    [目的]運動機能向上にはinternal focus(IF)指示よりもexternal focus(EF)指示が効果的である。しかし、対象者の熟練度が低い場合や、継続的な介入においてはIF指示が有効である可能性が考えられている。そこで、未習熟課題への継続的介入におけるIF・EF指示の効果を検証した。[対象と方法]健常若年者55名を2群に分け、非利き手での投球課題に対してIF・EF指示を用いた14日間の介入を行った。[結果]両群の球速変化に交互作用はなく、時間の主効果が認められた。また、対象者全体の球速に有意な向上が認められた。[結語]IF指示はEF指示と同等に効果的であったため、未習熟課題の継続的介入において、EF指示だけでなくIF指示も有効である可能性が考えられる。(著者抄録)

    【目的】人工膝関節全置換術(以下、TKA)前後の歩行中の両下肢間協調性の変化と影響を与える要因について、Phase coordination index(以下、PCI)を用いて検討した。【方法】対象はTKA患者55名とし、各対象者の術前と退院前日のPCIを比較した。また、各時期でPCIと基本属性、術側膝関節痛、両膝関節伸展筋力、両膝関節可動域の関係について、相関係数および重回帰分析を用いて検討した。【結果】TKA患者のPCIは術前6.43±3.20%で、退院前日(術後約17日)7.01±3.14%と同程度であった。また、術前はPCIと非術側膝関節伸展筋力、疼痛と有意な相関が認められ、非術側筋力がもっとも影響の強い項目として抽出された。一方、術後はPCIと年齢とにのみ有意な相関が認められた。【結論】TKA術前後の歩行中の両下肢間協調性は同程度であるが、各時期で関連する要因が異なることが明らかとなった。(著者抄録)

    人工膝関節置換術後早期の歩行機能と大腿四頭筋力における改善の関係性と、各時点での大腿四頭筋力と歩行機能の改善比を評価した。片側人工膝関節置換術を実施予定の49例(女性79.6%、年齢64.0〜88.0歳)に対して、術前と術後2、3週に歩行機能、筋力、疼痛を評価した。歩行機能については、通常の歩行速度とtimed up and go(TUG)テストで測定した。大腿四頭筋力は等尺性収縮時に両側を筋力計で定量化した。歩行速度検査時の膝痛強度は視覚的アナログスケールで測定した。術前と比較して術後2週では歩行速度は15.5%低下し、TUGテストの所要時間は24.5%長く、大腿四頭筋力は54.1%低下した。3週後では歩行速度が8.2%低下し、TUGテストの所要時間は12.9%長く、大腿四頭筋力は43.9%低下した。術前と術後3週の術側大腿四頭筋力は、歩行速度およびTUGテストと有意に関連した。非術側大腿四頭筋力は、全評価時点で歩行速度とTUGテストに有意に関連した。多重回帰分析では、非術側大腿四頭筋力が術後2週の歩行速度を除いて、歩行機能と有意に関連した。術側大腿四頭筋力は全評価時点で独立変数ではなかった。術側大腿四頭筋力は歩行機能の重要な決定因子ではなかった。
