    <p><b>Introduction</b></p><p>  The demographic composition of the ethnic Korean population in Japan (often referred to as<i> zainichi</i>) shows a declining birthrate. However, <i>zainichi</i> who live in <i>zainichi</i> Korean communities have more children than <i>zainichi</i> living in predominantly Japanese communities. Social capital (SC) can increase societal efficiency by promoting cooperative behavior within communities. This study clarified the family formation and SC that supports childcare for <i>zainichi</i> mothers living in <i>zainichi </i>Korean communities.</p><p><b>Methods</b></p><p>  We conducted semi-structured interviews with 10 <i>zainichi</i> mothers living in <i>zainichi</i> Korean communities in Japan. We qualitatively and descriptively analyzed the family formation and SC that support childcare in this population. The Ethical Committee of Osaka Prefecture University approved this study and the participants gave their written informed consent.</p><p><b>Results</b></p><p>  We extracted three categories and 12 subcategories for “thoughts on current family formation” and four categories and 11 subcategories for “SC supporting childcare.” <i>Zainichi </i>mothers living in Korean communities “think consciously about marriage” and “think positively about having multiple children and family formation,” but they have “concerns about childbearing and child-rearing.” “People who support childcare practically and mentally” and “childcare services” were also identified as important factors. Although <i>zainichi </i>mothers felt dissatisfied with “support that they wanted to use but found difficult to use,” they considered childcare support as human and physical SC. As an additional resource, they felt that “the <i>zainichi</i> community fosters security and bonding,” and thus promotes SC for child-rearing.</p><p><b>Discussion</b></p><p>  Networks with common norms, values, and understanding facilitated cooperation among <i>zainichi</i> mothers. “The <i>zainichi</i> community, which fosters security and bonding,” may support family formation through engendering a positive perception of child-rearing.</p>

  • Effectiveness of informational nursing support for nausea and vomiting of pregnancy: A systematic review Reviewed

    <p><b>Purpose</b></p><p>This study aimed to determine the effectiveness of informational nursing support for nausea and vomiting of pregnancy (NVP).</p><p><b>Methods</b></p><p>An exhaustive search was performed using five databases, and experimental or quasi-experimental studies with abstracts satisfying PICO were selected. We examined the texts of the selected articles and finally selected those that met our inclusion criteria. Using Cochrane's Risk of Bias ver.2, we independently assessed risk of bias in five domains and pooled the results to assess quality. Items with differing results were discussed and final ratings were combined.</p><p><b>Results</b></p><p>A total of six papers were adopted. Subjective symptoms of NVP were measured using the Index of Nausea, Vomiting, and Retching, The Motherisk Pregnancy-Unique Quantification of Emesis and Nausea, and the Visual Analogue Scale. Quality of Life (QOL) of pregnant women was measured using Health-Related Quality of Life for Nausea and Vomiting during Pregnancy. Additionally, a paper tracked the duration of NVP. Intervention methods included telephone visits, a combination of interviews and telephone visits, lectures in small groups, and a combination of lectures and discussions. All studies included a baseline survey, followed by surveys at two and four weeks post-intervention. One paper had weekly follow-ups until NVP converged. It was suggested that nursing support, which mainly included information provision for NVP, can be expected to have a certain effect in alleviating the pain experienced by pregnant women. However, as a result of quality evaluation using Risk of Bias ver.2, all papers were rated as “high risk of bias” or “some concern.”</p><p><b>Conclusion</b></p><p>In the existing studies examining the effects of nursing support for NVP, the risk of bias was inevitable because of the nature of the intervention. Therefore, it is imperative to increase the reliability of the results by conducting randomized controlled trials, publishing the protocol prior to the start of the study, unifying scales for measuring subjective symptoms of NVP, and limiting outcome indicators to one. Furthermore, the timing of the intervention and follow-up intervals that consider the physiological changes of NVP should be considered.</p>

    本研究は、思春期女子に日常的に接している小学校・中学校の養護教諭から見た思春期女子の月経に関する健康行動を構成している概念を明らかにすることを目的に、8名の養護教諭にグループインタビューを実施、SCATを用いて質的記述的に分析した。思春期女子の月経に関する健康行動の構成概念は「限られた環境での月経への対応」、「月経に関するキーパーソンである母親」、「養護教諭の対応」、「病院や医療職との連携」、「学校教育における反復学習の必要性」であり、思春期女子の月経に関する健康行動には「環境に合わせた具体的な対応の習得」「養護教諭と母親、病院、医療職との連携」「児童・生徒の個々の成長発達に合わせながらの学校教育とその反復学習」が重要である。限られた環境の中でも教諭が柔軟な姿勢でよりそうことなど思春期女子の月経に関する健康行動に必要なことを明示し、よりよい環境を整えていけるよう提言を行っていく必要性が示された。(著者抄録)

    新型コロナウイルス禍において子育て家庭が孤立化・密室化するなど、子育て家庭内外の環境が変化する中で、保護者の育児ストレスや子ども虐待の深刻化が懸念される。本研究では、今後の研究の予備的研究として、児童相談所(児相)や市区町村に対し、虐待相談などの実態や課題等に関する質問紙調査を実施した。その結果、例えば児相では前年同期に比して相談件数が相当減少しているが、虐待を含む養護相談は増加していること、自粛や在宅勤務の増加に伴い家庭内のトラブルが増大、DVや虐待に繋がっている可能性が大きいこと、児相や市区町村では子どもの保護や家庭への介入等に苦慮しており、財源・人的資源の確保、医療・保健分野によるバックアップ体制等を国や自治体に求めていることなどが分かった。そして、親子ともゲーム障害が深刻化している状況を踏まえ、SNSやゲーム依存に関するセミナーなどの啓発活動の必要性、児相や市区町村の体制整備や対応手法を標準化したマニュアル作成の必要性などについて提言を行った。(著者抄録)

  • Differences in gestational weight gain in accordance with Japanese and Institute of Medicine guidelines between Japanese and non-Japanese Asian pregnant women at a perinatal medical center in Japan

    <p><b>Objectives</b></p><p>  High gestational weight gain (GWG) is associated with perinatal risks to mother and child. Research shows that non-Japanese Asian women have higher GWG than Japanese women. However, no studies have compared GWG in these two populations using GWG recommendations in accordance with Japanese and Institute of Medicine (IOM) guidelines. The study aim was to compare GWG in non-Japanese Asian and Japanese pregnant women.</p><p><b>Methods</b></p><p>  This was a retrospective observational study. All participants were aged ≥20 years and gave birth between September 2019 and the end of October 2020 at one perinatal medical center in Japan. Medical record data were analyzed for 170 non-Japanese Asian and 316 Japanese pregnant women. We used t-tests and chi-square tests to examine differences in age, parity, smoking status, antenatal checkups, pre-pregnancy body mass index, and GWG. Logistic regression analysis was used to estimate odds ratios (95% confidence intervals) for above- and below-recommended GWG by non-Japanese Asian and Japanese status. We also analyzed differences in delivery type, abnormal blood loss, and birth size according to GWG.</p><p><b>Results</b></p><p>  After adjustment for confounding factors, the multivariable-adjusted OR and 95% CI for GWG above the Japanese guidelines recommendations was 1.86 (1.23-2.81) and that for GWG above IOM guidelines recommendations was 2.46 (1.45-4.16) for non-Japanese Asian women, as compared with Japanese women. Conversely, the multivariable-adjusted OR and 95% CI for GWG below Japanese guidelines recommendations was 1.55 (1.03-2.32) and that for GWG below IOM guidelines recommendations was 1.87 (1.26-2.76) for Japanese women, compared with non-Japanese Asian women. </p><p><b>Conclusion</b></p><p>  Because Japanese women tend to be below recommended GWG and non-Japanese Asian women tend to be above recommended GWG, midwives need to provide careful guidance to reduce perinatal risks.</p>

    目的:妊娠確定時と最も重い時期のつわり症状と妊娠前の食事内容との関連を検証する。対象と方法:52名の妊婦に妊娠確定時と妊娠14～20週の2回質問紙を配付した。妊娠確定時と最も重い時期のつわり症状は薄井の指数から嘔気、嘔吐の程度とつわり症状数、Visual Analogue Scale(VAS)によるつわりの辛さ・苦しさの程度、非妊時からの体重変化量とした。妊娠前の食事内容は半定量食物摂取頻度調査法を用いた。結果:つわりが最も重い時期のVASは、妊娠確定時のVAS、つわり症状数、嘔気と中程度に、嘔吐と弱く相関した。最も重い時期の嘔吐は、妊娠前ビタミンB1不足群が充足群より有意に症状が重く、最も重い時期のVASはビタミンB2不足群が充足群より有意に高かった。両時期の嘔気は、乳酸菌飲料を飲んでいなかった妊婦が、飲んでいた妊婦より有意に症状が重かった。結論:妊娠確定時のつわり症状の程度と、妊娠前のビタミンB1、B2、乳酸菌飲料の摂取状況を確認することで、つわり重症化の可能性がある妊婦を早期に支援できる可能性がある。(著者抄録)

    目的:小児期発症1型糖尿病(diabetes mellitus:DM)女性(以下、1型DM女性)が経験する月経周期に伴うセルフケアを明らかにする。方法:質的記述的研究。結果:1型DM女性7名への半構成的面接と分析で、169コード、28サブカテゴリー、9カテゴリーが示された。1型DM女性は【月経の影響を特に考える機会はない】状況と【1型DMを受容できないと月経は意識できない】時期があり、【みんなと同じような症状から月経周期に気づく】一方、1型DMでの妊娠を意識し【妊娠したいから月経周期を気にする】。1型DMの仲間と交流し【1型DMを受容する過程で月経を意識し始める】と、徐々に【血糖管理を通して月経の影響を捉える】。セルフケア行動は【みんなと同じような症状に自分なりに対処する】と共に【1型DMのコントロールによって月経への理解を深める】と【月経周期に伴う影響を踏まえて血糖管理を工夫する】ことが明らかとなった。考察:1型DM女性の月経周期に伴う血糖変動を意識した行動は、血糖管理の関連づけが影響していた。月経を意識したセルフケアは、まず1型DMの受容や妊娠への思いを把握する必要性が考えられた。(著者抄録)

    本研究の目的は、妊娠糖尿病の妊婦の関わりで支援の難しさを感じている助産師がどのように妊婦を捉えているかを明らかにし、妊娠糖尿病の妊婦に対してどのような支援ができるのか示唆を得ることである。助産師10名を対象に半構成的面接を行い、妊娠糖尿病の妊婦の支援において助産師が妊婦に対してどのようなことに困っているのかなど支援の難しさについて語られている部分を抽出し、質的記述的に分析した。その結果、35のコード、10のサブカテゴリー、4のカテゴリー【】が抽出された。助産師は妊娠糖尿病の妊婦について、【妊娠糖尿病の受容に伴う心理的落ち込み】【手応えのない反応】【あいまいな知識】【治療に専念できない】ことから支援が難しいと捉えていた。助産師は、自身のもつ妊娠糖尿病の妊婦への価値観を見直し、妊娠糖尿病を短期間で受容し療養生活に適応することを求められている妊婦の立場に立った理解が必要であると示唆された。そして、妊婦のレディネスにあわせた知識・技術の提供や妊婦の生活習慣を一緒に見直すなど、妊婦ができることから改善し、妊婦が自分の生活を継続できるよう支援することが重要である。(著者抄録)

    本プロジェクトでは、高校生が「自分を大切に思う気持ちを育て、命の尊さを感じる心と行動を身につけること」を目的に生と性の教育を実施している。時代の変化、環境の変化と共に高校生のセクシュアリティに関する価値観や課題には変化があり、近年ではスマートフォンやSNSの普及に伴う課題が多くみられる。そこで、これまで活動で使用してきた媒体を「今どき」高校生の課題に合わせて改訂し、ワークショップを実践した。「おつきあいのマナーかるた」改訂版では、SNSやLINEなど高校生が日常的に接するもののリスクをメッセージで伝えるように工夫した。実施後の高校生の反応からは、'相手を大切にすることの重要性''自分を大切にすることの重要性''話し合うことの重要性''SNSの危険性'が表出されていた。「今どき」高校生が楽しみながらも、自分や相手を思いやり、尊ぶ気持ちをもって行動できるように、今後も本活動を継続していきたい。(著者抄録)
