    ＜最近の動向とガイドライン＞●前頭側頭型認知症(FTD)の疾患概念は,広い病態が含まれるようになっている.社会行動障害や性格変化を主徴とする行動型前頭型認知症と会話や言語が障害される原発性進行性失語症のほかに,進行性核上性麻痺,皮質基底核症候群,筋萎縮性側索硬化症を合併,発症することがあり,これらもFTDに含まれることもある.●FTDは,従来若年者の認知症の原因としてもっとも多く考えられていたが,最近の疫学調査では,発症率のピークは71歳であり,加齢に伴い増加することが明らかとなった.●早期に治療研究への移行を可能にするために行動型前頭型認知症に対しても,MBCI-FTD(mild behavioural and/or cognitive impairment in bvFTD)と称する発症前駆期の診断基準が提唱された.●家族性FTDの主な原因遺伝子はC9orf72,GRN,MAPTが多数を占めるが,わが国では家族性FTDは少ない.●FTDのバイオマーカーとしては,血液中のニューロフィラメント軽鎖(NfL)が有用である.特に家族性FTDにおいては,未発症キャリアから発症のリスクを予測できる.●FTDの治療薬研究としては,GRN,C9orf72遺伝子異常を有する家族性FTDに対して治験が行われている.(著者抄録)
