    目的:人間ドック受診者における好酸球性食道(Eosinophilic esophagitis:EoE)の臨床像を明らかにすることを目的とした。方法:2015年4月~2020年10月の人間ドック受診者延べ33,877人を対象としEoEと診断したEoE群と、年齢・性別を合わせて無作為に抽出した対照群で検討を行った。標準的な健診項目と上部消化管内視鏡所見について単変量解析し、p値が0.1未満の因子に対して多変量解析した。またEoE群でEoEに特徴的な内視鏡所見についての検討も行った。結果:20,145例の解析を行いEoEの発見率は0.47%であった。単変量解析ではEoE群に飲酒歴・つかえ感や胸やけ症状・アレルギー歴があることが有意に多く、内視鏡所見と他の健診項目については有意差を認めなかった。多変量解析の結果、飲酒歴・つかえ感や胸やけ症状・アレルギー歴があること・内視鏡所見でバレット食道がないことがEoEの独立した危険因子であることが明らかになった。EoEに特徴的な内視鏡所見のなかでは縦走溝が93%と最も多く認められた。結論:アレルギー疾患の既往、つかえ感や胸やけ症状、飲酒歴があり、内視鏡所見でバレット食道を認めない受診者には、EoEである可能性を考え特徴的な内視鏡所見を探したうえで積極的に食道粘膜の好酸球数を評価することが人間ドックでのEoEの発見率向上のために重要であると考えられた。(著者抄録)

    <p><b>Objective:</b> This study aimed to elucidate the clinical features and endoscopic findings in patients with eosinophilic esophagitis (EoE) during a medical check-up.</p><p><b>Methods:</b> This observational study was conducted on patients diagnosed with EoE during a physical examination at the MedCity21, Osaka City University, and randomly recruited age- and sex-matched healthy controls (1:1 ratio). Clinical characteristics, gastrointestinal endoscopic findings, and standard physical examination findings were analyzed using univariate analyses and compared between the patient and control groups, using multivariate logistic regression analyses for items with a <i>p</i>-value of < 0.1. Additionally, the detection rate of unique endoscopic findings in the patient group was also examined.</p><p><b>Results:</b> A total of 94/20,145 participants were diagnosed with EoE, with a 0.47% detection rate. Longitudinal grooves were the most characteristic endoscopic findings of EoE observed in 93% of patients. Univariate analyses showed that the prevalence of alcohol consumption, dysphagia or heartburn symptoms, and serological allergy were significantly higher in the patient group than in the control group. Multivariate analyses indicated that alcohol consumption, dysphagia or heartburn symptoms, history of allergies, and the absence of Barrett’s esophagus were independent risk factors for EoE.</p><p><b>Conclusions:</b> It is critical to evaluate the eosinophil count in the esophageal mucosa of patients with a history of allergic diseases and alcohol consumption, symptoms of choking or heartburn, and the absence of Barrett’s esophagus on endoscopic examination, to improve the EoE detection rate during a medical check-up.</p>

    2017年産婦人科診療ガイドラインでは、妊娠24週以降34週未満の早産が1週間以内に予想される場合に母体へのベタメタゾン投与が推奨されている。ベタメタゾンの効果が最大限発揮される至適時期に投与するには分娩時期の予測が重要であり、分娩時予測の指標として破水時の子宮頸管長やAFIが有用と報告されている。今回、妊娠23週以上32週未満のpreterm PROM症例25例を対象に、母体ベタメタゾン投与時期と分娩時期について検討した。ベタメタゾン投与は、分娩を1週間以内に予想した場合に12mgを24時間ごとに計2回筋肉内投与した。初回投与後1~7日で分娩に至った症例を「至適時期に投与できた症例」と定義すると、該当したのは7例(28%)で、至適時期を過ぎて投与したものが10例(40%)、投与しなかったものが8例(32%)であった。至適投与できた症例と至適時期を過ぎて投与した症例の比率について、破水時の子宮頸管長が≦20mmの群と>20mm群で比較するとともに、破水時のAFIが<5mmの群と≧5mm群、破水時の子宮頸管長が>20mmかつAFIが>5mmの群とそうでない群とで比較した。結果、いずれの比較においても有意な群間差は認められなかった。

    <p><b>Objective </b>The association between functional dyspepsia (FD) and endoscopic findings has not been fully elucidated. <i>Helicobacter pylori</i> infection is considered a key factor in the pathophysiology of FD. The Kyoto Classification of Gastritis (KCG) was proposed in 2014 to evaluate endoscopic findings based on the <i>H. pylori</i> status. We investigated the endoscopic findings associated with FD according to the KCG. </p><p><b>Methods </b>This cross-sectional study included subjects who underwent esophagogastroduodenoscopy during a medical health check-up. We compared the endoscopic findings between subjects with FD and healthy controls (HCs) according to the KCG. </p><p><b>Results </b>A total of 456 subjects were analyzed. Among them, the detection rate of FD was 5.5% (25/456 persons). In a univariate analysis of the endoscopic findings, a significantly lower proportion of subjects with FD had gastric red streak in comparison to HCs (0% vs. 18.6%, respectively; p=0.0124). Subjects with FD were more likely to have gastric depressive erosion (20.0% vs. 7.9%; p=0.0522). A higher proportion of the erosion-positive subjects had FD in comparison to erosion-negative subjects (12.8% vs. 4.8%). There were no significant differences in the other endoscopic findings, including gastric atrophy, intestinal metaplasia, enlarged fold, nodularity, and diffuse redness. A multivariate analysis revealed that gastric depressive erosion was significantly and independently associated with FD (odds ratio, 2.92; 95% confidence interval, 1.03-8.26; p=0.0436). In contrast, gastric red streak was not associated with FD (p=0.989). </p><p><b>Conclusion </b>Gastric depressive erosions may be associated with dyspepsia. </p>

    当院では前置胎盤例に対し妊娠33〜34週頃から入院管理を行い、36〜37週での手術を予定し、胎盤剥離面からの出血に対してはZ縫合で止血を行っている。今回、2012〜2016年に前置胎盤で帝王切開術を施行した84例のうち、癒着胎盤のため子宮摘出を行った9例と多胎2例を除く73例を対象とし、手術時間、出血量、次回妊娠への影響などについて調査した。また、36週で分娩した群と37週で分娩した群に分け、「出血量」「Apgar score」「臍帯動脈pH」「新生児低血糖の頻度」「新生児呼吸障害の頻度」を群間比較した。調査の結果、手術時間は36〜155分(中央値60分)、出血量は羊水込みで610〜7050mL(中央値2060mL)であった。次回妊娠への影響については、分娩後に追跡しえた26例中24例で月経再開が確認され、再開した月経はいずれも通常どおりであった。また、次回妊娠が4例で確認され、いずれも妊娠中に大きな問題はなかった。36週分娩群に比べて37週分娩群は「新生児呼吸障害の頻度」が有意に低く、他の比較項目に有意な群間差は認めなかった。

    目的:人間ドックでの胃がん発見精度の評価方法として胃がん発見率が挙げられるが、母集団の性差、年齢分布により影響を受ける。そこで我々は標準化発見比を指標として用い、細径内視鏡を用いた当院人間ドックでの胃がん発見精度を調査した。方法:2016年1月4日から1年間、当院人間ドックで上部消化管内視鏡検査を施行された4,927例において、胃がんの標準化発見比、発見率等を検討した。結果:男性における胃がん存在期待値合計は3.48であり、胃がん発見症例数は13例であった。すなわち、男性での標準化発見比は3.74であった。特に65〜69歳では存在期待値0.95に対して7例の胃がんを発見した。一方、女性での胃がん存在期待値合計は1.12、胃がん発見症例数は1例であり、標準化発見比は0.89であった。また、男女合算では、存在期待値4.60に対して14例の胃がんを発見しており、標準化発見比は3.04であった。さらには、胃がん発見率は男女合算で0.28%、年代別に解析すると、50〜59歳では0.21%、60歳以上では0.78%であった。結論:当院での人間ドックにおいて、胃がんの標準化発見比を検討することにより、罹患率が高い年齢層で確実に胃がんの発見、診断がなされていることが示された。(著者抄録)

    目的:胸部単純X線検査は肺疾患、特に肺がんのスクリーニング検査として検診や健診において重要である。その一方で、胸部単純X線検査には診断の限界があり、陳旧性肺結核などの病変がある場合、肺がんの指摘が困難な場合もある。当施設では有所見者に対する積極的受診勧告を行っており、今回はその有効性について検討した。方法:2015年1月1日から1年間に、胸部単純X線検査を施行した5,603例の胸部D2判定者(要精密検査)の画像所見の内訳、最終診断病名、発見率を検討した。結果:D2判定者は230例(4.1%)、呼び出しシステムにより197例(85.7%)に電話にて受診勧告を行った。また、32例(13.9%)に対して健診当日に胸部X線検査に異常ありと説明した。174例(75.7%)が当施設にてCT検査を施行した。肺野陰影は156例(89.7%)、そのうち結節影は106例、すりガラス陰影は17例であった。経過観察必要な陰影は20例、4例が肺がんと診断された。肺がん発見率は0.07%であった。集積症例の肺がん存在期待値は3.93人であり、標準化発見比は1.02であった。結論:当施設における胸部単純X線検査にてD2判定となった症例の実態と呼び出しシステムの有効性を明らかにした。当施設のような最終診断、治療まで可能な施設では再受診率が高い傾向にあり、二次予防として肺がんの早期発見、早期治療に有効であると考えられた。(著者抄録)

    Aim The optimal treatment for pelvic organ prolapse has been the subject of much discussion. The aim of this study was to assess the utility of a combination of uterosacral colpopexy and anterior vaginal mesh implantation. Methods A single-center prospective cohort study was conducted. Twenty-eight patients with stage III-IV cystocele and uterine prolapse underwent reconstructive surgery. A combination of vaginal hysterectomy, McCall culdeplasty, and trocar-guided anterior vaginal mesh implantation was performed, and the patients' postoperative outcomes were analyzed. Patient satisfaction was investigated using the modified Short Form 12 version 2 (SF-12v2) questionnaire, and interviews regarding sexual behavior were conducted at 1 postoperative year. Results A bladder injury occurred during the dissection in one case (3.6%). Recurrent vaginal vault prolapse beyond the hymen was observed in one patient (cure rate: 96.4%), and further mesh augmentation was required in this case. Another patient developed mild cystocele (Ba = 0), but was simply observed because she did not complain of any symptoms caused by vaginal descent. We did not experience any other mesh-related complications, such as protrusion, chronic pain, or chronic inflammation, during the follow-up period. The patients' modified SF-12 scores at 12 months were significantly better than their preoperative scores in all eight domains. Conclusion The satisfactory correction of pelvic organ prolapse was achieved using a combination of vaginal hysterectomy and uterosacral ligament colpopexy augmented by anterior vaginal mesh implantation.

    Objective To investigate time intervals of the ductus venosus (DV) flow velocity waveform (FVW) and those of the cardiac cycle that correspond with each DV-FVW component in fetuses with intrauterine growth restriction (IUGR) due to placental insufficiency.
    Methods Women with a pregnancy complicated by IUGR were recruited into the study, as was a normal control group. Time intervals for systolic (S) and diastolic (D) components were measured in DV-FVW as follows: SDV, from the nadir of the a-wave during atrial contraction to the nadir between the S-wave and D-wave; DDV, from the nadir between S-wave and D-wave to the nadir of the a-wave. Regarding cardiac cycles, the following variables were measured from ventricular inflow through the tricuspid valve (TV) and mitral valve (MV): STV and SMV, from the second peak of ventricular inflow caused by atrial contraction (A-wave) to the opening of the atrioventricular valve; DTV and DMV, from the opening of the atrioventricular valve to the peak of the A-wave. In the IUGR group, only the last examination performed within 1 week of delivery was used for analysis. All variables were analyzed statistically using Z-scores.
    Results Data were obtained from 249 normal fetuses and 26 fetuses with IUGR. Compared to normal fetuses, SDV showed a significant decrease (P &lt; 0.001), while DDV showed a significant increase (P &lt; 0.001) in the IUGR group. Regarding cardiac cycles, STV and SMV showed significant decreases (P = 0.014 and P &lt; 0.001, respectively) and DTV and DMV showed significant increases (P = 0.008 and P = 0.002, respectively) in fetuses with IUGR.
    Conclusion Time-interval alterations of DV-FVW in growth-restricted fetuses reflect the hemodynamic events caused by placental insufficiency. Copyright (C) 2014 ISUOG. Published by John Wiley & Sons Ltd.

    Objective To investigate the differences in toll-like receptor (TLR)-mediated immune responses between human neonates and adults, focusing on the cytokine profiles of monocytes, dendritic cells (DCs), and monocyte-derived DCs (MoDCs) in cord and adult blood.Study design Purified monocytes, DCs, and MoDCs were stimulated with the following TLR ligands: lipopolysaccharide (TLR4), Pam3CSK4 (TLR1/ 2), flagellin (TLR5), zymosan (TLR2), polyinosinic: polycytidylic acid (TLR3), imiquimod (TLR7), and CpG (TLR9). Interleukin (IL)-8, IL-6, tumor necrosis factor, IL-1 beta, and IL-10 concentrations were analyzed in culture supernatants.Results Compared with the effects in adult blood, lipopolysaccharide-, Pam3CSK4-, flagellin-, and polyinosinic: polycytidylic acid-stimulated inflammatory cytokine production in cord blood was generally weak in monocytes, comparable in DCs, and elevated in MoDCs. CpG-and imiquimod-stimulated cytokine production in DCs was comparable in cord blood and adult blood, but cytokine production was almost absent in monocytes and MoDCs in both cord blood and adult blood. In contrast, zymosan stimulation produced comparable inflammatory cytokine profiles in the monocytes, DCs, and MoDCs of cord blood and adult blood.Conclusion The immaturity of TLR-mediated innate immunity in neonates depends on monocytes rather than on DCs. Our results indicate that zymosan-mediated TLR2 signaling may be useful for developing a neonatal vaccine adjuvant.

    41歳(2経産)。妊娠25週目に前医で胎盤上の嚢胞状病変を指摘、著者らの施設へ紹介後、妊娠31週目に嚢胞状病変の増大と胎児中大脳動脈収縮期最大血流速度(MCA-PSV)の高値を認め入院管理となった。入院時、羊膜下血腫が疑われ、胎児循環不全に留意し、頻回の超音波検査を行ない、主に嚢胞のサイズと性状・羊水量・MCA-PSVの変化を経時的に観察した。この間、妊娠32週から34週目に断続的にMCA-PSVの中央値が1.8MoMとなり胎児貧血の進行が疑われたが、胎児のwell-beingに関する他の評価では異常所見がなく、保存的に管理した。以後、MCA-PSV低下がみられた妊娠36週以降、胎児機能評価のためcontraction stress test(CST)を周期的に施行して、妊娠38週5日目に陣痛が発来、経腟分娩で3380g女児を娩出、Apgar scoreは1分8点、5分9点で、貧血はなく、出生後も経過良好であった。尚、嚢胞は病理組織学的に絨毛膜下血腫であった。

    To evaluate the efficacy and safety of imidafenacin (IM), a novel short half-life anticholinergic, as add-on therapy for male LUTS with nocturia and nocturnal polyuria.
    This multicenter, prospective, randomized, open-labelled study was conducted and involved men who had frequency, urgency, and nocturia despite receiving a stable dose of alpha 1-blocker for a parts per thousand yen1 month. Subjects were randomised to control (alpha 1-blocker alone), IM twice/day (alpha 1-blocker +0.1 mg imidafenacin twice daily), or IM nightly (alpha 1-blocker plus 0.1 mg imidafenacin nightly) group; the treatment period was 8 weeks. Primary endpoints included improvements in night-time frequency and Nocturia Quality of Life Questionnaire (N-QOL) scores. Secondary endpoints included changes from the baseline in frequency volume chart variables, and post-void residual volume.
    Compared with the controls, IM twice/day and IM nightly patients had a significantly lower night-time frequency (changes from baseline: 0.1 +/- A 0.8 in control, -0.6 +/- A 0.9 in IM twice/day, and -0.4 +/- A 1.0 in IM nightly, p = 0.5227, 0.0006 and 0.0143, respectively). The hours of undisturbed sleep and N-QOL score were significantly improved in IM twice/day group, though not IM nightly group. Nocturnal urine volume was significantly reduced in IM nightly group, although total urine volume remained unchanged.
    A short half-life anticholinergic is suggested to be safe and effective as an add-on therapy for residual nocturia in patients with male LUTS receiving alpha 1-blocker treatment. Anticholinergic administration nightly could reduce the nocturnal urine volume.

    ObjectivesWe aimed to construct reference ranges for time intervals of each component of cardiac flow velocity waveforms in normal fetuses, comparing those variables between right and left ventricles.
    MethodsIn 359 fetuses at the gestational age of 17-38weeks, the durations of atrioventricular (AV) valve opening (AVVO), AV valve closure (AVVC), total E- (total-E) and A- (total-A) waves, total ejection time (total-ET), acceleration time (acc-E for E-wave, acc-A for A-wave, and acc-ET for ejection time), and deceleration time (dec-E for E-wave, dec-A for A-wave, and dec-ET for ejection time) were studied cross sectionally.
    ResultsBoth right and left acc-E showed the strongest correlations with gestational age (r=0.478 and r=0.519, respectively). Left AVVO showed a stronger correlation (r=0.474) than right AVVO (r=0.282) and, conversely, right AVVC showed a stronger correlation (r=0.399) than left AVVC (r=0.195) with gestational age. Significant differences (all P values &lt;0.001) were observed for all right and left parameters other than total-A and acc-E.
    ConclusionsCharacteristic differences between right and left ventricles were found in the reference ranges, suggesting the developmental properties of the fetal heart. (c) 2014 John Wiley & Sons, Ltd.

    42歳(初産婦)。前医にて配偶者間人工授精で妊娠が成立するも、妊娠17週4日目に多発筋腫合併妊娠の診断を受け、高年初産婦であることから、著者らの施設へ紹介となった。経過では妊娠22週3目日より下痢や嘔吐の消化器症状を認め、更に腹部痛が出現して急性腹症にて緊急入院となった。入院後、内診および超音波所見では明らかな異常所見は確認されなかったが、血液検査では感染徴候と貧血の進行が認められた。また、MRI所見では急性虫垂炎破裂後が疑われ、以後、緊急開腹術施行となった。術中所見では消化管に異常所見は認められなかったものの、右側卵管付着部位では子宮静脈が破裂し持続出血していた。そこで、出血部位の子宮筋層の縫合結紮とフィブリン加第13因子製剤の散布を行ない止血し、RCC 6単位を輸血した。その結果、術後は麻痺性イレウスを発症したが保存的に軽快し、患者は術後26日目に退院となった。一方、妊娠経過は良好であったが、妊娠38週5日目に子宮収縮の増加を認め再入院となった。妊娠39週目のCTG所見にて遷延性徐脈が認められたため、同日胎児機能不全の適応にて緊急帝王切開術が施行され、2785gの男児(Apgar score 1分7点/5分9点、臍帯動脈血ガスpH 7.222、BE 3.0)を娩出した。尚、頸部と体幹の臍帯巻絡が遷延性徐脈の原因と考えられ、帝王切開と同時に有茎性筋腫核出術を施行することで、最終的に母児は術後6日目にともに軽快退院となった。

    76歳女性(3経妊3経産)。特に誘因なく左大腿内側から左外陰部にかけての違和感と疼痛を自覚し近医のペインクリニックを受診、仙骨部硬膜外ブロックが2回施行されたが軽快せず、発症から1ヵ月後に著者らの施設へ紹介となった。所見では全身状態は良好で、産婦人科診察や直腸診ほか、外陰部にも肉眼的な異常は認められなかった。ただ左大陰唇下に1cm程度の大きさでピンポイントに圧痛がみられた。以後、高齢であり、頻尿、痛み、しびれ、冷えという症状から排尿障害を伴う腎虚と考え、八味地黄丸エキス顆粒(7.5g/日分3)の処方を行なったところ、2ヵ月経過で頻尿や外陰部痛はかなり軽減した。しかし、外陰部痛は軽度に残存するため更に八味地黄丸を2ヵ月処方したが外陰部痛は依然残存した。また、足を伸ばすと足先に痛みが走り、こむら返りになるとの訴えもみられ、八味地黄丸に加え芍薬甘草湯エキス顆粒(2.5g)を頓用で追加処方した。その結果、1ヵ月後の再診時には症状は完全に消失して外陰部の圧痛も消失した。

    Hemoperitoneum by uterine venous rupture during pregnancy is a very rare and serious complication, and is often a dramatic event associated with high maternal and fetal mortality. We report a case of hemoperitoneum caused by spontaneous rupture of a superficial uterine vein at 22 weeks of gestation. A 42-year-old woman, gravida 1, para 0, was admitted at 22 weeks and 5 days of pregnancy with severe abdominal pain, but vital signs remained stable. Blood testing on admission showed mild inflammatory reaction and anemia. Emergency exploratory laparotomy was performed due to suspected appendicitis. Laparotomy revealed 1155 ml of hemoperitoneum. Bleeding was originating from a ruptured vein on the uterine surface, and was stopped by suturing, compressing, and attaching powdered medicine of human coagulation factor XIII. Six units of red cell concentrate were transfused. Postoperatively, treatment was required for paralytic ileus and threatened preterm labor. Postoperative course and subsequent antenatal period proved uneventful. She was admitted at 38 weeks and 5 days of pregnancy due to onset of labor. At 39 weeks and 0 days, prolonged decelerations were observed on cardiotocography (CTG) , and emergency cesarean section was performed due to non-reassuring fetal status. A male baby was born with a birth weight of 2795 g (Apgar scores: 7 at 1 min; 9 at 5 min ; pH of umbilical arterial blood, 7.222 ; base excess, -3.0). In this case involving active bleeding at 22 weeks, hemostatic sutures were applied and pregnancy continued to term normally. The present case suggests that treatment comprising maintenance of adequate circulation and rapid surgical intervention may improve the chance for favorable outcomes for both mother and child.     症例は39歳で、妊娠35週以降羊水過多を認め、下腿浮腫増強で入院管理となった。40週4日に胎児心拍数図で遅発一過性徐脈を認め、胎児機能不全の適応で緊急帝王切開術を施行し、女児を娩出した。新生児仮死で人工呼吸管理となり、胎盤後血腫などはなかったが、両側巨口症、高口蓋、耳介低位、小顎症、両側副耳頸耳を認め、鰓弓症候群と診断した。その他の奇形は認めず、染色体検査では正常核型で、呼吸窮迫症候群の所見はなく、呼吸状態は速やかに改善した。出生後3日目までは経管栄養とし、唇顎口蓋裂患児に対する授乳方法を参考にして授乳量の少ない時期はシリンジによる注入を、吸啜力の増強に伴い専用乳首での授乳を行った。哺乳状況を確認して生後6日目に母児とも退院した。生後3ヵ月時に形成外科で両側巨口症形成術、両側副耳頸耳形成術が行われ、追加手術は成長過程で検討予定とした。生後1年4ヵ月経過し、身体発育・精神発達とも良好である。

    分娩管理を行った双胎妊娠250例のうち、妊娠24週以降に生産に至り、経腟トライアルを実施した161症例の分娩様式について報告した。頭位-頭位試験経腟分娩99例のうち、第一子が経腟分娩成功は79例で、そのうち第二子も経腟分娩が73例、帝王切開は6例であった。頭位-非頭位試験経腟分娩62例では第一子経腟分娩49例のうち第二子も経腟分娩が47例、帝王切開が2例であった。骨盤位ガイドラインに準じた場合に第二子が経腟分娩適応外とされる頭位-非頭位症例(妊娠34週未満、児体重2000g未満)は13例で、そのうち10例は両児とも経腟分娩に至った。他の3例は第一子娩出前に帝王切開が施行され、ガイドラインに示された要因で帝王切開となった例はなかった。

    Branchial arch syndrome is a congenital anomaly that presents with hypoplasia of tissues derived from the first and second branchial arches and is characterized by microtia and mandibular hypoplasia. We describe a case of branchial arch syndrome diagnosed postnatally after detection of polyhydramnios during the third trimester of pregnancy. A 39-year-old primiparous woman with unremarkable history had been followed in our department since the first trimester of pregnancy. Polyhydramnios was detected, but there were not any anomalies suggesting gastrointestinal or other disorders of the fetus at 35 weeks of gestation. At 40 weeks and 0 days, the patient was admitted due to progressive edema of her leg. Amniotic fluid index (AFI) at the time of admission was 25.1 cm. At 40 weeks and four days, late decelerations of fetal heart rate were detected on cardiotocography (CTG), and emergency cesarean section was performed under general anesthesia due to non-reassuring fetal status. Female baby with a birth weight of 2602g was born (Apgar scores at 1 and 5 minutes were 1 and 6 points respectively; pH of umbilical arterial blood was 7.168; base excess was -9.6). The baby was intubated and supported by artificial respiration because of neonatal asphyxia. She exhibited bilateral macrostomia a high-arched palate, bilateral low-set ears, microgenia, and bilateral accessory auricles. These findings led to the diagnosis of the branchial arch syndrome. Respiratory status improved rapidly and she was extubated on the first day after birth. She had difficulties with swallowing milk due to bilateral macrostomia and therefore tube feeding was required until the third day after birth, followed by oral injection of milk with a syringe. At the age of three months surgical correction of the bilateral macrostomia and bilateral accessory auricles was performed. As of 16 months old, the child's physical and mental development appeared favorable. It has been reported in the scientific literature that nearly 40% of cases of branchial arch syndrome show polyhydramnios due to fetal dysphagia. The present case suggests that the fetal face should be examined in details in consideration of the present syndrome when polyhydramnios of unknown origin is detected.     The optimal management of acute hepatitis B in pregnant women remains to be fully evaluated. A case of hepatitis B virus (HBV) infection in a 20-year-old pregnant woman who initially presented with jaundice is reported. Serum samples were positive for anti-HBc IgM and HBe antigen. Her husband was infected with HBV genotype A and received entecavir because of prolonged hepatitis and a high viral load. The HBV DNA sequence of the wife completely matched that of her husband, indicating that he was the source of HBV infection. In accordance with guidelines for the treatment of chronic HBV carriers, the wife started to receive tenofovir disoproxil fumarate (TDF) in her third trimester. After 4 months of treatment, the HBV DNA load decreased from 7.6 to 3.5 log copies/ml. At delivery, the serum was found negative for the HBe antigen. Seven months after treatment began, the HBs antigens also disappeared. The baby, totally healthy, received passive - active immunoprophylaxis. At 3 months, the baby remained free of HBV infection. TDF thus prevented exacerbation and prolongation of acute HBV infection in a pregnant woman. Subsequent treatment also prevented mother-to-infant transmission of HBV.     The optimal management of acute hepatitis B in pregnant women remains to be fully evaluated. A case of hepatitis B virus (HBV) infection in a 20-year-old pregnant woman who initially presented with jaundice is reported. Serum samples were positive for anti-HBc IgM and HBe antigen.     2003年1月〜2008年に周産期管理をした特発性血小板減少性紫斑病(ITP)合併妊娠21例24妊娠の問題点について検討した。妊娠前にITPの診断を得られたのは21例中14例であった。7例は妊娠中に診断を得た。分娩に至ったのは23例で、1例は人工妊娠中絶を選択していた。妊娠中および分娩後の血小板数の推移について検討すると、無治療群ではその多くが妊娠中および分娩後も安定した血小板数保つことができた。一方、治療群では7例が妊娠初期よりも50000/μl以下、3例は分娩後も50000/μl以下で経過した。ITP治療を行った14例については7例は妊娠初期から血小板数が50000/μl以下で、2例は脾摘を行っていた。14例中12例で血小板輸血が行われていた。分娩直前の血小板数と分娩時出血量についてみると、全例で分娩直前の血小板数は50000/μl以上であった。血小板数低下に伴い出血量が増加する傾向であるが、赤血球濃厚液(RCC)や新鮮凍結血漿(FFP)を投与した例はなかった。検討より得られた問題点は妊娠初期或いは妊娠前から血小板数が50000/μl以下であっても、本人の挙児希望が強く、周産期管理に苦慮する症例が多いということであった。

    Uterine leiomyosarcomas (LMS) are difficult to distinguish from benign leiomyomas without surgery. In this study we performed transcervical needle biopsy on 475 patients, 8 LMS patients and 467 patients with non-sarcomas (non-LMS) in a high-risk group for LMS, and evaluated whether examinations performed with Ki-67 and CD34 immunohistochemical analyses in addition to the standard hematoxylin-eosin (H&E)-stained sections would improve preoperative diagnostic precision of the uterine smooth muscle tumors. Histopathologic analysis included three factors: degree of cytologic atypia, mitotic index and coagulative tumor cell necrosis (CTCN). We also evaluated cell proliferation with Ki-67 expression. In cases of suspected CTCN, we examined CD34 expression and counted positive blood vessels in the necrotic area. Three of the 8 LMS cases satisfied the diagnostic criteria of LMS by histopathologic evaluation with H&E-stained sections. We made a score list based on these analyses; scores for LMS specimens ranged from 6-14 points; non-LMS specimens scored 0-2 points. At the cut-off score of 6 points, the positive predictive value to distinguish LMS from non-LMS was 100%, showing that this scoring system, is a useful method for preoperative differentiation between LMS and non-LMS tumors.

    Aim: To clarify the effects on uterine arterial flow velocity waveforms of uterine contractions following oxytocin infusion and during spontaneous labor.
    Methods: Uterine arterial flow velocity waveforms were obtained by pulsed Doppler methods from 22 women during an oxytocin challenge test (OCT), 26 women during oxytocin-induced labor, and 40 women during spontaneous labor. Mean resistance index (RI) for bilateral arteries was used for analyses. After the onset of labor, flow velocity waveforms were assessed according to cervical dilatation. During OCT, Doppler flow velocimetry was performed when three uterine contractions occurred per 10-min period.
    Results: RI values did not differ significantly between induced and spontaneous labor during relaxations at any level of cervical dilatation. However, during contractions, RI was significantly higher for induced labor than for spontaneous labor. Absence or reversal of flow was more frequent in the OCT group than in the induced labor group (P &lt; 0.0001). However, no significant differences were found between spontaneous and induced labor groups.
    Conclusion: Interactions between the contracting uterine body and the relaxing lower segment in oxytocin-induced labor might be associated with differences in uterine arterial flow during contraction between oxytocin-induced and spontaneous labor. However, changes in the intensity of uterine contractions during labor progression might differ between oxytocin-induced and spontaneous labor.

    Protein S is an antithrombotic cofactor for protein C that also has multifunctional anti-inflammatory, cellular protective, apoptotic and mitogenic properties. Protein S levels are thought to decrease during pregnancy, but the underlying mechanism remains unknown. We compared protein S concentrations throughout normal pregnancy with those of nonpregnant women and measured plasma M-binding protein levels in nonpregnant women and in pregnant women at the 40th gestational week. We also examined protein S and M-binding protein in the placenta by immunohistochemical staining at early (20th gestational week) and late (40th gestational week) stages of pregnancy. Plasma protein S activity and free protein S-antigen levels significantly decreased from the 10th gestational week and total protein S antigen decreased from the 20th. M-binding protein levels between pregnant and nonpregnant women did not significantly differ. The stainable portion of protein S was located at the fetomaternal interface, particularly at degenerative villi. M-binding protein was weakly stained at the same areas as protein S. Neither protein S nor M-binding protein were stained at normal villi. These results indicated that protein S can protect or restore damaged villi via a physiological effect in addition to its anticoagulation properties.     Objective: To describe a practical procedure of transcervical needle biopsy of uterine myoma-like tumors using an automatic biopsy gun and its potential risks. Design: Description of new biopsy procedure. Setting: University teaching hospital. Patient(s): Three hundred twelve patients who had been recommended for surgery for uterine myoma-like tumors by the hospital's outside doctors. Intervention(s): Transcervical needle biopsy of uterine myoma-like tumors. Main Outcome Measure(s): Successful sampling rate, duration of procedure, estimated blood loss, patient discomfort, and complications. Result(s): Of the 312 patients who underwent transcervical needle biopsy, specimens were obtained from 311 (99.7%). The mean (± SD) duration of procedure was 6.3 ± 5.2 minutes. The mean estimated blood loss was 10.4 ± 10.9 g. There were two cases (0.6%) in which the blood loss during the procedure was in excess of 50 g, but bleeding can be conservatively controlled in such cases with an intrauterine tamponade using gauze or a balloon catheter. The level of patient pain and discomfort during the needle biopsy was significantly lower than that during endometrial curettage. No major complications requiring surgery occurred. Conclusion(s): Transcervical needle biopsies of uterine myoma-like tumors using an automatic biopsy gun are practical, simple, and safe. This new procedure can be of routine clinical use. © 2002 by American Society for Reproductive Medicine.

    BACKGROUND. The clinical differential diagnosis between uterine sarcoma and benign leiomyoma is difficult even with magnetic resonance imaging (MRI). Therefore, a considerable number of patients have undergone hysterectomies due to an indication of "suspected malignancy" based on tumor size alone. However, approximately 80% of these hysterectomies have been judged to have been recommended inappropriately. In such situations, reliable preoperative diagnostic tests are required. The authors have evaluated the accuracy of needle biopsy for uterine myoma-like tumors, a procedure that to the authors' knowledge has been performed infrequently. METHODS. Transcervical needle biopsy was performed in 435 patients with uterine myoma-like tumors. The biopsy specimens were scored for degree of malignancy according to the histopathologic criteria proposed by Bell et al. Histopathologic evaluation of surgical specimens and clinical outcome after 2 years of follow-up were used as the reference standards. RESULTS. Of 435 patients, 7 had uterine sarcomas, 4 of which were scored as ≥ 4 points and were diagnosed as "sarcoma" by needle biopsy alone. No sarcoma cases were included in the group of patients with a score of 0. The cutoff score combining the highest sensitivity and specificity with respect to distinguishing uterine leiomyosarcoma from uterine leiomyoma was 2
    sensitivity, specificity, and positive and negative predictive values were 100%, 98.6%, 58%, and 100.0%, respectively. CONCLUSIONS. Transcervical needle biopsy using histopathologic scoring is a reliable diagnostic test for the differential diagnosis between uterine sarcoma and leiomyoma. This diagnostic method, combined with MRI screening, could reduce the number of patients currently undergoing unnecessary surgery. © 2002 American Cancer Society.

    Specific chromosomal abnormalities, such as del(7q), t(12;14), 12 trisomy, or the rearrangement of Gp, are seen in approximately 30% of uterine leiomyomas despite their benign status. We investigated the association between the shrinkage of uterine leiomyomas treated with a GnRH agonist and the interstitial deletion of chromosome 7q, which is one of the most common chromosomal abnormalities in uterine leiomyomas. This study covered 29 women with uterine leiomyomas who were treated with a GnRH agonist before surgery. The volume of the largest myoma nodule was measured by means of MRI before and at 12 weeks after the beginning of GnRH agonist treatment, and the percentage of the reduction in volume was calculated. Genomic DNA was extracted from leiomyoma tissue and peripheral blood, and amplified by PCR using fluorescently-tagged oligonucleotide primers of twelve microsatellite loci on chromosome 7. The PCR products were analyzed for loss of heterozygosity (LOH) using an automated fluorescent DNA sequencer. Of the 29 informative tumors, five (17%) showed LOH with deletion of the common region, D7S491. The mean percentages of the reduction in volume of the largest myomas with LOH or without LOH were 32 +/- 13 and 18 +/- 58%, respectively (not significant). One tumor showing interstitial deletion of both alleles (homo-deletion) reduced in volume by 19%. Another tumor showing an extraband increased in volume by 13%. Although tumor specific chromosomal deletion suggested the existence of tumor suppressor genes in this region, there was no significant association between the shrinkage of uterine leiomyomas treated with a GnRH agonist and the interstitial deletion of chromosome 7q.

    Objective: To compare ultrastructural features of leiomyoma cells from the same uterine myoma nodule before and after GnRH agonist (GnRH-a) treatment and to examine the relation between these ultrastructural changes and the extent of myoma volume reduction with GnRH-a treatment.
    Design: Prospective clinical study.
    Setting: University teaching hospital.
    Patient(s): Twenty women with uterine leiomyomas who were scheduled for surgery.
    Intervention(s): Transcervical needle biopsy of uterine myoma, s.c. leuprolide acetate injection (3.75 mg) at least three times every 4 weeks before surgery, and hysterectomy or myomectomy.
    Main Outcome Measure(s): The changes in ultrastructural features of uterine leiomyoma cells and the percentage decrease in the volume of the largest myoma at 12 weeks of GnRH-a treatment measured by magnetic resonance imaging.
    Result(s): A decrease in myofilaments, mitochondrial swelling, and emergence of the lysosomal body were observed in relation to GnRH-a treatment. Furthermore, a positive correlation was observed between the decrease in myofilaments and myoma shrinkage.
    Conclusion(s): Cellular atrophy due to a decrease in myofilaments plays a major role in the reversible myoma shrinkage resulting from GnRH-a treatment. (C) 2001 by American Society for Reproductive Medicine.

    We previously reported on the efficacy of transcervical needle biopsy of a uterine muscle tumor. In this study, to examine diagnostic accuracy of our needle biopsy with small specimen size, previous slide preparations were used from 8 cases diagnosed with leiomyosarcoma and from 24 post-operative leiomyoma cases. Each tissue specimen was examined through needle biopsy-sized slit that had been randomly positioned over the slide. Three predictors: mitotic index, cytologic atypism, and tumor cell necrosis were evaluated on a 3-point scale (0, 1, or 2 points). When the diagnostic cut-off value was set to two, the sensitivity was 92.5% and the specificity was 100%.

    高齢社会でのQOL疾患である骨盤臓器脱の適切な治療は女性骨盤底医学会において重要課題である。合成メッシュを用いた骨盤底再建手術は耐久性、低侵襲性の観点からも本邦でも急速に普及したが、2008年、2011年の2回にわたる米国FDAのアラートによって腟式メッシュ手術の適応を再考すべき時期に来ている。Tension-free vaginal mesh(TVM)手術は手術手技の統一化が図れることや、子宮を温存し易い利点がありPOP手術の重要な術式の一つとなった。しかし、産婦人科で施行されてきた腟式子宮全摘、腟壁形成術に加えて、上部腟管の補強をすることでnative tissueによる骨盤臓器脱修復は解剖学的に適切な方法であり、経腟メッシュ法よりトラブルの少ない安全な手術法であることを認識してほしい。(著者抄録)

    超高齢社会での質の高い医療サービスを提供するため、女性骨盤底医学が産婦人科、泌尿器科のサブスペシャリティとして確立される必要がある。欧米ではすでに2階建ての専門医として確立されており、日本の産婦人科の3つのサブスペシャリティ(周産期、婦人科腫瘍、生殖内分泌)に加えて4つめの分野として認定されることが望まれる。専門医認定には国の実情を踏まえた取り決めが必要であり、骨盤底医学の診療ガイドライン、教育ガイドラインを産婦人科、泌尿器科の共通の認定学会で策定するシステムが構築されなければならない。日本における骨盤底医学の指導医の技術力向上と研修施設認定のための施設基準の均一化を早急に図る必要がある。(著者抄録)
