated mice deficient for two (βγ) or all dermokine isoforms (αβγ). Both variants, especially dermokine αβγ-deficient mice exhibited scale and wrinkle formation resembling ichthyosis accompanied by transepidermal water imbalance at the neonatal stage. Several dermokine αβγ-deficient mice died by postnatal day 21 when reared under low humidity. Moreover, the cornified envelope was vulnerable, and skin barrier lipid ceramides were reduced in the epidermis of dermokine αβγ-deficient mice. cDNA microarray and quantitative reverse transcriptase-PCR assays of the epidermis revealed the upregulation of small proline-rich protein and late cornified envelope family members, as well as antimicrobial peptides in the dermokine αβγ-deficient mice. These barrier gene signatures were similar to that seen in psoriasis, whereas recent studies demonstrated that congenital ichthyosis has gene profiles resembling psoriasis. In line with these findings, adult dermokine αβγ-deficient mice exhibited aggravated phenotypes in psoriasis-like dermatitis models but not in allergic dermatitis models. Dermokine may play a regulatory role in inflammatory dyskeratotic diseases, such as congenital ichthyosis and psoriasis, in the crosstalk between barrier dysfunction and inflammation.

＜文献概要＞70歳,女性.体幹,四肢の水疱を主訴に近医より紹介受診した.初診時,四肢を中心に発赤を伴わない緊満性の水疱が多発していた.表皮下水疱で,水疱内には好酸球を主体とした細胞が浸潤していた.蛍光抗体直接法で表皮基底膜部にIgG,C3が線状に沈着していた.抗BP180抗体は陰性であったが,水疱性類天疱瘡と診断し加療を開始した.しかし,プレドニゾロン,ジアミノジフェニルスルホンやミゾリビンとの併用は無効であった.その後の血清解析で後天性表皮水疱症と診断した.プレドニゾロン15mg/日にコルヒチン1mg/日の内服を追加したところ,コルヒチン開始後2週間で皮疹は著明に改善した.現在プレドニゾロン5mg隔日投与とコルヒチン1mgとの併用で増悪はみられず経過している.治療抵抗性の後天性表皮水疱症に対するステロイドとコルヒチンの併用投与は治療効果が比較的早期に現れ,また病理組織学的に好中球の浸潤がみられなくても有効である場合もあるため,積極的に併用してもよい薬剤であると考えた.

リン酸化Tyr705(pY705)によるSTAT3活性化、不活性化調節について調べるため、HepG2-stat3ノックアウト細胞にて再構成したSTAT3を用いて検討したところ、リン酸化Ser727(pS727)は、pY705の脱リン酸化とCRM1非依存性核外排出に対するSTAT3の迅速な不活性化を誘発した。N末端タグはSTAT3の核内蓄積を引き起こし、非活性化において立体配置が変化したことを示していた。pY705-SH2構造の詳細な分析により、L706からP715のC末鎖(CTT)がCTT-CTT分子間と、pY705-SH2結合を支持するCTT-SH2分子内相互作用にとって重要な調節因子であった。STAT3の変異体の解析でもこれらが支持された。このようにpS727はCTT修飾を介してpY705-SH2の解離を誘発し、STAT3の特有の機能であるSTAT3の活性化-不活性化の迅速なサイクルを誘導すると考えられた。

＜文献概要＞5歳,女児。初診1ヵ月前より体幹に水疱が出現するようになった。初診時,両下腿や前腕に拇指頭大までの緊満性水疱や淡褐色の色素沈着,淡紅斑が散在していた。下腿の水疱の皮膚生検で表皮下水疱を認め,蛍光抗体直接法にて基底膜部にIgG,C3が線状に陽性であった。血清中の抗BP180抗体と抗BP180のNC16a部位リコンビナント蛋白を用いた免疫ブロット法で陽性を示し,水疱性類天疱瘡と診断した。ジアフェニルスルホンを0.7mg/kg/日で開始するも両足背に水疱新生が続くため,プレドニゾロン0.5mg/kg/日より開始し皮疹は消退した。小児における水疱性類天疱瘡は比較的予後良好とされ,プレドニゾロン内服が必要となる症例はまれである。

日本人の乾癬性関節炎(PsA)患者に対するアダリムマブの有効性と安全性について検討した。NSAIDsに不応のPsA患者37例(男性27例、女性10例、平均56.2歳)を対象に非盲検単群試験を実施し、アダリムマブは初回用量80mg、その後は隔週で40mgを24週皮下投与した。主要評価項目は12週後におけるACR20反応率とした。ACR20反応率は12週後が40.5%、24週後が45.9%、ACR50反応率は12週後が24.3%、24週後が37.8%、ACR70反応率は12週後が16.2%、24週後が21.6%であった。生物学的製剤の投与歴からみると、ACR20、ACR50、ACR70の反応率に関して投与群と非投与群の間に著明な差は認められず、乾癬面積重症度指数反応率にも群間差はみられなかった。平均医師全般評価スコアは12週後と24週後に有意に減少し、血清中CRP、28関節疾患活動性スコア(DAS-28CRP)も有意に減少していた。また、平均健康評価アンケート障害指数スコアはベースライン時と比較して12週後、24週後に減少し、SF-36身体・精神的コンポーネントスコアは有意に上昇していた。安全性に関して、有害事象の発現は24例(64.9%)、治療関連有害事象の発現は19例(51.4%)であり、肝酵素値上昇、KL-6糖蛋白異常、LDH上昇、咽頭炎、一次症状悪化、白癬、頭痛、発熱が多く、いずれも軽度～中等度にとどまっていた。日本人の乾癬性関節炎患者に対してアダリムマブは有効で忍容性も良好であることが示された。

<p>A 77－year－old woman presented to us with a 13 cm skin tumor on the left cheek with trismus. She had first noticed symptoms in the buccal mucosa of the cheek one month prior to the emergence of the rapidly growing tumor. The histopathological diagnosis of the tumor was squamous cell carcinoma. Clinical imaging revealed tumoral invasion to the circumjacent muscle tissue and bilateral cervical lymph node swelling with no distant metastasis. She received cetuximab combined with radiation therapy, followed by oral tegafur, gimeracil and oteracil. Her oral function improved with tumor regression and closure of the oral cutaneous fistula. <p>A 60-year-old woman presented with a 1-year history of asymptomatic subcutaneous nodules on the lower extremities. Physical examination revealed multiple hard subcutaneous nodules with a smooth surface distributed on the bilateral lower extremities. Histopathological examination of a skin biopsy specimen from her left lower extremity revealed nodules with eosinophilic amorphous material in the dermis to the subcutaneous tissue. Congo red and direct fast scarlet staining tests revealed the amorphous material was amyloid, and immunohistochemical examination confirmed it to be light-chain (AL) amyloid. Additionally, a transbronchial biopsy specimen from a nodule of the left lung showed amyloid deposition. Further investigation showed involvement of no organ other than skin and the bilateral lungs. She has been stable over two years of follow-up. Systemic AL amyloidosis rarely presents with primary clinical features of multiple subcutaneous nodules of the extremities. Treatments and prognoses differ depending on disease types and organs affected by this condition. Wound healing is regulated by complex interactions between the keratinocytes and other cell types including fibroblasts. Recently, adipose-derived mesenchymal stromal/stem cells (ASCs) have been reported to influence wound healing positively via paracrine involvement. However, their roles in keratinocytes are still obscure. Therefore, investigation of the precise effects of ASCs on keratinocytes in an in vitro culture system is required. Our recent data indicate that the epidermal equivalents became thicker on a collagen vitrigel membrane co-cultured with human ASCs (hASCs). Co-culturing the human primary epidermal keratinocytes (HPEK) with hASCs on a collagen vitrigel membrane enhanced their abilities for cell proliferation and adhesion to the membrane but suppressed their differentiation suggesting that hASCs could maintain the undifferentiated status of HPEK. Contrarily, the effects of co-culture using polyethylene terephthalate or polycarbonate membranes for HPEK were completely opposite. These differences may depend on the protein permeability and/or structure of the membrane. Taken together, our data demonstrate that hASCs could be used as a substitute for fibroblasts in skin wound repair, aesthetic medicine, or tissue engineering. It is also important to note that a co-culture system using the collagen vitrigel membrane allows better understanding of the interactions between the keratinocytes and ASCs.

The pemphigoid group is a category of autoimmune subepidermal blistering diseases in which autoantibodies deposit linearly at the epidermal basement membrane zone (BMZ). The main subtypes of pemphigoid mediated by immunoglobulin G autoantibodies are bullous pemphigoid (BP), mucous membrane pemphigoid (MMP) and epidermolysis bullosa acquisita (EBA). To establish the first guidelines approved by the Japanese Dermatological Association for the management of pemphigoid diseases, the Committee for Guidelines for the Management of Pemphigoid Diseases (Including EBA) was founded as part of the Study Group for Rare Intractable Skin Diseases under the Ministry of Health, Labor and Welfare Research Project on Overcoming Intractable Diseases. These guidelines aim to provide current information for the management of BP, MMP and EBA in Japan. Based on evidence, the guidelines summarize the clinical and immunological manifestations, pathophysiologies, diagnostic criteria, disease severity determination criteria, treatment algorithms and treatment recommendations. Because of the rarity of these diseases, there are few clinical studies with a high degree of evidence, so several parts of these guidelines were established based on the opinions of the Committee. To further optimize these guidelines, periodic revision in line with the new evidence is necessary.

60歳代,男性。初診の6年前に右前腕部に熱傷を受傷。その後,一旦治癒したが,同部位にそう痒を伴う結節が出現し,徐々に拡大してきたため,約1年前に前医を受診した。ステロイド外用剤を処方されるも改善なく,当科へ紹介となった。初診時,右前腕に12×16cm大の紅色局面を認め,辺縁には馬蹄状に配列する一部疣贅状の痂皮を伴った結節を認めた。結節部から皮膚生検を施行したところ,病理組織像で類上皮細胞肉芽腫を伴う慢性炎症を認めるものの,乾酪壊死像は明らかではなかった。皮膚生検組織の結核菌培養検査にてMycobacterium tuberculosis complexが検出された。全身精査では皮膚以外に結核病巣は認められず,本症例を結核菌の直接接種により生じた皮膚疣状結核と考えた。その後,抗結核薬4剤の標準的治療を開始し,疣贅状結節は治療に伴い徐々に改善した。6ヵ月の標準治療で軽快し,その後再発を認めていない。本邦における皮膚疣状結核の報告では乾酪壊死を認める症例は稀であり,病理組織学的に乾酪壊死像の欠如は結核の除外診断に必ずしもつながらない。難治性の疣状病変は,皮膚疣状結核も鑑別の一つに入れ,病理組織検査に加えて抗酸菌培養検査も行うことが重要であると考える。(著者抄録)

＜文献概要＞67歳,男性。頭部打撲後に腫瘤が出現し,生検で血管肉腫と診断され,当科を紹介受診した。Weekly paclitaxelを3コース施行後,放射線治療中に倦怠感と嘔気が出現した。CTで閉塞性イレウスが疑われたため小腸部分切除を施行したところ,血管肉腫の転移と判明した。化学療法を予定するも通院を自己中断され,初診8ヵ月後に再度倦怠感と嘔気を訴え入院となった。造影CTにて小腸転移再発,腹膜播種,癌性腹膜炎,多発肝転移の所見があった。徐々に状態が悪化し,初診9ヵ月後に永眠された。血管肉腫の転移巣として頻度の高い肺転移がなく,消化管転移をきたした症例はまれであり,当科で経験した血管肉腫の症例のなかでも唯一であった。

<p>A 71-year-old man presented with itchy linear eruptions that had developed repeatedly on both sides of his lumber area and around the knees for about three years. Although they were treated with topical corticosteroids under the diagnosis of eczema, bullous lesions arose to accompany the erosions for approximately half a year. The clinical picture resembled the linear dermatitis. Histopathologically, a subepidermal bulla with an infiltration of eosinophils and lymphocytes was observed. Serum anti-BP180NC16a antibodies were negative. By direct immunofluorescence, granular and linear deposition of C3 on the basement membrane was noted, but IgG and IgA deposition was negative. IgG and IgA reactivity on indirect immunofluorescence using nomal human skin was also negative. By indirect immunofluorescence using 1M sodium chloride-split skin, weak reactivity for serum IgA on the epidermal side of the basement membrane was observed, whereas intra-bullous exudate showed no reactivety for either IgG or IgA. All immunoblottings and enzyme-linked immunosorbent assays examined also gave negative results. From these results, our patient was diagnosed with granular C3 dermatosis. To elucidate the etiology of granular C3 dermatosis, careful investigation of more cases in the future is required.</p>

<p>An infusion of vancomycin hydrochloride was administered to a 64-year-old woman with pneumonia. Blistering appeared immediately after stopping the infusion, and after re-administration, many blisters appeared in the erythema on the trunk and lower extremity. Anti-DSG1 antibodies, anti-DSG3 antibodies, and anti-BP antibodies were all negative. Histopathological examination revealed blisters and neutrophils, and slight eosinophil infiltration was observed in both the blisters and the superficial layers of the dermis. In direct immunofluorescence, linear IgA deposition was observed at the dermoepidermal junction. She was diagnosed with vancomycin-induced linear IgA bullous dermatosis (LABD). After the vancomycin hydrochloride was discontinued, no vesicle neogenesis was observed, and the erosion became epithelialized. The frequency of use of vancomycin hydrochloride for MRSA infections is expected to increase in the future, so it is necessary to keep this compilation in mind and accumulate further data from future cases.</p>

<p>A 74－year－old man presented with a left cheek nodule. He had a surgical history of excision of the cheek lesion nine years previously, which was diagnosed as a benign neoplasm with sweat gland differentiation. Histopathologically, the nodule consisted of nests embedded in mucin pools, while immunohistochemically, the tumor cells were positive for cytokeratin 7 (CK7) and gross cystic disease fluid protein 15 (GCDFP－15) and negative for CK20. No lesions were observed in the other visceral organs. Based on a diagnosis of primary cutaneous mucinous carcinoma (PMC) with left parotid lymph node metastasis, he underwent a wide local excision and radical neck dissection. He received additional excision and radiation therapy for local recurrence 20 months later. We retrospectively reviewed specimens from past excisions. The tumor consisted of sweat gland－like epithelium and was positive for CK7 and GCDFP－15. The pre－existing lesion was diagnosed as endocrine mucin－producing sweat gland carcinoma, which most likely progressed to PMC. [<i>Skin Cancer (Japan)</i> 2019 ; 34 : 57－62]</p>

<p>Tuberculosis verrucosa is warty plaque-like form of cutaneous tuberculosis resulting from the invasion of <i>Mycobacterium tuberculosis </i>into theskin in a previously <i>M. tuberculosis</i>-sensitized person. Cutaneous tuberculosis is classied into four subtypes by infection pattern : direct inoculation, direct inltration from internal lesion, hematogenous dissemination and tuberculid. We report a case of tuberculosis verrucosa that developed on a burn scar. A 67-year-old man presented with nodules and reddish plaqueon a burn scar on theright forearm. Hewas burned by a hot asphalt mixturesix years prior and treated the wound without medical intervention, but pruritic nodules developed on the burn scar later. He visited his previous doctor and received a topical steroid, which provided limited relief of pruritus, and he was referred to our hospital one year later. On physical examination, a 12×16 cm-sized reddish plaque on the right forearm with nodules arranged in a horseshoe shape was observed on the margin of the lesion. Skin biopsies were performed from the nodules, and the histopathological ndings were consistent with epithelioid cell granuloma without caseation necrosis. <i>M. tuberculosis </i>complex was detected in the mycobacterium culture of the biopsy specimen. As there was no tuberculous lesion other than the skin based on whole-body examination, we diagnosed him with tuberculosis verrucosa cutis caused by direct inoculation of <i>M. tuberculosis</i>. We started combination therapy with anti-tuberculosis drugs and the lesion was cured six months later. There have been 38 reported cases of tuberculosis verrucosa cutis in Japan in the past 35 years, and only 2 cases exhibited caseous necrosis histopathologically. This suggests that tuberculosis cannot be excluded by the absence of caseous necrosis in a biopsy specimen. Tuberculosis verrucosa cutis should be considered as a differential diagnosis of intractable warty lesions and we should perform mycobacterium culture in addition to pathological tissue examination. <i>Skin Research, 18 : 216</i>-<i>221, </i><i>2019 </i></p>

<p>A 40-year-old woman developed itching within the pharynx 20 minutes after taking a Precol^○Rlongacting capsule. Subsequently, itching edematous erythema spread throughout her body. The symptom disappeared in 1.5 hours without medical treatment. The scratch tests for the Precol^○Rlong-acting capsule(1% aq. and 10% aq.) and isopropylantipyrine(1% aq. and 10% aq.) werepositive. Isopropylantipyrineis oneof theconstituents of thePrecol^○Rlong-acting capsule. Oral provocation tests for ve drugs, including each constituent of the Precol ^○Rlong-acting capsule, except for isopropylantipyrine, were all negative. Background: Pseudomonas aeruginosa (PA) frequently develops antibiotic-resistant characteristics, which is clinically problematic. The main reason behind the rise of antibiotic-resistant PA is the extensive use of antibiotics. Therefore, a novel technique is needed to treat PA infections. Photodynamic therapy (PDT) is thought to have the potential to be a non-antibiotic treatment for infections. 5-Aminolevulinic acid (ALA), which works as a photosensitizer after being metabolized into protoporphyrin IX (PpIX) in the heme synthetic pathway, is used for PDT. Thus far, the in vivo effectiveness of PDT using ALA against PA is unknown. Objective: In this study, we investigated PDT using ALA both in vitro and in vivo. Methods and results: Although PDT with ALA alone did not show a bactericidal effect on PA, PDT with both ALA and EDTA-2Na had a bactericidal effect in vitro. In in vivo experiments, wounds healed faster in PA-infected mice treated with PDT using both EDTA-2Na and ALA compared to non-PDT. Conclusion: These results suggest that PDT with EDTA-2Na and ALA is a potential novel treatment option for PA-infected wounds.

410nm発光ダイオード(LED)ライトをエチレンジアミン四酢酸-2ナトリウム(EDTA-2Na)と5-アミノレブリン酸(ALA)とともに用いた光線力学的治療(PDT)による緑膿菌感染皮膚潰瘍の滅菌と治癒を解析した。緑膿菌上清にALAを加えてLEDライトに曝露して、コロニー形成単位(CFU)を計測した。緑膿菌をALAまたはEDTA-2Naとともにインキュベートして、プロトポルフィリンIXを高速液体クロマトグラフィーで測定した。緑膿菌をEDTA-2Naの有無で培養して、バイオフィルム形成を定量した。EDTA-2NaをHaCaT細胞に適用して、細胞生存率を評価した。マウス線維芽細胞にALAとEDTA-2Naを伴ったPDTを行い、細胞毒性を評価した。緑膿菌感染皮膚潰瘍マウスモデルを作製し、ALAおよびEDTA-2Naを含む軟膏の潰瘍への適用またはピペラシリン・タゾバクタム腹腔内中注射で治療し、潰瘍部を切除して培養し、CFUを計測した。EDTA-2Na/ALAと410nm LEDライトによるPDTはin vitroで殺菌効果があった。EDTA-2Naの追加によって、バイオフィルム産生を有意に減少させた。EDTA-2NA/ALAを用いたPDTは緑膿菌感染皮膚潰瘍の治癒を強化し、ピペラシリン・タゾバクタムと比較して、創傷治癒が早かった。

症例は3歳男児で、出生時よりまばらな縮毛を有していた。毛髪検査で薄い縮毛と乏毛症が認めた以外に他の症状は見られなかった。父親はわずかな頭髪の縮毛と男性型脱毛症が見られたが、母親には毛髪疾患の病歴はなかった。患者から血液を採取しゲノムDNAを分離した。両親の血液および患者毛髪の採取は拒否されたため、患者のゲノムDNAをテンプレートとしてLIPH遺伝子の突然変異を調査した。変異解析では、患者はLIPH遺伝子にc.736T>Aとc.1095-3C>Gの複合ヘテロ接合体を有することが明らかとなった。前者は日本人集団における一般的な発端者突然変異であったが、後者の突然変異の報告が無く、新たなスプラス部位の変異であった。更に培養細胞でのin vitro転写アッセイで、c.1095-3C>G突然変異はフレームシフトおよび蛋白質レベルでの未成熟終止コドンの原因となることが示された。これらの知見は、常染色体劣性縮毛症における分子基盤の手掛かりとなることが示唆された。

Autosomal recessive woolly hair is a relatively rare hereditary hair disorder characterized by sparse, short, curly hair. This condition is known to be caused by mutations in the LIPH gene, LPAR6 gene or KRT25 gene. In the Japanese population, most patients with autosomal recessive woolly hair carry one of two founder mutations in the LIPH gene, c.736T&gt
A (p.Cys246Ser) or c.742C&gt
A (p.His248Asn). However, occasionally, individuals with this condition carry compound heterozygous mutations, typically one founder mutation and another mutation. In this study, we describe a patient with a compound heterozygous mutation in the LIPH gene at c.736T&gt
A and c.1095-3C&gt
G. The latter mutation created a novel splice site. This was the fourth splice site mutation to be described in the LIPH gene. Furthermore, we performed an in vitro transcription assay in cultured cells, and demonstrated that the c.1095-3C&gt
G mutation led to a frame-shift, which created a premature termination codon at the protein level (p.Glu366Ilefs*7). Finally, we summarized the mutations previously reported for the LIPH gene. Our findings provide further clues as to the molecular basis of autosomal recessive woolly hair.

Background: Tuberous sclerosis complex (TSC) gene mutations lead to constitutive activation of the mammalian target of rapamycin (mTOR) pathway, resulting in a broad range of symptoms. Hypopigmented macules are the earliest sign. Although we have already confirmed that topical rapamycin treatment (an mTOR inhibitor) protects patients with TSC against macular hypopigmentation, the pathogenesis of such lesions remains poorly understood.Objective: Recently emerging evidence supports a role for autophagy in skin pigmentation. Herein, we investigated the impact of autophagic dysregulation on TSC-associated hypopigmentation.Methods: Skin samples from 10 patients with TSC, each bearing characteristic hypopigmented macules, and 6 healthy donors were subjected to immunohistochemical and electron microscopic analyses. In addition, TSC2-knockdown (KD) was investigated in human epidermal melanocytes by melanin content examination, real-time PCR, western blotting analyses, and intracellular immunofluorescence staining.Results: Activation of the mTOR signaling pathway decreased melanocytic pigmentation in hypopigmented macules of patients with TSC and in TSC2-KD melanocytes. In addition, LC3 expression (a marker of autophagy) and autophagosome counts increased, whereas, intracellular accumulation of autophagic degradative substrates (p62 and ubiquitinated proteins) was evident in TSC2-KD melanocytes. Furthermore, depigmentation in TSC2-KD melanocytes was accelerated by inhibiting autophagy (ATG7-KD or bafilomycin Al-pretreatment) and was completely reversed by induction of autophagy via mTOR-dependent (rapamycin) or mTOR-independent (SMER28) exposure. Finally, dysregulation of autophagy, marked by increased LC3 expression and accumulation of ubiquitinated proteins, was also observed in melanocytes of TSC-related hypopigmented macules.Conclusion: Our data demonstrate that melanocytes of patients with TSC display autophagic dysregulation, which thereby reduced pigmentation, serving as the basis for the hypomelanotic macules characteristic of TSC. 50歳代、女性。当科初診の約2年前より四肢に紅色丘疹が出現してきた。近医で皮膚生検を3回施行し、リンパ腫様丘疹症(LyP)の診断で加療されていた。自然消退と再発を繰り返していたが、当科初診の約1ヵ月前より潰瘍を伴う結節が新生し、体幹にも拡大し多発してきたため当科紹介受診となった。潰瘍化した結節の病理組織像では、真皮浅層から皮下脂肪織にかけて異型性の核を持つ大型の腫瘍細胞が密に増殖し、表皮向性はなく、CD30が腫瘍細胞の80%に陽性であった。以上より、LyPに原発性皮膚未分化大細胞リンパ腫(C-ALCL)が生じたと考えた。LyPは、多発性丘疹が消退と再発を繰り返し、病理組織学的には悪性所見を伴うが、予後良好な疾患と考えられていた。しかし、近年は菌状息肉症やC-ALCL、Hodgkin diseaseなどの悪性リンパ腫が経過中に高率にみられると報告されている。LyP経過中の悪性リンパ腫を早期発見・治療するために、長期的な経過観察が必要であると考えた。(著者抄録)

結節性硬化症(TSC)に伴う色素沈着低下に対するオートファジー調節不全の影響を調べた。TSC患者10例の皮膚病変部位と健康対照6例の対応部位を採取し、免疫組織化学的および電子顕微鏡的解析を行った。正常ヒト新生児表皮メラニン細胞のTSC2またはATG7ノックダウン(KD)を行い、細胞生存アッセイ、メラニン含量アッセイ、リアルタイムRT-PCR、ウエスタンブロット法、細胞内免疫蛍光染色法を実施した。ラパマイシンの哺乳類標的(mTOR)シグナル伝達経路の活性化によって、TSC患者の低色素斑におけるメラニン細胞色素含量が減少し、TSC2-KDメラニン細胞における色素沈着が減少した。TSC2-KDメラニン細胞でLC3発現とオートファゴソーム数は増加し、p62やユビキチン化蛋白質といったオートファジー分解性基質の細胞内蓄積が見られた。ATG7-KDまたはバフィロマイシンA1前処理によるオートファジーの抑制によって、TSC2-KDメラニン細胞における色素脱失が促進され、mTOR依存型のラパマイシンまたはmTOR非依存型のSMER28曝露によるオートファジーの導入によって完全に逆転した。TSCに伴う低色素斑のメラニン細胞ではオートファジー調節不全が観察された。

39歳,男性。4ヵ月前より過敏性肺炎に対してプレドニゾロンの内服治療中,2ヵ月前よりST合剤の内服を開始された。2週間前より皮疹が出現し,2016年4月初旬に当科を受診した。受診時,全身の浮腫性紅斑,38℃台の発熱,CRP,白血球,肝胆道系酵素,異型リンパ球の高値を認めたため,薬剤性過敏症症候群(DIHS)を疑い,入院のうえプレドニゾロン50mg/日の内服治療を開始した。同日,急性の腹痛が出現し,血清アミラーゼ値の上昇とCT所見から膵炎と診断されたが,絶食により軽快した。入院7日目,皮疹,発熱,肝機能は再度増悪したが,膵炎の再燃は認めなかった。ステロイド内服量を増量した直後より膵炎を発症し,DIHSの再燃時には膵炎の再燃を認めなかったことより,ステロイドによる薬剤性膵炎と診断した。(著者抄録)

A 56-year-old woman presented with a 2-year history of small reddish papules with spontaneous regression and recurrence on her extremities. The patient had previously undergone skin biopsies three times and was diagnosed with lymphomatoid papulosis (LyP). These lesions ulcerated and spread over the entire body one month before her first visit to our hospital. Skin biopsy of an ulcerated nodule revealed pleomorphic large cells throughout the dermis without epidermotropic infiltration. CD30 was expressed by 80% of the tumor cells. Based on these findings, we diagnosed this patient with LyP that had progressed to primary cutaneous anaplastic large cell lymphoma. LyP is a recurrent papulonodular skin eruption with malignant histopathological features, but it often follows a benign and indolent clinical course. LyP has been reported to be associated with other malignant lymphomas such as mycosis fungoides, C-ALCL, or Hodgkin disease. Long-term follow-up is required for early detection of secondary malignancies in patients with LyP.Skin Research, 17: 25-29, 2018

<p>Recently, bacteria resistant to many antibiotics have spread and become a problem. Because using antibiotic causes to produce antibiotic-resistant bacteria, it is thought that it is necessary to develop new mean which is different from antibiotic, in order not to increase the number of resistant bacteria anymore. We focus on and study photodynamic therapy (PDT) as new treatment of bacteria infections. Here, we report effect of PDT against infection ulcer we have done so far.</p>

多施設の乾癬性関節炎(PsA)患者に対する骨髄細胞系列白血球選択的除去の有効性を調べた。通常の生物学的疾患修飾性抗リウマチ薬に抵抗性を示す中等度から重度のPsA患者20例(男性12例、女性8例、平均48.1±9.5歳)に対し、アダカラムにて顆粒球単球吸着除去療法(GMA)を週に1回、計5回実施した。有効性の主要評価項目は米国リウマチ学会スコアの20%以上の改善(ACR20)とした。部分奏効患者にはGMAを5回追加した。20例中、2例が試験を完了せず、9例は5回のGMAで奏効し、9例はGMAを計10回施行した。最後のGMAから2週間後の初回評価において、20例中13例(65.0%)がACR20を達成した。ACR20は最後のGMAから8週間後の評価において10例中7例(70%)、20週後の評価では10例中5例(50%)で維持されていた。以上より、薬物療法抵抗性のPsA患者において、アダカラムによるGMAは有効かつ安全であることが示された。

マスト細胞(MC)に着目して、ソラフェニブによって様々な皮膚有害反応が生じる機序を調べた。良性皮膚腫瘍切除後に皮膚移植手術を施行した16例(男性6例、女性10例、年齢52～91歳)からヒト正常皮膚検体を採取して、ソラフェニブとともに培養した。器官培養はPD98059、ワートマニン、抗幹細胞因子(SCF)中和抗体を用いた。ソラフェニブによって誘発された皮膚有害反応のある患者8例(男性3例、女性5例、22～85歳)の皮膚検体を採取した。免疫組織化学検査/免疫蛍光顕微鏡法、トリプターゼ、c-Kit、SCF mRNA発現の定量的RT-PCR解析を施行した。ソラフェニブは皮膚型MC数と脱顆粒を増加させた。ソラフェニブによって皮膚型MC成熟が刺激され、SCFが発現亢進した。ワートマニンはソラフェニブによって誘発されたMC数増加を抑制したが、PD98059は抑制しなかった。ソラフェニブ関連皮膚有害反応の皮膚病変内では、脱顆粒および総MC数の増加が観察された。

Psoriatic arthritis (PsA), a chronic inflammatory arthropathy associated with psoriasis, is an intractable immune disorder and refractory to pharmacological intervention. We assessed efficacy of selective depletion of myeloid lineage leukocytes in patients with PsA in a multicenter setting. A total of 20 patients with moderate to severe PsA refractory to conventional and biological disease-modifying antirheumatic drugs were included. Eligible patients had 3 points or more in the classification criteria for PsA. Each patient received five sessions, once a week, of adsorptive granulocyte and monocyte apheresis (GMA) with the Adacolumn((R)). The primary efficacy outcome was 20% or more decrease in the American College of Rheumatology score 20 (ACR20). Partial responders could receive an additional five GMA sessions. Of 20 patients, two did not complete the study, nine responded to five GMA sessions and nine received 10 sessions. At the first evaluation 2 weeks after the last GMA session, 13 of the 20 (65.0%) patients achieved ACR20. ACR20 was maintained in seven of 10 (70%) and five of 10 (50%) patients at the follow-up evaluation points 8 and 20 weeks after the last GMA session, respectively. GMA was well tolerated without any safety concern. This study demonstrates that GMA with the Adacolumn was effective with good safety profile in patients with PsA refractory to pharmacologicals. The results indicate a major role for myeloid leukocytes in the immunopathogenesis of PsA. A large controlled study is warranted to fully evaluate the efficacy of Adacolumn GMA in patients with PsA.

Linear IgA bullous dermatosis is characterized by IgA autoantibodies reactive with LAD-1 and LABD97, truncated forms of BP180 (type XVII collagen), but not with full-length BP180. Toyonaga et al. determined that cleavage within both the C-terminal region and NC16A domain plays a role in exposure of neoepitopes on the 15th collagenous domain of BP180.

97歳、男性。当科初診の3年前に右手背に腫瘤出現し、有棘細胞癌と診断され、手術勧められるも拒否された。徐々に腫瘤増大し、出血するようになったため加療目的に当科紹介受診となった。有棘細胞癌のStage IIであり、5年生存率は96%であった。患者の全身状態、背景を考えて局所麻酔下での切除術と植皮術を行った。高齢化社会の到来により超高齢者の悪性腫瘍患者も増えてきている。超高齢者の治療では、ガイドラインを踏まえつつも局所コントロールを中心とすることにより、ADLを低下させることのない方針が治療には適切と考える。(著者抄録)

手掌足底の悪臭を主訴に来院し、遺伝子検索により長島型掌蹠角化症と確定診断した症例を報告する。症例は8歳の女児。出生時から手掌、足底に紅潮、過角化があり特に発汗後に異臭を伴っていた。皮膚科専門医である開業医で治療されるも改善乏しく当科を紹介受診した。手掌、足底に紅潮を伴った過角化があり、手背、足背側まで至っていた。皮疹部を水に10分浸すと白色化した。臨床症状より長島型掌蹠角化症を疑い、SERPINB7の変異検索を行った。父由来c.455-1G>Aスプライス変異と母由来c.796C>T(p.R266*)ナンセンス変異の複合ヘテロ接合体であることが同定され、長島型掌蹠角化症と診断した。過角化、悪臭に対して20%尿素含有クリーム、硫黄含有軟膏併用療法行うも効果乏しいため、過酸化ベンゾイルゲルへ変更したところ、悪臭は消失した。角化の状態は横ばいである。(著者抄録)

エクリン汗孔癌は、切除で約8割は根治されるが、再発や転移した症例では死亡率が高い疾患である。患者は68歳の女性、左側腹部に84×32mm、不整形で表面平滑な板状硬の茶褐色の皮膚腫瘤があり、7年前に同部の腫瘍切除がなされていた。病理組織学的には、腫瘍胞巣は好塩基性細胞と淡明な細胞からなり、管腔形成がみられた。細胞分裂数は400倍の視野で1.2個、リンパ管浸潤があり、腫瘍厚は17.7mmであった。免疫組織化学的所見では、好塩基性細胞はAE1/AE3陽性、淡明な細胞はCAM5.2陽性、CEAは一部に陽性、EMAは陰性で、エクリン汗孔癌と診断した。左側腹部腫瘍の拡大切除と色素法によるセンチネルリンパ節生検を行い、左鼠径部のセンチネルリンパ節1個に転移を認め、左鼠径リンパ節郭清術を行った。1年後に行ったCT検査で同側腋窩部に1個のリンパ節腫大を指摘された。生検で転移を認め腋窩部リンパ節郭清を行った。術後6年経過し再発や転移はみられなかった。エクリン汗孔癌のリンパ節評価の重要性を認識した症例であった。エクリン汗孔癌の臨床と病理診断の注意点、リンパ節評価、予後不良因子について考察を行った。(著者抄録)

In this study, we generated a new set of monoclonal antibodies (mAbs) to bovine and human type VII collagen (COL7) by immunizing mice with bovine cornea-derived basement membrane zone (BMZ) fraction. The four mAbs, tentatively named as COL7-like mAbs, showed speckled subepidermal staining in addition to linear BMZ staining of normal human skin and bovine cornea, a characteristic immunofluorescence feature of COL7, but showed no reactivity with COL7 by in vitro biochemical analyses. Taking advantage of the phenomenon that COL7-like mAbs did not react with mouse BMZ, we compared immunofluorescence reactivity between wild-type and COL7-rescued humanized mice and found that COL7-like mAbs reacted with BMZ of COL7-rescued humanized mice. In ELISAs, COL7-like mAbs reacted with intact triple-helical mammalian recombinant protein (RP) of COL7 but not with bacterial RP. Furthermore, COL7-like mAbs did not react with COL7 within either cultured DJM-1 cells or basal cells of skin of a bullous dermolysis of the newborn patient. These results confirmed that COL7-like mAbs reacted with human and bovine COL7. The epitopes for COL7-like mAbs were considered to be present only on mature COL7 after secretion from keratinocytes and deposition to BMZ and to be easily destroyed during immunoblotting procedure. Additional studies indicated association of the speckled subepidermal staining with both type IV collagen and elastin. These unique anti-COL7 mAbs should be useful in studies of both normal and diseased conditions, particularly dystrophic epidermolysis bullosa, which produces only immature COL7.

乾燥性皮膚疾患に対して、肌着が皮膚病変・自覚症状・生活の質に与える影響を主観・客観的に解析した。乾燥性皮膚疾患を有する20歳以上の患者33名を対象とし、被験試料はグンゼ株式会社が販売している完全無縫製の肌着(メディキュア)を使用し、開始後14日後および28日後に評価した。紅斑・乾燥・掻破痕は14日および28日後共に有意な改善が認められた。結節・苔癬化は28日後に有意な改善が認められた。経表皮水分蒸散量は28日後に有意に減少した。そう痒によるVAS scoreは14日および28日後に、疼痛によるVAS scoreは28日後に有意に減少した。DLQI scoreは14日後・28日後に有意に減少した。研究終了まで明らかな有害事象は認められなかった。本研究品の肌着を着用後、各皮膚病変はいずれも有意に改善し、また、経表皮水分蒸散量・VAS・DLQI scoreも有意に低下した。以上の結果より、本研究品の肌着が、皮膚との摩擦を軽減することで、バリア機能を改善させ、その結果、乾燥によるそう痒や疼痛・皮膚病変も軽快し、患者の日常生活の満足度が高くなったと考える。よって、乾燥を有する皮膚には、日常のスキンケアに加え、縫い目や肌着の素材などを考慮し、刺激の少ない肌着を選ぶことが重要であることが示唆された。(著者抄録)

20歳代、男性。初診10年前に眉間部にヒアルロン酸注射を行った。2ヵ月前より眉間から眉弓の腫脹を自覚し、異物肉芽腫が疑われ当科受診となった。初診時、眉間から眉弓にびまん性の腫脹があり、その腫脹は眼の周囲や頬部まで拡がっていた。眉間部から皮膚生検を行ったが、確定診断には至らなかった。3週間後の再診時に、両頬部や鼻尖部、鎖骨部に暗紫色局面がみられ、病歴から同性愛者であることが判明し、HIV抗原抗体検査は陽性であった。鎖骨部の皮膚生検では、Kaposi肉腫(KS)の診断であった。KSとKoebner現象の関連性は過去に報告されており、自験例では眉間部へのヒアルロン酸注射がKSの発症に関連した可能性が考えられる。本邦においてAIDS患者は年々増加傾向であり、早期発見において皮膚科医の果たす役割は大きい。皮疹に先行して浮腫や腫脹がみられるKSがあること、そしてHIV陽性の場合は外的刺激によってKS発症のリスクになりうることに留意しておく必要がある。(著者抄録)

60歳代、女性。初診1ヵ月前より両手足の爪甲下に水疱が出現し、次いで口腔内にもびらんが出現した。手背の紅斑および水疱の病理組織像では表皮真皮境界部に水疱を認め、真皮浅層にはリンパ球を主体とする炎症細胞の浸潤を認めた。蛍光抗体直接法では表皮基底膜部にIgGとIgAの線状沈着が認められ、正常ヒト皮膚切片を基質とした蛍光抗体間接法はIgG、IgAともに陰性であった。正常ヒト表皮抽出液を用いた免疫ブロット法で180kDaにIgGが陽性であった。そしてBP180のC末端部位リコンビナント蛋白を用いた免疫ブロット法で患者IgGはこの蛋白に反応したが、BP180のNC16a部位リコンビナント蛋白、BP230、ラミニン332には反応しなかった。以上の所見より抗BP180型粘膜類天疱瘡と診断した。ニコチン酸アミド、ドキシサイクリンおよびステロイドの全身投与で症状は改善した。粘膜類天疱瘡は標的抗原や重症度に応じて多彩な臨床症状を呈す。皮疹が口腔内などに限局している症例や逆に皮膚症状が広範囲に及ぶ症例では、他の疾患と考えられて治療されていることも多いと考えられる。見過ごさないように疑わしい場合には同疾患を鑑別に挙げることが重要で、また診断技術の向上と普及も待たれると考えた。(著者抄録)

Background IgE autoantibodies are considered to be involved in the pathogenesis of bullous pemphigoid (BP), particularly inflammatory and erythematous phenotypes. Objectives To develop reliable enzyme-linked immunosorbent assays (ELISAs) for the detection of IgE autoantibodies to both BP180 and BP230 in BP sera, and to compare the ELISA results with clinical features.
Methods We used commercially available IgG ELISAs to develop IgE ELISAs for both BP180 and BP230. To determine the influence of excess amounts of IgG autoantibodies, all normal and BP sera were tested before and after IgG adsorption. The results of the IgE ELISAs were statistically compared with various ELISAs and various clinical parameters, including our own severity scores and BP phenotypes.
Results IgG adsorption generally showed no changes in sensitivity and specificity for IgE ELISAs, although slight cross-reactivity of anti-IgE secondary antibody to IgG and interference of excess amounts of IgG autoantibodies to IgE reactivity were suggested. IgE autoantibodies to BP180 were found in 21 of 36 BP sera and IgE autoantibodies to BP230 were found in 18 of 36 BP sera. The results of IgG and IgE ELISAs for both BP180 and BP230 were well correlated. IgG and IgE anti-BP180 antibodies correlated with disease activity but IgG and IgE anti-BP230 autoantibodies did not. IgE anti-BP230 autoantibodies correlated with nodular phenotype but not erythematous phenotype.
Conclusions The results of this study indicated that IgE autoantibodies to both BP180 and BP230 are frequently detected in BP sera. IgE anti-BP180 autoantibodies seemed to be pathogenic, while an association between IgE autoantibodies and inflammatory BP phenotype was not indicated.

Background: Recent studies have demonstrated podoplanin expression in several tumors, which has been associated with lymph node metastasis and poor prognosis. Podoplanin expression in peritumoral cells such as cancer-associated fibroblasts also correlates with tumor progression in several cancers. However, podoplanin expression and its association with extramammary Paget's disease (EMPD) remain unclear.
Objective: In this study, we examined whether the presence of podoplanin expression in tumor cells or peritumoral basal keratinocytes correlated with aggressive behavior in patients with EMPD and investigated the mechanisms of podoplanin-mediated tumor invasion in this disorder.
Methods: Skin samples of 37 patients with EMPD were investigated by immunohistochemical analysis. The functions of podoplanin in keratinocytes were examined in vitro by RT-PCR and with invadopodia gelatin-degradation assays using HaCaT cells.
Results: Podoplanin was not identified in tumor cells in all cases. Podoplanin expression in peritumoral basal keratinocytes was observed in 25 patients (67.6%). In in situ EMPD, 50% of cases (9 in 18) exhibited podoplanin-positive keratinocytes, whereas 84.2% (16 in 19) demonstrated positive staining in invasive EMPD (P &lt; 0.05). Podoplanin expression in peritumoral keratinocytes was also associated with tumor thickness (P &lt; 0.005). By immunohistochemical analysis, podoplanin-positive peritumoral keratinocytes were found to be negative for E-cadherin, one of the major adhesion molecules of keratinocytes, which might contribute to tumor invasion into the dermis through a crack in the basal cell layer induced by down-regulation of cell adhesion therein. We further found that podoplanin-positive keratinocytes exhibited invadopodia, which are thought to function in the migration of cancer cells through tissue barriers, indicating that podoplanin-positive peritumoral basal keratinocytes might assist tumor invasion by degrading the extracellular matrix.
Conclusion: The presence of podoplanin expression in peritumoral keratinocytes correlates with aggressive behavior in EMPD and might therefore serve as a useful prognostic marker for patients with EMPD.
