    目的:本研究は,放課後等デイサービスおよび児童発達支援事業所における医療的ケア児の受入有無の関連要因を検討することを目的とした.方法:2018年12月末日時点で放課後等デイサービスおよび児童発達支援事業所の検索サイトである「放デイどっとこむ」に掲載された,全国15,560ヶ所の放課後等デイサービスおよび児童発達支援事業所のうち約10%に当たる1,556ヶ所の事業所を無作為抽出した郵送質問票調査を実施した.結果:二項ロジスティック回帰分析の結果,医療的ケア児の受入有無の関連要因は,「訪問看護の併設(オッズ比(OR)=4.55)」,「連携:地域のリソースが具体的にわかる(OR=1.18)」,「看護師の人数(OR=14.94)」であった.結論:放課後等デイサービスおよび児童発達支援における医療的ケア児の受入には,看護師の配置や医療的ケア児を地域で支えるための連携が関連していた.今後,医療福祉連携の推進と適切な職員配置の基準化が重要と示唆された.(著者抄録)

    目的:本研究は,機能強化型訪問看護事業所での利用者特性に応じた訪問看護ケアの実施実態を明らかにした.方法:全国の機能強化型訪問看護事業所と利用者515名を対象に開発したデータ入力システムを用いた調査を行った.結果:利用者特性に関連する実施回数の多い訪問看護ケア項目は,がん末期では「疾病・治療の説明・指導(オッズ比(OR)=4.535)」,神経難病利用者への「衣生活のケア・指導(OR=2.276)」,小児への「精神的援助(OR=3.062)」「意思決定支援(OR=3.701)」が特徴的であった.結論:利用者特性別での実施回数の多い特徴的な訪問看護ケアが明らかになった.訪問看護のケア実施には,利用者特性を考慮する必要がある.(著者抄録)

    目的:本研究の目的は,医療的ケア児のレスパイトケア(以下,RC)に焦点をあて,小児の訪問看護を実施している訪問看護ステーションおよびその併設事業所におけるRCの実施実態とその関連要因を明らかにする.方法:全国の小児の訪問看護を実施している事業所1,154ヶ所を対象に,郵送法による自記式質問票調査を実施した.分析はχ2検定およびMann-WhitneyのU検定を用いた.結果:381名(32.8%)からの有効回答を分析対象とした.RC実施率は57.1%であり,放課後等デイサービスの併設は実施率と関係がみられた(p=.004).また,RCの実施には,看護職員数,ニーズ,看護の質の統一化,人員確保,事業所スタッフの児とのコミュニケーション能力,家族との信頼関係構築能力が関連していた(p=.00-036).結論:RCの実施には,人員配置や看護の質の統一化,基本的看護スキルの習得などの整備を行う必要性が示唆された.(著者抄録)
