    <p>We herein describe the case of a 20-year-old Japanese woman with adult-onset Still's disease (AOSD) complicated by macrophage activation syndrome (MAS). Despite the administration of prednisolone and tocilizumab, MAS developed with hyperferritinemia, thrombocytopenia, hypertriglyceridemia, and liver dysfunction. Steroid pulse therapy, cyclosporine A, and plasma exchange (PE) improved the patient's condition. However, diabetes hinders glycemic control under high-dose steroids. Subcutaneous canakinumab (300 mg/month) was initiated, thus enabling rapid prednisolone tapering to 10 mg within one month without relapse. This case highlights that PE followed by canakinumab is a potentially effective strategy for treating severe AOSD with MAS, particularly when minimizing steroid exposure is critical. </p>

    症例は59歳女で、当院受診の2ヵ月前から倦怠感、2週前から腰痛を自覚し、市販の鎮痛剤(ロキソプロフェンナトリウム)を内服し自制内であった。経過中に検温をしていたが発熱はなかった。人間ドックを毎年受けており、3日前の人間ドックで炎症反応上昇と脾腫瘍の疑いを指摘され、当院を受診した。心雑音があり、鑑別疾患の一つとして感染性心内膜炎を考え、精査を行った。体幹部造影CTで脾梗塞と脾動脈瘤を認めた。血液培養で緑色連鎖球菌が検出された。経胸壁心臓超音波検査で僧帽弁に2cm大で可動性のある構造物を認め、疣贅に矛盾しない所見であった。経食道心臓超音波では僧帽弁の疣贅に加えて大動脈弁の壁肥厚を認めた。頭部MRIで多発する微小梗塞を認めた。これらの所見から、多発塞栓症(脳・脾臓)と脾動脈瘤を伴う感染性心内膜炎と診断した。同日に僧帽弁形成術+大動脈弁形成術を施行し、術後経過は良好で、入院後33日目に自宅退院した。本例は受診前にNSAIDsを内服していたために発熱が不顕在化した可能性が考えられた。

    大阪府の外来インフルエンザ患者における抗インフルエンザ薬の処方動向と有効性について検討した。本研究は大阪府で行われた多施設共同観察研究であり、2022～2023年および2023～2024年の流行期にインフルエンザA型と診断され、REDCapに回答した患者(2022～2023年73例、2023～2024年96例)を対象とした。2022～2023年にはオセルタミビルが最も多く処方され(54.8%)、2023～2024年にはラニナミビルが最多であった(46.9%)。バロキサビルの処方割合は11.0%から19.8%へ上昇した。発熱軽快までの期間(平均±SD)は2022～2023年でバロキサビル群1.92±0.54日、NAI群2.31±0.13日、2023～2024年でそれぞれ1.92±0.21日および2.32±0.14日であり、両季とも有意差を認めなかった。薬剤投与後の新規症状出現率も両群で差はなかった。2022～2023年の年齢別処方では10歳未満にオセルタミビルが多く、10歳以上ではラニナミビルが多かったが、翌季では年齢に応じた処方が安定し、バロキサビルの使用率も上昇した。

    症例は73歳男で、当院受診の4ヵ月前に両下腿浮腫を自覚し、近医で利尿薬を処方されたが浮腫は残存した。1ヵ月前から右眼の視力低下と嚥下困難感を自覚し、精査加療目的で当院に紹介された。血液検査でカルシウム高値、尿検査でβ-2MG高値を認めた。右眼に肉芽腫性前部ぶどう膜炎とテント状周辺虹彩前癒着を認めた。入院後、高カルシウム血症に対し生理食塩水とエルカトニンの投与を開始した。ガリウムシンチグラフィで、甲状腺、肺野、腎臓、腹部骨盤リンパ節、下肢皮膚に集積を認めた。エコー検査で甲状腺両葉の甲状腺下極側から広範囲に不整な低エコー域を認めた。皮膚と鼠径リンパ節の生検を行い、類上皮肉芽腫を認めた。これらの所見から、サルコイドーシスと診断し、プレドニゾロン投与を行い、症状・検査所見とも改善した。

    Introduction: Since the beginning of the coronavirus disease 2019 pandemic, there have been limited reports on the
    clinical outcomes of outpatients with influenza treated with baloxavir or neuraminidase inhibitors (NAIs). This study
    aimed to investigate the prescription trends and effectiveness of baloxavir and NAIs.
    Materials and Methods: This descriptive epidemiological study included data of patients diagnosed with influenza in
    Osaka Prefecture, Japan, during the 2022-2023 and 2023-2024 seasons who answered a questionnaire via Research
    Electronic Data Capture.
    Results: During the 2022-2023 and 2023-2024 seasons, respectively, we enrolled 73 and 96 patients diagnosed with
    influenza A at 26 facilities. Oseltamivir was most commonly prescribed (54.8%) in the 2022-2023 season, whereas
    laninamivir was most commonly prescribed (46.9%) in the 2023-2024 season. Baloxavir was prescribed to 11.0%
    and 19.8% of patients in the 2022-2023 and 2023-2024 seasons, respectively. The time to fever alleviation did not
    differ significantly between the baloxavir and NAI groups in either season (2022-2023: p = 0.47; 2023-2024: p =
    0.12). There were no significant differences between groups in new symptoms after drug administration.
    Conclusions: No significant differences were observed in the effects of baloxavir or the NAIs on fever.

    Reports of hypoalbuminemia in patients with syphilis are rare, except in cases complicated by nephrotic syndrome. We report a case of syphilis-associated hypoalbuminemia in the absence of nephrotic syndrome. In this patient, hypoalbuminemia was likely multifactorial, including albumin leakage into the extravascular space and decreased albumin production due to syphilitic inflammation, endothelial glycocalyx damage associated with long-standing diabetes mellitus, and malnutrition. These factors collectively impaired albumin synthesis. Patients with syphilis accompanied by unexplained hypoalbuminemia may experience improvement in their serum albumin levels following appropriate treatment for syphilis.

    症例は73歳女性で、60歳時に血清反応陰性関節リウマチ(RA)と診断されており、他院でメトトレキサート(MTX)、ブシラミン、プレドニゾロン、インフリキシマブによる治療を受けていた。4ヵ月前に全身倦怠感を訴え、造影CTで右副腎腫瘍と大動脈周囲リンパ節の腫大が認められ、当院紹介となった。血清中にEpstein-Barr(EB)ウイルスが検出された。リンパ増殖性疾患(LPD)を疑い骨髄生検を施行したが、悪性細胞は検出されなかった。白血球数、リンパ球数、ヘモグロビン値の低下が認められた。MTX投与を中止した結果、症状や血球数は改善し、右副腎腫瘍とリンパ節腫大も縮小した。臨床的LPDとして経過観察を行っていたが、初診から7ヵ月後に発熱と汎血球減少を認めた。再度の骨髄生検により、EBウイルス陽性のび漫性大細胞型B細胞リンパ腫に血球貪食症候群を合併していることが確認された。ステロイドパルス療法後、リツキシマブ、シクロホスファミド、ドキソルビシン、ビンクリスチン、プレドニゾンを投与し、完全寛解に至った。

    70 歳，男性。数日で体重が 13 kg 増加し，低Alb血症(1.4 g/dL)が出現し，当科に入院した。入院時，リウマチ因子・抗核抗体・抗SS-A抗体陽性および補体低値で，体幹部造影CT検査で腹部大動脈に壁在血栓を認めた。99mTc標識ヒト血清Albシンチグラフィ検査を行い， 胃下部~上部空腸にradioisotopeの集積があり，自己免疫性蛋白漏出性胃腸症と診断した。 プレドニンを開始したが，その10日後，強い上腹部痛が出現した。D-dimer が著明な高値で，体幹部造影CT検査で腹部大動脈，上腸管膜動脈等に血栓が多発していた。ヘパリンによる抗凝固療法を開始し，腹痛は軽快，D-dimer は低下した。血清Albも上昇傾向に転じ，その後，体幹部造影 CT検査で血栓は消失した。動脈硬化性疾患の危険因子を有している蛋白漏出性胃腸症の症例においては，血栓症の精査を行い抗凝固療法の導入を検討すべきである。

  • Meckel憩室への異物穿通による肝膿瘍の1例

    54歳男性。1ヵ月にわたり持続する発熱の精査目的で入院となった。腹部超音波検査では肝右葉に最大40mm大の境界不明瞭な低エコー領域が認められた。また、腹部造影CTでは肝右葉に辺縁のみ造影される境界明瞭な低濃度域に加え、小腸内に20mm大の異物がみられた。一方、小腸内視鏡では異物は確認できなかったが、2度目のCTでも小腸内の異物は確認された。以上、これらの所見を踏まえて、本症例は小腸の異物穿孔による肝膿瘍が疑われた。腹腔鏡下手術を行ったところ、手術所見ではMeckel憩室に硬結が触知され、同部位を切除した結果、最終的にMeckel憩室への異物(魚骨)穿通による肝膿瘍と確定診断された。術後は発熱および炎症反応の再燃はなく、経過は良好であった。

    症例は68歳男性で、左下腹部膨満感を主訴とした。13日前に左下腹部と左腰部に皮疹を認め、8日前に帯状疱疹と診断され、アシクロビル点滴後バラシクロビル、プレガバリン内服を開始し、5日前に帯状疱疹の瘢痕化を認めていた。立位と坐位では左下腹部に5×15cm大の膨隆を認めたが、腹部単純CT検査で異常を認めなかったことから標記の診断とした。腹圧をかけないことと便秘をしないように指導して経過観察した結果、4ヵ月後には左下腹部の膨隆は消失した。

  • 導管血管および指尖血管の内皮機能に対する禁煙効果の予測因子(Predictors of the effects of smoking cessation on the endothelial function of conduit and digital vessels)

    血管内皮機能に対する禁煙の効果を血流依存性拡張反応(FMD)と反応性充血末梢動脈圧測定(RH-PAT)により評価し、その効果に関連する予測因子について検討した。禁煙成功者58名(平均64±11歳)と年齢、性を一致させた喫煙継続者21名を対象に、禁煙前および禁煙20週後の臨床検査値、FMD、および反応性充血指数の自然対数変換値(Ln-RHI)を解析した。禁煙成功者はFMDが有意に改善したが、Ln-RHIに著明な変化はなかった。両者の変化の関連性は、Spearmanの順位相関係数が-0.004で、級内相関係数が<0.001(p=0.499)であった。ロジスティック回帰分析でFMD増加と関連がみられた項目はBrinkman指数(オッズ比0.99,p=0.043)と禁煙前後の収縮期血圧変化(0.94,p=0.040)であり、Ln-RHI増加に関連する項目は収縮期血圧変化(1.06,p=0.047)と禁煙前BMI(0.83,p=0.032)であった。これらの項目が内皮機能に対する禁煙効果を予測し得る可能性が示唆された。
